Growing Older, Feeling Better

 

Not long ago, when a man turned sixty-five, he became officially old – the best years of his life far behind him. The milestone meant his working days were done and if he was lucky, he might get four or five years to spend as he wished before illness and infirmity set in. It was simply expected and accepted that the older a man got, the sicker he got.

Well, not anymore. Today, a man age 65 is just as likely to be found hiking in the hills, running in a marathon, or even dancing in the streets than rocking in that proverbial front porch rocker. Because it’s becoming more and more evident that the older a man gets, the healthier that man has been.

Eating healthy, exercising, and kicking harmful habits (like smoking) can add years to a man’s life. Aging research is proving over and over again, that we can prevent and delay heart disease, cancer, diabetes, and Alzheimer’s disease – the major causes of disability and death in men over 50.

Now, it’s very true that good clean living from early on is preferable to sixty five years of bad habits and five years of good. But it’s also true that it’s never too late for men to make changes and vow to take better care of themselves. And one of the easiest and most effective ways men can improve their health is the addition of high quality nutritional supplements.

In this issue of Ask the Doctor, we’ll talk about specific dietary supplements that have been scientifically shown to improve the health of men over fifty, prevent the diseases that often strike at this crucial time in men’s lives, and actually slow the aging process.

 

Q. I just turned 50 and I’d like to begin taking nutritional supplements, but they seem so confusing. Where should I begin?

A. Many men feel the same way. There are literally hundreds and hundreds of nutritional supplements on health food store shelves. Figuring out which supplements provide the best health benefits for a 50+ man can be overwhelming.

The best foundation supplement is a high quality multivitamin. Research is repeatedly finding that even very healthy men who take daily multivitamins can significantly improve their health. In fact, an article published in the Journal of the American Medical Association (JAMA) recommends that all adult Americans take a vitamin supplement. Look for solid doses of vitamin supplement. Look for sol doses of vitamins and especially minerals. Multivitamins designed to be taken once a day are often woefully deficient in calcium, magnesium, and potassium. The only mineral a man should avoid is supplemental iron. Iron should only be in formulas for women prior to menopause. Men over 50 get all the iron they need from food and too much iron can cause health problems.

 

Look for men’s multivitamins that contain lycopene in the formula. Lycopene is the pigment that makes tomatoes red. The redder the tomato, the more lycopene is present. Numerous studies have shown that when men have high lycopene levels in their blood, they have a much lower risk of heart disease, age-related macular degeneration (a leading cause of vision loss) and prostate cancer.

 

Other important considerations are antioxidant Blends, especially fruit- and tea-derived extracts; ginseng for energy and stamina; and digestive enzymes to aid in absorption and compensate for age-related decreased enzyme levels.

Lycopene

Promising preliminary reports demonstrate that lycopene can actually kill prostate cancer cells, so there has been an explosion of lycopene and prostate cancer research.

 

Q. What exactly happens to men’s hormones as they get older?

A. Just as women experience significant hormonal changes as they age, so do men. In fact, the term andropause has been used to describe men’s mid-life changes. Similar to menopause in women (where the decline of estrogen causes a myriad of symptoms), andropause in men signals the slow decline of testosterone, the chief sex hormone in men. While estrogen levels decline faster and more abruptly in women than testosterone levels do in men, testosterone decline can cause a number of unpleasant symptoms. These include abdominal weight gain, hair loss, reduced energy and sex drive, heart disease, and prostate enlargement. Whether a man labels these age-related changes as andropause or just the consequences of aging, most men will unfortunately experience some or all of them as their birthdays mount.

 

Q. So, is there a supplement that can give me the hormone level of a 20 year old?

A. Sadly, no, at least not yet! But there is a nutrient that can help the testosterone in a man over fifty “behave” more like a younger man’s testosterone.

 

A study that took place at the Fred Hutchinson Center in Seattle found that men who ate three servings of cruciferous vegetables – broccoli, cauliflower, or Brussels sprouts – every day had a 48 percent lower risk of prostate cancer. It seems a cruciferous plant chemical called diindolylmethane (DIM) that’s formed when broccoli is eaten, is the substance responsible for this impressive prostate cancer prevention. Since not many men could be persuaded to ea t broccoli for breast, lunch, and dinner every day researchers tried to extract DIM from these vegetables and make into a nutritional supplement. After many years of trying, scientists finally discovered a stable, all natural, and absorbable from of DIM.

 

The secret of DIM’s prostate cancer prevention is its ability to metabolize estrogen. While estrogen is generally thought of as a “female” hormone, a precise ratio of testosterone-to-estrogen is needed to maintain a man’s healthy sexual response, effective sexual function (erection of the penis and intercourse), strong bones and muscles, viable sperm, and a well-functioning prostate gland. As men enter their fifties, this ratio begins to change.

 

When men take DIM, however, their estrogen metabolism improves, testosterone metabolism accelerates, and the unwanted conversion of testosterone into estrogen is eliminated. This results in higher testosterone levels, similar to those seen in young men. As a result, DIM may speed weight loss, reduce prostate gland enlargement (benign prostatic hyperplasia or BPH), and help men over 50 feel stronger and leaner.

 

Some supplements on the market today contain indole-3-carbinol (I3C), a precursor to DIM. However, I3C is unstable and requires activation in the stomach to be converted into DIM. This means I3C must be taken at a much higher amount and can undergo unpredictable and undesirable chemical reactions in your stomach and colon. DIM is by far the preferred supplement.

