Hyaluronic acid, also known as hyaluronan, is one of the main components of the extracellular matrix of connective tissue and joints, and is one of the main chemicals contained in the synovial fluid that lubricates your joints. Before we discuss how it can be used to help restore the function of damaged joints, let's have a look at what causes joint pain, and why joints can break down long before their time. You might have heard that arthritis is an inflammatory condition, or that it is due to the immune system. This is true to an extent, and rheumatoid arthritis is an immune system problem, although osteoarthritis is a different problem altogether. Arthritis is not the only cause of joint pain of course, but is by far the most common cause. As already inferred, there are two forms of arthritis, each with a different basic cause. Osteoarthritis is the most common form of joint disease, and is largely due to wear and tear, injury or can be hereditary. However it is usually initiated by damage, the joint surface becomes roughened and the bone around the damaged area gets thicker to compensate. A joint is where two bones meet, and not all joints move. You have the fixed joints in your skull, for example, and those between the ribs and the spine. However, arthritis generally affects the joints associated with movement, mainly the knees, hips, fingers, toes and elbows. The ends of the bones are covered with a thin layer of cartilage, which can absorb shocks and both cushion the joint and allow the ends of the bones to move smoothly over each other. Round each joint is a membrane known as the synovium that is filled with the thick synovial fluid, which lubricates the cartilage. The bone ligaments hold them close to the joint, and prevent them moving too much and dislocating. Finally, the joint is completed by the tendons that attach the muscles to the bones they control. Osteoarthritis causes a joint to deteriorate: the cartilage becomes rougher and wears, and the bone beneath the cartilage gets thicker. At the edge of the joint the bone tends to grow outwards, forming spurs and the synovium swells producing extra synovial fluid. This causes swelling and pain, and ultimately the joint can be damaged beyond repair. This process takes a number of years to reach a stage whereby pain and discomfort are felt. Before that situation occurs, however, the joint tries to repair itself, and hyaluronic acid can take part in this process. It is mainly involved in the reparation of connective tissues such as cartilage, although there are other connective tissues which hyaluronan has been found to repair. These include skin, eyes and heart valves and hyaluronic acid can arguably be used to help repair all of them. However, it is joint tissue with which we are concerned here, and hyaluronan is an important component of articular cartilage: the type of cartilage involved in movement (knees, hips, etc), as opposed the type that forms your nose or the external part of your ears. It is believed that injections of hyaluronic acid can help to repair damaged articular cartilage. However, osteoarthritis is only one of the two forms of arthritis. The other is rheumatoid arthritis, and this is much more sinister. Rheumatoid arthritis is caused by the attack of your joints by your own immune system, although why this happens is unknown. The most likely theory is that certain infections trigger the immune system, and it then proceeds to attack the synovial joints. Cytokines are responsible for inflammation of the synovial fluid, which can also cause fever, loss of weight and appetite, and inflammation of the blood vessels. The body tries to generate more synovial fluid which causes swelling round the whole joint, putting pressure on the damaged areas, and generating even more pain. The inflammatory response commences and the whole area becomes seriously painful. The smaller joints are most commonly affected, such as the fingers and toes, but it can progress to the elbows, hips and the knees. The joints become red, swollen and finally too stiff to be used. The sinusitis eventually leads erosion of the joint and deformation. Because hyaluronic acid behaves like the synovial fluid, it is believed that it can be used to treat both forms of arthritis. It is a glycosaminoglycan, an unbranched polysaccharide of the same type as chondroitin sulfate that is commonly used to treat arthritis. It can be used to increase the viscosity of the synovial fluid, and render a more effective lubricant. However, it is in treatment of osteoarthritis that hyaluronan is most likely to find success. It is used to improve the viscosity of the synovial fluid, so providing increased lubrication to the joint and helping reduce the pain. In fact it not escaped notice that those who eat a diet rich in hyaluronic acid tend to live longer and look younger than those that do not do so. Commercial preparations are currently in use or under evaluation for disorders such as glaucoma, fractures, and detached retinas, damage to cartilage, healing ligaments and osteoarthritis. These last three are particularly of interest to arthritis sufferers, and signs are that they are effective with many cases. However, it has also been established that smoking cigarettes can negate the effect of hyaluronic acid, and excessive levels of Vitamin C can also degrade it. Estrogen treatment, however, can