Garcinia cambogia is a small tree bearing a small pumpkin-like fruit of the citrus family, and is steadily appearing in an increasing number of weight loss supplements. It is indigenous to parts of Africa and Asia, and the rind is commonly used in Indian cookery. Although the fruit is particularly sour to the taste, it is used in form of an extract of the rind and claimed to act as an appetite suppressant and also to increase the body’s capacity to metabolize fat. Although many traditional herbal medicines have yet to find complete scientific corroboration in the West, it follows logically that people wouldn't use an herbal product for centuries if it didn't work. Many of the popular herbs we all recognize as having great health benefits were only recently considered pretty exotic. Even green tea - a staple in China for centuries -has only lately gone main stream. So it will likely be with herbs from the Amazon basin and its environs. The Amazon basin is one of the most bountiful environments on the planet. Explorers and botanists from the West have looked to this region for generations for the "next big thing." Of course, in many cases, the "next big thing" has already been in use for centuries. In this issue of Ask the Medicine Hunter, we're going to look at some energizing and life- stimulating herbs that also happen to have great antioxidant properties, too. Best yet, many of them are available to us here from companies that practice fair trade policies. Let's take a look at some of the herbal powerhouses coming out of the Amazon (and its nearby neighborhoods): Maca (Lepidium meyenii) has been cultivated for a long time at least 2000 years. Related to brassica family plants like radishes, mustard and cabbage, its foliage does actually look somewhat radish-like, but grows close to the ground. Maca is cultivated by the Andean people in Peru's central highlands, and contains a plethora of beneficial compounds that enhance overall health and vitality. The tradition of cultivating maca is an old one some strains have been found in Incan sites that date from 1600 B.C. During early European colonization, maca was used by the local native culture as a form of currency, much of the way cocoa was used by the Aztecs, further north in pre-Columbian Mexico. Maca thrives in high altitudes - between 10,000 and 16,000 feet. The harsher the conditions, the better it grows, or so it seems. In fact, efforts to grow the plant in Central Europe haven't been as successful - maca seems to enjoy its home turf the best. In Peru, maca is a popular and beloved nutrient-packed superfood, and is commonly powdered and mixed into drinks at roadside stands throughout the Andes. Q. I've heard of maca being used for healthy libido - are there any other benefits? A. Maca is a natural energizer, and although it is recognized for it's libido enhancing abilities, it has other uses, too, acting as an adaptogen - similar to rhodiola or ginseng. In fact, in South America, maca is known as "Peruvian Ginseng." Though maca is not ginseng at all, some of the benefits of both plants are similar. In any event, maca is recommended for boosting the immune system, menopause support, and hormonal balance in general. For daily use, maca is most recognized as a great source of energy and all-day endurance. Alkaloids from maca root may be partially responsible for both maca's energizing and libido boost. Although the same constituents specifically from maca haven't beentested, it's plausible that they could have the same effects. Q. I've heard a little about guarana extracts - is it just caffeine? A. Guarana is widely loved for its mild stimulating effect, which is due to caffeine. But this is by no means this Amazonian herb's sole beneficial compound. Guarana (Paullinia cupana) is so logically ingrained in the culture of Brazil that it's practically a rival (actually out-sells) Coca-Cola in its soft-drink form. Like many other indigenous herbs, guarana was in use locally well before European settlement. Its Latin name comes fromthe German botanist C.F. Paullini, who first encountered the herb in the 1700s. This evergreen vine typically climbs fairly far up the Brazilian forest trees. The seed is the part that gets used. In one clinical study, guarana Boosted the memory alertness of participants, even when the caffeine level per dose was a low 9 mg., as compared with approximately 100 mg for a cup of coffee. This effect suggests that other agents than caffeine contribute to a feeling of well being. Guarana also contains powerful antioxidants including catechin, epicatechin and proanthocyanidins, which protect cells against destruction from free radicals, and impart benefits to the body's tissues and blood. The small seed of this plant is powerful in its health benefits. Catuaba Bark: Catuaba (Erythroxylum catuaba) is a common tree