Migraines are severe, throbbing headaches that may or may not be accompanied by nausea, possibly with visual disturbances and other symptoms. Migraine incidences have increased by 50 percent within the last twenty years, with an estimated 25 to 30 million people suffering from them in the United States. Three out of four of these 25 to 30 million are women, with up to 24 percent of women experiencing at least one migraine in their lives and only 12 percent of men getting Migraines. Migraine headaches are one of the most severe types of headache.

Research has described Migraines as vascular headaches, as they involved excessive dilation or contraction of the brain’s blood vessels. Current research, however, gives other clues. The inflammation involved in Migraines is most noticeable in the meninges, which surround the brain and the spinal cord. However, the inflammation of the meninges is not what leads to the pain of migraine, but instead abnormal nerve activity. Stimulation of the trigeminal nerve, which goes from the brain to the head and face, triggers the release of substances that are known as calcitonin gene-related peptides. These induce inflammation and send messages to pain receptors in the meninges. Some researches even liken Migraines to meningitis in some ways, as the symptoms are very similar. However, meningitis is caused by viral or bacterial infection.

Migraines can occur anywhere from once a week to once or twice a year, often running in families. One factor that may contribute to the higher incidence of migraine in women may be fluctuations in the level of the hormone estrogen, as women typically get Migraines around the time of menstruation, when estrogen levels are low. Most often, Migraines occur in people between the ages of twenty and thirty-five, seeming to decline with age. Children can also suffer from Migraines, with pain tending to be more diffuse, rather than localized. Migraines can first show up in childhood as colic, periodic abdominal pain, vomiting, dizziness, and severe motion sickness, rather than as headaches.

There are typically five phases in a migraine. The first phase beings a day or so before the onset of a headache, as changes in mood, problems with memory, an alteration in one or all of the five senses, and speech problems occur. Secondly, some people will see flashes or patterns of light and experience numbness of the hands and mouth just before the headache. This is called an aura, and is a common sign of a classic migraine.

Those Migraines that occur without auras are called common Migraines. The third phase occurs when the headache starts with a severe, throbbing pain occurring on one or both sides of the head and can also move from side to side. Nausea can also set in this phase, along with tenderness in the neck and scalp, sensitivity to light, and possibly immobilization due to pain. The fourth phase occurs when the headache dissipates, although nausea may linger. During the fifth phase, the person may feel tired and lethargic, simply wanting to sleep.

A variety of things can trigger a migraine in an individual who is susceptible, including allergies, constipation, stress, liver malfunction, too much or too little sleep, emotional changes, hormonal changes, sun glare, flashing lights, lack of exercise, and changes in barometric pressure. Feverfew is most commonly used in the treatment and prevention of migraine headaches. Migraines are extremely common and can be very difficult to treat. Other methods of relieving pain have been explored because, for many migraine sufferers, conventional treatments have not been successful. Feverfew is one of the most successful forms of alternative treatments that have been found for treating Migraines. Many studies have been conducted which have validated the already existing knowledge of many herbalists: feverfew is a great tool for helping to prevent Migraines in many cases.

Although feverfew had long been used for treating headaches, its popularity declined with the advent of modern medicine. The herb has resurged into medical interest after the wife of a doctor who worked for the British National Coal Board had been suffering from Migraines and was told by a coal miner to trey chewing on two feverfew leaves a day. After trying the herb, the woman noticed improvement, with fewer and less severe Migraines. Her husband then urged a migraine specialist in London, Dr. E. Stewart Johnson, to test feverfew, with the results of several studies now showing it to be effective.

Dr. Johnson agreed to try the herb because he had many patients who had been suffering from migraine pain for years and had yet to find a successful treatment. Originally, he tried the herbal remedy on only ten of his patients. The results of this trial were so promising that it led to a study of another 270 of his patients, with seventy percent reporting that they received significant improvement when receiving feverfew for their headache pain.

Following this, a group of researchers in the City of London Migraine Clinic were led by Dr. Johnson to conduct a study, which was reported in the British Medical Journal, investigating seventeen patients who had already been using feverfew for at least three months. All were asked to discontinue use of their original method of treating with feverfew. Eight patients were given a capsule of feverfew, while the other nine were given a placebo.

Those that were given a placebo had an increase in frequency and severity of headaches, nausea, and vomiting, while those that were given feverfew capsules had no increase in frequency or severity of their Migraines. This research has led to the belief that feverfew is advantageous in the prevention of Migraines. Many other studies have been done to further test the effects of feverfew. It has been concluded that feverfew treatment is effective and there were always definite improvements in the group that used feverfew. Additionally, there were no serious side-effects that appeared to exist.

