Garcinia Cambogia is found naturally in India and parts of Asia, and also on the Pacific coast of South and Central America all the way from Peru up to Mexico, and likes a humid forested environment. Also known as Brindall berries, garcinia is believed to act as an appetite suppressant and allows you to lose weight by diminishing your desire for food. The Malabar tamarind, as it is also known, is about the size of an orange resembling a small pumpkin, and an extract from the fruit and rind is used in several weight loss products. The health risks presented by synthetic diet pills render a natural product extremely attractive were it to be effective. So can Garcia curb your appetite? What is the scientific evidence for it, and what biochemical route would it take? Although tests on animals have been very positive, human results have been inconsistent. In some double blind tests using a placebo, weight loss was up to three times that of the control, but in others there was no apparent difference between those taking garcinia, and those given a placebo. However, doubts have been raised of the validity of some of the negative tests, so what does science tell us? A study at Georgetown University in Washington found that after 8 weeks of taking the garcinia extract, there was a 5.4% reduction in body weight and body mass index, and a significant reduction in low density lipoproteins (LDL) and triglycerides with an associated rise in high density lipoproteins (HDL). This is good news for those suffering from high cholesterol levels, since the LDL lipoproteins are those that carry cholesterol to the major blood vessels, and which when oxidized by free radicals deposit fatty plaques on the artery walls. These plaques constrict the arteries and the resultant atherosclerosis can lead to cardiac problems and strokes. HDL lipoproteins carry cholesterol back to the liver for destruction, and is known popularly as ‘good cholesterol’. The studies also indicated modifications to certain indicators of the status of fat deposits in the body and of appetite modifiers in the brain. In this respect they found 38% decreases in serum leptin and increases in serotonin levels of 44%, and the excretion of fat metabolites in the urine increased from between 32% and 104%. These are significant findings, and further research has indicated that HCA helps to suppress appetite. Serum leptin is an indicator of the level of fat stores in the body, and as the leptin levels in the blood reduces, the hypothalamus is given an instruction to increase the appetite so as to increase the fat levels again. However, it is believed that HCA possesses leptin-like properties, and this signal is either not generated or is modulated. The increase in serotonin has the same effect. It is known that serotonin controls the appetite, although the exact mechanism has not yet been established. What is known is that serotonin activates certain neurons and melanocortin-4 receptors (MC4R) in the brain, that not only curb appetite but also block the effect of other neurons that would normally increase appetite by blocking the effect of MC4Rs. This is how the banned anti-obesity and serotonin inciting drug Fen-Phen operated, and it appears that the hydroxycitric acid in garcinia cambogia extract acts in a similar, but safer, way. The problem with drugs such as Feb-Phen was that they created cardiac problems which could be dangerous to obese people whose hearts might have been weakened. However, now that the biological pathway by which serotonin controls weight is believed to be known, if not fully understood, the way is becoming clearer as to how safer weight loss pills, acting through appetite suppression, might be developed. It also provides a valid scientific explanation for the effect of garcinia cambogia extract which might in itself prove to be that safer way. Bone Health Approximately 44 million American women and men aged 50 and older have osteoporosis (severe bone loss) or osteopenia (mild bone loss), with women being affected about twice as often as men. At least 1.5 million fractures of the hip, vertebra (back or neck), or wrist occur each year in the United States as a result of osteoporosis, and the annual cost of treating this disorder is nearly $14 billion and rising. Unfortunately, the toll in human suffering and loss of independence is even greater. In this issue of Ask the Doctor, we will discuss the risk factors for osteoporosis and some key nutrients you can add to your diet that can minimize bone loss and reduce your chances of developing this disease. Q. What are the risk factors for osteoporosis? A. Small body frame, underweight, Caucasian or Asian race, a sedentary lifestyle, cigarette smoking, excessive alcohol or caffeine intake, high intake of carbonated beverages (especially colas), and having other family members with osteoporosis all increase personal risk of developing the disease. Certain medical conditions, including diabetes, celiac disease, hyperthyroidism, rheumatoid arthritis, chronic obstructive lung disease, hyperadrenalism, and hyperparathyroidism, are all associated with an increased risk of osteoporosis. Some medications increase the rate at which bone is lost; these include drugs prescribed for the treatment of seizures, drugs used for blood thinning, steroids such as prednisone, aluminum-containing antacids, and loop diuretics (furosemide {Lasix}). Q. Isn’t bone loss just a normal consequence of aging? A. Although bone mass normally declines after the age of 35, bone loss severe enough to cause fractures after just minor trauma (such as bump or fall) seems to be a relatively new phenomenon. Osteoporosis was rare in the late 19th century, and it was not until around 1920 that the condition began to attract attention among doctors. Since that time, the percentage of people who develop osteoporosis has continued to increase. For example, the age-adjusted prevalence of osteoporosis in Q. Can osteoporosis be prevented? A. Engaging in regular weight bearing exercise, avoiding excessive consumption of alcohol and caffeine, and quitting smoking will slow the rate of bone loss. Eating adequate, but not excessive, amounts of protein also enhances bone health. In addition, a growing body of research has shown that supplementing with various vitamins and minerals may not only help prevent, but