found in South America from Brazil to Peru, in the same genus as the coca plant. Catuaba contains components known as alkaloids. These alkaloids (called catuabine A, B, and C) are probably responsible for themental boost most people get when they take catuabe-based supplements or mixes. There may be little confusion regarding catuaba, because various species and genus typesuse the common name. As a result, "catuaba" gets bandied around a lot, and one person'scatuaba may not be the next. Read labels carefully. The catuaba I've had the best luck with is Erythroxylum catuaba. Coffee Fruit: One of my favorite drinks in the world is coffee, and I'm sure at many people reading thisconsider it the essential part of their morning, too. The part of coffee that we use the most is the seed of the coffee fruit - which appears as a bright, red berry. Most of the time, this fruit is sloughed off and left behind in the process of making coffee - it's really too delicate to last long in hot conditions. But advances in technology have tapped a previously discarded resource. Though the fruit of coffee is available in any coffee-growing economy, a high antioxidant commercial extract of "coffee cherry" is now available from the fruits of coffee plants in Mexico. Coffee fruit has many of the attributes of other dark-colored, anthcyanin-rich fruits. Coffee fruit (also referred to as "coffee cherry") appears not to be just another antioxidant, however. Current research on this once-forgotten, former castoff shows impressive abilities to decrease tumor size, and possibly even prevent their formation in the first place. It seems that the elements in coffee berry activate T-lymphocytes in such a way that mammary tumors are shrunk or simply put on hold. It will be fascinating to see how this science plays out. Muira Puama Bark: Muira puama (Ptychopetalum olacoides) grows between 15 to 45 feet high. Native to theAmazon basin of Brazil, the dried bark has been used for centuries as a traditional energysupport. Components include beta-sitosterol, campesterol and lupeol. Muira puama, like other central nervous stimulants has been researched lately for its ability to boost memory retrieval and protect neural (brain) tissue. Who knows? Maybe this traditional ingredient could someday be on the cutting edge of natural medicines fighting Alzheimer's, much the way green tea and turmeric are currently. In one unpublished French study of 262 men with low libido and poor erectile function, 62% experienced significant improvement after taking an extract of Muira puama for two weeks. Acai Berry: Acai (Euterpe acai) berry is a traditional favorite (and readily available) food source for people in the Amazon. HISTORY For thousands of years amazing magical and medicinal powers have been attributed to garlic. Prized as a legendary protectant against vampires in Transylvania, it has also been used to enhance sexual prowess and fight off infections. Referred to as “the stinking rose,” it is mentioned in Bible, the Talmud, and in the Odyssey by Homer as well. The Egyptians looked to garlic as a tonic which Boosted physical strength and consumed it while building the pyramids. The Greeks utilized its laxative properties, and the Chinese prescribed it for high blood pressure. Vikings and Phoenicians alike extolled the virtues of garlic and used it both for flavoring foods and treating disease. Garlic is a hardy, perennial bulb which is native to the Mediterranean regions of Africa and Europe. Along with onions, leeks, chives and shallots, garlic is a member of the lily family. The botanical name for garlic, allium sativum may have been derived from the celtic word all which refers to “pungent.” The edible portion of the garlic plant grows underground and consists of a cloved bulb. Hippocrates believed that garlic could treat uterine cancer and Native Americans used it for stomach cancer. During the Bubonic Plague years in Europe, garlic was used to boost immunity against the infectious organism responsible for so many deaths. Several accounts relate that survivors of the plague were frequently those who had routinely ingested large amounts of garlic. A sixteenth- century herbalist writes, referring to garlic, “The virtue of this herb is thus. It will unbind all wicked winds within a man’s body.”1 During the eighteenth century, Russians utilized garlic to treat influenza. Eventually, garlic would become known as “Russian penicillin.” American colonists regarded garlic for its ability to kill parasites. In the nineteenth century, Louis Pasteur finally proved scientifically that garlic contains antibiotic properties. His discovery led to the initiation of hundreds of studies which have substantiated his findings. What was thought to be nothing more than a culinary ingredient has medicinal value. Garlic can effectively kill bacteria, fungi, viruses and parasites. In the late nineteenth century, garlic was routinely used by physicians as an effective treatment for typhus, cholera and whooping cough. It was highly recommended by medical practitioners and considered as staple treatment for infection. Albert Schweitzer used garlic for treating amebic dysentery in Africa. Early in this century, tuberculosis was treated with garlic and it was also used as an antibiotic/antiseptic for wounds during World War II. American and European doctors alike noted a remarkable high cure rate in tuberculosis patients treated with garlic. 