Feverfew seems to have similar properties as NSAIDs such as aspirin and ibuprofen. Also, the parthenolide that is found in feverfew is the main inhibitor for the formation of compounds that promote inflammation. Parthenolide reduces the secretory activity of blood platelets and white blood cells. This is important because Migraines are thought to be associated with abnormal platelet behavior, as they are responsible for the release of serotonin, which constricts blood vessels and leads to migraine pain.

Feverfew is now recognized by the British and Canadian governments as a migraine treatment and is often prescribed by doctors. Butterbur extract is taken mainly from the rhizome, root and leaves of the butterbur, a member of the daisy family. They are very hardy and have creeping underground rhizomes and large leaves like those of rhubarb. Another name given to it is the sweet Coltsfoot, and they generally grow in the temperate climates of Europe, North Africa and South west Asia. They like damp conditions, specifically marshes and ditches, and also riverbanks where there are always plentiful supplies of moisture.

It has been used by Native Americans for headaches and inflammation, and has been shown to be an effective remedy for hay fever and to provide relief from painful menstrual cramps. Butterbur has also been used throughout the middle Ages to treat fever and the plague, and has been recorded in the seventeenth century as being used for asthma, wounds and coughs. However, one of its most important applications is in restore bladder function in the incontinent and semi-incontinent.

Urinary incontinence is typified by an unusually high frequency of urination – more than 8 times a day, an immediate strong urge to pass water or leaking and involuntary urination. Any two of these three indicates urinary incontinence. As people age their bladders become smaller, and by definition the periods between urination will reduce. This does not, however, suggest that bladder size is the cause of urinary incontinence.

Urination is caused by the contraction of the smooth layered muscle that surrounds the bladder, called the detrusor, a contraction in turn caused by neurons both in the brain and in the detrusor itself. This naturally contracts and expands according to the volume of urine in the bladder, and once the bladder is about half full the brain will tell you that the detrusor is ready to contract to expel the urine. However, if the time is not convenient, the cortex will suppress this desire until a more convenient time.

In incontinence, the desire is suppressed but the neurons still fire to contract the detrusor, expelling urine at inconvenient moments. Butterbur contains the sesquiterpenes petasin and isopetasin, which are known to reduce spasms in smooth muscle tissue and in vascular walls. It can therefore be used to control the involuntary spasms that cause urine leakage or expulsion against the patient’s wishes. These sesquiterpenes are at highest concentration in the roots of the plant.

The effect that the sesquiterpenes have in inhibiting the synthesis of leukotriene in leukocytes tends to support this effect, since leukotrienes can cause contraction of vascular and smooth muscle tissue. Not only this, but the spasmolytic effect could also be explained by the inhibition of cellular calcium caused by the petasin isomers.

Many studies have indicated that the effectiveness of butterbur extract is also useful in the prevention of Migraines. There has been a lot of research carried out on the use of butterbur extract on migraine sufferers, and the effective dose appears to about 75 mg twice daily. There is little evidence of it being a cure but as a prophylactic there appears no doubt of its efficacy: there have been too many positive results against placebos for its effect to be deniable.

It is significant that leukotriene can cause constriction of the small blood vessels in the veins, and so affect the flow of blood. Butterbur, in inhibiting its biochemical production, helps to keep these blood vessels open. Lekotrienes are also important components of inflammation, and altogether it appears that whatever the real cause of migraine, the petasin isomers in butterbur have an effect in inhibiting its initiation. Add to that the potential reduction in calcium content that can cause blood vessels to become less flexible, and the argument for its effectiveness is both irrefutable and well explained.

In one example of such a double blind study that is representative of many, a group of patients given 50 mg butterbur extract twice a day for twelve weeks experienced a 60% reduction in the frequency of attacks, a reduction in the severity of the attacks they did have, and a reduction in the length of the attacks. Although the vascular theory of the cause of migraine is no longer supported, maintenance of the vascular system appears to at least reduce the likelihood of attacks.

The effect of butterbur on asthma and other allergic reactions is also well documented. This again is due to its anti-spasmodic properties and inhibitory effect on the inflammatory immune response through the inhibition of leukotriene synthesis and the consequent positive effect on the metabolism of prostaglandin. Prostaglandins also constrict vascular smooth muscle cells, regulate the mediation of the inflammatory response and constrict general smooth muscle cells. All of these can lead a to a variety of disorders cause by smooth muscle spasms in additional to urinary incontinence, such as menstrual cramps, liver and gastrointestinal disorders and asthmatic conditions.

