Recent research has shed new light on dietary factors that can help to promote acne symptoms, if not being the sole cause of them. Before discussing that, it is important to understand why acne gives rise to the symptoms that it does: lesions in the form of whiteheads and blackheads, pustules and cysts. While not necessary to discuss the biochemical details, the part that your diet has to play will not be understood without considering the effect of hormones on acne. The reason that teenagers in particular appear to be more prone to acne is that an increased production of hormones has an effect on the condition that causes the symptoms of acne. Fundamentally, acne is characterized by the infection and inflammation of a mass of oil and dead skin cells within the pores of the skin, particularly on the face, neck, chest buttocks and back. If we discuss each of these elements first, and how they are created, then the relationship between diet and acne will become clear. At a certain time in their lives, people experience a spurt of growth and develop sexually. This is initiated by the secretion of hormones, particularly of male Sex Hormones collectively known as androgens, and by various hormonal 'Growth Factors'. This stage of human development is known as puberty, although there is also an increase in androgen secretion by the adrenal glands just before menstruation and during pregnancy and menopause. Androgens such as testosterone are reserved not only for the male of the species! An effect of androgens is to increase the rate of secretion of sebum from the sebaceous glands in the skin. The reason for this is unknown, though it has been hypothesized that its purpose is to waterproof the additional hair that is grown on the body at this time. Another suggested reason is as an olfactory warning to others to deter from sexual activity, in teenagers until their sexual development is complete, and in pre-menstrual, pregnant and menopausal women for obvious reasons. There is no substantial proof for any of these hypotheses, though the latter appears to make more sense than the former. Irrespective of this, androgens also interfere with desquamation, and the dead skin cells within the pores tend to fall off irregularly and in clumps. This mix of dead skin cells and excess sebum clogs up the pores of the sebaceous follicles. Once this plug becomes infected with bacteria, the immune system is activated, inflammation occurs, and leukocyte action leads to pus formation. That is what is known as acne. In order to determine how diet and acne are connected, it would be necessary to determine what components of our diet can either stimulate sebum production, or stimulate androgen secretion. If no such link could be found, then it would be fair to descry any connection between acne and the food you eat. However, there is a connection, and it is a positive one. In addition to their main function, insulin and a hormone known as IGF-1 (Insulin-like Growth Factor 1 that helps promote growth in children) promote the secretion of testosterone, a male hormone or androgen. Knowing, as we now do, that androgens promote the secretion of sebum, then anything that increases the levels of insulin or IGF-1 within the body will also lead to sebum production and hence to acne. What that infers is that any foodstuffs that increase the insulin levels in the blood can also lead to acne. This inference is supported in many ways. For example, it has been found that while drinking milk promotes a greater risk of acne, eating yoghurt does not. Why is this? It is known that milk can increase insulin levels because of its high sugar content. The effect of bacterial activity to produce yoghurt reduces the amount of sugars in the milk it is made from because the bacteria live on the lactose. The same argument applies to cheese, which promotes lower insulin levels than milk, if not as low as yoghurt. This being the case, then a diet low in sugars and carbohydrates should reduce the incidence of acne generally. Recent research has indicated that a diet high in refined carbohydrates and sugar increases both IGF-1 and insulin levels in the blood. This then creates a surge in male hormones which in turn leads to excessive sebum secretion and intermittent shedding of skin cells and so on to the growth of bacteria and acne. It is a logical progression, supported both by theory and by observation. So how should a person with a propensity for acne change their diet? Switch to fruits, vegetables and grains. Non-fatty meats are also acceptable, and .lots of fish and other seafood. Studies have concluded that diets rich in seafood lead to very low acne rates. The Japanese and coastal Chinese suffer very little acne in comparison with those taking a Western diet, particularly an American