2 Septic poisoning and gangrene, which can so quickly develop in battlefield wounds were prevented to a significant degree by using garlic. During the 1950’s Chinese scientists used garlic to successfully treat influenza. Subsequently, western studies found that garlic was an effective treatment for the common cold. Today the widespread use of antibiotics have relegated garlic to the back burner of medicinal therapies for infection. The discovery of penicillin resulted in classifying garlic as nothing more than a folk remedy. Unfortunately, for several decades its medicinal potential was no longer taken seriously by scientists. Over the last decade, scientific interest in garlic has dramatically escalated. In 1990, the First World Congress on the Health Significance of garlic and Garlic Constituents was held in Washington D.C. Herbalists have always considered garlic as an effective treatment and preventative agent against colds, flu and other infectious diseases. The present focus on garlic as a medicinal agent promises to support the notion that garlic should be utilized by medical practitioners much more than it currently is. Recently, medical research has focused on garlic’s potential value in treating cardiovascular disorders and as an anti-cancer agent. This renewed interest in garlic has contributed to the development of the “Designer Foods Program” which is sponsored by the National Cancer Institute.3 This agency investigates foods that may be effective cancer preventatives. Garlic is one of those foods which may have profound cancer prevention potential.

Americans don't move around and burn off the calories they eat. Instead, our sedentary ways are causing big stomach bulges and a big bulge in health problems linked to being overweight. Want to lose those pounds? Consuming the right nutrients and pounding the pavement are a great start. Number of Large Americans Grows Larger Fifty-five out of every 100 Americans are now overweight. And the numbers continue to surge. To take yourself out of these alarming statistics, plan a consistent exercise program. If the thought of sweating during strenuous workouts at the gym scares you, relax. You don't have to do that much. According to a long list of research, a few half-hour walks a week can put you on the road to weight loss. For instance, researchers at Duke found that walking a total of only 12 miles a week can help you control your weight even if you're not dieting (Archives of Internal Medicine 1/12/04). Of course dieting and taking the right supplements will help you lose weight a little faster, but walking burns off the pounds. In the study at Duke, scientists gathered about 180 overweight, sedentary people and assigned them to one of four groups. The first group had to jog about 20 miles a week. The second group jogged 12 miles a week. The third group walked 12 miles, and the last group didn't do a thing. " We found that the two low-exercise groups lost both weight and fat, while those in the more intensive group lost more of each in a 'dose-response' manner," says Cris Slentz, PhD, one of the researchers. " Simply put, the more you exercise, the more you benefit. Just as importantly, the control group of participants who performed no exercise gained weight over the period of the trial." Everybody who was exercising lost weight; their stomachs and hips shrank. The folks who did nothing didn't just stay at the same weight; their weight went up. Dr. Slentz adds, "From the perspective of prevention, it appears that the 30 minutes per day will keep most people from gaining the additional weight associated with inactivity. Given the increase in obesity in the US, it would seem likely that many in our society may have fallen below this minimal level of physical activity required to maintain body weight." Enzyme Help Of course, limiting the carbohydrates you consume and eating more protein at the same time as you exercise can help you lose weight more effectively. To help your body utilize protein more fully to fuel your exercise, many experts recommend taking enzyme supplements. The Vermont researchers found that chromium increases the activity of a protein called Akt phosphorylation. This is a natural substance made by the body that makes