in some cases actually reverse, bone loss. At least 15 different nutrients have been found to play a role in bone health. Q. What type of calcium is best? A. For most people, calcium salts are absorbed about the same, between 30% and 40% of the administered dose. People who low stomach acid (hypochlorhydria) should not use calcium carbonate, because that form of calcium is absorbed poorly in the absence of stomach acid. Calcium phosphate may be preferable for many older people, because phosphorus is necessary for normal bone formation, the phosphorus intake of older people is often low, and calcium supplements inhibit the absorption of phosphorus. Also, calcium bound to phosphorus is the form in which calcium in the bone is stored, and it has a much greater bone activity than other forms. Q. How much vitamin D is needed to promote strong bones? A. Because vitamin D is produced when the ultraviolet rays from the sun hit skin, people who stay out of the sun, wear sunscreen, or live in a northern latitude (such as Boston or Seattle) where less ultraviolet light reaches the skin, are at increased risk of vitamin D deficiency. In addition, aging decreases a person’s ability to synthesize vitamin D in the skin. Results from five research trials on vitamin D found that supplementation with 700-800 IU of vitamin D per day decreased the number of hip fractures by 26%, but 400 IU per day was ineffective. In addition to enhancing bone health, vitamin D improves nerve and muscle function in older people, thereby reducing their chances of falling down. Supplementation of elderly women with 800 IU of vitamin D per day has been shown to decrease the number of falls by about 50%. Q. Is that much vitamin D safe? A. Bone is living tissue, constantly remodeling itself and engaging in numerous biological functions. Like other tissues in the body, bone has a wide range of nutritional needs. The typical refined and processed American diet has been depleted of many different vitamins and minerals, some of which play a key role in promoting bone health. Not getting enough of one or more of these micronutrients may be and important contributing factor to the modern epidemic of osteoporosis. In addition, supplementing with calcium may cause a loss of magnesium, zinc, silicon, manganese, and phosphorus, unless these nutrients are also provided. Q. What nutrients besides calcium and vitamin D promote healthy bones? A. Magnesium, zinc, copper, manganese, vitamin K, boron, strontium, silicon, folic acid, vitamin B6, vitamin B12, phosphorus, and vitamin C have all been shown to play a role in bone health. Following is a brief description of the role that each of these 15 nutrients play in building healthy bones. Calcium: A component of the mineral crystals that make up bone. Vitamin D: Enhances calcium absorption, prevents falls by improving nerve and muscle function. Magnesium: Important for bone mineralization (accumulation of minerals which form bones). Magnesium deficiency is associated with abnormal bone mineral crystals in humans. In an open clinical trial, magnesium supplementation increased bone mineral density by an average 5% after 1-2 years in postmenopausal women. Copper: Laboratory research has found that copper promotes bone mineralization and decreases bone loss, and that osteoporosis can develop if the diet is deficient in copper. Western diets often contain less copper than the amount recommended by the National Academy of Sciences. In a 2-year double-blind trail, copper supplementation reduced bone loss by 90% in middle-aged women, compared with a placebo. Zinc: Like magnesium, zinc is important for bone mineralization, and also has been shown to decrease bone loss. Low dietary zinc intake was associated with increased fracture risk in a study of middle-aged and elderly men. The zinc content of the diet is frequently low; a study of elderly low-income people found they were consuming only half the Recommended Dietary Allowance for this mineral. Manganese: Plays a role in the creation of the connective-tissue components of bone. Manganese deficiency in laboratory tests resulted in low bone mineral density and weak bones. Manganese deficiency may be associated with the development of osteoporosis. Boron: Supports creation of bone-protecting hormones such as estrogen, testosterone, and DHEA. Boron supplementation prevented bone loss in experimental studies. In human volunteers consuming a low-boron diet, boron supplementation decreased urinary calcium excretion by 25-33%, a change that may indicate reduced bone loss. Silicon: Plays a role in the synthesis of the connective-tissue components of bone. Silicon deficiency has been associated with bone abnormalities. In an observational study, higher dietary silicon intake correlated with higher bone mineral density. In a clinical trial, administration of an organic silicon compound increased bone mineral density of the femur (or thigh bone) in postmenopausal women. B vitamins (folic acid, vitamin B6, and vitamin B12): These three B vitamins have been shown to lower blood levels of homocysteine, a breakdown product of the amino acid methionine. An elevated homocysteine concentration is a strong and independent risk factor for fractures in older men and women. Homocysteine levels increase around the time of menopause, which may explain in part why bone loss accelerates at that time. In a 2-year double-blind trial, supplementation of elderly stroke patients with folic acid and vitamin B12 reduced the number of hip fractures by 78%, compared with a placebo. Strontium: This trace mineral is incorporated into bone and appears to increase bone strength. It also stimulates bone formation and inhibits bone breakdown. Controlled trials have demonstrated that strontium supplementation of postmenopausal women increases bone mineral density and decreases fracture risk. Vitamin K: Best known for its effect on blood clotting, vitamin K is also required for the creation of osteocalcin, a unique protein found in bone that participates in the mineralization process. The amount of vitamin K needed for optimal bone health appears to be greater than the amount needed to prevent bleeding. Vitamin K levels tend to be low in people with osteoporosis. In randomized clinical trials, supplementation of postmenopausal women with vitamin K prevented bone