diet. One of the reasons for this is that omega-3 fatty acids help to reduce both inflammation and sebum production. The same is true of green tea that contains antioxidants that reduce the blood levels of dihydotetosterone and hence of sebum secretion by the sebaceous glands. We could go on, and list supplement after supplement that contain antioxidants and other substances that can reduce the production of sebum and hence of the symptoms of acne. Vegetable oils, on the other hand, with their high omega-6 fatty acid content, can drive up sebum production and the activity of the immune system and the inflammatory response. It can also lead to uterine, ovarian and breast cancers, and cervical dysplasia which, while not cancer, can give rise to cervical cancer if not treated. Estrogen imbalance, therefore, is not good news and something to be avoided if it possibly can. So what can be done to avoid it? Dioscorea villosa, commonly known as wild yam, is a tuberous vine native to North America and parts of Central America. It was used by the Mayan and Aztec civilizations for pain relief and birth control and has also been given the names colic root and rheumatism root, demonstrating this early use of the plant. Progesterone levels continue to rise for about 8 days, and then if fertilization has not occurred, the progesterone levels trigger menstruation, and the lining of the womb is detached and expelled. During certain phases of the menstrual cycle, the levels of the hormones estrogen and progesterone drop off, although the exact reason for the many and varied symptoms of pre-menstrual syndrome (PMS) are still unknown to a large extend. There appear to be several contributing factors, though estrogen and progesterone certainly appear to be two of the major ones. We shall concentrate here on the progesterone factor because that is what wild yam is claimed to address. Although the main function of progesterone is to generate estrogen and testosterone, to restore the libido after menstruation, and maintain the integrity of the lining of the uterus and aid in gestation, it has many other purposes such as in thermogenesis (burning of body fats), protection against osteoporosis and is also believed to have a natural anti-depressant action. These are all supported by the symptoms that appear when the body is low in progesterone levels: depression, sudden increase in body fat, mood swings, loss of sex drive and a susceptibility to weak bones if the deficiency is long-term. In other words, women that usually suffer from PMS, also generally suffer low progesterone levels. It is believed in some quarters that a progesterone deficiency causes an excess of estrogen that leads to these symptoms in addition to heart disease. No one really knows for sure, but a deficiency of progesterone seems to be the determining factor. Women with menstrual problems continue to be issued with prescribed synthetic progesterone even though they do not have the same effect as natural progesterone. They can also lead to some unpleasant side effects such as fluid retention, strokes, jaundice, blood clots and depression: some of the symptoms it is intended to alleviate. Growing Older, Feeling Better Not long ago, when a man turned sixty-five, he became officially old – the best years of his life far behind him. The milestone meant his working days were done and if he was lucky, he might get four or five years to spend as he wished before illness and infirmity set in. It was simply expected and accepted that the older a man got, the sicker he got. Well, not anymore. Today, a man age 65 is just as likely to be found hiking in the hills, running in a marathon, or even dancing in the streets than rocking in that proverbial front porch rocker. Because it’s becoming more and more evident that the older a man gets, the healthier that man has been. Eating healthy, exercising, and kicking harmful habits (like smoking) can add years to a man’s life. Aging research is proving over and over again, that we can prevent and delay heart disease, cancer, diabetes, and Alzheimer’s disease – the major causes of disability and death in men over 50. Now, it’s very true that good clean living from early on is preferable to sixty five years of bad habits and five years of good. But it’s also true that it’s never too late for men to make changes and vow to take better care of themselves. And one of the easiest and most effective ways men can improve their health is the addition of high quality nutritional supplements. In this issue of Ask the Doctor, we’ll talk about specific dietary supplements that have been scientifically shown to improve the health of men over fifty, prevent the diseases that often strike at this crucial time in men’s lives, and actually slow the aging process. Q. I just turned 50 and I’d like to begin taking nutritional supplements, but they seem so confusing. Where should I begin? A. Many men feel the same way. There are literally hundreds and hundreds of nutritional supplements on health food store shelves. Figuring out which supplements provide the best health benefits for a 50+ man can be overwhelming.