In one study of allergic rhinitis, administration of butterbur extract appeared to result in a reduction in the histamine and leukotriene content of nasal fluids and no difference was noticed between this treatment and histamine treatment. This was a useful study because histamines causes drowsiness and butterbur can be used as a substitute for histamine without the sedative effect. A study in Germany in 1993 has shown that the stomach ulceration caused by the anti-inflammatory medications for arthritis was reduced by the administration of butterbur extract

Cetirizine is a commonly prescribed prescription treatment for allergic conditions, and studies comparing that with butterbur demonstrated them to be equally effecting in reducing the symptoms typical of allergic reactions such as sneezing, runny nose and nasal congestion. 50% of the patients in the group took each and there was no difference in results. Again it was explained by the petasin limiting the production of leukotriene and histamine, both of which are produced by the immune response and promote mucous secretions and inflammation. Before you can understand how to manage Migraines naturally, it is necessary to understand exactly what they are and how certain treatments could work. FEVERFEW

Feverfew is the best known of the herbal remedies for migraine, and has been used for centuries in the treatment of fevers, headache, migraine and other painful conditions such as arthritis. The active ingredient is parthenolide, an anti-inflammatory that is also an anti-hyperalgesic, and that would explain its effect on reducing the pain. It also blocks lipolysaccharide-induced osteolysis by suppressing the activity of a eukaryotic transcription factor protein known as NF-kappaB that can cause a number of immune response problems, including inflammatory conditions such as arthritis, hence its use to relieve that condition.

Its effect on Migraines could be explained by this activity in repressing the inflammatory response to certain stimuli that could be brought about by hormonal activity. Because it must be taken for a few weeks before the actual problem arises in order to be effective against it, feverfew is not effective to treat an existing attack, but to prevent future attacks. Double blind studies have proved its effectiveness in many tests, but not in all. However, if you take feverfew as a regular supplement in you diet, you might never have to try magnesium or vitamin B-2 supplements.

Some people swear by the leaves of feverfew, eating one per day which is easy way to take it. It is important, however, that you do so regularly, since it can take for up to 6 – 8 weeks for you to notice a reduction in your attacks. Don’t give up after 5 or 6 weeks, but continue, since the results will be well worth it. If you stop for a week or two you will have to start all over again. You might get a sore mouth and perhaps mouth ulcers to start with, but these will eventually go once your body is used to it.

You are probably best not to take if you are pregnant since it could cause you to abort. Otherwise, however, it has been shown to be effective in many severe cases. Feverfew is bitter, and can be used as a tea with other herbs, such as rosemary, lavender and sage. If you are taking warfarin, aspirin or other blood-thinning drugs, see your doctor first, and the same is true of NSAIDs (non-steroidal anti-inflammatory drugs). If you are taking these for arthritis, the feverfew itself might be more effective!

If you are taking feverfew as a supplement using commercially available dried leaf then the recommended dosage is around 125mg of the dried leaf. It should contain at least 0.2% of parthenolide (0.25mg/daily), although other studies have indicated that a total of 0.7 mg parthenolide a day should be taken (100mg at 0.7%). Parthenolide is unstable so don’t take it after its sell by or best by date.

BUTTERBUR

Butterbur is also useful in the treatment of Migraines. The active ingredient here is petasin, a sesquiterpene ester that also has anti-inflammatory properties, all though it works in a different manner to feverfew. Here, it inhibits the activities of leukotriene generation in neutrophils and eosinophils, which indicates that it also possesses anti-allergenic properties that could help asthmatics that also suffer from Migraines. It does not prevent the release of histamine, but does appear to reduce smooth muscle spasm.

You should never take the raw herb, only the extract, because it contains alkaloids that can harm you. The butterbur extract, however, is a perfectly safe supplement and found effective by many people in the treatment of migraine.

GINGER

If your migraine is at the front of the head, then you should try ginger. Unlike feverfew, ginger works on a migraine as it is occurring, and improvement can be achieved for a bad migraine within half an hour. In fact if, like many people, you can feel a migraine coming on, half a teaspoon of freshly ground ginger can stop the pain before it starts.

Many herbal treatments for migraine include a combination of some of the above remedies, and many people swear by them. However, migraine treatment is very personal, and you should use what works best for you. Understanding EFA

Soon after this constriction occurs, the blood vessels then reflexively open, or dilate. When the vessels become dilated, they occupy more space in the brain, activating nearby pain receptors. It is speculated that an imbalance of mineral stores in the body can lead to this spasm of the blood vessels. Many researchers have suggested magnesium plays an important role in migraine attacks. The activities of magnesium in the body include preventing blood vessel spasm, inhibiting blood clotting, and stabilizing cell membranes, all of which are involved in migraine develoment4. Magnesium concentration exerts an effect on neurotransmitter production and receptors, pro-inflammatory molecules, and other migraine-related chemicals in the brain.5 Recent evidence suggests up to 50 percent of migraine patients have lowered levels of tissue magnesium during an acute migraine attack.6 Another study discovered brain magnesium concentrations were 19 percent lower in patients during migraine attack compared to healthy controls.7 Because recent research strongly indicates a magnesium deficiency in migraine headaches, natural medicine practitioners prescribe magnesium along with other trace minerals as a primary treatment for this condition with great success.

