Cetyl myristoleate is a fatty acid ester used largely as a dietary supplement to support joint mobility and the immune system. Chemically it is a natural hexadecyl ester of cis-9-tetradecanoic acid, an unsaturated fatty acid commonly known as myristoleic acid. The acid is found naturally in fish and whale oils, and also in butter and kombo butter from the kombo nut. The ester is of a buttery consistency at normal room temperatures, and turns waxy when cooled, and it has no known uses other than as a health supplement, particularly for the treatment of arthritis and other inflammatory and immune system problems. The substances appear to have similar therapeutic characteristics to unsaturated essential fatty acids, but with a stronger and more long lasting effect. It also appears to work in three distinctly different ways in that it acts as a joint lubricant, supports the immune system and functions in the same way as essential fatty acids. Arthritis is a disease that has reached epidemic proportions worldwide, and it is estimated that more than 50 million Americans are affected by it. Until recently it was believed to be a disease of the aged, or associated with physical injury, but is now known to be more complex than that, with many manifestations and causes. Because of this, no single treatment is effective with all forms of arthritis, but cetyl myristoleate is the new kid on the block that is believe to make a significant contribution to the treatment of non-infective forms of the disease. He had been unable to induce arthritis in mice, and believed that if he found out how to do that, then he should also be able to establish what made the mice immune to the disease, and so establish a cure. He soon established that it was this cetyl myristoleate circulating in the blood that protected the mice, and then found a way of synthesizing it in the lab by esterifying myristoleic acid. Since the compound was unknown at that time Diehl patented it in 1977, hence the date. Apart from in mice, it has subsequently been found only in whale oil and in one of the glands of the male beaver, with no other known sources at the time of writing. There have been several studies that have supported the effect of cetyl myristoleate on arthritis, the first human study being in 1995 when 46 out of 48 arthritis patients showed significant improvement in articular mobility and reduction in pain. Others since have demonstrated similar results. So how does it work? What is the mechanism by which this hitherto unknown substance helps to relieve the symptoms of osteo and rheumatoid arthritis? Being a fatty acid ester, cetyl myristoleate possesses lubricant properties, and one of its properties is to supplement the synovial fluid that lubricates the joints and tendons. This helps to keep the joints moving freely, and reduces their tendency to grind when the connective tissue wears. It also lubricates entire muscle groups, allowing them to slide over each other more freely, and also over bones allowing a smoother movement of the joints. The second way, in which it operates, by supporting the immune system, is particularly appropriate to the treatment of rheumatoid arthritis which is caused by the immune system attacking the body’s own joint and connective tissue. In this respect, cetyl myristoleate has also been found effective in treating other immune system conditions such as multiple sclerosis and lupus erythematosus. Finally, it also possesses the mediation effect on the inflammatory reaction possessed by other essential fatty acids. This helps to reduce the effect of inflammation that causes much of the swelling and pain of arthritic conditions, although it occurs much faster than with other fatty acids. When taken together, all of these effects have a powerful effect in reducing the unpleasant symptoms of both osteoarthritis and rheumatoid arthritis. Swelling is reduced, mobility is improved and pain is alleviated. Improvements have been reported by sufferers of both types of arthritis, and in fact it is reported that Harry Diehl successfully treated his own arthritis with the substance before any other human tests had been carried out. Cetyl Myristoleate is now used word-wide in significant quantities, and there have as yet been no confirmed reports of adverse side effects. However, as with many otherwise safe supplements, it is recommended not to be taken by pregnant or lactating women until it has been proved to definitely have no side effects on the fetus or young babies. Although there is no definitive scientific evidence of the effectiveness of the substance in relieving the symptoms of arthritis, there is an overwhelming body of evidence from those that have used it world wide. The accepted treatment is by non-steroidal anti-inflammatory drugs (NSAIDs), and with over 16,0000 NSAID-related deaths in the USA alone each year, cetyl myristoleate is certainly worth considering as an alternative for arthritis sufferers. A. You may not know if you have a serious B-vitamin deficiency. Routine lab work does not measure the amount of B vitamins in your blood. You might want to ask your health care practitioner to have your B vitamin level in your blood measured, especially if you are having memory and focus problems. Keep in mind that this type of lab work is fairly expensive, however.
Supplements do not replace the need for a healthy diet, especially a diet with high levels of fruits, vegetables, whole grains and omega-3 fatty acids. Keeping your body healthy helps keep your brain healthy as well.