The best foundation supplement is a high quality multivitamin. Research is repeatedly finding that even very healthy men who take daily multivitamins can significantly improve their health. In fact, an article published in the Journal of the American Medical Association (JAMA) recommends that all adult Americans take a vitamin supplement. Look for solid doses of vitamin supplement. Look for sol doses of vitamins and especially minerals. Multivitamins designed to be taken once a day are often woefully deficient in calcium, magnesium, and potassium. The only mineral a man should avoid is supplemental iron. Iron should only be in formulas for women prior to menopause. Men over 50 get all the iron they need from food and too much iron can cause health problems. Look for men’s multivitamins that contain lycopene in the formula. Lycopene is the pigment that makes tomatoes red. The redder the tomato, the more lycopene is present. Numerous studies have shown that when men have high lycopene levels in their blood, they have a much lower risk of heart disease, age-related macular degeneration (a leading cause of vision loss) and prostate cancer. Other important considerations are antioxidant blends, especially fruit- and tea-derived extracts; ginseng for energy and stamina; and digestive enzymes to aid in absorption and compensate for age-related decreased enzyme levels. In fact, years of research has shown the foods a man chooses to eat (or not to eat) can have a profound impact on the health of his prostate gland. Because of this close nutritional link, prostate cancer may be the most preventable type of non-smoking related cancers. Q. Aside from taking a quality multivitamin for general health, what nutritional supplements prevent and treat prostate cancer? A. Calcium D-Glucarate When men are exposed to excess levels of hormones, their risk of prostate cancer increases. What exactly happens to men’s hormones as they get older? A. Just as women experience significant hormonal changes as they age, so do men. In fact, the term andropause has been used to describe men’s mid-life changes. Similar to menopause in women (where the decline of estrogen causes a myriad of symptoms), andropause in men signals the slow decline of testosterone, the chief Sex Hormone in men. While estrogen levels decline faster and more abruptly in women than testosterone levels do in men, testosterone decline can cause a number of unpleasant symptoms. The prostate gland is about the size of a walnut and is located below the bladder and in front of the rectum. It wraps around the upper part of the urethra and its primary job is the production and storage of semen, the milky fluid that nourishes sperm. BPH is one of the most common health conditions in older men. Half of all men aged 40-60 and more than 90 percent in men over 80 have BPH. BPH causes the prostate gland to enlarge, putting pressure on the urethra. Men have trouble starting or maintaining a stream of urine, find they can’t completely empty their bladders, and have to urinate frequently, even during the night. They may also have episodes of uncontrollable dribbling or complete loss of urine. BPH is caused by the conversion of estrogen to a very potent form of testosterone called, dihydrotestosterone (or DHT). When prostate cells are exposed to DHT, they multiply in number and get much larger. BPH rarely improves. It most often remains the same for years or gets gradually worse. The need to continually urinate, interrupted sleep, dribbling, and loss of urine can significantly interfere with a man’s quality of life. Prescription medications that have been developed to treat BPH are only partially effective. And surgical removal of the prostate gland may result in even more persistent urinary incontinence and the inability to achieve an erection (ED). However, saw palmetto berry extract relieves the symptoms of BPH by inhibiting the production of DHT. And, in study after study after study, saw palmetto caused none of the side effects that happen with prostate surgery or medications. Q. There seem to be plenty of ads for supplements that claim they make men into Sexual Superheroes. Is there an “honest” nutritional supplement to help me sexually? A. That’s a very good observation. And yes, there are honest nutritional supplements for men’s sexual health. Sexual intimacy is an important, complex, and lifelong need. It makes us feel better physically and mentally and adds to