Because of their widespread distribution throughout the metabolic workings of the human body, trace minerals are integral to the functioning of one of the body’s largest organ systems, the muscles. Mainly, magnesium plays a large role in the relaxation of muscles following their contraction. Without this vital nutrient, it would be impossible for the muscles of the human body to function. Muscle cramps are prevalent in western society due to lack of intake of an appropriate amount of minerals. One easy, straightforward cure for muscle cramping is supplementation with magnesium and other trace minerals, as they allow the muscles to function smoothly and correctly. The role of magnesium in relieving cramped muscles also makes it a highly appropriate therapy for the muscle pain associated with fibromyalgia, a condition that is often treated successfully by practitioners of natural medicine. These practitioners often use high doses of magnesium and other trace mineral combinations to reduce the painful and tender muscles that are so common in fibromyalgia patients.

Another condition that is successfully treated with magnesium and trace minerals is Chronic Fatigue Syndrome. People with this condition often experience profound muscle aches and weakness. It has been shown that in order for proper muscle contraction and relaxation to occur, magnesium and calcium need to be present in proper amounts in the body, which can be difficult to achieve even on a standard healthy diet. Premarin and Provera are two of the most commonly prescribed progestin drugs and come with a wide variety of side effects and health risks. Over 30 negative side effects are listed for Provera alone. Taking Provera during early pregnancy may actually cause a miscarriage or deformity of the developing fetus. Premarin is comprised of two different types of estrogen called estrone and estradiol, which have been linked with the development of certain kinds of cancer.

The risks involved with taking these synthetic progestins is due to their chemical structures. For the most part, the compound resembles natural progesterone, therefore it binds to the same receptor sites as to natural progesterone. The altered portions of the molecule, however, convey a totally different signal to the cells involved. It is this atomic manipulation which poses alarming health risks to the body.     While white sugar, turbinado, fructose, honey and corn syrup all qualify as natural sweeteners, none of these are calorie- free nor can they be used by people who suffer from blood sugar disorders. They can encourage weight gain, tooth decay, raise blood sugar quickly, and can also predispose certain individuals to yeast infections. These sugars can also contribute to indigestion, bowel disorders and, possibly, hyperactivity or ADD in children. Pharmaceutical sweeteners like aspartame and saccharin qualify as calorie-free but come with significant limitations and health risks. Saccharin has been labeled with a warning that it has caused the development of cancer in laboratory animals but is still available for purchase. In 1970, cyclamates, another class of artificial sweeteners, were banned because of the strong possibility that they are, in fact, carcinogenic. Aspartame has been marketed as a safe substance for the general public, except for those few individuals who suffer from PKU (phenylketonuria), a relatively rare disorder. Most consumers assume that aspartame is a perfectly benign compound and use it liberally. It is, in fact, comprised of phenylalanine, aspartic acid, and methanol (wood alcohol). As previously mentioned, various side effects have been associated with the ingestion of aspartame and include Migraines, memory loss, slurred speech, dizziness, stomach pain, and even seizures.

    In addition, because aspartame contains chemicals which affect brain cell function, significant questions have been raised concerning its link to increased incidence of brain tumors (Olney). Acesulfame K, another artificial sweetener on the market, has also been linked to cancer by the Center for Science in the Public Interest. Despite the protest of various organizations and health professionals, these pharmaceutical s weeteners have been approved by the FDA and are recognized as safe.

    Studies have been ongoing as to the benefits of feverfew in relieving migraine pain since the 1970’s. The latest clinical study confirms the beneficial effects of feverfew. Researchers in Israel reported on a clinical trial involving 57 individuals, none of which had ever tried feverfew. Half of the group were given the feverfew supplement while the others a placebo. Those taking the feverfew reported a significant reduction in migraine pain and accompanying symptoms such as sensitivity to light, vomiting and nausea. The treatment was switched during the study giving the placebo group the feverfew supplements. Again, the group taking the feverfew had a reduction in Migraines (D. Palevitch, G. Earon and R. Carasso. “Feverfew as a prophylactic treatment for migraine: A double-blinded placebo-controlled study.” Phytotherapy Research, 1997, 11 (7): 506-11).

    EXERCISE MAY PREVENT DIABETES

    Exercise is important for health and prevention of illness. One study lead by Michael Brown of the Un i versity of Pittsburgh followed 12 overweight women who had a condition which often leads to diabetes. Exercise has been thought to help prevent diabetes, but just how much was necessary has not been determined. Exercise such as brisk walking practiced for 30 minutes for at least four days a week is thought to help.

