To start with, it is important to understand what causes the condition so that one may know how to combat it. High blood pressure is primarily caused by the constriction of the arteries which makes it hard to pump blood. The arteries are thinner than the veins and this means that the blood in them is at a higher pressure than in the veins. Due to the narrowness, they are able to pump blood faster and for longer distances.

When a person feeds on unhealthy food that is rich in cholesterol and other harmful substances, this causes a deposition of the harmful substances on the endothelium (the interior surface of the arteries). When this happens, the arteries become more constricted thus increasing the pressure of blood that flows in them (hence the term high blood pressure).

When the pressure increases, there is insufficient supply of blood to vital organs of the body like the brain. When the brain is not supplied with enough nutrients in the blood, this could lead to brain damage or stroke that may cause paralysis. Again, to ensure enough blood gets to the brain, the heart is forced to pump blood faster. This overworks the heart which may lead to heart attacks. Heart attacks are usually fatal.

Glaucoma is a group of diseases that affect the optic nerve and can lead to irreversible vision loss. It is usually associated with elevated fluid pressure within the eye. All forms of glaucoma can cause damage to the optic nerve and lead to vision loss, even blindness, if left untreated. About 2.2 million Americans have been diagnosed with glaucoma and as many as 2 million more could have it and not yet know. It is one of the lading causes of blindness and is expected to become more prevalent in years to come due to the growing population of older adults.

Those people who are at greatest risk for developing glaucoma are people over the age of sixty, people of African ancestry, and people with diabetes, high blood pressure, severe myopia, or a family history of glaucoma. Smokers also have an elevated risk, as do those who have sustained eye injuries or who have used steroids for an extended period of time.

About 3 percent of Americans are believed to have open-angle glaucoma, which is the most common form of this disease. Because this disorder causes no symptoms until it is quite advanced, only about half of those who have it are actually aware of it. In open-angle glaucoma, there is no physical blockage and the structure of the eye appear to be normal. However, the drainage of fluid is inadequate to keep the intraocular pressure at a normal level.

The most pronounced symptoms of open-angle glaucoma are the gradual loss or darkening of peripheral vision and a marked decrease in night vision or the ability of the eye to adjust to darkness. Of the 20 amino acids we need, only 10 can be produced by our body: the other 10 must be included in our diet and are termed 'essential' because they are an essential part of our diet, just as vitamins and minerals are. Without an adequate supply of essential components, we cannot survive, and if the essential amino acids are depleted in our diet then the body will break down muscle tissue to release them.

Although L-arginine is termed a 'conditionally' essential amino acid, it is included by many among the 10 regarded as being essential. Hence, depending upon who you read, it can be either essential or non-essential. It is known that aging is largely a result of the effect of free radicals on our body cells, and that green coffee bean extract can be used to fight against these. While we intuitively understand what the term ‘aging’ means, very few people can actually describe it in words, although there are several theories of why it occurs.

The most viable of these include the Error Catastrophe Theory, related to faulty molecular transcription and errors in cellular function, the Crosslinkage Theory, in which progressively increasing cross-linking between proteins slows the body functions down, and the Neuroendocrine Theory, in which changes in homeostasis and hormone levels occur through time due to an increasing loss of sensitivity of receptors to feedback inhibition.

However, by far the most acceptable and best understood theory is the Free Radical Theory of Aging, which green coffee beans have been found to help fight, and it is on that which we shall focus here.

The Free Radical Theory of Aging

Free radicals are oxidants created by unpaired electrons. Electrons generally go around in pairs, but occasionally molecules lose one of these electrons, creating a situation where it possesses an unpaired electron. In this condition, that molecule has only one purpose in life and that is to oxidize other molecules by stealing an electron from them.

Oxidation can cause untold damage to cell membranes, and also to other molecules that are vital to life, such as DNA. The end result is aging, and the onset of many diseases and conditions connected with aging. Free radicals are believed to be behind inflammatory diseases such as arthritis and Crohn’s disease, strokes, cardiovascular disease, Parkinson’s disease, Alzheimer’s disease and cancer among many others. Each of these is associated with aging.

Free radicals are generated in the body in four different ways:

1. Energy is generated by the intercellular mitochondria by the production of ATP (adenosine triphosphate). By-products of the mechanism by which this is done include hydrogen peroxide, the superoxide anion, and a hydroxyl radical. Over 20 billion molecules of antioxidant are produced in each individual cell daily, and every one of these has the capability to do damage to your body. The figure for inefficient cell metabolism is significantly higher.

2. Peroxisomes are eukaryotic cell components that contain oxidative enzymes, whose function is to produce hydrogen peroxide that is then used by another enzyme, catalase, to oxidize other toxic substances. It is used by the liver, for example, to oxidize about a quarter of all the alcohol we drink to acetaldehyde, and also to remove other toxins from the body. The down side is that the hydrogen peroxide can escape and degrade the cell membranes.

3. Chronic infections give rise to a high activity of white blood cells, which utilize oxidants of various kinds to destroy viruses, bacteria and parasites. These include hydrogen peroxide, superoxide and Nitric oxide which can also destroy the cells they are protecting.

4. Cytochrome P450 is an enzyme used to clear the body of toxic chemicals in our food such as pesticides and drugs. They also give rise to oxidative by-products.

In addition to these, free radicals are also produced by air pollution consisting of smoking, factory emissions and traffic fumes. Trace metals such as lead, iron and copper, are rich free radical sources, as is the ultraviolet component of sunlight, and caffeine, from tea and coffee, can also contribute to the store of free radicals in your body.

So where does green coffee extract come into this, and how should it be used. Free radicals tend to react very rapidly to accelerate aging, and in order to counter them, and hold the effects of aging at bay, it is necessary to destroy them almost as quickly as they are produced. This is carried out by antioxidants, of which there must be a plentiful supply available in each body cell.

Antioxidants donate electrons to free radicals, and so effectively neutralize them before they can attack the membranes of the cells in your body, or any of the other tissues that they can degrade. Many of the vitamins have a powerful antioxidant effect, among them vitamins A, C and E. Other antioxidants available in our diet include beta carotene and other carotenoids, flavonoids and glutathione, and also cofactors such as lipoic acid. All of these can destroy free radicals by the donation of an electron.