 

Q. What is saw palmetto? Does it reduce symptoms of Benign Prostate Hyperplasia (BPH)?

A. Yes it does and very effectively too. Saw palmetto is a small palm tree native to Florida and North Carolina. The tree’s dark red berries contain many beneficial compounds. Nutritional supplements that contain saw palmetto are highly effective in the treatment of BPH.

 

The prostate gland is about the size of a walnut and is located below the bladder and in front of the rectum. It wraps around the upper part of the urethra and its primary job is the production and storage of semen, the milky fluid that nourishes sperm. BPH is one of the most common health conditions in older men. Half of all men aged 40-60 and more than 90 percent in men over 80 have BPH. BPH causes the prostate gland to enlarge, putting pressure on the urethra.

 

Men have trouble starting or maintaining a stream of urine, find they can’t completely empty their bladders, and have to urinate frequently, even during the night. They may also have episodes of uncontrollable dribbling or complete loss of urine. BPH is caused by the conversion of estrogen to a very potent form of testosterone called, dihydrotestosterone (or DHT). When prostate cells are exposed to DHT, they multiply in number and get much larger.

 

BPH rarely improves. It most often remains the same for years or gets gradually worse. The need to continually urinate, interrupted sleep, dribbling, and loss of urine can significantly interfere with a man’s quality of life. Prescription medications that have been developed to treat BPH are only partially effective. And surgical removal of the prostate gland may result in even more persistent urinary incontinence and the inability to achieve an erection (ED).

 

However, saw palmetto berry extract relieves the symptoms of BPH by inhibiting the production of DHT. And, in study after study after study, saw palmetto caused none of the side effects that happen with prostate surgery or medications.

 

Q. There seem to be plenty of ads for supplements that claim they make men into Sexual Superheroes. Is there an “honest” nutritional supplement to help me sexually?

A. That’s a very good observation. And yes, there are honest nutritional supplements for men’s sexual health.

 

Sexual intimacy is an important, complex, and lifelong need. It makes us feel better physically and mentally and adds to our sense of security, belonging, and self-esteem. But just like other changes that happen to men as they get older, men’s sexual response most often changes, too. Declining testosterone levels, changes in blood flow to the penis, certain medications that older men are prescribed, and the presence of diabetes or heart disease can all affect men’s ability to engage in sexual activity.

 

When men have a chronic inability in obtaining and/or maintaining an erection, it’s called erectile dysfunction (ED). While ED is not an inevitable part of getting older, it does occur more frequently as men age. About 5% of 40-year-old men have ED, but more than 23% of 65-year-old men have difficulty maintaining erections.

 

The development of prescription medication Viagra (sildenafil citrate) has revolutionized ED treatment. When a man is sexually stimulated, Viagra helps the penis fill with enough blood to cause an erection.

 

Like all medicines, Viagra can cause some side effects, including headache, flushing of the face, and upset stomach. But because Viagra is a prescription medication, it requires a visit to a licensed healthcare practitioner. For many men, telling anyone (even a professional) that they are having trouble getting or keeping an erection is simply too embarrassing. Viagra is also fairly expensive and many older men do not have prescription drug health insurance.

 

These reasons may explain that while an estimated 30 million men in the United States – 10% of the male population – experience chronic ED, as few as 5% of men with chronic ED seek treatment.

 

Not every man can take Viagra, either. Men who use nitrate drugs, often used to control chest pain (also known as angina), must not take Viagra. This combination can cause their blood pressure to drop to an unsafe or life-threatening level. Men with serious liver and kidney problems who take Viagra must be monitored closely for possible serious side effects.

Almost all practitioners understand how difficult this problem is for men to discuss and are experienced in getting the information as quickly and as painlessly as possible.

 

Conclusion

No man has the power to stop the passage of time. But every man has the power to make aging more healthy and less harmful. Research conducted on men who live to be 100 and beyond, has determined that those who reach extreme old age do so by avoiding ill health, rather than by enduring it. As I like to remind my patients, “Age is not determined by years, but by function.” And it’s never too late for men to detour around the major illnesses of getting older. With good nutrition, healthy habits, and high quality nutritional supplements, the best years of a man’s life can absolutely and positively be those he spends in his 70s, 80s and even his 90s.

What are essential oils?

Essential Oils are the naturally occurring volatile oils obtained by distillation or expression having the characteristic aroma of the plant part from which it was derived. These 100% pure oils are neat, meaning they have not been processed or manipulated in any way with solvents or other additives. Though a particular species of plant harvested and distilled for its essential oil during a particular growing season in a specific region may produce a fragrance that differs from the same species grown that season in a different region, many of the main chemical markers and physical specifications may be similar.

Do essential oils have a grading system to tell me which is better?