    In a related study reported by CNN March, 4, 1998, walking was found to help improve insulin sensitivity. Elizabeth Mayer-Davie of the University of South Carolina reported the result of her research. The study published in the Journal of the American Medical Association followed 1,400 women between the ages of 40 to 69. Some had normal blood sugar levels and others had a mild form of diabetes. Those who exercised moderately and regularly were less likely to have impaired insulin-using capacity which is involved with diabetes.

    CLINICAL APPLICATIONS OF CAPSICUM

    Capsicum is a remarkable whole body stimulant that can boost blood flow, tone the nervous system, relieve indigestion, promote sweating, help to cauterize and heal ulcers, ease persistent pain and fight off infection. One very authoritative work on African plants suggests that Capsicum’s “regular ingestion is highly beneficial in hemorrhoids, varicose veins, anorexia, liver congestion and vascular conditions . . .the indigenous inhabitants of Africa and of the Antilles are remarkably free form all of these conditions as they use Capsicum fruit in their diet.”10 Most of the therapeutic actions of Capsicum are attributed to the alkaloid or glucoside content of the herb.11 The latest scientific studies conducted with Capsicum will be discussed in subsequent sections.

    Herbal Catalyst

    Because Capsicum boosts peripheral circulation and stimulates organ secretion, it expedites the therapeutic delivery and action of other herbs. In other words, the medicinal benefits of these herbs reach infected or inflamed tissue more rapidly due to enhanced blood flow.12 Consider the following statement: “Cayenne will insure the rapid and even distribution of the active principles of the rest of the herbs to critical function - al centers of the body, including those involved in cellular respiration, metabolism, data transmission, and neural-hormonal activation. Cayenne is included in several other blends for this reason. In extremely small quantities it can dramatically increase the efficiency of most other herbs.”13 Many health practitioners believe that the key to healing is CAPSICUM stimulation. Capsicum stimulates eve rything from blood flow to peristaltic action in the stomach, to intestinal transit time. The re m a rkable ability of Capsicum to stimulate organ secretion and even heart action makes it one of the strongest natural stimulants known. Se veral different kinds of herbal blends targeting various body systems will utilize Capsicum to boost the formula’s efficacy.

    Cardiovascular Tonic

    Capsicum is said to be unequaled for its ability to boost circulation and increase heart action. Interestingly, cultures who consume significant amounts of cayenne pepper in their diet have much lower rates of cardiovascular disease.14 Capsicum exerts a variety of desirable actions on the entire card i ovascular system. Murray, N.D., has stated, “ Cayenne pepper [Capsicum] should be recommended as a food for its beneficial antioxidant and cardiovascular effects.”17 Herbalists have considered Capsicum as a superior “f o o d” for the heart. In fact, in cases where a heart attack is suspected administering capsicum in hot water has been thought to help lessen the severity of the attack. Capsicum can also be placed on or under the tongue in emergencies involving heart attack, stroke or hemorrhaging. 18 Note: Using Capsicum for any heart-related problem, especially a suspected heart attack should never take the place of medical attention or a physician’s care.

    CAPSICUM Blood Cholesterol Reducer

    Various studies have conclusively demonstrated that Capsicum reduces the risk of developing atherosclerosis (hardening of the a rteries) by reducing blood cholesterol and triglyceride levels .19 Additional clinical studies conducted in India found that when cayenne was ingested along with dietary cholesterol, the typical rise in liver and blood serum cholesterol levels was significantly inhibited. In addition, bile acids and free cholesterol were subsequently eliminated from the body through the stool.20 Interestingly, these tests revealed that using Capsicum was actually more effective in reducing cholesterol that capsaicin alone.2 1 Daniel Mowrey, Ph.D., emphatically points out that this is just one of many examples of the superiority of whole botanicals as opposed to their isolated components.22 Note: Using Capsicum in combination with Hawthorn is a particularly good cardiovascular tonic.

    Blood Pressure Equalizer

    While an added bonus of Capsicum’s capability to lower blood serum cholesterol is a decrease in blood pressure, additional evidence strongly suggests that the herb initiates other mechanisms that fight hypertension .23 “Cayenne, according to another study, also reduces the blood pressure in an even more direct manner: a number of years ago, a team of researchers discove red that capsaicin acts in a reflexive manner to reduce systemic blood pressure, a kind of coronary chemoreflex.”24 Adding Garlic to Capsicum creates an even better therapeutic blend for treating hypertension.