enhance its effect in repairing connective tissue. Zinc deficiencies have also been found to have negative results, so make sure that these factors are addressed if you are using hyaluronic acid to treat arthritis. Something to keep in mind if you are taking hyaluronic acid orally is the size of the molecule. It is a very large molecule, and suffers from the same absorption problems as chondroitin sulfate. These compounds are not only responsible for the pungent odor of garlic, but also for its benefits to your health. Among the other components of garlic are selenium, manganese and vitamins B6 and C. Before considering the other effects of garlic on your health, we shall first consider how it benefits the immune system. The immune system is an essential part of human biology, and protects your body from invasion by pathogenic organisms. Without the immune system your body would rapidly be overcome by bacteria, viruses, fungi, parasites and other foreign bodies, and your body would rapidly fail to function. The immune system consists of several components that can act in concert to protect you from these foreign invaders. So what does garlic do to help your immune system? Let's first have a look at the inflammatory reaction of the immune system, a prime example of which is rheumatoid arthritis. The inflammation is caused by compounds known as prostaglandins and thromboxanes, the biosynthesis in your body of which requires the enzymes lipoxygenase and cyclooxygenase (LOX and COX). If these enzymes can be inhibited, then the inflammatory response can be modulated, and LOX and COX inhibition is one of the studies currently being carried out into the treatment of some forms of cancers. However, where garlic comes in here is that two effective non-reversible inhibitors of LOX and COX are the chemicals Di(1-propenyl) sulfide and ajoene, and both of these are components of garlic. Garlic can therefore be used, not to stop the inflammatory response altogether since it is an essential part of the immune system for certain infections, but to modulate it and protect you from the more severe effects of conditions such as arthritis - both osteo and rheumatoid - and asthma, which is also an immune response. Allicin has been shown to work with vitamin C to kill certain types of bacteria and viruses, and can help the immune system to protect you from colds and flu, Candida and some gastroenteric viruses. It can also be effective against some of the more powerful pathogens such as tuberculosis. It should be stressed that garlic will not cure these conditions, but help the immune system to deal with them. In fact with respect to the common cold, a study at Munich University has shown that garlic significantly reduces the activity of kappa-B, which is a nuclear transcription factor that mediates the inflammatory response. In other words, the cold symptoms are greatly reduced. This is significant, since increased kappa-B levels can be triggered off by any pathogen that causes an inflammatory response by the immune system (e.g. infection, allergens, physical trauma). The study showed that unfertilized garlic provided a reduction of 25% in kappa-B activity, while garlic fertilized with sulfur reduced it by 41%. There have been other studies carried out that demonstrated that Helicobacter pylori, the organism responsible for gastritis and peptic ulcers, was less active in those that took a regular amount of garlic in their diet. This was shown by measuring the antibody concentration, and while H.pylori was found in both sets (with and without garlic in the diet), the antibody count in the garlic-eating set was much lower indicating a significantly lower population of the bacterium. Another unexpected result was that a group taking both cooked and uncooked garlic had a lower antibody count than those taking either cooked or uncooked. This appears to indicate that cooking changes the chemical nature of garlic, so that both forms work together to provide a more potent effect that cooked and uncooked separately. What has also been established is that odorless garlic has less of an effect on the immune system that natural garlic, so while the odorless type is more socially acceptable, it is not so good at supporting your immune system. The allicin levels in odorless garlic are very much lower than in the natural bulb. Garlic has also been found to be able to help with certain types of cancer. Two servings weekly have been found effective in protecting from colon cancer. Allicin has been found to protect colon cells from the toxic effect of various chemicals, and also reduce the growth rate of any cancerous cells that develop. People in Southern Europe consuming large quantities of garlic have been shown to be 39% less liable to contract cancer of the mouth and pharynx, and 57% less liable to contract cancer of the esophagus. It also had an effect on other cancers, including breast and ovarian cancer. However, the effect of onions on such cancers is even greater. Most people are aware of the cardiovascular benefits of garlic, and it can reduce blood pressure, cholesterol levels and serum triglyceride levels, thus protecting against the harmful condition of atherosclerosis and also of diabetic heart disease. Reduced atherosclerosis means a reduced chance of heart attacks or strokes. It also appears to possess antioxidant properties.

