A recent study has found that Pycnogenol, which is an antioxidant plant extract from the bark of the French maritime pine tree, helps individuals by enhancing the production of nitric oxide (NO), which in turn leads to an increase in blood flow and oxygen supply to muscles. Nitric oxide is a key cardiovascular chemical that is produced by the body. It increases blood flow which allows more nutrients and oxygen to be delivered to the muscles. This helps muscles to cope with increased physical activity. The study also suggests that taking Pycnogenol provides more NO in response to neurotransmitters which allows for better expansion of arteries, which can then carry more blood. When the subjects being tested had taken Pycnogenol, the arteries relaxed and blood flow increased by Fourty two percent, as compared to the placebo group.
Although more research is warranted, this breakthrough is especially encouraging to athletes since Pycnogenol seems to allow people to move faster when they are exercising. Other studies have compared 5-htp with antidepressants such as chloripramine and imipramine. 5-htp was at least as effective if not more so than the conventional drugs. Moreover, 5-htp has no reported side effects, although some patients have experienced mild nausea when they first take 5-htp. If this happens, merely back off and reduce the daily dose to 50mg and gradually increase it over a four-day period.
5-htp has an advantage over its precursor amino acid L-Tryptophan (LT). it is more readily absorbed than LT and is immune to meals without reducing its effectiveness. 5-htp, unlike LT, is not shunted into niacin, melatonin, picolonic acid and other amino acids. Seventy percent of oral 5-htp ends up in the bloodstream, crosses into the brain and is directly converted into serotonin.
It’s best not to combine 5-htp with antidepressant medications, although there have been no reports of adverse events. Suggested doses is 100mg 3 times a day or 200 to 200 mg taken at bedtime for insomnia.

Each day, we’re subjected to millions of elements that affect us in ways seldom seen, felt or noticed. From the moment we’re born, we rely on our cells to work around the clock - producing energy, fighting infections and sustaining life. These same cells eventually determine the rate at which we show (or hide) our age.
Taking into consideration that our planet has no shortage of toxins & germs, the need to safeguard our cells becomes very real. Constant exposure to exhaust, secondhand smoke, heavy metals, lead, fluoride and uncountable other noxious compounds should ideally provide us with nothing more than a routine immune system workout. Unfortunately, over long periods they hinder our “resistance” abilities and become stepping stones to accelerated aging.
Antioxidants. We’ve all heard the term before, and may even have a general understanding of their role. But to better grasp just how vital they are, it helps to know what’s happening at the cellular level. Free radicals are unstable molecular thieves that often lack electrons. To compensate, they rob healthy cells - a process better known as oxidation. Antioxidants work with the immune system to prevent oxidation, and clean up the mess it leaves behind. Hence, the name.
There are two types. Exogenous antioxidants are derived from our diet and include vitamins A, E, and C along with others such as alpha lipoic acid, selenium, CoQ10, grape seed, pycnogenol and zinc. To date, we’ve been limited to exogenous antioxidants as a way to increase the rate at which our body wards off oxidation. They are not, however, our first line of defense. The action of lignans in sparing testosterone may hold greater implications than the obvious. Lack or loss of sexual desire in men and women is oftentimes associated with low testosterone levels. By preserving testosterone, many men and women may be able to retain or regain their sexual vigor. In fact, agents that block the conversion of testosterone to estrogen have been showing to increase testosterone levels by as much as 10 percent. (Speaking of sexuality, the surgical treatment of choice for prostatic disease (prostectomy) is the leading cause of impotency in men over 50. This fact alone should be powerful motivation for men of all ages to consider a supplemental regime to include lignans to lower the incidence of prostate cancer and benign prostatic hypertrophy.)