    Blood Detoxification CAPSICUM

    “Cayenne is a kind of catalyst in the blood purification process . . . it acts as a diaphoretic, stimulating the excretion of wastes in the swe a t . ”25 Because Capsicum stimulates organ secretion and boosts peripheral blood flow, it would only stand to reason that it would also facilitate the faster removal of toxins from the bloodstream and lymphatic system. You may have already noticed that Capsicum is frequently added to blood-purifying herbal combinations. Circulatory Booster Researchers have found that the simulating action of Capsicum on surface capillaries can help to pre vent cold hands and feet.2 6 For this reason, it may be helpful for Reynaud’s Syndrome. Old remedies using Capsicum have even recommended placing it in socks to warm the feet and to help prevent frostbite. An old folk cure for a chilled body was a steaming hot cup of Capsicum tea. Free Radical Scavenger The rich flavonoid content of Capsicum gives it significant antioxidant capabilities. A recent study conducted in 1995 showed that Capsicum has a higher ascorbic acid content than chiles from the jalapeno or serrano varieties .27 Vitamin C and bioflavonoids can scavenge for dangerous free radicals which cause tissue damage and can predispose organs to degenerative diseases. Free radicals are found everywhere and are created as by-products of metabolic p rocesses including the act of breathing itself. Pollutants can expose the body to free radicals. An interesting study done in Mexico City and published in 1993 found that Capsicum extract was able to modulate the mutagenic activity of urban air samples.28 In other words, these potentially dangerous nitro - a romatic compounds found in polluted air were kept from mutating by red chile extract.29 Chemical breakdowns of Capsicum have also found that CAPSICUM the pepper is high in Provitamin A, which significantly contributes to its healing ability and immune fortification.30 Anti-Carcinogenic Compound Anti-cancer research recently tested Capsicum on laboratory rats and found that it does indeed demonstrate anti-cancer properties by inhibiting certain enzymes which can initiate the mutation of cells.31 What this implies is that taking Capsicum can afford the body some protection against the cellular mutation which occurs in malignant growths. Capsicum actually inhibited the formation of dangerous metabolites under laboratory conditions where they should have normally been activa t e d .3 2 This study implies that Capsicum may have many more sophisticated bio-chemical actions than previously thought.

    An Impressive Pain Killer

    Capsaicin has recently emerged as a remarkably effective pain reliever and has become the subject of recent clinical research . Applying capsaicin in cream or ointment form to painful joints, scar tissue or other painful conditions involving peripheral nerves confuses pain transmitters. In other worlds, capsaicin temporarily disrupts sensory nerve cell biochemistry there by impeding the relay of pain sensations from the skin surface. It does this by inhibiting a neurotransmitter called substance P. This specific compound is thought to be the main mediator of pain impulses from peripheral nerve endings.33 Substance P has also demonstrated its ability to inhibit inflammatory pain generated in arthritic joints in much the same way.34 Today, several over-the-counter topical preparations utilize capsaicin for the pain of arthritic joints. The ability of Capsicum to control severe and unresponsive pain is significant, to say the least. Modern clinical utilization of topical capsaicin may offer signifi-cant relief for a number of painful conditions including: diabetic neuropathy, cluster headaches, post-amputation pain, post-mastectomy pain, shingles and painful scar tissue.35

    POST-SURGICAL PAIN

    In the early spring of 1996, prime time national news show s reported that scientists had found that individuals who had suffered from chronic pain in post-surgical scars (heart bypass, arterial grafts, etc.) were successfully treated with topical preparations containing capsaicin. While this may have been news to many of us, clinical studies had been already published for several years that capsaicin held profound value for various kinds of pain which did not respond to established medical treatments. Typically surgical scars and regions around them can produce persistent pain or can be very sensitive to the touch even when completely healed. This type of pain phenomenon seems to respond well to capsaicin ointments and creams.

    POST-MASTECTOMY PAIN

    When capsaicin preparations were applied following mastectomy or breast reconstruction, pain was significantly relieved. Se veral double blind studies found that using capsaicin creams four times daily for 4 to 6 weeks resulted in much less frequent occurrence of sharp, jabbing pain.3 6 All thirteen patients studied had a 50 percent or greater improve m e n t .3 7 Various unpleasant sensations other than pain also improved with topical applications of capsaicin creams.38

    MOUTH SORES FROM RADIATION OR CHEMOTHERAPY

    A fascinating study conducted at the Yale Pain Management Center discove red that capsaicin could ve ry significantly lessen pain caused by mouth sores which frequently develop after chemotherapy or radiation.39 Apparently delivering the capsaicin in the form of soft candy (taffy) enabled the substance to be retained in the mouth long enough to desensitize the nerve endings causing the pain. Each one of the eleven case studies re p o rted that their pain had decreased and in two patients, it stopped entirely.40