He soon established that it was this cetyl myristoleate circulating in the blood that protected the mice, and then found a way of synthesizing it in the lab by esterifying myristoleic acid. Since the compound was unknown at that time Diehl patented it in 1977, hence the date. Apart from in mice, it has subsequently been found only in whale oil and in one of the glands of the male beaver, with no other known sources at the time of writing. There have been several studies that have supported the effect of cetyl myristoleate on arthritis, the first human study being in 1995 when 46 out of 48 arthritis patients showed significant improvement in articular mobility and reduction in pain. Others since have demonstrated similar results. So how does it work? What is the mechanism by which this hitherto unknown substance helps to relieve the symptoms of osteo and rheumatoid arthritis? Being a fatty acid ester, cetyl myristoleate possesses lubricant properties, and one of its properties is to supplement the synovial fluid that lubricates the joints and tendons. This helps to keep the joints moving freely, and reduces their tendency to grind when the connective tissue wears. It also lubricates entire muscle groups, allowing them to slide over each other more freely, and also over bones allowing a smoother movement of the joints. The second way, in which it operates, by supporting the immune system, is particularly appropriate to the treatment of rheumatoid arthritis which is caused by the immune system attacking the body’s own joint and connective tissue. In this respect, cetyl myristoleate has also been found effective in treating other immune system conditions such as multiple sclerosis and lupus erythematosus. Finally, it also possesses the mediation effect on the inflammatory reaction possessed by other essential fatty acids. This helps to reduce the effect of inflammation that causes much of the swelling and pain of arthritic conditions, although it occurs much faster than with other fatty acids. Sharks do not have skeleton of bone but of cartilage, which is a dense form of connective tissue. Rheumatoid arthritis, on the other hand, is completely different, and is caused by the immune system rather than wear and tear. The immune system mistakes parts of your joints as being foreign, and attacks the synovial membrane, or lining of the joint. It might also attack the sheath around the tendons. This eventually causes the cartilage to thin and the joints to wear, and the inflammatory response can cause painful inflammations. There is no apparent cause, though heredity, lifestyle and hormones might all be connected. The effect of shark cartilage on arthritis is well documented, and in one study in the 1970s, only 11% of patients did not respond well to a treatment of cartilage injections. The pain relief the injections provided lasted from six weeks to over a year, though no reason could be provided for the vast difference. However, it did seem to demonstrate that the remedy was more than just a placebo. In another study involving bedridden osteoarthritis patients, eight out of ten could leave their beds after only three weeks of oral cartilage treatment. Shark cartilage appears to be effective when administered both intravenously and orally. In a later placebo study involving 147 arthritis patients, they were given either shark cartilage or a placebo. Those with the placebo were encouraged to use other treatments when their pain became severe. After five years the placebo group reported a 5% drop in pain scores compared to 85% of the group taking shark cartilage. The joint deterioration in each group was assessed, and was considerably less in the cartilage group, who also lost less time at work through pain. All of these results indicate that shark cartilage can be used to relieve osteo- and rheumatoid arthritis pain in at least 60% of sufferers. Since rheumatoid arthritis has been linked to psoriasis, this could also explain its effects on psoriasis. What the studies did indicate is that you need not wait five years to find if the treatment is effective or not: if you do not experience positive results within between four and six weeks, then shark cartilage treatment will likely not work for you. There is no doubt that shark cartilage can provide relief to painful, swollen joints, and this is likely due to the mucopolysaccharides. It can also prevent the undesirable growth of new blood vessels into the cartilage and appears to help to regulate the immune system. These are also two of the reasons why it is believed by some to be an effective treatment for cancer by preventing metastasis. It was the publication of the book “Sharks Don’t get Cancer” that started the rush for shark cartilage as a a treatment for cancer, but the problem is that sharks do get cancer – they even get cancer of the cartilage! However, that does not mean that the scientific reasons for the inhibition of metastsis are invalid. They are valid, and it is metastsis rather than the original cancer that ferquently leads to death. Metastsis is the spread of the disease round the body by the bloodstream, and shark cartilage appears to be able to help to prevent that. It also prevents the growth of blood cells into cancerous areas to feed the cancers with oxygen and other nutrients. Although there are few reported side effects of shark cartilage, in the interest of safety it is advised that children and pregnant women should avoid it. The same is true of people recovering from recent surgery since it could slow healing due to its effect on repairing blood vessels. If you have a low white blood cell count, do not have a shark cartilage enema since it can cause a potentially fatal infection.