cells more sensitive to insulin and speeds the movement of blood sugar into cells, where it can be burned for energy. So, in combination with exercise, which also helps the body become more responsive to insulin, chromium may help keep blood sugar under control. As Sandra Woodruff points out in her book Secrets of Good Carb, Low Carb Living (Avery/Penguin), "[P]hysical activity boosts the body's ability to process carbohydrate and helps reduce insulin resistance....Exercise acts much like an insulin-sensitizing drug to make the body's cells more responsive to insulin, which enables them to remove sugar from the blood more efficiently." The results of exercise and getting sufficient chromium can help support weight loss and healthy blood sugar levels. The Vermont research team found that the exercising chromium takers Boosted their insulin sensitivity by almost 9%. Building a Lean Body A prime benefit of exercising while you lose weight is that physical activity builds muscle tissue. Muscle tissue, even at rest, burns more calories than fat cells, so having extra muscle means you can eat more and still maintain or lose weight. Research shows that forskohlii furthers molecular energy processes within cells that support the development of lean body tissue (muscle) and the enzymatic reactions that break down fat. In a study of about two dozen women who took forskohlii extract, researchers found that taking this supplement was associated with feeling more energetic and less hungry (Experimental Biology meeting, 2002). According to Vladimir Badmaev, PhD, who has studied forskohlii, this herb's help in supporting lean body tissue is crucial to maintaining a healthy weight. " In highly overweight individuals, fat tissue can constitute up to 70% of body weight. The remaining proportion of body composition corresponds to the lean body mass. Lean body mass is composed of muscle, vital organs, bone, connective and other non-fatty tissues in the body," says Dr. Badmaev. " The body's metabolic rate is in direct proportion to the amount of lean body mass. Therefore, safely maintaining or increasing lean body mass is an important consideration for any weight-loss strategy." The Skinny on Calcium Calcium, long known for its ability to strengthen bones, is now turning out to play a key role in weight maintenance as well. In one study, obese mice who were put on calorie-restricted diets lost body fat when fed extra calcium (Experimental Biology meeting 2000). Both supplemental calcium and low-fat dairy proved effective. What's more, low-fat dairy's ability to burn fat and provoke fat loss have been confirmed in additional research (Obesity Research 4/04; 12:582-90). Burn off body fat! by Mimi Facher Energy Times, June 1, 1997 So you're feeling a little blah, a little overweight, and you're looking to drop a few of those winter pounds gained during the colder months. Maybe you've dabbled with diets and jogged around the neighborhood a few times but you're still packing unsightly bulges. If so, you may be considering the idea of turning to supplements to help you drop those pounds. Well, two types of diet supplements now generally available, combined with a diet and exercise program, may be able to help you trim those stubborn pounds. The first type of supplement, called metabolic optimizers, which include ephedra, caffeine and salicin (derived from willow bark), boost your metabolic rate, causing your body to burn calories faster. The second class, lipotropic substances, aid the body in fat mobilization, causing greater utilization of stored fat. These products include chromium, carnitine and hydroxycitric acid (HCA). Both classes of supplements have been around in various forms for quite a while but are now enjoying greater popularity among dieters. Trying to cope with a weight problem is a dilemma expanding throughout modern society. According to a 1995 Harris poll, nearly 75% of Americans are overweight. Although it's well known that the way to lose weight is to expend more calories than you take in, supplements may be able to help you burn off extra calories. Thermogenesis and You Metabolic optimizers are supposed to aid weight loss through a process called thermogenesis. Thermogenesis is a natural process in which fat is burned to produce body heat. Fat that isn't burned is stored on the hips, thighs, stomach, etc. Thermogenic agents are designed to counteract your body's fat storage mechanisms by causing your body to maintain a higher metabolic rate-turning your internal thermostat up to burn fat faster. The thermogenic process can be jump-started by a number of factors including cold, exercise, certain dietary nutrients and metabolic optimizers. Ephedra The ephedra herb, also known as ma huang is one of nature's earliest medicines, known for over 5000 years to the Chinese, who used it to relieve