There are many companies selling Essential Oils today spinning many tall tales regarding the quality or grade of their products. To my knowledge there exists NO official grading system in any of the more respected sources of essential oil literature. Neither The Federal Register and Code of Regulations, FEMA nor AFNOR has to date adopted a system that grades these oils as an A, B or C grade. By Nilesh Patel, NOW Quality Assurance, April 20, 2005 Why are FRUITS AND VEGETABLES important? “Diets rich in FRUITS AND VEGETABLES may reduce the risk of some types of cancer and other chronic diseases.”- National Cancer Institute. OXYGEN AND ANTIOXIDANTS As we all know, “Oxygen is critical to life,” but is itself a double-edged sword. While oxygen is necessary to sustain life and for natural defense against microbes, too much oxygen in our cells can lead to the production of “free radicals” (mitochondrial respiratory chain) or ROS (Reactive Oxygen Species). Free radicals come in many forms - singlet oxygen, hydrogen peroxide, superoxideperoxynitrite, to name a few - but all have one commonality. Each has an unpaired (unbalanced) electron, a situation it remedies by stealing an electron from a stable molecule. This sets off a domino effect of oxidation, a chain reaction that usually ends up damaging cellular integrity and compromising overall health. Nature has a defense system in place to protect these processes in the form of antioxidants. Whether endogenous (produced by the body, such as liver enzymes, SOD, coenzymes and sulfur-containing compounds) or exogenous (obtained through the diet, such as vitamins C & E, bioflavonoids, carotenes, etc.), antioxidants “quench” free radicals by donating an electron to stabilize a molecule, thus controling the chain reaction and stopping the oxidation “domino effect”. ANTIOXIDANT-RICH FOODS Research suggests that eating plenty of foods high in antioxidants helps to slow the processes associated with aging and protect against many chronic diseases. Maximizing one’s antioxidant power will enhance overall health. Fruit and vegetables contain both nutritive and non-nutritive factors that can affect oxidative damage and enzymatic defense and might contribute to redox (antioxidant and prooxidant) actions. A new “6-a-day” study looked into the effects of fruits and vegetables on markers of oxidative stress and antioxidative defense in healthy nonsmokers by The Danish Institute for Food and Veterinary Research in Denmark. The study found that fruits and vegetables increase erythrocyte glutathione peroxidase activity and resistance of plasma lipoproteins to oxidation more efficiently than do the nutritive factors (vitamins and minerals) that the fruits and vegetables are also known to contain. Certain berries, such as blackberries, also contain salicylates, which are also linked to heart health and prevention of atherosclerosis. The protective effects of fruits and vegetables intake on both heart disease death and deaths in general have previously been demonstrated but researchers at the Human Nutrition Research Center on Aging at Tufts University, Boston. Quercetin is an anti-oxidizing flavonoid found in many berries (such as cranberries, bilberries, blueberries, strawberries, etc.) and can prevent CVDs (coronary vascular diseases), according to a recent Finnish study. All these natural plant polyphenols are responsible for the colors of many red and purple berries, fruits, vegetables and flowers. GOVERNMENT GUIDELINES The new federal guidelines released earlier this year by the U.S. Department of Health and Human Services (HHS) and U.S. Department of Agriculture (USDA) recommend eating more fruits and vegetables, combined, than any other food group -- five cups or about 10 servings a day for most adults. The amount of fruits and vegetables recommended has increased for men and women of every age. “Fruits and vegetables are the "good news" story of the new Dietary Guidelines for Americans for food-loving consumers, the industry and America's public health”, stated the Produce for Better Health Foundation (PBH). Eating a variety of colorful phytochemical-rich fruits and vegetables has been associated with lower risk of some chronic diseases such as cancer and heart disease. Many authoritative organizations such as the National Cancer Institute and The American Heart Association recommend getting phytochemicals from whole foods, such as fruits and vegetables, rather than from individual component supplements. The Scottish government is promoting healthy eating through a scheme designed to increase purchasing of fruit and nutritional foods. Scottish health minister Andy Kerr said, "This initiative shows that healthy eating can be good for customers and good for business." Scottish women are said to have the highest rates of death from lung cancer in the world as well as the highest rates in Europe for coronary heart disease. They also have low consumption of fruits and vegetables, shown in studies to help protect against some cancers and benefit heart health. ROS (Reactive Oxygen Species) Free radicals and oxygen free radicals play an important role in the development and progression of many brain disorders such as brain injury, neurodegenerative disease, and Down syndrome. Oxidative stress is an important factor in the etiology and pathogenesis of diabetes & is also linked to other host of degenerative health conditions. Fortunately, antioxidants are available to support the body’s defense and fight disease and aging. Examples of “Fast acting antioxidants” in the body (serum) are: uric acid (polyphenols), ascorbate, bilirubin, vitamin E (the later two are lipid soluble). Examples of “Slow acting antioxidants” are glucose, urea nitrogen etc. In short, free radicals, reactive oxygen species (ROS) and reactive nitrogen species (RNS) are generated as by-products of normal cellular metabolism. Their deleterious effects are minimized in vivo (in the body) by the presence of antioxidant systems. How do Antioxidants work? Antioxidants are substances in plants that help maintain health. Antioxidants protect against damage to cells caused by too many “free oxygen radicals,” which form because of the effects of oxidation. Smoking, sunlight, heavy exercise, and pollution all increase oxidation in the body. Most people would benefit by eating more (five to nine or more servings) fruits and vegetables & colorful plant foods, such as purple, dark green, yellow, orange, blue, and red ones, each day. These have healthful pigments along with antioxidant nutrients such as vitamin C, carotenoids, beta-carotene, lycopene, lutein, zeaxanthin, vitamin E, selenium, flavonoids, and other beneficial substances. There are numerous ways in which these antioxidants affect, but can be explained in two groups: Alpha (a) Effects: This refers to the scavenging or neutralizing of free radicals. These effects do not change the way humans (or animals) feel. There are also no noticeable health, psychological or emotional benefits. While there are no obvious changes, increased total antioxidant intakes are associated with decreased tumor rates, prevention of heart attacks and increased longevity. Beta (ß) Effects: These are the changes on health, psychological or emotional state that you or others will notice. In this case, the antioxidant is affecting metabolic processes (enzymes) with consequent changes in the physical (improvement in joint movements, improved skin condition, tissue damage recovery), emotional (better ability to cope with stress) or psychological state (increased alertness). The ORAC value Because most of the active nutritional components in fruits and vegetables are antioxidants, accurate measurement of antioxidant activity serves as a good indicator of potential health benefit. Scientific opinion runs high that ORAC (Oxygen Radical Absorption Capacity) will eventually become a government standard of reference for overall daily fruits and vegetables intake. ORAC units are a measurement of the ability of food to stop oxidation. It is most generally expressed in terms of Trolox equivalent per gram (µmole Trolox equivalents (TE)/g).     Stevia has traditionally been used in either a powder or raw liquid form. Powdered forms can either be crude green or fine and white. Powders come in bulk or in tea bags. White stevia powder is the most common type and usually has more sweetening power than other forms. Countries like Japan use a filler substance along with stevia powder in order to give it more substance and make it easier to package. Powdered forms can be somewhat difficult to measure, although they are considered quite practical. Liquid formulas which are often brown in color frequently add other compounds to counteract bitterness. Alcohol based extracts are also available, as well as new concentrated liquid varieties. White stevia powder is the most popular form of the sweetener, although the leaf, ground or whole, can be purchased loose or in tea bags. Fresh leaves can be chewed but they are not practical for sweetening other foods. Dried leaves can used used for teas or in tea Blends. Stevia tablets are also available for those who want to use the herb as a therapeutic rather than sweetening agent. Ground stevia can be sprinkled over cereals, salads, and other ready-toserve foods.     Different Ginsengs