The area swells up due to the concentration of fluid and protein, and becomes painful due to pressure on nerve endings. Anti-inflammatories, such as the flavanoids, work to relieve the symptoms of inflammation without interfering with the way the immune system deals with infection. Many inflammatory conditions occur without any apparent reason, and while anti-inflammatories might not be used to suppress the immune system, it is used in such conditions as rheumatoid arthritis where the inflammation is a mis-firing of the immune system. Inflammation occurs because of the reaction of your immune system to what it perceives as being the presence in your tissues of dangerous foreign particles. Your immune system generally reacts to particles. Typical particles are bacteria and viruses, pollen, smoke, your own body cells that are not doing what they should, moulds and fungi, and various other things that can circulate in your blood. There are several reasons for the immune system being activated, some of them historical due to your body having an imprinted memory of previous bacteria and viruses that it has dealt with, and others the reaction to new invaders that it has never met before, and for which your immune system creates antibodies to destroy. Dioscorea villosa, commonly known as wild yam, is a tuberous vine native to North America and parts of Central America. It was used by the Mayan and Aztec civilizations for pain relief and birth control and has also been given the names colic root and rheumatism root, demonstrating this early use of the plant. However, whether it is nature’s progesterone or not is another question that requires close analysis. These Central American civilizations, of course, did not understand the reason why they were effective. Some of the symptoms treated can be caused by the menopause, and the wild yam is believed to contain natural forms of progesterone that can alleviate some of the adverse effects of the menopause on the body. Like most plants, they also likely act as anti-inflammatories, so relieving the pain of rheumatoid arthritis and other inflammatory conditions. Like any natural remedy, wild yam had its adherents and its detractors, though in recent years the plant has been used as the raw material for the production of cortisone and a form of progesterone: or at least so it is claimed, so let’s have a closer look at these claims and determine whether or not wild yam is indeed nature’s progesterone. The human body uses organic compounds called hormones to regulate many of the chemical activities of the body. The hormones catalyze the chemical reactions and changes needed for the proper functioning of our bodies. Hormones are produced in small chemical factories dotted throughout our body called glands. Each gland is devoted to producing a specific hormone, or a range of them, and each hormone is designed for a specific purpose. Each has an associated hormone ‘receptor’ at their targeted destination that fits the hormone like a jigsaw piece. When it turns up, the hormone attaches to the receptor and the reaction proceeds. That might be the initiation of energy production in cells, the activation of certain genes or even the stimulation of hair growth by the follicles or of the libido and the natural desire to procreate. Hormones are manufactured from only three constituents: proteins, amino acids and cholesterol, and the steroidal sex hormones are created from cholesterol. Therefore, don’t reduce your cholesterol level too much! Prior to puberty, all of the sex hormones are manufactured by the adrenal glands, and after puberty by the ovaries in women and the testes in men. Progesterone is produced initially, and is then used as the building block for all the other sex hormones (hence the prefix ‘pro’). In women these are the estrogens and in males the androgens. Progesterone is made from the start of the menstrual cycle, and after day 12 they have reached a high enough level to halt ovulation. Progesterone levels continue to rise for about 8 days, and then if fertilization has not occurred, the progesterone levels trigger menstruation, and the lining of the womb is detached and expelled. During certain phases of the menstrual cycle, the levels of the hormones estrogen and progesterone drop off, although the exact reason for the many and varied symptoms of pre-menstrual syndrome (PMS) are still unknown to a large extend. There appear to be several contributing factors, though estrogen and progesterone certainly appear to be two of the major ones. We shall concentrate here on the progesterone factor because that is what wild yam is claimed to address. Although the main function of progesterone is to generate estrogen and testosterone, to restore the libido after menstruation, and maintain the integrity of the lining of the uterus and aid in gestation, it has many other purposes such as in thermogenesis (burning of body fats), protection against osteoporosis and is also believed to have a natural anti-depressant action. These are all supported by the symptoms that appear when the body is low in progesterone levels: depression, sudden increase in body fat, mood swings, loss of sex drive and a susceptibility to weak bones if the deficiency is long-term. In other words, women that usually suffer from PMS, also generally suffer low progesterone levels. It is believed in some quarters that a progesterone deficiency causes an excess of estrogen that leads to these symptoms in addition to heart disease. No one really knows for sure, but a deficiency of progesterone seems to be the determining factor. Women with menstrual problems continue to be issued with prescribed synthetic progesterone even though they do not have the same effect as natural progesterone. They can also lead to some unpleasant side effects such as fluid retention, strokes, jaundice, blood clots and depression: some of the symptoms it is intended to alleviate. Some take borage or evening primrose oil for the GLA that helps some of the effects, but this is taken to counter the over-production of the hormone prolactin in some women prior to menstruation, not a deficiency of progesterone. Wild yam does not cause these symptoms, and is claimed by many to be very effective in alleviating the symptoms of PMS. The problem with the artificial forms of progesterone is that the liver’s job is to destroy foreign hormones, and send them to the digestive tract for expulsion. Natural hormones are not affected in this way, even if they come from a non-human source. The chemical diosgenin in wild yam is very similar in structure to progesterone, and is believed to be the active principle. It is claimed that it can be used to produce not only progesterone but also other related hormones. However, some medical authorities and practitioners dispute this claim, and there is still a question as to how wild yam works. Because it does work, many people swear by it and claim that they could not live without it.
This is especially true of those that suffer from the more severe effects of PMS, and since wild yam does not work for everybody, it could be connected with the severity of the condition and the symptoms. Whichever is true, there appears to be little doubt in the eyes of those that use it as a cream that wild yam is indeed nature’s progesterone.