loss and reduced the incidence of fractures. Q. Which form of vitamin K is best? A. Two forms of vitamin K compounds are present in food: vitamin K1 and vitamin K2. Vitamin K1 (also called phylloquinone) is present in leafy green vegetables and some vegetable oils, and vitamin K2 is found in much smaller amounts in meat, cheese, eggs, and natto (fermented soybeans). To make things a little more complicated, Vitamin K2 itself can occur in more than one form. The two most important to this discussion are menaquinine-4 (MK-4, also called menatetrenone), which is licensed as a prescription drug in Research suggests that MK-7 from natto may be an ideal form of vitamin K. The biological activity of MK-7 in laboratory studies was 17 times higher than that of vitamin K1 and 130 times higher than that of MK-4. After oral administration, MK-7 was better absorbed and persisted in the body longer, compared with MK-4 and vitamin K1. Although both have shown ability to prevent osteoporosis in laboratory research, a much lower dosage (600 times lower) of MK-7 is required, compared to MK-4, to obtain beneficial effects. Thus, MK-7 has greater biological activity, greater bioavailability, and possibly more potent effects on bone, compared with other forms of vitamin K. The potential value of MK-7 for bone health is supported by an observational study from Q. Why is strontium so important in building strong bones? A. Strontium is of great interest to bone health researchers and has been studied in very high doses. Surprisingly, lower doses are not only safer for long-term supplementation, but may in fact have a greater impact on bone health than very high doses. Too little, and bone density is impaired; too much and health may be impaired. This is a case where dosing needs to be just right for optimal impact. Therefore, until more is known, it is wise to keep supplemental strontium at less than 6 mg per day. Q. Can people taking osteoporosis medications also take bone-building nutrients? A. Because nutrients work by a different mechanism than osteoporosis drugs, nutritional supplements are likely to enhance the beneficial effect of these medications. Calcium or other minerals may interfere with the absorption of biphophonates such as alendronate (Fosamax) or etidronate (Didronel). For that reason, calcium and other minerals should be taken at least two hours before or two hours after these medications. Irritable bowel syndrome (IBS) is a painful and frequently frustrating disorder of the intestines that’s often difficult to treat. Fortunately, there are scientifically studied natural products that effectively reduce the distressing symptoms of IBS. Q. What is IBS? A. IBS causes crampy pain, gassiness, bloating, and alterations in bowel habits. IBS is termed a functional disorder, because when the colon is examined, there is no visible sign of disease. While IBS causes significant pain and distress, no actual damage is occurring in the intestines. There is a wide variability in IBS. Symptoms may be mild and fairly well tolerated. Or, the pain, disComfort, and bowel dysfunction may be disabling, limiting social interactions, employment, or travel.
While some individuals with IBS have diarrhea (frequent, loose stools with an urgent need to move the bowels), others have constipation (hard, infrequent stools that are difficult to pass). And, still others may experience both. Individuals with IBS also may have painful abdominal cramps and feel an urgent need to move the bowels, but are unable to do so. A. The small intestine receives digestive material from the stomach and delivers it to the large intestine (colon). About two quarts (2,000 ml) of digestive material enter the colon from the small intestine every day. The colon absorbs water and salts from the material, which is progressively moved through the colon. This progressive movement continues until most of the fluid and salts are absorbed into the body and stool is formed. The stool passes to the left side of the colon, where it is stored until a bowel movement occurs. Because researchers haven’t been able to find actual damage in the colon, it once was suggested that individuals with IBS have emotional problems or are overly susceptible to stress. While stress may cause symptoms of IBS to intensify, it doesn’t cause the condition. Recent study has determined the colon muscle of an individual with IBS spasms after only mild stimulation. It’s thought the symptoms of IBS are produced by hyperactivity of the intestines. In other words, the intestines of individuals with IBS are more reactive to stressors and diet than usual. Almost everyone has experienced abdominal queasiness in response to everyday stress or certain foods. This may result in a brief bout of diarrhea or an upset stomach. However, this response is exaggerated in individuals with IBS. Q. How prevalent is IBS?
A. IBS is very common. In fact, it’s one of the most frequent problems seen by family physicians. It’s the most common disorder diagnosed by gastroenterologists (physicians specializing in the treatment of digestive disorders). The overall prevalence rates range from 10% to 20% of the general population in most industrialized countries. As a result, the pain and disabling symptoms associated with IBS result in significant socioeconomic costs, as wall as reduction in quality of life for many individuals. A. Normal bowel function varies from person to person. Some people move their bowels daily, while others may only have two to three stools a week. A normal bowel movement is soft, formed, and is easily passed without cramping or pain. IBS, however, causes abdominal cramps and pain, which are often severe and disabling. Bowel movements may be irregular and alternate between diarrhea and constipation. The diarrhea may be quite loose and watery. Mucous may be passed. There is often much straining, urgency, and feeling of incomplete evacuation (emptying). Abdominal bloating and passing of gas is common. Nausea, lack of appetite, heartburn, and belching may also be present. Sleep may be disrupted resulting in fatigue and lack of energy. Understandably, persons with IBS often feel anxious and depressed. Diagnosis of IBS is usually based on the continuous presence or recurrence of these symptoms for at least three months. Other intestinal conditions must be ruled