Green coffee beans have also been found to possess a strong antioxidant effect, due largely to the plant phenols, such as caffeic acid that forms chlorogenic acid with quinic acid, both cholorgenic and caffeic acid being string antioxidants. Green coffee bean extract is standardized to 99% chlorogenic acid. This substances not only reacts rapidly with free oxygen radicals but also helps to prevent to formation of hydroxyl radicals.

It has been established that green coffee bean reacts twice as fast as green tea or grape seed extracts, and speed of reaction is critical in the destruction of free radicals that have to be destroyed before they do damage. Other antioxidants found in extracts of green coffee beans include heterocyclic compounds such as pyrroles, furans and maltol.

The extract is made from beans of Coffea Arabica, this containing higher concentrations of chlorogenic and caffeic acids than the Arabica plant. The extract is also produced to be naturally low in caffeine, thus avoiding the negative effects of drinking coffee for its stimulating properties. When the green coffee bean is roasted, the antioxidant effect is found to decrease, and after roasting and brewing both the Arabica and the Robusta beans have reduced in activity to much the same level.

Studies on some of the conditions exacerbated by free radicals have indicated the effectiveness of green coffee beans as an antioxidant. It is believed to help reduce atherosclerosis caused by the oxidation of low density lipids (LDL). Oxidized LDLs tend to be easily absorbed by phagocytes to form plaques and foam cells in artery walls, thus narrowing and hardening the arteries, causing a deprivation of oxygen and nutrients to the heart and also increased blood pressure. Antioxidants from the green coffee bean prevent this from happening, and so help to reduce this serious effect of aging.

A good supply of antioxidants will also prevent the cell membranes from being destroyed, one effect of which is to age the skin. Antioxidants in the form of green coffee bean extract can help to maintain a youthful appearance while also aiding in the prevention of the more serious effects of free radicals that can shorten life.

There are no doubts that free radicals contribute significantly to accelerated aging, and that the antioxidants contained in green coffee beans can help hold back the physical signs of aging, while also helping to destroy those free radicals that threaten life by promoting cancer, atherosclerosis, and other similar conditions.

A recent study has found that Pycnogenol, which is an antioxidant plant extract from the bark of the French maritime pine tree, helps individuals by enhancing the production of Nitric oxide (NO), which in turn leads to an increase in blood flow and oxygen supply to muscles. Nitric oxide is a key cardiovascular chemical that is produced by the body. It increases blood flow which allows more nutrients and oxygen to be delivered to the muscles. This helps muscles to cope with increased physical activity. The study also suggests that taking Pycnogenol provides more NO in response to neurotransmitters which allows for better expansion of arteries, which can then carry more blood. This process meets the enhanced oxygen demand of muscles and helps to avoid anaerobic metabolism. The results of this study also lead researchers to believe that Pycnogenol can be a natural alternative therapy in diseases involving oxidative stress.

The study was held at the Hiroshima University Graduate School of Bio-medical Sciences in Japan. Each day for two weeks, healthy, young men were given either 180 mg of Pycnogenol or a placebo. In order to identify Pycnogenol's effect on the release of Nitric oxide, patients were infused with an inhibitor of L-arginine, which restricts arteries from expanding in response to the neurotransmitter acytelcholine. After two weeks of supplementation, the results revealed that blood flow had increased by forty two percent. Additionally, the group receiving a placebo did not show a significant blood flow increase at all. NO causes the muscle surrounding the arteries to relax, resulting in an increase in the diameter of the blood vessel, while acetylcholine stimulates the cells of arteries to produce NO from L-arginine faster. When the subjects being tested had taken Pycnogenol, the arteries relaxed and blood flow increased by Fourty two percent, as compared to the placebo group.

Although more research is warranted, this breakthrough is especially encouraging to athletes since Pycnogenol seems to allow people to move faster when they are exercising. It does this by satisfying the enhanced muscle oxygen demand and also increases the blood flow to the active muscles. People who are performing heavy physical activity experience the release of acetylcholine by nerves to arteries that are supplying the active muscles. This acetylcholine makes them expand, while the whole process requires that there is an enhanced production of Nitric oxide.

A great variety of studies about Pycnogenol and sports nutrition have been conducted over the past years. The most recent one occurred a year ago. In this study, Pycnogenol was found to improve blood circulation even in extended aerobic muscle activity. Tongkat Ali is also known as Longjack, and is a well known aphrodisiac in South East Asia, the root of which has been found effective both as an aphrodisiac and in treating certain sexual problems including failure to achieve an erection. What Viagra does in the west, Longjack does in the east, only cheaper.

The tree is also called Pasak Bumi, and had originally been used for many years as a treatment for malaria, the side effects being accepted but not understood as originating from the malaria medication. Some though that the malaria itself perhaps caused them, and was a long time before the effect of tongkat ali on the testosterone levels in the body was understood.

The name literally means Ali’s walking stick, and is named after the long roots from which it is extracted. The tree itself is about ten metres high, and grows beneath the canopy of the Indonesian rainforests. However, due to a heavy demand for the product, the older trees are increasingly more difficult to find, and most of the herbal preparation is extracted from younger trees. The tree itself is not easy to cultivate outside its natural environment, and is very slow growing.

HOW DOES IT WORK?

The Liver

Every day, the liver must process an almost unbelievable amount of blood – at a rate of three pints every minute. All the while, the liver performs over 500 physiologic functions, including protein and glucose synthesis, carbohydrate metabolism, vitamin and mineral storage, synthesis of clotting factors, urea formation, metabolism of medications, and the production of bile. The liver also assists in hormonal regulation, blood glucose control, and other regulatory functions.

Harmful substances that have been neutralized by the liver are carried to the intestines and kidneys for excretion. They are transported by bile, a greenish, watery solution that is synthesized, and continuously being excreted, by the liver. Burdock is originally native to Europe and Asia, but was introduced to North America, probably during colonial times. The plant is commonly found in the northern United States, and is very recognizable, with large, heart-shaped leaves. It has a long history of traditional use for gastrointestinal support.

Burdock root (Arctium lappa) supports the natural physiologic processes of organs involved in detoxification and elimination: notably, the liver, kidneys, and intestines.