    What's the difference between Chinese (white root), Chinese (red root), Eleuthero and American Ginsengs? Which one is best for me? There are actually many different "ginsengs." We will discuss those mentioned above since they are the most widely available. All of these Ginsengs are considered to be potent adaptogens, which means that they are: 1) harmless to the body 2) non-specific in their actions 3) have balancing or normalizing effects. An adaptogen helps the body adapt to stress - both mental and physical. It is in this area that ginseng excels.

    Chinese Ginseng (Panax ginseng) is what most of us think of when Ginseng is mentioned. It is indigenous to the forests of northeast China, Manchuria and Korea. Red Ginseng is often referred to as "Korean" Ginseng. In traditional Chinese Medicine Ginseng is used to tonify the "Chi" (vital energy or life energy force). Modern scientific studies indicate Panax Ginseng stimulates the immune system, has antifatigue, antistress, antitumor, anticancer and anti-aging properties, balances blood sugar levels, enhances mental performance and memory, lowers cholesterol, strengthens the heart muscle and protects against radiation damage. Panax ginseng has had a notorious reputation as a sexual rejuvenator which studies give some credence to; albeit not to the degree of its reputation. Ginseng "overuse syndrome", although rare, is characterized by irritability, insomnia and rapid heart beat, and is associated with using too much Chinese Ginseng, especially by healthy, active men.

    American Ginseng (Panax quinquefolium) is indigenous to the eastern woodlands ranging from Georgia to Quebec and was used by many Native Americans. Jesuit Priests were reported to be trading American Ginseng to the Chinese as early as 1718. Ironically, American Ginseng is highly sought after in China, while Americans chase after Chinese ginseng. While having much the same adaptogenic qualities of Chinese Ginseng, American Ginseng is believed to have a more "yin" or cooler nature. What this means is that American Ginseng is excellent for the high-paced, stressed, not enough time culture that we live in. While still energizing the body, American Ginseng calms the central nervous system, quiets the brain and lowers blood pressure. Also, because of its more "yin" nature, it is generally better to use on a day-to-day, long term basis than Chinese Ginseng. American Ginseng is one of the best tonics for all-around health and vitality, particularly well suited for the hectic world we've created.

    Eleutherococcus senticosus (known as Siberian Ginseng in Herbs of Commerce) is native to Siberia, Japan, Korea and China. Although not a "true ginseng", this variety is most highly prized. Eleuthero was traditionally used to promote longevity and general health. Many herbalists prefer Eluthero for helping with women's health issues. It is particularly useful with depression associated with PMS and menopause.     CLINICAL APPLICATIONS OF CAPSICUM

    Capsicum is a remarkable whole body stimulant that can boost blood flow, tone the nervous system, relieve indigestion, promote sweating, help to cauterize and heal ulcers, ease persistent pain and fight off infection. One very authoritative work on African plants suggests that Capsicum’s “regular ingestion is highly beneficial in hemorrhoids, varicose veins, anorexia, liver congestion and vascular conditions . . .the indigenous inhabitants of Africa and of the Antilles are remarkably free form all of these conditions as they use Capsicum fruit in their diet.”10 Most of the therapeutic actions of Capsicum are attributed to the alkaloid or glucoside content of the herb.11 The latest scientific studies conducted with Capsicum will be discussed in subsequent sections.

    Herbal Catalyst

    Because Capsicum boosts peripheral circulation and stimulates organ secretion, it expedites the therapeutic delivery and action of other herbs. In fact, in cases where a heart attack is suspected administering capsicum in hot water has been thought to help lessen the severity of the attack. Capsicum can also be placed on or under the tongue in emergencies involving heart attack, stroke or hemorrhaging. 18 Note: Using Capsicum for any heart-related problem, especially a suspected heart attack should never take the place of medical attention or a physician’s care.