    DIABETIC NEUROPATHY

    Diabetic neuropathy is a painful nerve condition which can develop in cases of prolonged diabetes. Several double-blind studies have supported the considerable value of capsaicin creams for relieving the pain associated with this disorder.41 The results of a controlled study using Capsicum for seve re cases of diabetic neuropathy which did not respond to conventional therapy were published in 1992. A cream containing Capsicum was applied to painful areas four time a day and pain was carefully e valuated for 8 weeks at two-week intervals. The results we re impressive, to say the least. In a follow-up study, approximately 50 percent of the subjects reported improved pain control or were cured . . .”42 No t e : While there was a burning sensation when the Capsicum c ream was first applied, some subjects found that its magnitude and duration lessened with continued application.43

    SHINGLES

    The FDA has approved capsaicin-based ointments for the treatment of pain that results from diseases like shingles. Again, numerous studies have documented the value of capsaicin for decreasing the miserable nerve-related pain associated with shingles. The general consensus derived from these tests were that approximately 50 p e rcent of people suffering from shingles responded well to capsaicin creams, some even after 10 to 12 months.44

    Note: If blisters accompany a shingles outbreak, it is better to wait until they have healed before using any capsaicin-based ointments or creams.

    RELIEF FOR BURNING FEET

    Frequently an uncomfortable “burning” sensation in the feet will occur in many people, particularly in diabetics. As ironic as it may seem, using capsaicin creams may actually alleviate this burning. “In various studies, diabetics who treated their burning feet with capsaicin got greater improvement and we re able to walk more easily than those not using the cream.”45 In addition, using topical applications of capsaicin as opposed to strong, oral drugs is much more preferable.

    ARTHRITIS PAIN

    Clinical tests have confirmed that topical capsaicin ointments substantially alleviate the miserable pain that characterizes osteoand rheumatoid arthritis.46 These studies revealed that using 0.075 capsaicin cream reduced tenderness and pain.47 Dr. Michael T. Murray writes: “ . . . seventy patients with osteoarthritis and thirty - one with rheumatoid arthritis received capsaicin or placebo for 4 weeks. The patients were instructed to apply 0.025 percent capsaicin cream or its placebo to painful knees four times daily. Significantly more relief of pain was reported by the capsaicin-treated patients than by the placebo patients throughout the study . . .”48 Anyone suffering from osteo or rheumatoid arthritis should evaluate the effectiveness of capsaicin ointments for joint pain. Ester Lipstein-Kresch, M.D., has studied the effectiveness of capsaicin creams for arthritis and has stated: “You need to apply it three or four times a day on the affected area for at least two weeks before you’ll see any improvement. An initial burning sensation at the site is not unusual for the first few days, but this goes away with continued application.”49 Note: Capsaicin is also useful for tennis elbow due to its ability to block the transmission of pain.

    MIGRAINE HEADACHES (CLUSTER TYPE)

    Topical applications of capsaicin ointments intranasally may also help to relieve the pain of a specific kind of migraine headache called cluster headaches. Cluster headaches are characterized by s e ve re pain which typically radiates around one eye.     How many of us give the red hot chile pepper the respect it d e s e rves? Mo re often than not, most of us re g a rd red pepper or Capsicum as nothing more than the spice added to give Cajun and Mexican cuisine its piquant kick. Technically speaking, caye n n e pepper is the strongest red pepper variety of the Capsicum family, with paprika being the mildest.

    Throughout this discussion, the terms capsicum and cayenne pepper will be used interchangeably. For our purposes, it’s important to know that herbalists have designated both of these terms for the same botanical agent. Health practitioners have known for centuries that Capsicum is much more than a culinary spice. Because they considered it a “ h o t” plant, Chinese physicians utilized it for physiologic conditions that needed stimulation. Capsicum or Cayenne Pepper is one of the few herbs that can be measured by its BTU or thermal units. In other words, it is a hot and stimulating pepper plant that can generate heat.

    What's in a name? A headache by any other name hurts just as much. But categorizing your headache can be as overwhelming as finding an obscure breakfast cereal at the supermarket. Medical folks pigeonhole headaches as tension headaches, allergy headaches, morning headaches and sinus headaches, plus the organic, migraine, cluster, trauma, TMJ, eyestrain, rebound, exertion, hormonal and muscle tension varieties. You may also suffer the self-induced hangover and toxicity headaches.

    Americans are no strangers to self-medication, and as a society we battle these headaches by consuming nearly 80 billion tablets of aspirin each year, about 20 million aspirins a day, according to Burton Goldberg, co-author of An Alternative Medicine Definitive Guide to Headaches (Future Medicine Publishing). And while we drown ourselves in over-the-counter and powerful prescription products, our tolerance, just like a drug addict's, grows. So what begins as a twodose headache slowly evolves into three, four or five doses, until you realize that no dosage can make your headache go away.