our sense of security, belonging, and self-esteem. But just like other changes that happen to men as they get older, men’s sexual response most often changes, too. Declining testosterone levels, changes in blood flow to the penis, certain medications that older men are prescribed, and the presence of diabetes or heart disease can all affect men’s ability to engage in sexual activity. When men have a chronic inability in obtaining and/or maintaining an erection, it’s called erectile dysfunction (ED). While ED is not an inevitable part of getting older, it does occur more frequently as men age. About 5% of 40-year-old men have ED, but more than 23% of 65-year-old men have difficulty maintaining erections. The development of prescription medication Viagra (sildenafil citrate) has revolutionized ED treatment. When a man is sexually stimulated, Viagra helps the penis fill with enough blood to cause an erection. Like all medicines, Viagra can cause some side effects, including headache, flushing of the face, and upset stomach. But because Viagra is a prescription medication, it requires a visit to a licensed healthcare practitioner. For many men, telling anyone (even a professional) that they are having trouble getting or keeping an erection is simply too embarrassing. Viagra is also fairly expensive and many older men do not have prescription drug health insurance. These reasons may explain that while an estimated 30 million men in the United States – 10% of the male population – experience chronic ED, as few as 5% of men with chronic ED seek treatment. Not every man can take Viagra, either. Men who use nitrate drugs, often used to control chest pain (also known as angina), must not take Viagra. This combination can cause their blood pressure to drop to an unsafe or life-threatening level. Men with serious liver and kidney problems who take Viagra must be monitored closely for possible serious side effects. The good news is there is a nutritional supplement that’s formulated with vitamins, herbs, and glandular products that targets male sexual organs. As I like to remind my patients, “Age is not determined by years, but by function.” And it’s never too late for men to detour around the major illnesses of getting older. With good nutrition, healthy habits, and high quality nutritional supplements, the best years of a man’s life can absolutely and positively be those he spends in his 70s, 80s and even his 90s.

Lowering cholesterol safely. By Kim Vanderlinden, N.D., D.T.C.M. Atherosclerosis and its complications are major causes of death in the United States and have reached epidemic proportions throughout all of the Western world. Heart disease accounts for 36% of all deaths among Americans and ranks as the number-one killer; stroke; another complication of atherosclerosis; is the third most common cause of death. Foremost in the prevention and treatment of heart disease is the reduction of blood cholesterol levels. The evidence overwhelmingly demonstrates that elevated cholesterol levels greatly increase the risk of death due to heart disease. The first step in reducing risk for heart disease is keeping your total blood cholesterol level below 200 mg/dl (milligrams per deciliter). Not all cholesterol is bad; it serves many functions in the body, including the manufacture of Sex Hormones and bile acids. Without cholesterol, many body processed would not function properly. Cholesterol is transported in the blood by molecules known as lipoproteins. Cholesterol bound to low density lipoprotein, or LDL, is often referred to as the “bad” cholesterol, while cholesterol bound to high-density lipoprotein, or HDL, is referred to as the “good” cholesterol. LDL cholesterol increases the risk of heart disease, strokes, and high blood pressure, while HDL cholesterol actually protects against heart disease. LDL transports cholesterol to the tissues. HDL, on the other hand, transports cholesterol to the liver for metabolism and excretion from the body. Therefore, the HDL-to-LDL ratio largely determines whether cholesterol is being deposited into tissues or broken down and excreted. The risk for heart disease can be reduced dramatically by lowering LDL cholesterol while simultaneously raising HDL cholesterol levels. Research has shown that for every one percent increase in HDL levels, the risk for a heart attack drops three to four percent. Dietary cholesterol Dietary cholesterol is a major risk factor in developing atherosclerosis. The evidence is substantial. However, several studies have shown that a lower dietary cholesterol intake was associated with up to a 37% lower risk of death from any cause, or an increased life expectancy of roughly 3.4 years. Although dietary cholesterol intake is an important contributor to atherosclerosis, most of the