out. These include Chron’s disease, ulcerative colitis, inflammatory bowel disease, colon cancer, inflammatory conditions of the stomach or pancreas, ulcers, infectious disease, or gastroesphageal reflux disease. Q. Are there clinically demonstrated natural alternatives to the over-the-counter drugs prescribed by my doctor? A. Yes, both enteric-coated peppermint oil and clown’s mustard (in combination with other herbs) have significant scientific research behind them. Both have been demonstrated to benefit individuals with IBS. Q. What is clown’s mustard and what does it do? A. The scientific name for clown’s mustard is Iberis amara. Other names for this herb are wild candytuft and bitter candytuft. Clown’s mustard is a white-flowering plant from Spain, where it grows in dry soil on hillsides and in cornfields. It is also grown in Britain, France, and the United States. Iberis amara is a member of the Brassicaceae family. Iberis refers to its place of origin, the Iberian Peninsula. Amara means bitter. The key components of clown’s mustard are glycosides and flavonoids that have specific actions on gastrointestinal tract tone. Q. Is there scientific evidence that clown’s mustard benefits people with IBS? A. There has been very impressive research on clown’s mustard (in combination with other herbs). And, it has been used with great success in Germany for many years to treat IBS and other gastrointestinal diseases. In a study of an herbal combination containing clown’s mustard, 20 patients were given the herbal combination for three to 32 days. They all had been diagnosed with chronic functional disorders for at least one to 20 years. The symptoms the patients experienced included pressure and pain in the abdomen, belching, heartburn, vomiting, nausea, fullness, lack of appetite, constipation, and diarrhea. The patients have been treated for their problems with a variety of antacids, anti-spasmodic agents, and motility-inducing substances. For the purposes of the study, the patients stopped taking these medications and received treatment only with the herbal combination. Abdominal pressure and pain in the abdomen was the most common of all the experienced symptoms, with 11 of the patients rating it as severe. After six days of treatment, only sic of the patients continued to rate their abdominal pain and pressure as severe. After two weeks, this symptom had completely resolved for 16 of the patients. Diarrhea had been rated as severe in five of the patients. By day 14, only one patient continued to have moderate diarrhea. Medications prescribed and taken for cardiovascular diseases, arthritis, and autoimmune diseases often cause gastrointestinal problems. Because these conditions are chronic, these medications must be taken for a long time, often for life. With long-term use, these medications can cause erosion of the stomach lining and actual ulcers. Many of these medication-caused symptoms are similar to IBS symptoms: pressure and pain in the upper abdomen, nausea, abdominal fullness, and lack of appetite. Most, if not all, of the individuals who have gastrointestinal problems caused from medications experience two or more of these IBS symptoms. Forty patients who were taking medications for various types of cardiovascular disease and arthritis, and who are experiencing gastrointestinal problems related to their medications, were enrolled in a study. These symptoms included pressure and pain in the upper abdomen, nausea, abdominal fullness, and lack of appetite. Twenty patients received clown’s mustard combined with other herbs that support gastrointestinal motility. Three days after the trial started, a significant improvement of all s symptoms was noted in those taking this combination. By day 14, abdominal pressure and pain, nausea, and heartburn were completely eliminated in the herbal combination group. Several other clinical trials that were conducted in Germany report similar results. Q. How does this herb compare to prescription drugs? A. A study compared clown’s mustard (combined with other herbs) to Reglan (metoclopramide), which is frequently prescribed to reduce the symptoms of IBS. While metoclopramide is a very effective medication, it also has numerous side effects. Metoclopramide can cause fatigue, anxiety, agitation, jitteriness, insomnia, yellowing of the skin or eyes, changes in vision, hallucinations, and seizures. Because of these serious side effects, metoclopramide must not be taken longer than 12 weeks. In comparison study, 77 subjects were randomized to receive treatment of either clown’s mustard in a combination with other herbs, or metoclopramide. All subjects had pain and pressure in the abdomen, cramping, abdominal fullness, nausea, heartburn, and lack of appetite. The subjects took 20 drops of their assigned treatment after meals three times daily. The duration of treatment was one to two weeks. In both groups, a parallel improvement of all symptoms was observed. At no point in the study was a statistically significant difference in symptoms found. Both treatments significantly reduced pain and pressure in the abdomen, cramping, abdominal fullness, nausea, heartburn, and lack of appetite. In short, both metoclopramide and the clown’s mustard herbal combination worked well at reducing the symptoms of IBS.
However, side effects occurred more frequently and severely in the metoclopramide group. Given the lack of differences noted between the products at reducing symptoms of IBS, it would seem sensible to choose the treatment with the fewest reported side effects and no limits on duration of use. A recent study compared the use of peppermint oil and commonly used medications to reduce the colonic spasming in colonoscopy. The peppermint oil was introduced directly into the colon. Effective reduction of colon spasming was observed in 88% of the patients. In a critical review and meta-analysis of peppermint oil for irritable bowel syndrome published in The American Journal of Gastroenterology, eight randomized controlled trials were identified. The studies collectively showed peppermint oil is superior to placebo in improvement of the symptoms of IBS. Because of the good results of these trials, the authors of the review urged additional study of peppermint oil in IBS. However, straight peppermint oil is rapidly absorbed into the blood stream