allergies, coughing, wheezing and cold and flu symptoms. In the US, ephedra has been available since the 1800s. The ingredients in ephedra include the alkaloids ephedrine, pseudoephedrine and norephedrine. Concentrated forms of these substances are used in today's over-the-counter cold, allergy and asthma relief formulas. Ma huang's effectiveness as a weight loss aid is tied to its appetite suppressant and stimulant properties. By speeding up action of the thyroid gland, the ephedrine found in the herb acts a thermogenic agent, boosting the rate at which the body metabolizes fat and promoting weight loss. According to Mark Blumenthal, Executive Director of the American Botanical Council, "When used as part of a total package that includes diet modification and exercise, ma huang can be highly effective in the short run because it increases the speed of the body's metabolism and suppresses appetite." Because of their strong stimulant effect, ephedra and its derivatives have engendered some controversy. However, in its long history, billions of doses of ephedra have been consumed without problem. But ephedra supplements should only be used as directed on product labels. People with cardiovascular problems, diabetes, thyroid or prostate dysfunction, high blood pressure and those taking MAO inhibitors, pregnant or nursing should avoid this herb. Salicin Burns Fat Salicin, a substance derived from willow bark-which is also the original source for aspirin, a related compound-can boost the burning of fat when combined with ephedra. An animal study in the American Journal of Clinical Nutrition found that while ephedra Boosted calorie burning by almost 10%, when ephedra was combined with aspirin, extra calorie burning just about doubled. Another study in the Internatioanl Journal of Obesity showed that when overweight women took aspirin and ephedrine during a meal, their bodies burned off more calories than normal. (Eating a meal produces a thermogenic effect as your body expends energy in digestion. That's why dieters are told not to skip meals. Skipping meals lowers your metabolic rate, decreasing your calorie expenditure.) Similar studies also show that caffeine, the stimulant that gives coffee its eye-opening kick, can also boost ephedra's thermogenic properties. But before using these combinations check with a health practitioner knowledgeable about nutrition. Aspirin or salicin may cause stomach upset in some people (although salicin is generally tolerated well.) Carnitine: Lipotropic Amino Acid To get carnitine into your system, you don't have to take it as a supplement. Your body already makes this vitamin-like substance. However, your body doesn't make that much. And it is said to be especially low in people with heart disease. This non-essential amino acid (said to be non-essential because human bodies produce it) is a key ingredient in the formation of mitochondria membranes. Mitochondria are tiny structures in your cells that burn fats for energy. Consequently, sufficient carnitine is necessary for the movement of fat into the mitochondria where it is consumed. When not enough carnitine is present, the breakdown of long chain fatty acids slows down. Said to improve the recovery rate for athletes (it may limit the production of lactic acid, a waste product in muscle tissue), carnitine can also lower cholesterol levels, boost levels of HDL (the good cholesterol) and decrease serum triglycerides (blood fats linked to heart disease). Not bad for a nutrient that coaxes fat into those teeny, ceullular, mitochondrial furnaces. Go for the Chrome Chromium-based supplements work as lipotropic agents by aiding insulin use in the body. This essential trace mineral is required for normal protein, fat, and carbohydrate metabolism. According to Dr. Michael Janson, author of The Vitamin Revolution in Healthcare and President of the American Preventive Medical Association (APMA), "Chromium is important for proper insulin activity. Insulin moves sugar into the muscle cells, where it is burned off as energy. Chromium improves the activity of insulin, and since insulin causes fat deposition, less of it means less fat deposition." Chromium has also been shown to build muscle tissue and to reduce LDL cholesterol, which has been linked to heart disease. Although the body's minimum requirement is low, the American diet tends to be deficient in chromium, in part because the mineral can be difficult for the body to absorb. The fact that, in nature, chromium is most powerfully concentrated in brewer's yeast, wheat germ and liver-items most Americans rarely eat-probably hasn't helped either. Other natural sources of chromium include whole grains, molasses and beef. But it is estimated that 50% of Americans are chromium deficient. An early study found that overweight adults taking a chromium supplement