    CAPSICUM Blood Cholesterol Reducer

    Various studies have conclusively demonstrated that Capsicum reduces the risk of developing atherosclerosis (hardening of the a rteries) by reducing blood cholesterol and triglyceride levels .19 Additional clinical studies conducted in India found that when cayenne was ingested along with dietary cholesterol, the typical rise in liver and blood serum cholesterol levels was significantly inhibited. In addition, bile acids and free cholesterol were subsequently eliminated from the body through the stool.20 Interestingly, these tests revealed that using Capsicum was actually more effective in reducing cholesterol that capsaicin alone.2 1 Daniel Mowrey, Ph.D., emphatically points out that this is just one of many examples of the superiority of whole botanicals as opposed to their isolated components.22 Note: Using Capsicum in combination with Hawthorn is a particularly good cardiovascular tonic.

    Blood Pressure Equalizer

    While an added bonus of Capsicum’s capability to lower blood serum cholesterol is a decrease in blood pressure, additional evidence strongly suggests that the herb initiates other mechanisms that fight hypertension .23 “Cayenne, according to another study, also reduces the blood pressure in an even more direct manner: a number of years ago, a team of researchers discove red that capsaicin acts in a reflexive manner to reduce systemic blood pressure, a kind of coronary chemoreflex.”24 Adding Garlic to Capsicum creates an even better therapeutic blend for treating hypertension.

    Blood Detoxification CAPSICUM

    “Cayenne is a kind of catalyst in the blood purification process . . . it acts as a diaphoretic, stimulating the excretion of wastes in the swe a t . ”25 Because Capsicum stimulates organ secretion and boosts peripheral blood flow, it would only stand to reason that it would also facilitate the faster removal of toxins from the bloodstream and lymphatic system. You may have already noticed that Capsicum is frequently added to blood-purifying herbal combinations. Circulatory Booster Researchers have found that the simulating action of Capsicum on surface capillaries can help to pre vent cold hands and feet.2 6 For this reason, it may be helpful for Reynaud’s Syndrome. Old remedies using Capsicum have even recommended placing it in socks to warm the feet and to help prevent frostbite. An old folk cure for a chilled body was a steaming hot cup of Capsicum tea. Free Radical Scavenger The rich flavonoid content of Capsicum gives it significant antioxidant capabilities. A recent study conducted in 1995 showed that Capsicum has a higher ascorbic acid content than chiles from the jalapeno or serrano varieties .27 Vitamin C and bioflavonoids can scavenge for dangerous free radicals which cause tissue damage and can predispose organs to degenerative diseases. Free radicals are found everywhere and are created as by-products of metabolic p rocesses including the act of breathing itself. Pollutants can expose the body to free radicals. An interesting study done in Mexico City and published in 1993 found that Capsicum extract was able to modulate the mutagenic activity of urban air samples.28 In other words, these potentially dangerous nitro - a romatic compounds found in polluted air were kept from mutating by red chile extract.29 Chemical breakdowns of Capsicum have also found that CAPSICUM the pepper is high in Provitamin A, which significantly contributes to its healing ability and immune fortification.30 Anti-Carcinogenic Compound Anti-cancer research recently tested Capsicum on laboratory rats and found that it does indeed demonstrate anti-cancer properties by inhibiting certain enzymes which can initiate the mutation of cells.31 What this implies is that taking Capsicum can afford the body some protection against the cellular mutation which occurs in malignant growths. Capsicum actually inhibited the formation of dangerous metabolites under laboratory conditions where they should have normally been activa t e d .3 2 This study implies that Capsicum may have many more sophisticated bio-chemical actions than previously thought.

    An Impressive Pain Killer

    Capsaicin has recently emerged as a remarkably effective pain reliever and has become the subject of recent clinical research . Applying capsaicin in cream or ointment form to painful joints, scar tissue or other painful conditions involving peripheral nerves confuses pain transmitters. In other worlds, capsaicin temporarily disrupts sensory nerve cell biochemistry there by impeding the relay of pain sensations from the skin surface. It does this by inhibiting a neurotransmitter called substance P. This specific compound is thought to be the main mediator of pain impulses from peripheral nerve endings.33 Substance P has also demonstrated its ability to inhibit inflammatory pain generated in arthritic joints in much the same way.34 Today, several over-the-counter topical preparations utilize capsaicin for the pain of arthritic joints. The ability of Capsicum to control severe and unresponsive pain is significant, to say the least. Modern clinical utilization of topical capsaicin may offer signifi-cant relief for a number of painful conditions including: diabetic neuropathy, cluster headaches, post-amputation pain, post-mastectomy pain, shingles and painful scar tissue.35

    POST-SURGICAL PAIN

    In the early spring of 1996, prime time national news show s reported that scientists had found that individuals who had suffered from chronic pain in post-surgical scars (heart bypass, arterial grafts, etc.) were successfully treated with topical preparations containing capsaicin. While this may have been news to many of us, clinical studies had been already published for several years that capsaicin held profound value for various kinds of pain which did not respond to established medical treatments. Typically surgical scars and regions around them can produce persistent pain or can be very sensitive to the touch even when completely healed. This type of pain phenomenon seems to respond well to capsaicin ointments and creams.