from the stomach. In recent studies comparing enteric-coated peppermint oil capsules and non-enteric coated oil, both preparations provided effective symptom relief. However, the studies concluded the enteric-coated capsules delivered the benefit of the peppermint oil directly to the intestines. In the treatment of IBS, enteric-coated supplemental peppermint is most definitely preferred. In fact, an enteric-coated peppermint oil capsule containing rosemary and thyme is extremely effective in the treatment of IBS. All three of these oils are classified as volatile oils, derivatives found in plants that impart taste and aroma. Betaine HCI and Pepsin Betaine HCl is a form of HCl used as a nutrient to supplement the stomach’s own production of HCl, or stomach acid. While occasional indigestion may be a result of acid irritating tissue in the structure above the stomach known as the esophagus, a line of research suggests that the cause of this irritation may actually be less than optimal stomach acid production. A major trigger for the process of tightening the sphincter is the presence of sufficient stomach acid. When sufficient stomach acid is sensed, the LES will close. However, in conditions where there is a lack of stomach acid, the sphincter remains open, allowing stomach contents, including acid, to flow back through the opening, potentially creating a sense of irritation and disComfort. Adequate stomach acid production is an essential criterion for the sphincter to function properly and prevent the backflow of stomach contents.1 A recent study assessed the incidence and causes of low vitamin B12 levels in elderly patients. The researchers suggest that the incidence of decreased vitamin B12 in the elderly, based on results of some epidemiological studies, is as high as 30-40%. When they looked at the possible causes of low B12 levels in 200 individuals that they followed, they found that food-B12 malabsorption accounted for 60-70% of the cases.2 In other words dietary B12 is bound to foods, generally animal proteins. The protein is normally broken down in conjunction with acid and pepsin in the stomach. However, low production of stomach acid may decrease the efficiency of this process and vitamin B12 remains bound to the protein source, leaving it unavailable to be absorbed. To understand glucosamine's role, it is important to understand joint structure and function. Cartilage in the joints acts as a shock absorber to cushion the blows of daily wear and tear. Surrounding the joint cartilage is synovial fluid, which contains many substances including its chief component, hyaluronic acid. Hyaluronic acid forms the backbone of other proteoglycans and is responsible for the thickness of synovial fluid as well as its lubricating and shock-absorbing properties. Synovial fluid also provides nutrients for the joint cartilage. Glucosamine sulfate is a normal constituent of glycosaminoglycans in cartilage and synovial fluid. In essence, glucosamine sulfate provides important building blocks for cartilage production. Laboratory studies suggest that glucosamine may also function to stimulate production of cartilage-building proteins. It is also thought that the sulfate portion of the molecule contributes to the efficacy of glucosamine sulfate in the synovial fluid by providing the elemental sulfur needed for strengthening cartilage and aiding glycosaminoglycan synthesis. 1,2,3 Glucosamine sulfate has been the subject of research for over twenty years. Clinical trials as well as experimental studies have repeatedly supported the efficacy of oral glucosamine sulfate in supporting joint function. In one large open trial, over 1200 people took oral glucosamine sulfate for periods ranging from 36 to 64 days. In this multi-center trial, ninety-five percent of the subjects experienced greater joint Comfort and increased mobility. The physicians reported "good" results in 59%, and "sufficient" results in 36%. This activity of Curcumin is thought to play a major part in its role as a joint protective nutrient. In fact, the numerous beneficial effects attributed of whole turmeric are thought to stem in large measure from the antioxidant properties of curcuminoids. Antioxidants neutralize free radicals, which are highly unstable molecules that can damage cellular structures through abnormal oxidative reactions. Curcumin is not toxic to cells, even at high concentrations. Pure Curcumin was shown to be less protective than a mixture of curcuminoids, indicating a possible synergism among the curcuminoids.8 Curcumin demonstrates several other in vitro effects linked to free radical scavenging. Curcumin scavenges nitric oxide, a compound associated with the body’s inflammatory response.9 Curcumin also demonstrates in vitro inhibition of certain enzymes involved in promoting inflammatory reactions in the body. Together these results strongly suggest that Curcumin is a potent bioprotectant with a potentially wide range of therapeutic applications.9,10,11 Preliminary human trials have assessed the therapeutic potential of Curcumin, with results that verify the traditional use of turmeric as an herb to enhance joint health. In a short-term double-blind, cross-over, comparative study, eighteen people were randomized to receive Curcumin (1200 mg daily) or an alternative therapy for two-week periods. Two groups of researchers have conducted analyses of trials that utilized SAM-e for mood enhancement. One meta-analysis was published in 1994. The researchers analyzed the efficacy of SAM-e in oral or injection forms based on published trials dated between 1973 and 1992. The authors concluded that there was a significant improvement of 17 to 38% seen in trials of SAM-e compared to placebo response. They state that the efficacy of SAM-e was superior to placebo and its administration caused few side effects.5 A second review was published in 2002. The authors analyzed studies in which SAM-e doses ranged from 200 to 1600 mg daily. They also found a significant effect of SAM-e in comparison to placebo, with an evident rapid onset of effect at enhancing mood.6 Promotes Joint Comfort and Mobility* As a sulfur donor to connective tissue, SAM-e plays a major role in protecting the integrity of cartilage tissue. An in vitro trial assessed the actions of SAM-e in cultured human articular chondrocytes. At