Doctors say they will not be recommending a drug that has been found to improve insulin resistance in type 2 diabetics.
In a study published in the current issue of the Journal of the American College of Nutrition, patients with type 2 diabetes had less insulin resistance after undergoing an infusion of the amino acid L-carnitine.
Oral and injectable forms of this amino acid _ which is produced naturally in the liver_ are already prescribed as the drug levocarnitine for patients with carnitine deficiency. The drug also is available as a dietary supplement. Makers claim it can enhance athletic performance and protect against liver, kidney and heart disease.
Unlike patients with type 1 diabetes, who do not produce insulin, patients with the type 2 variation may manufacture adequate levels of insulin but develop resistance to its effects. Insulin is essential for the transportation of blood sugar, or glucose, into cells so they can produce energy.
Dr. Geltrude Mingrone and colleagues from the Catholic University in Rome evaluated the effects of insulin and L-carnitine on 15 type 2 diabetics and 20 healthy volunteers.
Earlier studies cited in the report found that L-carnitine improved heart function in diabetics and increased the level of glucose oxidation, a process that helps cells make use of glucose. The researchers found that both the diabetic group and the healthy volunteers experienced an 8-percent increase in glucose use with L-carnitine compared with a placebo.
This 8-percent increase compares modestly with a previous study using a higher dose of L-carnitine that found glucose use was increased by 17 percent. Dr. Richard K. Bernstein, director of the New York Diabetes Center in Mamaroneck, said the study demonstrated benefits that were ``only slightly higher than marginal."
Bernstein said that he prescribed L-carnitine with some success to patients with poor circulation, but did not expect to recommend it for insulin resistance. ``If we were to see an increase of 50 percent in glucose utilization, then we might want to look at the study more closely," he said. A leading national diabetes expert, who refused to be named, described the results of the study as not impressive and ``certainly no breakthrough." He said the study indicated that L-carnitine was a long way from being clinically recommended for type 2 diabetics.
Doctors said they would continue to prescribe drugs like metformin and troglitazone for insulin resistance. Dr. David M. Nathan, director of the Diabetes Center and the General Clinical Research Center at Massachusetts General Hospital in Boston, said he was satisfied with the safety and effectiveness of both drugs provided the patient was monitored at regular intervals for kidney and liver function.

Essential fatty acids are found in both plant and animal sources, although primarily in plants. The EFA family is composed of two main forms, Omega-3 and Omega-6. The following explains exactly what these forms are.
OMEGA-3: The most common forms of Omega-3 are eicosapentaenioic acid (EPA), docosahexaenoic acid (DHA) and alpha-linolenic acid, which comes from plants and helps create EPA and DHA. Omega-3 is usually derived from fish oils. Dr. Roger Illingworth, associate professor of medicine and biochemistry at Oregon Health Sciences University, explains that Omega-3 fatty acids are “long-chained metabolic products from linolenic acid. . . When animals consume and metabolize plants rich in linolenic acid, they produce Omega-3.” EPA and DHA are liquid and remain that way, even at room temperature. It is said that they protect fish by providing a body fat that stays fluid even in cold temperatures. OMEGA-6: The most common form of Omega-6 is is gammalinolenic acid (GLA). GLA is known to provide the following benefits, among many others:

FLAX: Flax is a plant said to date back as far as 5000 B.C. It has been used since approximately 5000 B.C., making it one of the oldest cultivated crops. The mucilaginous seed is, of course, called flaxseed. More than one of four Americans suffers from high blood pressure, also known as hypertension. This so-called silent killer is often the first step in developing long-term problems like heart disease and stroke. According to the American Heart Association, high blood pressure leads to about 45,000 deaths a year and contributes to another 210,000. Hypertension is more common in women beginning at age 50, particularly African-American women. And since high blood pressure rarely causes obvious physical distress, unless your health practitioner monitors your blood pressure on a regular basis, it's easy to miss. The famous study by the National Heart, Lung and Blood Institute (NHLBI), known as the Framingham Heart Study, found that half of all people who suffered a first heart attack and two-thirds of first-time stroke victims also had moderate to high blood pressure. What's more, left untreated, high blood pressure can also increase the risk of atherosclerosis (hardening of the arteries), aneurysms, loss of vision and kidney failure. Normal blood pressure is considered 120/80. When blood pressure reaches 140/90 or above on a consistent basis, you have high blood pressure. What do the numbers mean? The top number, systolic pressure, represents the peak pressure generated in your arteries when your heart beats. The bottom number, diastolic pressure, indicates the pressure when your heart is at rest between heartbeats. Among 95% of all people with high blood pressure, health practitioners can generally pinpoint no specific, single cause.
For decades, the most common recommendation for people with high blood pressure was to eat less salt. Experts have advocated reducing our salt intake to no more than three teaspoons a day: six grams (2400 mg), which is four grams less than the current national average. This recommendation was largely based on a study conducted by Northwestern University Medical School in Chicago, Illinois, known as INTERSALT. The study tested more than 10,000 men and women from 32 countries. The researchers concluded that eating a lot of salt was linked to rises in blood pressure. Other scientists haven't always found the same results. One review of 56 clinical trials by the Integrative and Behavioral Cardiology Program at the Mount Sinai School of Medicine in New York found only a modest reduction in blood pressure when the salt shaker was left unshaken. And an analysis of 58 studies by academics at the University of Copenhagen, Denmark found that, overall, studies did not support a general recommendation to reduce the amount of salt we consume. Added to all this confusion, many people are salt sensitive: their bodies retain excess salt instead of flushing it out of their systems. Unfortunately, only medical tests can reveal this sensitivity. Consequently, experts still recommend that you eat fewer foods containing salt. That means going easy on processed foods, lunch meats and soft drinks. In addition, increasing your intake of potassium, calcium and magnesium may help your blood pressure.
Foods rich in potassium and magnesium not only help regulate blood pressure, but may boost overall cardiovascular health and reduce the risk of stroke. Vegetarian items such as bananas, baked potatoes and oranges are rich in these minerals. Research that looked at 30,000 doctors found that those who ate diets rich in fiber, potassium and magnesium had lower blood pressure than the men who ate few of these mineral-rich foods (Circ, 1992; vol 86:1475-1484). A study of 40,000 female nurses found that their pressure decreased when they consumed fibrous and magnesium-filled foods (Hypertension, 1996, vol 27:1065-1072).
The nutrient CoQ10 is a vitamin-like substance which acts as an antioxidant in the body, decreasing the harm caused by caustic substances known as free radicals. Found in every part of the body, CoQ10 is necessary for producing energy in every cell. But it is estimated that nearly 40% of people with high blood pressure are deficient in CoQ10. Tests of CoQ10 seem to show that it can often reduce blood pressure by almost 10% (Cur Ther Res 1990;47: 841-845). It also appears to reduce blood triglycerides, blood fats linked to heart disease, and insulin, while slightly increasing HDL (good) cholesterol.
Perhaps the biggest breakthrough in lowering blood pressure without the use of prescription medicine came with a study known as DASH (Dietary Approaches to Stop Hypertension). Funded by NHLBI and the National Institutes of Health, the multicenter study examined more than 400 people with high blood pressure. These folks were divided into three groups. One ate the standard high-sodium, high-fat American diet, the second a diet high in fruits and vegetables, and the third a combination diet rich in fruits, vegetables and low-fat dairy products (the DASH diet). While the group eating plenty of fruits and vegetables enjoyed a modest reduction in blood pressure, the study found that combining low-fat dairy with produce lowered both systolic and diastolic blood pressure by 11.4 and 5.5 points, respectively. And the benefits came quickly. Many of the people on the combination diet lowered their blood pressure within two weeks. The results were so impressive that researchers at Brigham and Women's Hospital in Boston, Massachusetts suggested that the DASH diet may offer an alternative to drug therapy for people with hypertension and may even serve to prevent high blood pressure altogether. The DASH diet is low in saturated fat and rich in whole grains, fruits and vegetables. Similar to the diet found in Mediterranean cultures, DASH also includes nuts, seeds and legumes, and is supplemented by non- or low-fat dairy products. Moderate amounts of protein-in the form of fish, poultry and soy-are also eaten. Eating in the DASH may also spur weight loss. Since being overweight can increase your blood pressure, the NHLBI strongly recommends a low-calorie diet such as DASH to take off extra pounds. Exercise and stress relief play critical roles in most pressure-reducing plans. Working out not only helps shed weight, it can also lower your blood pressure. Low to moderate aerobic exercise four days a week may lower blood pressure just as effectively as a higher intensity workout. And learning how to manage stress has helped dropped pressures in people with hypertension (Arch Intern Med 2001; 161:1071-80). Presented at the 2005 World Garlic Symposium
Many of the world’s top-level scientists gathered in Washington D.C. this week for the 2005 Garlic Symposium, entitled, “Significance of Garlic and its Constituents in Cancer and Cardiovascular Disease.” The conference provided current scientific information about the effect of garlic and its constituents on health and performance. The symposium was held at the Georgetown University Conference Center on April 9-11, 2005.
“For the first time in seven years authorities in various fields of garlic research from all over the world to provide the latest updates, specifically regarding aged garlic extract and its actions in diseased states including heart disease and cancer,” commented Dr. Matthew Budoff, M.D. cardiovascular researcher at UCLA. “Garlic has been used medicinally for thousands of years in virtually all ancient cultures. Now, new metabolic roles for garlic are being proposed and there are many promising lines of research.”

Effect of aged garlic extract (AGE) has been tested in the placebo-controlled double blind randomized clinical study that determined that the atherosclerotic plaque burden detected by electron beam tomography (EBT) changed significantly with the use of aged garlic extract, Patients in Dr. Budoff’s study were able to significantly lower their total cholesterol, blood pressure, homocysteine and LDL cholesterol oxidation levels with aged garlic extract supplementation.
“Garlic is turning out to be a major player in cancer and heart disease prevention and control, especially in combination with drug treatments,” said Richard Rivlin, M.D. of Strang Cancer Prevention Center at Cornell. “It’s also showing us that we can start early. It’s madness to treat cancer and heart disease in their advanced stages. We need to start early and aged garlic extract is an excellent way to do that.”
Almost 400 scientific studies have been completed on aged garlic extract, done in major universities worldwide. These studies have focused on a variety of heart disease risk factors such as cholesterol, high blood pressure, homocysteine levels, inhibiting LDL oxidation, anti-platelet aggregation and adhesion, stimulating blood circulation; in addition to other studies on immune stimulation, cognitive effects, liver function and anti-tumor effects. .