    POST-MASTECTOMY PAIN

    When capsaicin preparations were applied following mastectomy or breast reconstruction, pain was significantly relieved. Se veral double blind studies found that using capsaicin creams four times daily for 4 to 6 weeks resulted in much less frequent occurrence of sharp, jabbing pain.3 6 All thirteen patients studied had a 50 percent or greater improve m e n t .3 7 Various unpleasant sensations other than pain also improved with topical applications of capsaicin creams.38

    MOUTH SORES FROM RADIATION OR CHEMOTHERAPY

    A fascinating study conducted at the Yale Pain Management Center discove red that capsaicin could ve ry significantly lessen pain caused by mouth sores which frequently develop after chemotherapy or radiation.39 Apparently delivering the capsaicin in the form of soft candy (taffy) enabled the substance to be retained in the mouth long enough to desensitize the nerve endings causing the pain. Each one of the eleven case studies re p o rted that their pain had decreased and in two patients, it stopped entirely.40

    DIABETIC NEUROPATHY

    Diabetic neuropathy is a painful nerve condition which can develop in cases of prolonged diabetes. Several double-blind studies have supported the considerable value of capsaicin creams for relieving the pain associated with this disorder.41 The results of a controlled study using Capsicum for seve re cases of diabetic neuropathy which did not respond to conventional therapy were published in 1992. A cream containing Capsicum was applied to painful areas four time a day and pain was carefully e valuated for 8 weeks at two-week intervals. The results we re impressive, to say the least. In the 22 patients who used the Capsicum the following results we re re c o rded: “Capsaicin tre a tment was more beneficial than vehicle treatment in the overall clinical improvement of pain status, as measured by physician’s global evaluation and by a categorical pain severity scale . . . In a follow-up study, approximately 50 percent of the subjects reported improved pain control or were cured . . .”42 No t e : While there was a burning sensation when the Capsicum c ream was first applied, some subjects found that its magnitude and duration lessened with continued application.43

    SHINGLES

    The FDA has approved capsaicin-based ointments for the treatment of pain that results from diseases like shingles. Again, numerous studies have documented the value of capsaicin for decreasing the miserable nerve-related pain associated with shingles. The general consensus derived from these tests were that approximately 50 p e rcent of people suffering from shingles responded well to capsaicin creams, some even after 10 to 12 months.44

    Note: If blisters accompany a shingles outbreak, it is better to wait until they have healed before using any capsaicin-based ointments or creams.

    RELIEF FOR BURNING FEET

    Frequently an uncomfortable “burning” sensation in the feet will occur in many people, particularly in diabetics. As ironic as it may seem, using capsaicin creams may actually alleviate this burning. “In various studies, diabetics who treated their burning feet with capsaicin got greater improvement and we re able to walk more easily than those not using the cream.”45 In addition, using topical applications of capsaicin as opposed to strong, oral drugs is much more preferable.

    ARTHRITIS PAIN

    Clinical tests have confirmed that topical capsaicin ointments substantially alleviate the miserable pain that characterizes osteoand rheumatoid arthritis.46 These studies revealed that using 0.075 capsaicin cream reduced tenderness and pain.47 Dr. Michael T. Murray writes: “ . . . seventy patients with osteoarthritis and thirty - one with rheumatoid arthritis received capsaicin or placebo for 4 weeks. The patients were instructed to apply 0.025 percent capsaicin cream or its placebo to painful knees four times daily. Significantly more relief of pain was reported by the capsaicin-treated patients than by the placebo patients throughout the study . . .”48 Anyone suffering from osteo or rheumatoid arthritis should evaluate the effectiveness of capsaicin ointments for joint pain. Ester Lipstein-Kresch, M.D., has studied the effectiveness of capsaicin creams for arthritis and has stated: “You need to apply it three or four times a day on the affected area for at least two weeks before you’ll see any improvement. An initial burning sensation at the site is not unusual for the first few days, but this goes away with continued application.”49 Note: Capsaicin is also useful for tennis elbow due to its ability to block the transmission of pain.

    MIGRAINE HEADACHES (CLUSTER TYPE)

    Topical applications of capsaicin ointments intranasally may also help to relieve the pain of a specific kind of migraine headache called cluster headaches. Cluster headaches are characterized by s e ve re pain which typically radiates around one eye. The term “cluster” refers to the fact that these headaches tend to occur in clusters of one to three per day and can recur at intervals. Headache pain and severity we re reducing in groups using intranasal capsaicin.5 0 This type of capsaicin treatment should be done under a physician’s care. There is some speculation that capsaicin may be more effective in pre venting migraines before they develop into a full blown attack.51

    Known to the natives of the tropical Americas for millennia, Capsicum, or Cayenne Pepper, was introduced to Europe by Christopher Columbus and became known as “Guinea Pepper. ” Originally used by Native Americans located south of the Mexican border, archeological evidence supports its cultivation from 7000 B.C. Apparently, mixing chocolate and red chiles was a taste treat exclusively reserved for Aztec royalty.5 The exact origin of the word Capsicum remains somewhat of a mystery. However, it is assumed to be a derivative of the Greek word kapto, meaning “to bite,” an appropriate reference to its fiery pods. Capsicum is the fruit of a shrub-like tropical plant and is technically considered a berry. Its designation as a “pepper” can be traced back to Columbus, who equated its hot taste sensation with that of black pepper.