lost an average of 22% body fat, while maintaining or gaining lean body mass. In another study, athletes consuming 200 mcg. of chromium a day showed an average loss of 7.5 lbs. of body fat after six weeks, without a corresponding loss of muscle tisue. Overall, although some studies question chromium's precise effects, many experts are optimistic about this substance because of its relationship to insulin in the body's metabolism. Hydroxycitric Acid (HCA) Another possible addition to the dieter's arsenal is HCA. In nature, HCA appears chiefly in a fruit called garcinia cambogia (sometimes also called Malabar tamarind or brindall berry), a citrus plant found primarily in Asia, where the rind is often used as a flavoring agent. HCA works by inhibiting the enzyme in the body responsible for converting carbohydrates into fat. HCA causes calories to be burned in an energy cycle similar to thermogenesis and acts as somewhat of an appetite suppressant. HCA is also said to have a role in reducing triglycerides and LDL cholesterol levels. Several animal studies have shown that HCA caused significant weight loss without a reduction in lean body mass. In other words, the pounds that came off came out of fat stores, and not out of energy or muscle reserves. This means that HCA takes off not just weight but body fat, making it a potentially effective tool against weight regain. Dr. Elson Haas, director of the Preventive Medical Center of Marin in San Rafael, CA, and author of Staying Healthy With Nutrition: The Complete Guide to Diet and Nutritional Medicine, believes that HCA can be a helpful aid for dieters when used in combination with eating habit changes and exercise. He recommends an HCA and chromium blend for optimum appetite suppression. "This combination can keep the appetite down and reduce sugar cravings," he says. Although human research data on HCA is still in the preliminary stages, the animal study results are positive, and the supplement seems to have minimal side effects in most people. Some Overall Recommendations You are likely to lose weight faster if you eat sensibly. This means avoiding foods high in fat or sugar (which are the most likely to add to stored body fat), but it doesn't mean starving yourself. A sensible balanced diet, along with moderate exercise, is still the best prescription for weight loss. As Dr. Haas puts it, "I'm a firm believer in diet and exercise. Using supplements responsibly can help you to lose weight provided they're combined with dietary changes and exercise. They won't work if you don't change anything." No one is suggesting that dietary supplements are a miracle cure for being overweight-as always in self-health care, there are no magic wands. But, used as directed and combined with a good diet and exercise plan, you could find that these supplements might help you work your way to a slimmer you. Summer Sports Nutrition Guide by Joyce Dewon Energy Times, June 18, 2004 If you're hooked on exercise you're probably just as hooked on using top-notch equipment when you work out. Those who are serious about staying in shape buy the best running shoes, carefully pick out the best bikes and tread on durable treadmills. But do you pay just as much attention to your nutrition? Scientists who have studied exercise have found that what you eat before, during and after workouts is crucial to maintaining your health, getting into shape and staying fit. To achieve your best athletic performance without getting injured or sick depends on optimum nutrition. When you carefully plan what to feed your exercised body, it rewards you by feeling and looking better. Short 'n Sweet If you thought long exercise sessions were the only ways to get decent exercise benefits, take notice: small doses of exercise during the week can go a long way. " The important thing, apparently, is just do it," says Howard D. Sesso, ScD, author of an American Heart Association study on exercise and heart disease. In his study, exercisers demonstrated that several short sessions of exercise were as good for the body as a single long session (Circ 8/00; 102:975-80). " Short sessions lasting 15 minutes long appear to be helpful,"Dr. Sesso explains. Even walking about three miles per week, which is a moderate level of exercise, lowers your risk of heart disease by 10%. No Sweat? Some people glorify in working up a sweat; others curse the dampness. But putting in extra effort in even short bursts of activity pays off: experts have found that intense exercise burns more calories than more relaxed sessions, more effectively reduces your risk of cardiovascular disease and helps stabilize blood sugar levels. In addition, it stimulates production of human growth hormone, which offsets some of the effects of aging (Exp Biol Med 2004 Mar; 229(3):240-6). But don't go crazy if you haven't worked out in