cholesterol in the body is actually manufactured in the liver. Reducing dietary cholesterol alone is not always sufficient to lower blood cholesterol levels. Common drugs In an attempt to reduce blood cholesterol levels, many physicians are ignoring the need to give dietary recommendations and are instead utilizing drugs as the primary treatment. Using drugs before diet is clearly not the best approach, in terms of both effectiveness and cost. Women and Depression by Lisa James Energy Times, March 11, 2004 Just as fog veils a beautiful landscape, so depression veils life itself: rendering existence dark and dreary, narrowing the scope of one's dreams. And women are particularly prone to this lingering sadness. The good news: Depression doesn't have to linger forever. With proper nutrition, lifestyle changes and a revived outlook, you can break through that fog into a sunnier emotional clime. Women are more likely than men to fall prey to depression throughout their lifetimes, with women being twice as likely as men to experience major depression. While the greatest risk for both sexes falls at midlife, the gender difference appears early; one in ten teenage girls was found to suffer from major depression in one study (International Journal of Behavioral Development 2004; 28:16-25). What's more, childhood depression leaves a person more susceptible to mood problems in adulthood. One reason for the gender difference in depression, according to researchers, is that women tend to dwell on depressed feelings to a greater degree than men. Some scientists believe a family history of depression carries greater weight for women. Others theorize that the inner fluctuations of a woman's monthly cycle can leave her susceptible to stresses emanating from the outer world. Studies indicate that almost three-quarters of all premenstrual women experience some level of mood difficulties (Summit on Women and Depression, APA, April 02), and a woman's hormonal ebb and flow may even make her more vulnerable to seasonal affective disorder (SAD), the kind of depression linked to a lack of natural light. Warning Signs Not surprisingly, many depressed folks feel sad and lethargic, down on themselves and the world. But in some people, depression is marked by agitation and concentration difficulties, or is accompanied by anxiety. Sleep disturbances-either insomnia or excessive sleepiness-often ensue, and activities that used to provide pleasure lose their appeal. Other factors are physical problems and medication side effects. That's why your first step should be a consultation with your health care practitioner (if your moods are especially dark, seek professional assistance as soon as possible). Life's worries and cares also weigh more heavily on some people than on others. " [N]ot only will certain stressors [adverse events] cause depression as a direct response," notes Dr. Cass, "but they may predispose an individual to future episodes of depression." If you're like many women, breast cancer stands near the top of that dreaded list. But that fear doesn't permeate other cultures the way it does ours. A woman like Mariko Mori, for instance, 52 years old, Japanese, worries about intense pressures beginning to burden her toddler grandson. But worry about breast cancer? Hardly. In Indiana, Mary Lou Marks, 50, has similar family frets, mulling over her 28-year-old daughter's career choice. But on top of that, when Mary Lou tabulates her other worries, she recoils at the thought of breast cancer. She's heard about her lifetime risk: 1 in 8. Meanwhile, Mariko's is merely 1 in 40, according to Bob Arnot's Breast Cancer Prevention Diet (Little, Brown).
American Problem New studies have found the effect of carrying the gene linked to breast cancer, which is responsible for only 5 to 10% of breast cancer incidence, is not as great as first suspected. Earlier estimates that the gene reflects an 80% chance of incurring breast cancer by age 70 has been recalculated to be only 37% (The Lancet, 1998;352:1337-1339). Complex Causesbr> Researchers agree: No one factor is solely responsible for breast cancer. Risk depends on many factors, including diet, weight, smoking, alcohol consumption, activity level and, of course, those genes. Regardless of their actual chance of getting breast cancer, women worry. Mary Lou faces no factors that would place her in particular jeopardy. But her anxieties about radical therapies and medical expenses paralyze her: She forgets to visit her health care provider and skips her annual mammogram appointments. Mary Lou's daughter, perhaps in reaction to her mother's gripping fears, campaigns ardently for cancer prevention, educating herself and mobilizing against the cumulative effects of known cancer risks. Smart young woman: A malignancy, after all, can take years to develop. A tumor must swell to one billion cells before it is detectable by a mammogram.