Procyanidin B-2 seems to promote hair growth by down regulating PKC in both the anagen (active growth phase) and telogen (resting phase) of the hair follicle. When the anagen phase is prolonged, and the telogen phase is shortened, increased hair growth results.

Analysis of the different parameters set up in the study shows that the hair diameter gradually increased with ketoconazole use (+8.46%) over a 6 month period, whereas the diameter showed a trend to decrease with zinc pyrithione use over the same period (-2.28%). The sebum excretion rate was reduced with ketoconazole (-6.54%) while it increased with zinc pyrithione (+8.2%) over the same period of time. The number of hairs shed over a 24-hour period was reduced by 16.46% with ketoconazole and 6.02% with zinc pyrithione after 6 months. Finally, the percentage of hairs in the anagen phase increased by 6.4% and 8.4% respectively during the study.

The results are similar to a previous study done on 2% prescription strength Ketokonazole where it was shown that use of 2% ketoconazol yielded an increase in hair shaft diameter similar to what was achieved by the control group using 2% Minoxidil and a non-medicated shampoo.

Rooibos - Rooibos or Red Bush Tea - a hardy shrub indigenous to the North Western Cape of South Africa – is an exciting new botanical ingredient with potent antioxidant and anti-inflammatory properties well documented in medical literature. To understand glucosamine's role, it is important to understand joint structure and function. Cartilage in the joints acts as a shock absorber to cushion the blows of daily wear and tear. Joint cartilage is made of a unique connective tissue that consists of collagen and proteoglycans. Collagen is a strong, fibrous, insoluble protein. Proteoglycans are large, carbohydrate-rich protein chains made up of 95 percent polysaccharides and 5 percent protein called glycosaminoglycans (GAGs). GAGs are composed of repeating two-sugar units (disaccharides) that contain glucosamine sulfate and other amino sugars. Surrounding the joint cartilage is synovial fluid, which contains many substances including its chief component, hyaluronic acid. Hyaluronic acid forms the backbone of other proteoglycans and is responsible for the thickness of synovial fluid as well as its lubricating and shock-absorbing properties. Synovial fluid also provides nutrients for the joint cartilage.

Glucosamine sulfate is a normal constituent of glycosaminoglycans in cartilage and synovial fluid. In essence, glucosamine sulfate provides important building blocks for cartilage production. Curcumin scavenges Nitric oxide, a compound associated with the body’s inflammatory response.9 Curcumin also demonstrates in vitro inhibition of certain enzymes involved in promoting inflammatory reactions in the body. Pycnogenol (pronounced pic-nojen-all) is a natural plant extract originating from the bark of the maritime pine that grows along the coast of southwest France. A recent study supported by Kemin Food Asia and conducted at the College of Medicine, Catholic University of Korea suggested that lutein protects the retina from cellular damage caused by eye conditions, including diabetic retinopathy and age-related macular degeneration. The researchers injected rats with lutein prior to retinal ischemia, or reduced blood flow to the retina caused by obstruction of the blood vessels. Results showed the lutein inhibited retinal degeneration, which is marked by reduced expression levels of neuronal Nitric oxide synthase and Cyclo-oxygenase-2, or COX-2. In addition, the reduction of these proteins appeared to be dose-dependent, which would imply benefit from increased exposure to this carotenoid. According to the lead author of the study, “These results suggest that a lutein supplement may protect against ischemia mediated cell death in the retina."

INTRODUCTION:

 

Cholesterol, the soft, waxy substance present among the lipids (fats) in the bloodstream and in all cells, is important for wide variety of physiological functions. It is essential for the formation of cellular membranes, necessary for the production of bile salts, and also plays a role in the synthesis of certain hormones.3-5

 

Cholesterol is both produced by the body and obtained from food. Endogenous cholesterol is formed by human cells, particularly liver cells, whereas exogenous cholesterol is absorbed through the gastrointestinal tract from food.3,4

 

Because cholesterol can not be metabolized for energy, it must be removed from the body once it has served its function. The major route of removal is through the liver, where it is processed and subsequently excreted from the body.3,4

 

Types of Cholesterol

 

Cholesterol is lipophilic (“fat loving” or water insoluble) by nature. It can not be dissolved in the blood, and must, therefore, be transported by carriers known as lipoproteins. These carriers are classified by density, with LDL (low density lipoproteins) and HDL (high density lipoproteins) being the most common.4,5

 

LDL is often referred to as “bad” cholesterol. LDLs carry cholesterol throughout the body. Conversely, HDL, or “good” cholesterol, is responsible for carrying cholesterol away from the arteries to the liver where it is eventually processed and eliminated from the body.3,4,6

 

Scientific studies have shown that both types of cholesterol are important indicators of cardiovascular health. But recent research, focusing on the beneficial subtypes of HDL, has found that certain fractions of HDL may be more supportive of cardiovascular health than others. The two most notably supportive HDL fractions are HDL-2 and HDL-3.7

 

The smaller HDL-3 is synthesized by the liver and intestines. This form, which is known as “free cholesterol-rich” HDL, scavenges or “scoops up” free cholesterol. The cholesterol is then chemically altered by the addition of an ester group. When sufficient cholesterol is esterified, HDL-3 becomes HDL-2, which is therefore referred to as “cholesterol ester-rich” HDL. HDL-2 is larger in size and has been shown to be more cardiosupportive than HDL-3.*7  

 

HOW IT WORKS:

 

HDL is known to possess antioxidant activity and to help balance the body’s natural anti-inflammatory response, both of which are important for cardiovascular health, but its most important function is the role it plays in cholesterol transport.6,8 High levels of HDL cholesterol are also associated with reduced platelet activity, another key indicator of arterial and venous health.9

Curcumin

Turmeric- History and Traditional Usage

Native to Southeast Asia, Curcuma longa is a tall
tropical shrub with large oblong leaves and pale yellow flowers.
The genus “Curcuma” belongs to the Zingiberaceae family, which
includes ginger.1 The plant possesses a large root structure
with fleshy, bulbous underground parts called “rhizomes.” These
rhizomes, known as turmeric root, are harvested at maturity,
dried and cured for commercial use. Chemical analysis shows that
dried turmeric contains essential and volatile oils, with a
curcuminoid content of 2.5 to 5.0 %.2