a long time. The intensity of the workout should match your physical fitness. According to the American Heart Association, when people exercise at a comfortable pace, their heart rate and level of exertion stay within a safe range, but still high enough to benefit their health. Strenuous activities, for those who can handle them, produce the most physiological bang for the jog. But brisk walking within your own level of fitness still offers significant benefits. Feeding Your Muscles When you exercise, you work and develop your muscles, which are made primarily out of protein. Despite this fact, many exercise experts have advocated high-carb diets for athletes. But, as John Ivy, PhD, and Robert Portman, PhD, point out in their book The Performance Zone (Basic Health), "[While] there is no doubt that aerobic athletes require more carbohydrate than strength athletes...we are now discovering that the addition of protein to a carbohydrate supplement offers significant benefits to aerobic athletes." That is why researchers believe that consuming plenty of protein along with carbohydrates offers the best fitness benefits. Protein helps fuel activity more efficiently and aids in recovery after a session at the gym, allowing your body to repair muscle damage and build up muscle fibers. During exercise, you break down muscle tissue. It is during recovery, after your exercise session ends, that muscles are rebuilt. At the same time, other cellular processes take place that adapt the body to working out. According to Ivy and Portman, timing your intake of nutrients after exercise is crucial: "The ability of the muscle machinery to regenerate itself decreases very rapidly after a workout, so that the nutrients consumed more than 45 minutes after exercise will have far less impact in helping the muscles regenerate than nutrients consumed earlier." Stresses and Tears Engaging in athletics can cause microscopic muscle tears. These tears can cause a range of problems that, when you exercise excessively, can cause pain and injury. Inflammation is the body's response to cellular damage. The damaged area can swell as the body sends white blood cells and other cells to repair the injured area. Unfortunately, the swelling can further damage the muscle cells. Since inflammation can take 24 hours or more to cause the collection of cells in the injured area, it can be a day or two before the resulting muscle soreness reaches its peak painfulness and then starts to subside. Cortisol, a hormone produced when you exercise strenuously, which can result in muscle fiber damage. Cortisol boosts protein breakdown, so it can be used to fuel muscle movement. But the more protein breaks down, the more potential exists for muscle fiber injury. Free radicals are caustic molecules that are created when the mitochondria (small structures in cells) create energy; these marauders can also cause microscopic shredding of muscle strands. As you increase your use of energy during exercise, you simultaneously increase the production of free radicals. This collection of free radicals can outstrip the body's antioxidant defenses, leading to extensive muscle damage and dampening of the immune system. All of these cellular events can make you sore. They are also the reasons that athletes who overdo it day after day are liable to come down with nagging colds and a variety of infections. Muscle Fuel Your muscles use different substances for fuel depending on what you ask them to do. Lift a heavy weight and muscles recruit two processes called the creatine phosphate system and glycolysis to generate a large amount of quick energy. These are known as anaerobic types of energy production. But if you jog, swim, bike or perform any other aerobic activity, the cells use oxygen in what is called cellular respiration to supply energy to working muscles. When you exercise aerobically for extended periods of time, the energy available is generally limited by how much oxygen your body is capable of taking in and supplying to the muscles, where it takes part in energy production. In athletic circles, this upper limit is known as your VO2max. The carbohydrates your body burns for energy during aerobic activity are taken from blood sugar and carbohydrate reserves called glycogen. (The muscles store glycogen, as does the liver.) During a workout session, your glycogen supply is limited to what is stored with your muscles. But blood glucose can be Boosted by carbohydrate drinks, energy gels or bars. Most people who work out have enough glycogen and blood sugar to fuel moderate aerobic activity for about two hours. After that, the body turns mostly to fat and protein stores to fuel exercise. Fat Into the Fire In contrast to the body's quickly diminishing supply of glycogen and blood sugar, fat can last for hours and hours of exercise. According to