Dietary Benefits The soy-rich regimen of Japanese women like Mariko Mori, for example, helps to explain the low breast cancer rates in Asian countries (see box at center of the page). Tomatoes, because of their high quotient of the carotenoid lycopene, have been found to protect cells from the corrosive clutches of oxidants that have been linked with cancer in 57 out of 72 studies (The Santa Rosa Press Democrat, February 17, 1999, page A6, reporting on a Harvard Medical School study). For more on tomatoes see page 16. But there's no one magic anti-cancer food or diet. Eating to prevent breast cancer requires a balanced menu with fiber, healthy fats, phytoestrogens and antioxidants, all fresh and free of chemical additives. Modifying the balance and type of estrogen, the female Sex Hormone produced by the ovaries, offers an important breast cancer safeguard. "When men go through andropause in their late 40s (like women go through menopause) and testosterone drops, these hormonal changes are associated with heart attacks, high cholesterol and diabetes," says Jacob E. Teitelbaum, MD, Director of the Annapolis Research Center for Effective Chronic Fatigue and Fibromyalgia Therapy in Maryland. And although cardiovascular disease and cancer account for about two-thirds of men's deaths, says Michael Castleman, author of Blended Medicine (Rodale), men are also plagued by chronic pain (arthritis, especially from old athletic injuries), sexual problems and mental decline. But men, and the women who love them, need not accept decline as an inevitable sign of aging. A natural man lifestyle makeover can make a difference. "Prostate problems can be significant [for men as they age]," says Jamey Wallace, ND, clinic medical director at the Bastyr Center for Natural Health in Seattle. A recent study Williams led, published in the American Journal of Public Health, confirms that men's behavior is indeed a contributing factor to longevity or lack thereof. "Men take more risks than women," says Castleman. "Men ride motorcycles, go skydiving and do 'death-defying' things. Sometimes, death wins." A macho attitude can prompt men to practice risky behavior by, say, driving without a seat belt. Men also typically engage in more dangerous occupations like construction or fire fighting. "Men care less about their health, so they don't take care of themselves as well as women do," says Castleman. Get Him to Take the Long View "Health problems are a result of decades of poor diet and not enough exercise," says Dr. Wallace. Dr. Teitelbaum, author of From Fatigued to Fantastic (Avery/Penguin) says, as a rule of thumb, "Things that make you feel good are generally good for you." But there is a difference between a craving, say for sugar, and what makes you feel good. The difference is how you feel an hour after you've eaten something. Sugar may make you feel fatigued; a high-protein diet may make you feel energized. "If you have low energy, that's the time to add eggs and meat. Others need to be vegans. It's really individualized. Listen to your body." Zinc is an important nutrient for men's health, particularly for the prostate, and can be found in pumpkin seeds. "Sprinkling a small handful on salads on a daily basis or bringing a small bag to the office and nibbling on those can be a helpful adjunct," says Dr. Wallace. Don't overdo zinc supplementation because high levels can lowers HDL-the good cholesterol-levels. If you do use supplements, follow package directions. "You'll get a variety of nutrients so the body can select what it needs from the different vegetables," says Dr. Wallace. "Serve more vegetables, at least two with dinner and add fruit into your man's (and your own) diet," says Dr. Zimmerman. "Eating three each of protein, vegetables and fruits per day goes a very long way to improving health." So does drinking plenty of water, eight to ten glasses a day. Besides providing a good example by eating healthy foods, a woman can do the same thing with exercise. "If a woman wants to start walking she can invite her husband to go along. Thirty minutes of walking every day can be very helpful," says Dr. Wallace. Walking, like sex, keeps the pelvic area active and improves prostate health by stimulating blood flow. Under-Reported (and Underappreciated) Solutions for Cholesterol and Triglyceride Controlby Richard Conant, L.Ac., C.N.Fat and human existence are inseparable. Setting aside the fear and loathing over fat in the body that pervades our culture, we understand that fat is our friend. We cannot live without fat. The human body contains many different kinds of fats and fat-like molecules. Collectively known as "lipids" these fatty substances