a concentration of 10 micrograms/ml, proteoglycan synthesis and sulfate residue incorporation in chondrocytes was shown to be 60% higher than control levels. The treatment was rated as “very good” or “good” in 71% of the participants, with an additional 21% rating the treatment effect as “moderate”.9 In a long-term trial lasting 24 months, SAM-e was given to 108 participants with compromised joint function. Individuals were given 600 mg orally per day for the first two weeks followed by 400 mg daily for the remainder of the trial. Individuals experienced significant enhancements in joint Comfort, with dramatic improvements noted after 2-4 weeks of treatment. Improvements continued to 6 months and beyond.10 In addition to the above studies, a review was conducted in 1987 to assess the results of SAM-e supplementation in clinical trials for enhancing joint mobility and function. Over 22,000 individuals had participated in the clinical trials that were the subject of this review. The author concluded from his analysis that SAM-e was shown to be highly efficacious, rivaling or surpassing the effectiveness of other treatments, and also possessing a high level of safety.11 Because of this, SAM-e may be the treatment of choice for enhancing joint function. The green in plants possesses unique powers. Green landscapes soothe the soul. A verdant expanse of green vegetation offers Comfort, peace and ecological consolation. " In the area of China in which we did our study about one in 10 adults die from liver cancer, and it's the third leading cause of cancer deaths worldwide," says George Bailey, PhD, a professor of environmental and molecular toxicology at OSU. "The findings of this research could be enormously important to many areas of China, Southeast Asia and Africa, where aflatoxin-related liver cancer is a real concern. Many of these deaths might be preventable with supplements that cost pennies a day." This research looked at about 180 people in Qidong, China. When people in the study were given supplements containing chlorophyll derivatives, they had less than half the DNA damage of people who didn't take supplements. According to the scientists, chlorophyll and similar substances may act as interceptor molecules, blocking the absorption of carcinogens. As John Groopman, professor at the Johns Hopkins School of Public Health, observes, the supplements these people took "...can effectively reduce aflatoxin levels, which should reduce the risk of liver cancer." Closer to home, other researchers point out that chlorophyll-rich vegetarian foods may help protect us from carcinogens in the typical American diet. If you've ever enjoyed a hunk of grilled meat, you've consumed substances scientists call heterocyclic amines, which are contained in the charred part of meat cooked on a grill. Studies have shown that these tasty tidbits can increase your risk of breast and other types of cancer. (Your risk from charred meat greatly increases if you are also a smoker.) However, if you eat a food like spirulina, a blue-green algae high in antioxidants that also contains plenty of chlorophyll, its protective substances can bind with these carcinogens within your digestive tract and keep them from being absorbed. Green Keeps You Younger While we always hear that eating more fruits and vegetables enhances our health, new research shows that eating green foods adds extra power to an anti-aging program. Two experiments at the University of South Florida Center for Aging and Brain Repair, published in the Journal of Neurobiology (7/15/02), show that spirulina and other greens can help shield the brain from the antioxidant damage that accumulates as one ages and may help reverse declines in learning and memory. The first study found that a diet rich in spinach helped lab animals stay smart as they grew older. Spinach's benefits, according to the researchers, are due to its rich antioxidant content, which can counteract free radicals (caustic molecules) created in the body during normal metabolism and increased by exposure to environmental pollutants, sunlight and radiation. When free radicals attack, cell walls and other cellular structures are compromised and DNA can malfunction. A lifetime of free-radical damage can slow your thinking and may be one of the causes of Alzheimer's and Parkinson's diseases, says Dr. Paula Bickford, lead author on the project. The second study found that the protective effect of green plants may be linked to their ability to reverse age-related accumulations of potentially harmful inflammatory substances in the brain. In fact, spirulina didn't just slow the deterioration of neurotransmitter interactions caused by aging, it actually improved their function. " Not all foods are created equal," says Dr. Bickford. "Cucumbers taste good and have lots of fiber. But unlike spirulina and apples, they are not rich in phytochemicals that have antioxidant or anti-inflammatory effects in the brain." Green Immunity Aside from assisting brain function, spirulina also seems able to help pump up the immune system. Researchers at the University of California at Davis found that adding spirulina to cultured immune system cells significantly increases the production of infection-fighting cells called cytokines. A number of previous laboratory studies have found that spirulina can balance immune response: While easing allergic reactions, this powerful green food also was found to enhance the ability of immune cells called macrophages to both destroy bacteria and eliminate cancerous cells. " We found that nutrient-rich spirulina is a potent inducer of interferon-g (13.6-fold increase) and a moderate stimulator of both interleukin-4 and interleukin-1b (3.3-fold increase)," notes Eric Gershwin, professor at UC Davis. "Together, increases in these cytokines suggest that spirulina is a strong proponent for protecting against intracellular pathogens and parasites, and can potentially increase the expression of agents that stimulate inflammation, which also helps to protect the body against infectious and potentially harmful micro-organisms." Today, the average American consumes about 10 mg of soy protein a day, even though the American Heart Association (AHA) recommends taking in at least 25 mg of soy to benefit your heart. Even the Food and Drug Administration (FDA) has signaled