In addition to its
popularity as a spice, turmeric is used as a dye for cloth and
coloring agent in foods and cosmetics, thanks to its rich yellow
color. Turmeric also serves as a preservative, probably owing to
the antioxidant and antimicrobial properties of curcumin.
Extracts of Curcuma longa have demonstrated in vitro
antibacterial and anti-fungal effects.3

Turmeric is named in
ancient Ayurvedic and Chinese herbal texts as a traditional folk
remedy. Historically, turmeric was used externally for wounds,
and sprains, and internally for digestive complaints,
rheumatism, liver disorders, coughs and colds.4
Benefits

The numerous beneficial
effects attributed to turmeric stem in large measure from the
antioxidant properties of curcumin. Antioxidants neutralize free
radicals, which are highly unstable molecules that can damage
cellular structures through abnormal oxidative reactions.
Curcumin is a potent “scavenger” of the superoxide radical, a
free radical that initiates potentially harmful oxidative
processes such as lipid peroxidation.5 Through this activity,
curcumin has been shown to protect skin cells from the injurious
effect of nitroblue tetrazolium, a toxin that generates
superoxide radicals. Curcumin also increases survival of cells
exposed in vitro to the enzyme hypoxanthine/xanthine oxidase,
which stimulates superoxide and hydrogen peroxide production.
Curcumin itself is not toxic to cells, even at high
concentrations. Pure curcumin was shown to be less protective
than a mixture of curcuminoids, indicating a possible synergism
among curcuminoids.6 Because free radicals are involved in aging
and exert harmful effects on skin, these results suggest
curcumin may help slow skin aging.

Curcumin demonstrates
several other in vitro effects linked to free radical
scavenging. Curcumin scavenges Nitric oxide, a compound
associated with the body’s inflammatory response.7 Pure curcumin
and turmeric extracts protect red blood cells from lipid
peroxidation induced by hydrogen peroxide.8 Curcumin has been
shown to protect DNA from oxidative damage, inhibit binding of
toxic metabolites to DNA, and reduce DNA mutations in the Ames’
test.9 Although additional studies suggest an anticarcinogenic
effect of curcumin, through protection of DNA,10 one in vitro
study found that curcumin induced DNA damage in human gastric
mucosal cells.11 It is speculated that curcumin may act as a
pro-oxidant in the presence of transition metal ions such as
copper and iron. (This is true for other antioxidants, including
vitamin C.) Curcumin also demonstrates in vitro inhibition of
COX-I and COX-II enzymes, which are involved in the inflammatory
reaction.12 Together these results strongly suggest that
curcumin is a potent bioprotectant with a potentially wide range
of therapeutic applications.

Animal studies- In vivo protective effects

Through its free radical scavenging
properties, curcumin has shown bioprotective effects in animals.
In one study, rats were treated with isoproterenol, a chemical
that causes cardiac hypertrophy (enlargement of the heart) due
to abnormal collagen metabolism. Co-treatment with curcumin
reversed the degradation of collagen and cardiac hypertrophy
induced by isoproterenol.13 Curcumin protects mice from
detrimental effects of radiation, by stabilizing the glyoxalase
system, a biological system that regulates cell division.14
Curcumin protects livers of rats from the damaging effects of
carbon tetrachloride (CCl4), a potent hepatoxin that injures the
liver via its free radical metabolite, CCl3.15,16 Curcumin
protected rats from alcohol-induced brain damage, in a study in
which oral administration of curcumin reversed lipid
peroxidation, reduced levels of free-radical metabolites and
increased levels of glutathione, a major physiologic
antioxidant.17 Curcuma longa extracts have shown
anti-inflammatory effects in rats.18

Human Trials

Curcumin exhibits free-radical scavenging ability when
administered to humans. In an open trial (uncontrolled), 18
healthy individuals ranging in age from 27 to 67 years consumed
a Curcuma longa extract, at a dose supplying 20 mg curcuminoids,
for 45 days. Before and after blood tests showed a statistically
significant decrease in lipid peroxides.19 Preliminary trials
have tested the anti-inflammatory action of curcumin, with
results that verify the traditional use of turmeric as an
anti-rheumatic herb. In a short-term double-blind, cross-over,
comparative study, 18 people received curcumin (1200 mg daily)
or phenylbutazone for two week periods. Both curcumin and
phenylbutazone produced measurable improvements in joint
flexibility and walking time. The subjects reported results only
with phenylbutazone, which may be explained by the short
duration of the trial.20 In a small placebo-controlled trial
comparing curcumin to phenylbutazone, 45 patients with
post-operative inflammation received curcumin, phenylbutazone or
placebo. The anti-inflammatory effects of curcumin and
phenylbutazone were comparable and superior to placebo.21
Curcumin has not been found to produce an analgesic (pain
relieving) effect.

From time immemorial, people have tuned into life's lessons that come from the heart. Sadly, times are changing: If you're like most inhabitants of today's harried world, you may be too distracted to detect important clues about your cardiovascular circumstances.

And while heart lessons may be more complicated than simply connecting the physiological dots, understanding those heart messages are imperative for improving and maintaining your heart health.

Every cell in your body relies on heart-powered blood flow to keep it supplied with nutrients, oxygen, hormones and other natural chemicals necessary for survival. Without that supply of life-giving substances, few cells in the body-including those within the heart itself-can survive very long.

And just as damage to a major roadway can cause mayhem with traffic patterns, damage to blood vessels and the heart can wreak a lumpy cardiovascular havoc that blocks the passage of blood and endangers your heart's well-being.

Your Heart Disease Chances

Within the last ten years, scientific research performed by investigators around the world has focused on the specific factors that most strongly influence your chances of developing heart disease and suffering either a heart attack or a stroke.

While much of your risk depends on your genetic inheritance and family history, several factors that determine your heart health are within your control.