include fatty acids, lipoproteins, phospholipids, glycolipids, triglycerides, steroid hormones and the infamous, dreaded cholesterol. Lipids (fats) are found everywhere in the body, performing a variety of vital functions. The brain is a fat-rich organ. Brain neurons and all other nerve cells are protected by a myelin sheath, made largely out of fatty material. Cell membranes consist almost entirely of phospholipids (lipids that contain phosphorus) arranged in a sandwich-like double layer embedded with proteins. Sex Hormones are lipids, belonging to the group of complex lipid molecules known as "steroids." Vitamin D is a lipid. The body stores and transports fatty acids in the form of triglycerides. A triglyceride contains three fatty acid molecules, which have a chain-like structure, linked to glycerol. (There are also mono- and di-glycerides, which have one and two fatty acid chains, respectively, attached to glycerol.) Like many other things necessary to life, fat is a two-edged sword. Fat insulates us from the cold, cushions and protects our vital organs and serves as a storehouse for energy. Yet, when present in excess to the point of obesity, fat threatens health, happiness, self-esteem, social standing and longevity. The same is true of other lipids, most notably triglycerides and cholesterol. Transported throughout the body in the bloodstream, these essential lipids become a health liability when the blood contains too much of them. Keeping fat in it its proper place, not eliminating or drastically reducing it, is the goal we should seek. In the blood, lipids must be maintained at healthy levels and ratios. When they are, an important foundation of good health is established. How do we keep the blood lipids we need——triglycerides and the various forms of cholesterol——balanced at healthy levels? Diet and exercise are indispensable, these basics must come first. Along with the recommended dietary practices, a number of nutritional approaches offer help for maintaining healthy blood lipids. We will now give several of these a closer look. In 1990, an herb used for centuries in the Far East was introduced to U.S. consumers. This herb, called "gum guggul," is proving to be one of the most effective natural cholesterol-lowering agents ever discovered. It also brings triglycerides down and raises HDL, the "good" cholesterol. The changes are substantial; gum guggul single-handedly normalizes the entire blood lipid profile, even in people with high starting levels of cholesterol and triglycerides. Gum guggul, also called simply "guggul," is a gummy resin tapped from the Commiphora tree. A cousin of myrrh gum, guggul has been used by Ayurvedic herbalists of India for at least 3,000 years; texts dating from around 1,000 B.C. mention the herb. Guggul was traditionally given for rheumatism and poor health caused by excess consumption of fatty foods. One ancient Sanskrit text describes in detail what happens in the body when blood fats are out of balance, due to sedentary lifestyle and overeating. The name of this condition has been translated as "coating and obstruction of channels." Intrigued by the obvious similarity between "coating and obstruction of channels" and arteries clogged by fatty plaque, Indian researchers initiated a series of experimental and clinical studies in the 1960's to see if gum guggul would lower excess blood lipids.1 Both human and animal studies consistently showed cholesterol and triglyceride reductions. Detailed pharmacological studies showed that guggul's lipid-lowering effects are produced by compounds in the resin called "guggulsterones."2 An Indian pharmaceutical firm then patented a standardized extract of gum guggul under the trade name "Gugulipid." The product contains a uniform 2.5 percent guggulsterones, which is higher than guggul resin in its natural state. Because Gugulipid guarantees the necessary intake of guggulsterones needed for blood fat reduction, it has become the product used in clinical research. Phase I efficacy safety trials and Phase II efficacy trials have yielded more positive data.3,4,5 Most of the studies on gum guggul have used relatively small numbers of subjects; this tends to make mainstream medical scientists reluctant about natural remedies. A large, well-publicized double-blind Gugulipid trial on 400 to 500 people would go a long way toward giving this herb the credibility it deserves. Another effective natural solution for blood fat control that should be better known is a relative of pantothenic acid (vitamin B5). Pantethine is the active form of pantothenic acid in the body. Pantethine forms CoA, an essential co-enzyme for utilization of fat. CoA transports "active acetate," an important byproduct of fat metabolism that provides fuel for generating cellular energy. By