its approval of the soy bandwagon, allowing claims that daily soy can help lower the risk of cardiovascular complications. As Good as a Drug Researchers who have investigated how soy can help lower cholesterol and shrink the risk of heart disease have concluded that soy, in a diet with fruits and vegetables, can be as effective as cholesterol-reducing drugs (JAMA 7/22/03). Researchers at the University of Toronto and St. Michael's Hospital compared the cholesterol-lowering power of soy and other vegetarian foods with that of lovastatin, a standard pharmaceutical used to reduce cholesterol. In the study, scientists fed people a diet that, along with soy, had large amounts of nuts, such as almonds and walnuts, and high-fiber foods like oats and barley plus margarine made with plant sterols (natural substances derived from leafy greens and vegetable oils). Researcher David Jenkins, PhD, a nutrition science professor, thinks these foods may be good at dropping cholesterol because human evolution makes us well-adapted for an "ape diet," one high in fiber, vegetable protein, nuts and plant sterols. According to Dr. Jenkins, "As we age, we tend to get raised cholesterol, which in turn increases our risk of heart disease. This study shows that people now have a dietary alternative to drugs to control their cholesterol, at least initially." Dr. Jenkins also thinks that soy and a vegetarian diet can be used to maintain normal cholesterol levels. Soy Meals Dr. Jenkins' heart-healthy diet, designed to be easily prepared and consumed, includes oat bran bread and cereal, soy drinks, fruit and soy deli slices. For instance, in his study, a typical dinner consisted of tofu baked with eggplant, onions and sweet peppers, pearled barley and vegetables. Dr. Jenkins adds, "The Food and Drug Administration has approved these cholesterol-lowering foods as having legitimate health claims for heart disease risk reduction. They're also being recommended by the American Heart Association and the National Cholesterol Education Program as foods that should be incorporated into the diet. And we have now proven that these foods have an almost identical effect on lowering cholesterol as the original cholesterol-reducing drugs." Dr. Jenkins regrets that health practitioners often give drugs to people with high cholesterol instead of trying to control the problem with soy and other vegetarian foods. Soy Safety Affirmed Recently, some doctors have spread the story that soy may increase the risk of cancer because natural chemicals in soy act like estrogens, hormones that may contribute to breast and other cancers. However, research has failed to support this supposition. As Dr. Jenkins points out, "the concerns have been whether soy estrogen might lead to hormone-dependent breast cancer or abnormal sexual development in children, yet we found no evidence to support this." In another of Dr. Jenkins' studies, people were put on diets high in soy to see how their estrogen levels were affected. Then, the researchers measured estrogen byproducts in their urine. Since estrogen stimulates breast cancer cells to produce a special protein, the researchers measured the amount of this protein produced by each urine sample to calculate how much estrogen was present. The total estrogenic activity in the urine of women on soy dropped to lower levels than it had been before they ate soy. "This finding suggests that soy may not have the estrogenic effects that were thought to alleviate menopausal symptoms but it refutes claims about its purported hormone risks," Dr. Jenkins says. The study also demonstrated that soy can reduce the risk of heart disease by lowering levels of oxidized cholesterol, which is thought to stick to coronary artery walls and form dangerous plaques. Dr. Jenkins' other research demonstrates that soy consumption reduces cholesterol in general while also decreasing the amount of LDL (bad) cholesterol in the body and maintaining the HDL (good) cholesterol. According to Dr. Jenkins, this confirms that soy should be promoted for its important role in preventing heart disease without fear that it will promote cancer. In another study in China, researchers compared the dietary habits of more than 350 women with breast cancer to the foods eaten by more than 1,000 women who did not have cancer (Amer Assoc Canc Res Second Annual Intl Conf Fron Can Prev Res 10/27/03, Abst 1274). They found that eating large amounts of soy did not raise the risk of breast cancer. Of course, anyone can develop allergies to almost any food, soy included. Isoflavone Benefit Isoflavones, soy's plant estrogens, are believed to create some of the most significant heart-healthy soy benefits. Health Movements by Sylvia Whitefeather Energy Times, December 6, 2003 Mind/body exercises like yoga (especially the super-popular Bikram variety), tai chi and Pilates aren't just trendy, they're custom made to soothe the rough edges of modern life. So often does today's fast-paced world emphasize the mental and competitive aspects of existence that its inhabitants frequently neglect the necessity of gentle movement for the body. But these exercises are an antidote to the tendency to view the mind and body as separate entities. Modern science is validating what traditional teachers have always known: The mind dwells in every cell. Joining mind and body with healthy movement generates harmony, lowers your chance of chronic illness and promotes emotional stability. Yoga No one knows when yoga first appeared. Historians and archaeologists figure the practice was initiated in India somewhere between 3,000 and 1,500 BCE. But the father of the modern forms of yoga is considered to be a man named Patanjali, who wrote the Yoga Sutra around 200 CE. The literal translation of the word yoga is "union." As Jennifer Schwamm Willis notes in her book The Joy of Yoga (Marlowe), this practice represents "the union of body, mind and spirit." The purpose of learning the fundamental movements of yoga is to connect with your body, release knots of tension and improve strength and flexibility. In that way, the physical balance during a yoga session translates into inner balance during times of crisis or distress. Schwamm points out that ancient yoga practitioners believed "the aim of yoga is to quiet the fluctuations of the mind, to create stillness in