Portman and Ivy, a 200-pound man with 15% body fat has, theoretically, enough fat energy to run from Washington DC down to Miami Beach-and still has enough energy left over to jump into the ocean. But using fat for energy is complicated; fat is stored in fat tissue and not readily available to working muscles. Plus, to burn fat for energy, the body needs carbohydrate-it cannot burn fat all by itself. What's more, the conversion of fat into energy doesn't go as quickly as carb conversion. Protein is also used for energy when carbs run low. But the more you use protein for energy, the more you risk soreness as muscle fibers break down. Prepare to Energize To maximize your energy during exercise and minimize soreness, Portman and Ivy recommend some simple nutritional steps: Limit Soreness Taking protein and carbs while working out can limit muscle damage and curtail soreness. Carbs apparently drop your cortisol levels, and thereby limit muscle injuries linked to this hormone. While the mechanism that helps protein limit muscle soreness is not completely understood, it is possible that taking in protein while working out keeps the body from shredding muscle tissue in search of fuel. Supplements that contain antioxidants such as natural vitamin E and vitamin C (Portman and Ivy think you should take these during exercise) may limit free radical damage to muscle fibers. Muscle Reconstruction Plan If you want to help your exercise plan make you stronger, you should focus your after-exercise sports nutrition plan on these steps: The protein part of the equation is vital: don't merely indulge in only carbs after exercising. A recent study found that while carbs could help muscles rebuild, adding protein can make a big difference in improving your fitness (J App Phys 2/04). This combination of nutrients stimulates the pancreas so that it releases insulin. The release of insulin is the key, initial step that sets off a cascade of physiological events that speeds muscle recovery. Although many people think of insulin as an undesirable hormone-if you never exercise, too much insulin may help drive your blood sugar down and cause other problems-for exercisers, this hormone plays a crucial function in benefiting from exercise. By eating carbohydrate and protein soon after working out and stimulating insulin, according to Ivy and Portman, you help your body boost its synthesis of protein by: Drinking for Exercise The most obvious nutrient you lose during intensive exercise is water in your perspiration. However, that perspiration also contains an array of minerals known as electrolytes. So, for optimal performance and health, experts recommend you replace both the water and its minerals. Merely drinking water-instead of electrolyte-filled sports drinks-during prolonged aerobic activity can be dangerous. It leaves you vulnerable to a condition called hyponatremia, which can occur when your blood levels of sodium and other electrolytes drop, but your blood volume stays steady or increases because you drink lots of water. According to Edmund Burke, PhD, in his book Optimal Muscle Performance and Recovery (Avery), one out of four athletes who seek medical attention after a long race are suffering hyponatremia. " Typically," he says, "conscientious athletes get in trouble because they adhere too diligently to one recommendation: the need to drink lots of fluids. They tend to ignore another recommendation: The need to keep electrolytes up...for most endurance athletes the real problem is drinking too much water." Dr. Burke warns that you can possibly suffer hyponatremia even if you don't drink a lot of water. Signs of hyponatremia can be similar to those of heat exhaustion. But, while resting and cooling down can help alleviate heat exhaustion, that doesn't help hyponatremia. " To protect yourself against hyponatremia, start by paying attention to how much you sweat," Dr. Burke says. If your sweat seems very salty, burns your eyes or leaves an evident, white residue on your skin, you may be losing a great deal of sodium and should be diligent about eating salty foods. " You can also make sure you're getting enough sodium by drinking sports drinks instead of plain water during long (exercise) events," Dr. Burke notes. Exercise Matters Of course, no matter what you decide to eat or drink while exercising, the most important factor for your well-being is to get out to the gym, onto the track, or just on to the sidewalk, and do something, even if you only want to go out for a walk. No matter how old you are or what kind of shape you're in, you'll benefit from exercise. " It's solid evidence that across-the-board declines occur when people stop exercising," says Charles Emery, PhD, professor of psychology at Ohio State University (Health Psychology 3/04). Don't decline or remain supine. Let your fitness climb.