The most important factors you can do something about include:

* Smoking: free radicals generated by burning tobacco causes significant damage to blood vessels and other cells

* Lack of exercise: the human body is designed for consistent, moderate physical activity; without exercise, the body slacks off in creating antioxidant protection for arteries

* Diabetes: when excess blood sugar persists, physiological processes begin that endanger the heart and arteries

Heart Attack Signs

Do you think you know what a heart attack feels like? Well, if you think it feels like a dramatic pain somewhere in your chest that knocks you to the floor, you're probably wrong. "Most heart attacks do not look at all like what one of my colleagues calls the 'Hollywood' attack-the heart attack you see on television or in the movies," warns Julie Zerwic, MD, professor of surgical nursing who has studied what happens when people develop heart disease and suffer damage to their hearts.

"The symptoms [of heart problems] are not necessarily dramatic. People don't fall down on the floor. They don't always experience a knife-like, very sharp pain. In fact, many people describe the sensation as heaviness and tightness in the chest rather than pain," she says. And, if you're a woman experiencing a heart attack, you may not even feel discomfort specifically in your chest. Instead you may experience a severe shortness of breath. The apparent ambiguity of the discomforts caused by a heart attack lead many people to either ignore them or take hours to realize they need to go to the emergency room at the hospital.

Consequently, much fewer than half of all individuals undergoing a heart attack actually go to a hospital within an hour of the start of the attack. That delay can be a fatal mistake.

"Timing is absolutely critical," laments Dr. Zerwic. "If treatment starts within a hour after the onset of symptoms, drugs that reestablish blood flow through the blocked coronary artery can reduce mortality by as much as 50%. That number drops to 23% if treatment begins three hours later. The goal is to introduce therapy within two hours."

However, in Dr. Zerwic's research, only 35% of non-Hispanic whites go to the hospital within an hour of the start of a heart attack. And among African-Americans, the number of people going to the hospital right away drops to a frighteningly low 13%.

Often, people will lie down or use a heating pad to relieve the tightness they feel in the chest," says Dr. Zerwic. "They may take some medicine and wait to see if that works. All these steps postpone needed treatment."

Signs of a possible heart attack include:

* Chest discomfort: Heart attacks most frequently cause discomfort in the center of the chest that can either go away after a couple of minutes (and come back) or persist. The discomfort may feel like strong pressure, fullness or pain.

* Upper body discomfort: An attack may set off pain or discomfort in either or both arms, and/or the back, neck, jaw or stomach.

* Shortness of breath: Chest discomfort is frequently accompanied by shortness of breath. But it's important to note that shortness of breath can take place even in the absence of chest discomfort.

* Other signs: You can also break out in a cold sweat, or feel nauseated or light-headed.

A Woman's Sleep Signs

If you are a woman who suddenly experiences a marked increase in insomnia and puzzling, intense fatigue, you may be in danger of an imminent heart attack.

In an attempt to understand how women's symptoms of heart problems differ from those of men, researchers talked to more than 500 women in Arkansas, North Carolina and Ohio who had suffered heart attacks. (Technically, what they had experienced is referred to as acute myocardial infarction.)

They found that chest pain prior to a heart attack was only reported by about 30% of the women surveyed.

More common were unusual fatigue, sleep disturbances and shortness of breath (Circulation Rapid Access, 11/3/01).

"Since women reported experiencing early warning signs more than a month prior to the heart attack, this [fatigue and sleep problems] could allow time to treat these symptoms and to possibly delay or prevent the heart attack," says researcher Jean C. McSweeney, PhD, RN, nursing professor at the University of Arkansas for Medical Sciences in Little Rock. In Dr. McSweeney's study, more than nine out of ten women who had heart attacks reported that they had had new, disturbing physical problems more than a month before they had infarctions.

Almost three in four suffered from unusual fatigue, about half had sleep disturbances, while two in five found themselves short of breath.

Other common signs included indigestion and anxiety.

"Women need to be educated that the appearance of new symptoms may be associated with heart disease and that they need to seek medical care to determine the cause of the symptoms, especially if they have known cardiovascular risks such as smoking, high blood pressure, high cholesterol, diabetes, overweight or a family history of heart diseases," says Dr. McSweeney.

Dr. McSweeney warns that, until now, little has been known about signs that women are having heart trouble or heart attacks. The fact that most of Western medicine's past attention has been on heart problems in men has obscured the warning signs in women. As part of Dr. McSweeney's studies, she and her fellow researchers have discovered that more than 40% of all women who suffer a heart attack never feel any chest discomfort before or during the attack.

"Lack of significant chest pain may be a major reason why women have more unrecognized heart attacks than men or are mistakenly diagnosed and discharged from emergency departments," she notes. "Many clinicians still consider chest pain as the primary symptom of a heart attack."

Vitamins for Diabetes and Heart Disease

Having diabetes significantly raises your chance of heart disease, which means that keeping your blood sugar levels under control can reduce your chances of suffering a heart attack.

Today, 17 million Americans have diabetes and, as the country's population in general gains weight and fails to exercise, the number of people suffering this problem continues to grow.

The first line of defense against diabetes consists of exercise and weight control. All you have to do is take a brisk walk for 30 minutes a day to drop your chances of diabetes (American Journal of Epidemiology 10/1/03).

"We have found that men and women who incorporate activity into their lifestyles are less likely to develop type 2 diabetes than those who are sedentary. This finding holds no matter what their initial weight," said Andrea Kriska, PhD, professor of epidemiology at University of Pittsburgh Graduate School of Public Health.

To help your body fight the development of diabetes, researchers also recommend vitamin C and natural vitamin E.

Researchers working with lab animals at the University of California at Irvine have found that these antioxidant vitamins can help insulin (the hormone-like substance secreted by the pancreas) reduce harmful blood sugar. In addition, these vitamins shrink the chances of organ damage that can be caused by diabetes (Kidney International 1/03).

In this investigation, these vitamins also helped reduce blood pressure, another risk factor that raises heart disease risk.

"Blood pressure was lowered to normal, and free radicals were not in sufficient numbers to degrade the sugars, proteins and Nitric oxide," notes Nick Vaziri, MD, professor of medicine at the University of California. "We think this shows that a diet rich in antioxidants may help diabetics prevent the devastating cardiovascular, kidney, neurological and other damage that are common complications of diabetes."

Free Radical Blues

Dr. Vaziri and his group of researchers found that untreated diabetes raised blood pressure and increased the production of free radicals, caustic molecules that can damage arteries and the heart. Free radicals can change blood sugar and other proteins into harmful substances, boosting tissue and heart destruction.