Mounting evidence points to the anticancer properties of fresh garlic extracts, aged garlic, garlic oil, and a number of specific organosulfur compounds from garlic. These prevention characteristics arise through both a dose and temporal related change in several cellular events including those involving drug metabolism, immunocompetence, cell cycle regulation, apoptosis and angiogenesis. A block in carcinogen activation through modulation of cytochrome P450-dependent monooxygenases and/or acceleration of carcinogen detoxification via induction of phase II enzymes likely account for some of this protection. The block in preneoplastic lesions and/or tumors in several sites suggests a generalizable mechanism. The efficacy of water- and lipid-soluble allyl sulfur compounds against chemical carcinogenesis appears comparable, although more studies are needed. A shift in sulfhydryl groups, redox status or enzyme catalysis may account for some of the phenotypic changes. They may also account for the observed hyperphosphorylation of specific cell cycle related proteins and histone hyperacetylation; both of which have been correlated with suppressed tumor cell proliferation. Several forms of allyl sulfur compounds are effective in blocking cell division and inducing apoptosis, but notable differences in the efficacy among these various compounds and across tumor types are evident. While the expression of many genes and proteins can be influenced by allyl sulfides; the challenge is to determine which is responsible for a phenotypic change. Additional studies are needed with more modest exposures and over prolonged periods and that utilize transgenic and knockout models to assist in the identification of molecular targets. Finally, additional research is needed to identify sensitive “effect” and “susceptibility” biomarkers that can ultimately be used to identify responders from non-responders.

Effect of Aged garlic extract (AGE) has been tested in the placebo-controlled double blind randomized clinical study to determine whether the atherosclerotic plaque burden detected by electron beam tomography (EBT) will change at a different rate under the influence of AGE or placebo. EBT can non-invasively quantitate the amount of coronary calcification and track atherosclerotic plaque over time. Nineteen of 23 patients completed the study protocol. The patients were well matched for age, gender, statin use and cardiac risk factors. Patients underwent EBT and blood testing at baseline, and then again after 12 months of randomization. The average change in the calcium score (Volumetric method) ± SD for the AGE group (n = 9) was 7.5 ± 9.4% over the one year. The placebo group (n = 10) demonstrated 22.2 ± 18.5% annual progression, significantly greater than the treated cohort (p = 0.01). While there were no significant changes in cholesterol parameters, or C Reactive protein between the groups, high density lipoproteins and plasma homocysteine in the AGE group demonstrated a trend toward improvement compared to the placebo patients. Thus, although this is a small-scale trial, it demonstrates the potential of AGE to inhibit the rate of atherosclerosis (progression of coronary calcium), as compared to placebo over one year. Larger studies need to be performed to assess this potential anti-atherosclerotic therapy and the impact on coronary events.

Endothelial dysfunction (ED) due to decreased bioavailable nitric oxide (NO) by increased vascular oxidant stress plays a critical role in the vascular pathobiology of hyperhomocysteinemia (hhcy). Aged Garlic Extract (AGE) minimizes intracellular oxidant stress and stimulates NO generation in endothelial cells. We performed a placebo-controlled, blinded, cross-over study to examine whether AGE prevents macro- and microvascular ED during acute hhcy induced by an oral methionine challenge in healthy subjects. Acute hhcy leads to a significant decrease in flow-mediated vasodilation of the brachial artery as determined by vascular ultrasound, indicative of macrovascular ED, as well as a decreased number of recruited nailfold capillaries during postischemic reactive hyperemia as determined by videomicroscopy, and to a decreased ratio of acetylcholine (endothelium-dependent) vs. sodium nitroprusside (endothelium-independent) iontophoresis induced skin perfusion as measured by laser doppler flowmetry, indicative of microvascular ED. Preliminary results show that pretreatment with AGE for six weeks diminishes the adverse effects of acute hhcy on endothelium-dependent brachial artery vasodilation and on acetylcholine-induced stimulation of skin perfusion. Whether or not this is accompanied by changes in biochemical parameters of ED is still under investigation. It is concluded that AGE may at least partly prevent a decrease in bioavailable NO during acute hhcy.

Matthew Budoff, MD, FACC, is an associate professor of medicine at the UCLA School of Medicine and program director for the Division of Cardiology, as well as director of the Electron Beam CT Laboratory at Harbor-UCLA Medical Center in Torrance, Calif. He completed his undergraduate work at University of California, Riverside, and earned his medical degree at George Washington University in Washington D.C. Dr. Budoff’s efforts to identify and modify risk factors for cardiovascular disease using electron beam CT have been extensively published. His latest research focuses on the progression of arteriosclerosis.