    In 1597, Gerard referred to Capsicum as extremely hot and dry and prescribed it for throat and skin infections. Health practitioners of the nineteenth century called phsysiomedicalists used Capsicum to counteract rheumatism, arthritis, depression and chills. In the early 1800s, Dr. Samuel Thompson utilize d Capsicum as a potent and safe natural stimulant. His followe r s , who would become known as Thomsonians, believed that Capsicum should be used to treat a wide variety of diseases. It was used orally and as a poultice to treat tumors, toothaches, feve r s , and respiratory ailments.

    In 1804, Dr. John St e vens introduced the red pepper to England where it became the catalyst component in a variety of herbal Blends. Subsequently, herbal and medical practitioners used Capsicum to fight infection and sustain the natural heat of the body. It became well known in American dispensatories and pharmacopeia. In 1943, The Dispensary of the United States recorded that, “Capsicum is a powe rful local stimulant, producing when CAPSICUM swallowed, a sense of heat in the stomach and a general glow over the body without narcotic effect.”6 Twentieth-century physicians recognized the medicinal value of Capsicum which eventually found its way to the American Illust rated Medical Dictionary, the Merck Manual and Materia Medica, where it was referred to as a rubefacient, local stimulant, counter-irritant, gastric stimulant, and diaphoretic.7

    Today Mexican Indians continue to use Capsicum as an internal disinfectant and protectant against contaminated food and also to treat fevers.8 “Today the pepper is nowhere in the world more appreciated and more widely used than in Mexico and certain other Latin American countries, which together form the original home of all the peppers. Both at morning and at evening, practically eve ry dish the Indians eat included Capsicum, just as their food did 2,000 years ago. The diet of the Indians was, and still is, rather bland . . . maize, beans, squash, pumpkin, yucca, potatoes . . . little wonder that the pepper was so highly regarded. And of course . . . the peppers were a wonderful source of essential vitamins in a diet otherwise lacking in them.”9 Capsicum continues to be a source of vitality and health in numerous countries including the Bahamas and Costa Rica, where it is used to overcome colic or indigestion, in Africa for vascular disorders and by North Americans who use it as a tonic and natural stimulant.

    Capsicum is currently experiencing a renaissance in that a number of recent studies have emerged adding to its already impressive list of actions. Scientists are taking notice and looking at Capsicum with new respect and interest.     Life can feel like an emotional rollercoaster, with the high-stress jitters following the low-mood blues. But aromatherapy-the healing power of scent-can restore equilibrium.

    The use of volatile plant oils, including essential oils, for psychological and physical well-being dates back thousands of years. The ancient Chinese, Egyptians, Greeks and Romans used infused oils and herbal preparations for medicinal, fragrant, cosmetic, even spiritual reasons.

    During the late 20th century, people started to relearn the benefits of aromatherapy and these days, aromatherapy's reputation as a soothing, healing art continues to grow. Once you've experienced the odiferous power of aromatherapy's essential oils, you'll keep coming back for more: These gently wafting odors have the power to stimulate or calm, invigorate or relax.

    When you enter this scented world, "there you will find nature in one of its most powerful forms-aromatic liquid substances known as 'essential oils,'" says Valerie Ann Worwood in The Complete Book of Essential Oils and Aromatherapy (Thorsons). Essential oils form what Worwood refers to as the "fragrant pharmacy," a collection of concentrated substances used in pharmaceuticals, foods and cosmetics.

    When you sniff the aromas of essential oils, "they enter and leave the body with great efficiency, leaving no toxins behind," Worwood points out. "The most effective way to use essential oils is...by external application or inhalation. The methods used include body oils, compresses, cosmetic lotions, baths-including sitz, hand and foot baths-hair rinses...perfumes...and a whole range of room [scenting] methods."

    Plant Essences

    As Worwood explains, essential oils are produced in various parts of different plants. As a result, it takes a great deal of specialized work to extract essential oils. About 60,000 rose blossoms are consumed in the production of an ounce (!) of rose oil.

    Just as the antioxidant phytonutrients we eat in vegetarian foods link our bodies to the health-promoting chemistry of plants, the penetrating nature of essential oils are thought to connect our souls to the essences of flora. "From inside comes the voice and from inside comes the scent," observed the 19th century German doctor Gustav Fechner, quoted by Robert Tisserand in The Art of Aromatherapy (Healing Arts Press). "Just as one can tell human beings in the dark from the tone of voice, so, in the dark, every flower can be recognized by its scent. Each carries the soul of its progenitor."

    Fechner believed that the power of essential oils to stir our deepest emotions derives from their function as a vital means of communication in the plant world. As Tisserand asks, can't we imagine that flowers "communicate with each other by the very perfumes they exude, becoming aware of each other's presence?"

    The Science Behind the Scent

    While alternative medical practitioners have acknowledged the effectiveness of aromatherapy for thousands of years, only recently have conventional medical researchers begun seriously looking into how this technique works.

    For instance, a study of estragole, a chemical found in basil, fennel and tarragon, determined that it could potentially ease back pain by inhibiting inflammation of the sciatic nerve. (The sciatic nerve, the longest nerve in the body, runs from the back down the leg.) The researchers discovered that estragole is "active on nerves," a conclusion that aromatherapy practitioners, who employ the scent of these oils to soothe pain, already knew. Science is verifying another piece of information long known to practitioners-that while certain essential oils can calm you down, others prod your alertness. In a study performed at the University of Northumbria in England, scientists found that sniffing the scent of lavender lulls the human brain into a comfortable, rather stupefied state, while rosemary, in contrast, can sharpen recall.