In Dr. Vaziri's work with lab animals, he found that treating diabetes with insulin lowered blood pressure and helped keep sugar and protein from changing into dangerous chemicals, but allowed the free radicals to subvert Nitric oxide, a chemical the body uses to protect itself from free radicals.

In this investigation, adding vitamins C and E to insulin insulated the body's sugars, proteins and Nitric oxide from oxidative assault. This produces a double advantage: Lowering the risk of heart disease and other damage to the body from diabetes.

Maitake, an Oriental mushroom that has been shown to have many health benefits, can also be useful for people with diabetes who are trying to avoid cardiovascular complications. Laboratory studies in Japan demonstrate that maitake may help lower blood pressure while reducing cholesterol (Biological & Pharmaceutical Bulletin 1997; 20(7):781-5). In producing these effects, the mushroom may also help the body reduce blood sugar levels and decrease the risk of tissue damage.

No Smoking!

Tobacco smoke is one of the most notorious causes of heart problems. In the same way a hard frost exerts a death grip on a highway, the smoke from cigarettes can freeze up arteries and hamper their proper function. A healthy artery must stay flexible to comfortably allow adequate circulation.

But "...when blood vessels are exposed to cigarette smoke it causes the vessels to behave like a rigid pipe rather than a flexible tube, thus the vessels can't dilate in response to increased blood flow," says David J. Bouchier-Hayes, MD, professor of surgery at the Royal College of Surgeons in Ireland, who has studied the deleterious effects of tobacco.

This rigidity is called endothelial dysfunction. When arteries are rigid, blockages gum up vessels, clots and other impediments to blood flow appear, and your risk of heart attack and stroke increases (Circulation 2001 Nov 27; 104(22):2673).

This condition can also cause chest pain (angina) similar to that caused by a heart attack, and should be evaluated by a knowledgeable health practitioner.

Although all experts recommend you stop smoking to lower your heart disease risk, some studies have found that Pycnogenol(r), a pine bark extract that helps the body fight inflammation, may ease some of smoking's ill effects.

In a study of platelets, special cells in the blood that can form dangerous blood clots, researchers found that Pycnogenol(r) discouraged platelets from sticking together (American Society for Biochemical and Molecular Biology 5/19/98). By keeping platelets flowing freely, this supplement may alleviate some of the heart-threatening clots that tobacco smoke can cause.

In Ayurvedic medicine, a traditional therapy from India, an herb called guggul has also been used to lower the risk of blockages in arteries. This herb, derived from the resin of the mukul tree, has been shown to reduce cholesterol by about 25%. People taking this herb have also reduced their triglycerides (harmful blood fats) by the same amount (Journal Postgraduate Medicine 1991 37(3):132).

The Female Version of Heart Disease

  • Medical experts who have examined heart disease in men and women have found some striking differences.
  • For one thing, women often don't suffer from the crushing chest pain that for most people characterizes a heart attack; instead, many women experience back pain, sweating, extreme fatigue, lightheadedness, anxiety or indigestion, signs that can be easily misread as digestive troubles, menopausal symptoms or indicators of aging.

    The genders also differ in how heart disease poses a threat. While men seem most endangered by the buildup of blockages in arteries, women apparently are more at risk from endothelial dysfunction. But more study needs to be done since, in many cases, researchers have been unable to pin down the precise mechanism that causes many women to die of heart disease.

    Scientists have found that the number of women in their 30s and 40s who are dying from sudden cardiac arrest is growing much faster than the number of men of the same age who die of this cause. But research by the Oregon Health & Sciences University and Jesse E. Edwards Cardiovascular Registry in St. Paul, Minnesota, shows that while doctors can pinpoint the coronary blockages that kill men, they can't find specific blockages in half of the female fatalities they have studied (American Heart Journal 10/03).

    "This was an unexpected finding. However, the study underscores the need to focus on what is causing these younger women to die unexpectedly because the number of deaths continues to increase," says Sumeet Chugh, MD, a medical professor at Oregon.

    Since the failure of arteries to relax probably contributes to heart disease in many women, eating red berries, or consuming supplements from berries such as chokeberry, bilberry or elderberry, may be important in lowering women's heart disease risk. These fruits help arteries expand and allow blood to flow freely.

    Red berries are rich sources of flavonoids, polyphenols and anthocynanins. The anthocyanins are strong antioxidants that give the berries their color. Research at the Indiana University School of Medicine have found that these chemicals can interact with nitrous oxide, a chemical produced by the body, to relax blood vessels (Experimental Biology conference 5/20/02).

    Working Out

    As researchers work to devise lifestyle roadmaps that can steer you around the perils of heart disease, they are finding that exercise is a key path to avoiding cardiovascular complications.

    A 17-year study of about 10,000 Americans found that those who exercised and kept their weight down (or took weight off and kept it off) experienced a significantly lower risk of heart problems (Preventive Medicine 11/03).

    "The fact is that those who both exercised more and ate more nevertheless had low cardiovascular mortality," says Jing Fang, MD, of the Albert Einstein College of Medicine in the Bronx, New York. Burning calories in physical activity may be the secret to reducing heart disease risk and living longer, she says.

    Dr. Fang's research used information collected from the First National Health and Nutrition Examination Survey in 1975 and then computed how much people exercised, how their body mass indices varied and which of these folks died of heart disease during the next two decades.

    In the study, more than 1,500 people died of heart disease. Those who worked out and consumed more calories cut their risk of heart disease death in half.

    Exercise Is Essential

    "Subjects with the lowest caloric intake, least physical activity, and who were overweight or obese had significantly higher cardiovascular mortality rates than those with high caloric intake, most physical activity, and normal weight," Dr. Fang notes. The individuals in the study who were overweight and didn't exercise had a bigger risk of heart disease even if they tried (and succeeded) at eating less.

    "This suggests that heart disease outcome was not determined by a single factor, but rather by a compound of behavioral, socioeconomic, genetic and clinical characteristics," according to Dr. Fang.

    According to researchers, if your job requires a great deal of physical activity, your health will be better if you get another job. Exercise on the job not only doesn't decrease your risk of heart disease, it may actually raise it. The reason: On-the-job activity is linked to heart-endangering increases in job stress.