promoting the burning of fats for energy, pantethine helps keep triglyceride levels down.6 Pantethine also helps regulate cholesterol production, by facilitating the conversion of fat into other lipid-based molecules needed in the body.6 Japanese researchers began studying the effect of pantethine on blood fats nearly twenty years ago. They reported their promising results at the Seventh International Symposium on Drugs Affecting Lipid Metabolism, held in Milan, Italy in 1980.7 Few in the medical or scientific communities took notice. Italian researchers followed up with several small clinical trials that confirmed the preliminary reports.6,8,9 An excellent cholesterol and triglyceride lowering agent that is safe and free of side-effects, pantethine remains, for the most part, ignored by mainstream science, although its usage is growing in alternative medicine circles. Pantethine it will no doubt prove to be one of the most important supplements for maintaining healthy blood fat levels. When taken in high enough doses, niacin (vitamin B3) substantially lowers cholesterol. This has been known to medical science for many years.10 studies on niacin as a cholesterol-lowering agent go back to the 1950's. There was a fair amount of initial enthusiasm for niacin because it improves, unlike most lipid-lowering drugs, all parameters of the blood lipid profile. Niacin reduces total cholesterol, LDL cholesterol and triglycerides. It also raises HDL cholesterol quite well. Interest in niacin has faded, in part because the necessary dose, 1200 milligrams a day or more, can cause flushing and gastrointestinal disturbances. Very high doses may be harmful to the liver if taken for too long. There is a solution to the side-effect problem with niacin which, again, has failed to gain widespread attention. Inositol hexanicotinate is a flush-free form of niacin composed of six niacin molecules bonded to one molecule of inositol, another B-complex nutrient. Absorbed as an intact structure, inositol hexanicotinate is metabolized slowly, releasing free niacin into the bloodstream over a period of hours following ingestion.11 Inositol hexanicotinate has all the benefits of niacin for controlling blood fats. The flushing effect of ordinary niacin, which metabolizes much more rapidly, does not occur. Taking as much as four grams per day has not been reported to raise liver enzymes or cause other side-effects, but prudence dictates that people with liver problems should avoid very high doses of inositol hexanicotinate, or any form of niacin.12 We often think of vitamin E as synonymous with d-alpha tocopherol. Vitamin E is actually a whole family of compounds that includes various tocopherols and a group of lesser known but highly beneficial substances called "tocotrienols." All have vitamin E activity. Tocotrienols are similar in chemical structure to tocopherols, but they have important differences which give them unique and highly beneficial properties for human health. Vitamin E is one of the most recognized antioxidants, nutrients that deactivate potentially toxic byproducts of oxygen metabolism known as free radicals. Vitamin E neutralizes peroxides, which result from the free radical oxidation of lipids, making it a key antioxidant in cell membranes. While d-alpha tocopherol has generally been regarded as the form of vitamin E with the strongest antioxidant activity, tocotrienols are even stronger. The tocotrienol story is another example of a natural product slow to gain recognition. A Univeristy of California research team discovered that d-alpha tocotrienol is over six times more effective than d-alpha tocopherol at protecting cell membranes against free radical damage.13 In the presence of vitamin C, which recycles vitamin E-like compounds, its antioxidant activity is 40 to 60 times higher than d-alpha tocopherol. This study was published in 1991. Its safe to say few cardiac physicians know about tocotrienols, and we have yet to see 60 Minutes do a piece on "the powerful new form of vitamin E." It would be a tremendous service to public health if they did, because the benefits of tocotrienols go far beyond their stellar antioxidant ability. Tocotrienols also lower total cholesterol and LDL, by impressive percentages. In one double-blind controlled study, tocotrienols reduced total cholesterol by 16 percent and LDL by 21 percent after twelve weeks. Another study recorded drops of 15 to 22 percent in total cholesterol along with 10 to 20 percent decreases in LDL levels.14 Now appearing on health food store shelves, tocotrienols are a health-protecting nutrients whose long overdue time has come. Derived from food oils such as palm oil and rice bran oil, tocotrienols have the same lack of toxicity as ordinary vitamin E. References 1. Satyavati, G.