    Unless you're fond of medieval practices such as ritualistic healings and bloodletting, drugs seem the only answer to headache misery. But even modern medicine has been unable to solve headaches. Rather than a cure, medications provide only temporary relief, and even that isn't guaranteed. Additionally, over-reliance on medications can lead to chronic head pain. According to Goldberg, rebound headaches often result from the consumption and withdrawal of drugs.

    Halt Your Headaches

    Rather than strive to simply eliminate headache pain, why not identify the underlying cause and prevent the headache? Almost always, every type of headache results from a health or lifestyle-related activity. In an ideal world, the best approach would be to eliminate the tensions, stress, frustrations, anger, insufficient sleep, excessive drinking and poor diet that contribute to your headaches. The study doesn't expect you to change your entire diet, join a gym and eliminate everything that triggers stress, but teaches techniques to make subtle lifestyle changes and reduce the impact of daily stress. These same techniques can also improve sleep patterns, a factor known to affect headaches. The lifestyle modification approach clarifies that while very few headaches directly result from existing acute conditions, recurring headaches often derive from a larger lifestyle factor. If these issues are ignored, the body's natural defense mechanisms may kick into overdrive and become exhausted, leaving you susceptible to other chronic diseases. By finding and eliminating the cause of your headaches, you may be saving yourself from other eventual illnesses.

    Never underestimate the old-fashioned ice pack to the forehead as an effective treatment that works by constricting the swollen blood vessels that cause your head to ache. If the ice pack isn't enough, try putting your arms in icy water up to your elbows to constrict additional blood vessels. But if it's a sinus headache you're fighting, take the opposite approach and try hot compresses.

    The herbal, and original, form of aspirin is white willow bark. Used by Chinese practitioners 2,500 years ago, it contains salicin, nearly the same pain reliever found in aspirin. Other herbal aspirins include meadowsweet tea, just as effective as aspirin with few side effects. For all headaches, Goldberg suggests essential fatty acid supplementation with evening primrose oil (EPO), which improves circulation, helps regulate inflammation and relieves pain.

    The Curse of the Migraine

    Migraines, a debilitating headache distinguished by a throbbing pain, may be humanity's oldest malady. The name is derived from the word the Greek physician Galen used to describe the disorder in 200 A.D. Six-thousand-year-old Sumerian writings refer to the ravages of Migraines, and prehistoric skeletons bear testimony to a crude form of trephination-holes chiseled in skulls to allow the escape of pain-creating demons, according to Lifetime Encyclopedia of Natural Remedies by Myra Cameron (Parker Publishing).

    In the common migraine, throbbing pain develops gradually from distended veins around the brain and may be accompanied by nausea, vomiting and sensitivity to light. And while some doctors summarily attribute Migraines to diet, other experts attribute at least half of all Migraines to food sensitivities. The most common edible villains are aged cheeses, yeast breads, chocolate, cured meats, citrus fruits, eggs, fermented foods, wheat, milk, milk products, alcoholic drinks and food additives, including nitrates and MSG.

    Suggested daily supplements for migraine-susceptible individuals include a multivitamin, one capsule of B complex to help maintain normal vascular control, 3,000 to 6,000 milligrams in divided doses of vitamin C with bioflavonoids to assist the production of anti-stress hormones, and 500 to 1,000 milligrams of magnesium to reduce nerve excitability and pain. At the onset of a migraine, says Cameron, try homeopathic remedies, dilutions of natural substances from plants, minerals and animals. For throbbing pain, take natrum muriaticum according to package directions. Other homeopathic options to explore include iris versicolar, lac defloratum and sanguinaria.

    Headache management involves managing your life. Practice relaxation techniques to reduce stress. Calmly discuss your feelings. Take a leisurely stroll at lunch. Walk your dog, or a neighbor's dog, after dinner. Keep a food diary. Sit and stand tall. Skip the fast food tonight. Join a yoga class. Take vitamins and supplements. Get a two-hour massage. Turn the television off earlier and sleep in a little later.

    Taking the time to care may take effort, but it will soon become second nature. Invest in yourself. It's your most precious possession.

    Abstract

    Petasides hybridus (butterbur) is a perennial shrub, found throughout Europe as well as parts of Asia and North America, that has been used medicinally for centuries. During the Middle Ages butterbur was used to treat plague and fever; in the 17th century its use was noted in treating cough, asthma, and skin wounds. The plant can grow to a height of three feet and is usually found in wet, marshy ground, in damp forests, and adjacent to rivers or streams. Its downy leaves can attain a diameter of three feet, making it the largest of all indigenous floras, and their unique characteristics are responsible for the plant's botanical and common names. The genus name, Petasites, is derived from the Greek word petasos, which is the felt hat worn by shepherds. The common name of butterbur is attributed to the large leaves being used to wrap butter during warm weather. Other common names include pestwurz (German), blatterdock, bog rhubarb, and butter-dock.