    Research into this subject, performed at the Keck School of Medicine of the University of Southern California, found that while recreational exercise slowed hardening of the arteries, workers who had to exert themselves during the workday had arteries that were blocked at a younger age (American Journal of Medicine 7/03).

    In this study, researchers examined about 500 middle-aged employees as part of what is called the Los Angeles Atherosclerosis Study.

    "We found that atherosclerosis progressed significantly faster in people with greater stress, and people who were under more stress also were the ones who exercised more in their jobs," says James Dwyer, PhD, professor of preventive medicine at the Keck School. According to Dr. Dwyer, "This suggests that the apparent harmful effect of physical activity at work on atherosclerosis-and heart disease risk-may be due to the tendency of high-activity jobs to be more stressful in modern workplaces.

    "It appears from our findings that the psychological stresses associated with physically active jobs overcomes any biological benefit of the activity itself."

    Playful Workouts

    On the other hand, the scientists found that heart disease drops dramatically among those who exercise the most in their spare time. In the study, people who vigorously worked out at least three times a week had the lowest risk. But even those who just took walks enjoyed better heart health than people whose most strenuous activity was working the TV remote. Dr. Dwyer says, "These results are important because they demonstrate the very substantial and almost immediate-within one or two years-cardiovascular benefit of greater physical activity."

    Lowering your risk of heart disease is substantially up to you. Listen to what your heart tells you it needs; then, exercise your right to fetch some cardiovascular necessities.

    Many of the world’s top-level scientists gathered in Washington D.C. this week for the 2005 Garlic Symposium, entitled, “Significance of Garlic and its Constituents in Cancer and Cardiovascular Disease.” The conference provided current scientific information about the effect of garlic and its constituents on health and performance. The symposium was held at the Georgetown University Conference Center on April 9-11, 2005.

    “For the first time in seven years authorities in various fields of garlic research from all over the world to provide the latest updates, specifically regarding aged garlic extract and its actions in diseased states including heart disease and cancer,” commented Dr. Matthew Budoff, M.D. cardiovascular researcher at UCLA. “Garlic has been used medicinally for thousands of years in virtually all ancient cultures. Now, new metabolic roles for garlic are being proposed and there are many promising lines of research.”

    Presentation highlights included:

  • • Clinical Intervention Trial and pre-clinical substantiation on Cancer using Garlic, presented by National Cancer Institute scientists, Mitchell Gail and John Milner Mounting evidence points to the anticancer properties of aged garlic extract and a number of specific organosulfur compounds from garlic. These prevention characteristics arise through both a dose and temporal related change in several cellular events including those involving drug metabolism, immunocompetence, cell cycle regulation, apoptosis and angiogenesis.

  • • Inhibition of Coronary Arterial Plaque Accumulation by Garlic, presented by Matthew Budoff, Harbor-UCLA Medical Center

    Effect of aged garlic extract (AGE) has been tested in the placebo-controlled double blind randomized clinical study that determined that the atherosclerotic plaque burden detected by electron beam tomography (EBT) changed significantly with the use of aged garlic extract, Patients in Dr. Budoff’s study were able to significantly lower their total cholesterol, blood pressure, homocysteine and LDL cholesterol oxidation levels with aged garlic extract supplementation.

  • • Influence of Garlic on Endothelial Dysfunction in Hyperhomocysteinemia, presented by Norbert Weiss, University of Munich in Germany Aged Garlic Extract (AGE) minimizes intracellular oxidant stress and stimulates NO generation in endothelial cells. Preliminary results show that pretreatment with AGE for six weeks diminishes the adverse effects of acute high homocysteine on endothelium-dependent brachial artery vasodilatation and on acetylcholine-induced stimulation of skin perfusion.

  • • Anti-glycation properties of aged garlic extract: possible role in prevention of diabetic complications, presented by Nessar Ahmed, Manchester Metropolitan University in England Aged garlic extract inhibited the formation of advanced glycation end products, which have been previously shown to increase the risk of diabetic complications ranging from heart disease to retinopathy, kidney failure, impaired wound healing and many more.

    “Garlic is turning out to be a major player in cancer and heart disease prevention and control, especially in combination with drug treatments,” said Richard Rivlin, M.D. of Strang Cancer Prevention Center at Cornell. “It’s also showing us that we can start early. It’s madness to treat cancer and heart disease in their advanced stages. We need to start early and aged garlic extract is an excellent way to do that.”

    Almost 400 scientific studies have been completed on aged garlic extract, done in major universities worldwide. These studies have focused on a variety of heart disease risk factors such as cholesterol, high blood pressure, homocysteine levels, inhibiting LDL oxidation, anti-platelet aggregation and adhesion, stimulating blood circulation; in addition to other studies on immune stimulation, cognitive effects, liver function and anti-tumor effects. .

    Abstracts

    PRECLINICAL PERSPECTIVE ON GARLIC AND CANCER. John A. Milner, National Cancer Institute, Rockville, MD 20892

    Mounting evidence points to the anticancer properties of fresh garlic extracts, aged garlic, garlic oil, and a number of specific organosulfur compounds from garlic. These prevention characteristics arise through both a dose and temporal related change in several cellular events including those involving drug metabolism, immunocompetence, cell cycle regulation, apoptosis and angiogenesis. A block in carcinogen activation through modulation of cytochrome P450-dependent monooxygenases and/or acceleration of carcinogen detoxification via induction of phase II enzymes likely account for some of this protection. The block in preneoplastic lesions and/or tumors in several sites suggests a generalizable mechanism. The efficacy of water- and lipid-soluble allyl sulfur compounds against chemical carcinogenesis appears comparable, although more studies are needed. A shift in sulfhydryl groups, redox status or enzyme catalysis may account for some of the phenotypic changes. They may also account for the observed hyperphosphorylation of specific cell cycle related proteins and histone hyperacetylation; both of which have been correlated with suppressed tumor cell proliferation. Several forms of allyl sulfur compounds are effective in blocking cell division and inducing apoptosis, but notable differences in the efficacy among these various compounds and across tumor types are evident. While the expression of many genes and proteins can be influenced by allyl sulfides; the challenge is to determine which is responsible for a phenotypic change. Additional studies are needed with more modest exposures and over prolonged periods and that utilize transgenic and knockout models to assist in the identification of molecular targets. Finally, additional research is needed to identify sensitive “effect” and “susceptibility” biomarkers that can ultimately be used to identify responders from non-responders.