    As the English researchers noted, lavender "produced a significant decrement in performance of working memory, and impaired reaction times for both memory and attention-based tasks." That's probably why the odor of lavender is noted for enhancing sleep. On the other hand, the scientists found that rosemary "produced a significant enhancement of performance for overall quality of memory and secondary memory factors." However, they did point out that under the influence of both of these oils, performance slowed when tackling a battery of memory tests. Apparently, the oils mellowed people so that they had little motivation to rush through the paperwork.

    As Frazesca Watson notes in Aromatherapy Blends & Remedies (Thorsons): "The aroma of the oils directly affects our moods and emotions and sometimes our short- and long-term memory. Together with a wide range of physiological benefits, the aroma can help with emotional upsets such as depression, anxiety, nervous tension, anger, apathy, confusion, indecision, fear, grief, hypersensitivity, impatience, irritability, panic and hysteria."

    Essential oils are especially helpful at defusing stress. Watson notes, "Treatments with essential oils are therefore very helpful for all sorts of stress-related problems, so common in our modern life."

    As scientific research into the effects of these oils continue, conventional medical practitioners are sure to embrace them in increasing numbers. But before there were scientists around to confirm the effects of these wonderful scents, the ancient medical practitioners in Egypt and Greece attributed the origins of aromatherapy to the gods. For many people in today's overstressed world, the relaxing assurance of essential oils certainly seems heaven-s(c)ent.

    But aromatherapy is more than just a '90s-style novelty. The practice of using aromatic essential oils for psychological and physical well-being dates back more than 4,000 years to medicinal practices in Egypt and India.

    The term "aromatherapy" was coined in 1937 by French cosmetic chemist R.M. Gattefosse, who discovered the benefits of essential oil after burning his hand in a laboratory accident. Gattefosse immersed his hand into the nearest available cool liquid: a vat of lavender oil. The near miraculous soothing of his pain and rapid healing spurred him to dedicate his life to the study of aromatic plants and their therapeutic effects.

    How it Works

    For those who turn their noses up at this most seemingly-subtle of senses, keep in mind that the perception of smell is 10,000 times more sensitive than the sense of taste. "The sense of smell is the sense of the imagination," noted French philosopher Jean-Jacques Rousseau; this emotional connection lies at the heart of aromatherapy.

    Aromas are transmitted rapidly from olfactory cells in the nose to the limbic system in the brain which perceives and responds to emotion, pleasure and memory. Scents trigger the limbic system to release neurochemicals which influence mood. Well-known neurochemicals like endorphins and serotonin help create a sense of well-being.

    When you inhale essential oils, some of the molecules travel to the lungs, where they proceed to enter the bloodstream and circulate throughout the body.

    Oils applied to the skin are absorbed into the bloodstream as well. Because they are oil/fat soluble, essential oils are highly absorbed by the body, where they circulate for anywhere from 20 minutes to 24 hours and are eventually eliminated through sweat and other bodily secretions.

    Plant Power

    Essential oils are extremely potent and volatile: approximately 75 to 100 times more concentrated than dried herbs.

    Most essential oils are steam distilled from herbs, flowers and plants. Others are cold expressed from the rind of the fruit, which produces the purest essential oils because no heat or chemical treatment is involved.

    The components of various oils are beneficial for a wide variety of beauty and hygiene conditions. Some of the more indispensable essential oils include:

    Chamomile (anthemis nobilis): soothing properties for sensitive and inflamed skin; calming, balancing and relaxing.

    Clary Sage (salvia sclarea): warming, female balancing herb used for PMS; calms anxiety, tension and stress; also used as a muscle relaxant for aches and pains.

    Eucalyptus (eucalyptus globulus): antibacterial; fresh, herbal menthol aroma; widely used as an inhalant for colds, coughs and congestion; excellent for massaging tired or sore muscles.

    Geranium (pelargonium graveolens): one of the best all-around tonic oils for mind and body; soothes nervous tension and mood swings; balances female hormones and PMS; gently astringent and antiseptic, it improves general tone and texture of skin.

    Jasmine (jasminum grandiflorum): a warm, rich, sensual floral scent used historically as an aphrodisiac; moisturizing for dry/mature skin.

    Lemon (citrus limonum): refreshing and invigorating; eases tension and depression; useful for oily skin and treatment of acne.

    Peppermint (mentha piperita): cool, menthol, invigorating stimulant; cleans and purifies the skin.

    Rosemary (rosmarinus officinalis): stimulating and uplifting; purifying and cleansing for all skin types; warm and penetrating for massage to ease muscular aches and pains.

    Tea Tree (melaleuca alternifolia): an antiseptic from the leaves of the Australian tea tree; antifungal, antibacterial and antiviral; excellent for skin irritations like cold sores, insect bites and acne.

    Ylang Ylang (cananga odorata): enticing and sensual; helps alleviate anger, stress, insomnia and hypertension; helps balance the skin's sebaceous secretions.

    Essential oils can be utilized in a variety of ways: in electric or candle-based diffusers, to spread the aroma through a room; in sachets and air fresheners; added to shampoos and lotions; or diluted and applied to pulse points like the temples, on neck or on wrists. Undiluted essential oils should never be applied to the skin. First mix them with carrier oils: pure vegetable oils such as sweet almond oil, grapeseed oil and apricot kernel oil. Use a general guideline of six to 18 drops of essential oil per one ounce of vegetable oil. Blended, diluted oils are also available which can be used directly on your skin.