order to hear one's inner voice..." Yoga is used by many for stress relief. But it has other important uses: In a study presented by Oregon Health & Science University at the American Academy of Neurology meeting in April 2003, yoga was shown to benefit folks with multiple sclerosis. The researchers found that participants who regularly attended yoga class for six months suffered less fatigue and improved their quality of life. A yoga class generally begins with warm-up postures, moves on to a core group of basic postures, and ends with poses meant to cool you down. An important aspect of yoga is breath work and control. Movement in and out of poses involves carefully orchestrated breath work. Inhalation and exhalation in timed sync with movement lies at the heart of yoga's benefits. Yoga beginners often feel stiff and inflexible. But with gentle, patient, regular practice, greater flexibility, strength and balance can be had. Experts say that a yoga session does not demand struggle; it asks for surrender. If one pose causes disComfort, try another. Bikram Yoga One particular form of yoga, Bikram, is hot in terms of both popularity and room temperature: Not only is it one of the biggest trends in the fitness world, this demanding, aerobic take on yoga is conducted in heated rooms designed to maximize muscle relaxation and minimize injury risk. The heat also helps facilitate cleansing and detoxification. It was created by Bikram Choudhury, a four-time Indian yoga champ who founded the Yoga College of India in Beverly Hills, California. As in other types of yoga, Bikram uses asanas, or poses, handed down through generations of yoga teachers. In this case, though, 26 asanas are done in a prescribed order over a 90-minute period. Everyone, from novice to expert, works out together, the idea being that each individual is working to stretch his or her own limits by becoming stronger, more flexible and less prone to illness. Bikram yoga stresses the tourniquet effect, in which blood floods through vessels after they've momentarily been squeezed shut. This pressurizing effect is supposed to flush out debris, quickening circulation and releasing stress. The tourniquet effect also helps cleanse the lymphatic system. Proponents say Bikram improves balance, concentration and posture; increases energy; and eases sleep. Like any other exercise program, Bikram yoga requires diligence: one center says a minimum of 10 classes over 30 days is needed for maximum benefits. And while hydration is important during all fitness routines, consuming adequate water is crucial when you're exercising in a hot room. Tai Chi Tai chi (also known as taiji or tai chi ch'uan) consists of a series of fluid movements that build endurance, increase flexibility and balance, and foster alertness of mind and spirit. Tai chi developed around the 13th century as a form of martial arts in China based on the power of flow and grace, rooting and yielding, flexibility and endurance. To the onlooker, a person practicing the movements of tai chi has the quality of someone swimming in air. This gentle form of movement can be practiced by people of almost all ages and physical conditions. Tai chi does not require special equipment, props or a floor mat. As a non-impact form of exercise, tai chi delivers minimal stress to the joints. Tai chi emphasizes proper body alignment and uses the large muscles in the legs to relieve stress from the hips, back and shoulders. It strengthens joints, increases range of motion and improves circulation of all body fluids. Like many other forms of mind/body exercise, tai chi relieves stress. Tricia Yu, author of Tai Chi: Mind and Body (DK), has been practicing tai chi for over 30 years. She believes that tai chi not only has benefits as a health exercise, but that it "can have a beneficial effect on your mental and emotional states, as well as help you to feel connected with your surroundings." Yu adds, "Like yoga, tai chi originated in a culture that views the mind and body not as separate but rather as different expressions or states of qi-vital energy or life force." Current research has validated the health benefits of tai chi. One study found that the knee strength of elderly people practicing tai chi improves significantly (J Gerontol A Biol Med Sci 2003 August; 58:M763-6). Participants in this study, whose average age was 72 years, benefited significantly after five months of tai chi. For the elderly, this extra strength and control translates into fewer falls and injuries. Tai chi may help immunity. In a study published in the September 2003 issue of Psychosomatic Medicine, researchers reported that elderly folks who participated in a tai chi class for a period of 15 weeks "saw an improvement in factors that suppress shingles [a painful viral condition] increase by 50%." They also showed an increased ability to move throughout the day and a significant improvement in their general health. Pilates Joseph Pilates (1880-1968) was a sickly child afflicted with asthma, rickets and rheumatic fever. Determined to recover his health, Pilates studied both Eastern and Western forms of exercise, incorporating moves from gymnastics, yoga and wrestling, along with controlled breathing. With his wife, Clara, Joseph Pilates developed the form of exercise known today as simply Pilates. In the 1920s, Joseph left his native Germany and came to New York City and began teaching his exercise style in dance studios. Today, Pilates has gained acceptance both as an exercise style for fitness and as a system for physical rehabilitation. Because of its benefits, Pilates is practiced in hospitals, wellness centers, gyms and specialized Pilates studios. It is used by athletes, dancers and anyone looking to increase endurance and improve flexibility, balance and muscle tone. The basic principle of Pilates focuses on increasing what is called core strength. Core muscle groups include the abdominal, pelvic floor and back muscles. If these muscle groups are strong, then the body is balanced and strong. The Pilates method also encourages flexibility by building long, strong muscles without bulk. The Stott method is one of the most popular forms of Pilates. This technique combines traditional Pilates exercises with movements updated to conform with modern knowledge about the biomechanics of the human body.