    INHIBITION OF CORONARY ARTERIAL PLAQUE ACCUMULATION BY GARLIC. Matthew Budoff, Harbor-UCLA Medical Center, UCLA School of Medicine, California, USA

    Effect of Aged garlic extract (AGE) has been tested in the placebo-controlled double blind randomized clinical study to determine whether the atherosclerotic plaque burden detected by electron beam tomography (EBT) will change at a different rate under the influence of AGE or placebo. EBT can non-invasively quantitate the amount of coronary calcification and track atherosclerotic plaque over time. Nineteen of 23 patients completed the study protocol. The patients were well matched for age, gender, statin use and cardiac risk factors. Patients underwent EBT and blood testing at baseline, and then again after 12 months of randomization. The average change in the calcium score (Volumetric method) ± SD for the AGE group (n = 9) was 7.5 ± 9.4% over the one year. The placebo group (n = 10) demonstrated 22.2 ± 18.5% annual progression, significantly greater than the treated cohort (p = 0.01). While there were no significant changes in cholesterol parameters, or C Reactive protein between the groups, high density lipoproteins and plasma homocysteine in the AGE group demonstrated a trend toward improvement compared to the placebo patients. Thus, although this is a small-scale trial, it demonstrates the potential of AGE to inhibit the rate of atherosclerosis (progression of coronary calcium), as compared to placebo over one year. Larger studies need to be performed to assess this potential anti-atherosclerotic therapy and the impact on coronary events.

    Endothelial dysfunction (ED) due to decreased bioavailable Nitric oxide (NO) by increased vascular oxidant stress plays a critical role in the vascular pathobiology of hyperhomocysteinemia (hhcy). Aged Garlic Extract (AGE) minimizes intracellular oxidant stress and stimulates NO generation in endothelial cells. We performed a placebo-controlled, blinded, cross-over study to examine whether AGE prevents macro- and microvascular ED during acute hhcy induced by an oral methionine challenge in healthy subjects. Acute hhcy leads to a significant decrease in flow-mediated vasodilation of the brachial artery as determined by vascular ultrasound, indicative of macrovascular ED, as well as a decreased number of recruited nailfold capillaries during postischemic reactive hyperemia as determined by videomicroscopy, and to a decreased ratio of acetylcholine (endothelium-dependent) vs. sodium nitroprusside (endothelium-independent) iontophoresis induced skin perfusion as measured by laser doppler flowmetry, indicative of microvascular ED. Preliminary results show that pretreatment with AGE for six weeks diminishes the adverse effects of acute hhcy on endothelium-dependent brachial artery vasodilation and on acetylcholine-induced stimulation of skin perfusion. Whether or not this is accompanied by changes in biochemical parameters of ED is still under investigation. It is concluded that AGE may at least partly prevent a decrease in bioavailable NO during acute hhcy.

    Abstract:
    It has been reported that garlic activates Nitric oxide synthase in vitro and that chronic inhibition of Nitric oxide (NO) synthesis by N omega-nitro-L-arginine-methyl-ester (L-NAME) induces arterial hypertension in rats. In this work, we studied the effect of oral administration of L-NAME for 4 weeks on control and garlic-fed rats. Basal systolic blood pressure was recorded 4 weeks after garlic supplementation, and on weeks 1, 2, 3, and 4 after L-NAME treatment. At the end of the study, the in vivo NO production was evaluated indirectly by measuring the urinary excretion of the stable end products of NO metabolism, nitrite (N02-) and nitrate (N03-). It was found that L-NAME induced arterial hypertension on weeks 1-4 in control rats but not in garlic-fed rats, whose blood pressure remained essentially as the basal values. Also, during this time period, blood pressure remained unchanged in garlic-fed rats without L-NAME treatment. Urinary excretion of N02-/NO3- decreased in L-NAME-treated rats, increased in garlic-fed rats, and remained unchanged in garlic-fed rats treated with L-NAME. It was concluded that garlic blocks the L-NAME -induced hypertension by antagonizing in vivo the inhibitory effect of L-NAME on NO production.

    Abstract:

    1. The aims of the present study were to determine the characteristics of garlic extract-induced relaxation in rat isolated pulmonary arteries, its susceptibility to changes in oxygen tension and its protective effect against hypoxic pulmonary vasoconstriction.


    2. In normoxia, garlic extract (3-500 µ/mL) produced a dose- and Nitric oxide (NO)-dependent relaxation. Following 60 min hypoxia, maximum garlic relaxation was reduced compared with control (mean (-SEM) -86 +/- 3 vs-69 +/- 2% of phenylephrine (PE) precontraction, respectively), but recovered after 60 min reoxygenation (-85 +/- 3% PE precontraction).

    3. Acetylcholine (0.1 micromol/L)-induced NO-dependent relaxation was reduced from a control value of -76 +/- 1% to -46 +/- 4% during hypoxia and was further reduced to -35 +/- 2 % after reoxygenation.


    4. In endothelium-intact arteries, hypoxic exposure resulted in a triphasic response: early transient contraction (+24 +/- 4%), followed by transient relaxation (-37 +/- 7%) and then sustained contraction (+62 +/- 5%).

    5. Pretreatment with NG-nitro-L-arginine methyl ester abolished the early transient contraction, moderately attenuated the sustained contraction and had no effect on the transient relaxation. Mechanical endothelial disruption inhibited all hypoxia-induced vascular changes.


    6. Garlic pretreatment had no effect on the early transient contraction (+25 +/- 4%), but inhibited the transient relaxation (-5 +/- 3%; P < 0.05) and the sustained contraction (+26 +/- 5%;P < 0.05).

    7. Garlic also significantly inhibited endothelin-l-induced contractions in a dose-dependent manner.

    8. These findings show that garlic extract modulates the production and function of both endothelium-derived relaxing and constricting factors and this may contribute to its protective effect against hypoxic pulmonary vasoconstriction.