Bone Health Approximately 44 million American women and men aged 50 and older have osteoporosis (severe bone loss) or osteopenia (mild bone loss), with women being affected about twice as often as men. At least 1.5 million fractures of the hip, vertebra (back or neck), or wrist occur each year in the United States as a result of osteoporosis, and the annual cost of treating this disorder is nearly $14 billion and rising. Unfortunately, the toll in human suffering and loss of independence is even greater. In this issue of Ask the Doctor, we will discuss the risk factors for osteoporosis and some key nutrients you can add to your diet that can minimize bone loss and reduce your chances of developing this disease. Q. What are the risk factors for osteoporosis? A. Small body frame, underweight, Caucasian or Asian race, a sedentary lifestyle, cigarette smoking, excessive alcohol or caffeine intake, high intake of carbonated beverages (especially colas), and having other family members with osteoporosis all increase personal risk of developing the disease. Certain medical conditions, including diabetes, celiac disease, hyperthyroidism, rheumatoid arthritis, chronic obstructive lung disease, hyperadrenalism, and hyperparathyroidism, are all associated with an increased risk of osteoporosis. Some medications increase the rate at which bone is lost; these include drugs prescribed for the treatment of seizures, drugs used for blood thinning, steroids such as prednisone, aluminum-containing antacids, and loop diuretics (furosemide {Lasix}). Q. Isn’t bone loss just a normal consequence of aging? A. Although bone mass normally declines after the age of 35, bone loss severe enough to cause fractures after just minor trauma (such as bump or fall) seems to be a relatively new phenomenon. Osteoporosis was rare in the late 19th century, and it was not until around 1920 that the condition began to attract attention among doctors. Since that time, the percentage of people who develop osteoporosis has continued to increase. For example, the age-adjusted prevalence of osteoporosis in Q. Can osteoporosis be prevented? A. Engaging in regular weight bearing exercise, avoiding excessive consumption of alcohol and caffeine, and quitting smoking will slow the rate of bone loss. Eating adequate, but not excessive, amounts of protein also enhances bone health. In addition, a growing body of research has shown that supplementing with various vitamins and minerals may not only help prevent, but in some cases actually reverse, bone loss. At least 15 different nutrients have been found to play a role in bone health. Q. What type of calcium is best? A. For most people, calcium salts are absorbed about the same, between 30% and 40% of the administered dose. People who low stomach acid (hypochlorhydria) should not use calcium carbonate, because that form of calcium is absorbed poorly in the absence of stomach acid. Calcium phosphate may be preferable for many older people, because phosphorus is necessary for normal bone formation, the phosphorus intake of older people is often low, and calcium supplements inhibit the absorption of phosphorus. Also, calcium bound to phosphorus is the form in which calcium in the bone is stored, and it has a much greater bone activity than other forms. Q. How much vitamin D is needed to promote strong bones? A. Because vitamin D is produced when the ultraviolet rays from the sun hit skin, people who stay out of the sun, wear sunscreen, or live in a northern latitude (such as Boston or Seattle) where less ultraviolet light reaches the skin, are at increased risk of vitamin D deficiency. In addition, aging decreases a person’s ability to synthesize vitamin D in the skin. Results from five research trials on vitamin D found that supplementation with 700-800 IU of vitamin D per day decreased the number of hip fractures by 26%, but 400 IU per day was ineffective. In addition to enhancing bone health, vitamin D improves nerve and muscle function in older people, thereby reducing their chances of falling down. Supplementation of elderly women with 800 IU of vitamin D per day has been shown to decrease the number of falls by about 50%. Q. Is that much vitamin D safe? A. Bone is living tissue, constantly remodeling itself and engaging in numerous biological functions. Like other tissues in the body, bone has a wide range of nutritional needs. The typical refined and processed American diet has been depleted of many different vitamins and minerals, some of which play a key role in promoting bone health. Not getting enough of one or more of these micronutrients may be and important contributing factor to the modern epidemic of osteoporosis. In addition, supplementing with calcium may cause a loss of magnesium, zinc, silicon, manganese, and phosphorus, unless these nutrients are also provided. Q. What nutrients besides calcium and vitamin D promote healthy bones? A. Magnesium, zinc, copper, manganese, vitamin K, boron, strontium, silicon, folic acid, vitamin B6, vitamin B12, phosphorus, and vitamin C have all been shown to play a role in bone health. Following is a brief description of the role that each of these 15 nutrients play in building healthy bones. Calcium: A component of the mineral crystals that make up bone. Vitamin D: Enhances calcium absorption, prevents falls by improving nerve and muscle function. Magnesium: Important for bone mineralization (accumulation of minerals which form bones). Magnesium deficiency is associated with abnormal bone mineral crystals in humans. In an open clinical trial, magnesium supplementation increased bone mineral density by an average 5% after 1-2 years in postmenopausal women. Copper: Laboratory research has found that copper promotes bone mineralization and decreases bone loss, and that osteoporosis can develop if the diet is deficient in copper. Western diets often contain less copper than the amount recommended by the National Academy of Sciences. In a 2-year double-blind trail, copper supplementation reduced bone loss by 90% in middle-aged women, compared with a placebo. Zinc: Like magnesium, zinc is important for bone mineralization, and also has been shown to decrease bone loss. Low dietary zinc intake was associated with increased fracture risk in a study of middle-aged and elderly men. The zinc content of the diet is frequently low; a study of elderly low-income people found they were consuming only half the Recommended Dietary Allowance for this mineral. Manganese: Plays a role in the creation of the connective-tissue components of bone. Manganese deficiency in laboratory tests resulted in low bone mineral density and weak bones. Manganese deficiency may be associated with the development of osteoporosis. Boron: Supports creation of bone-protecting hormones such as estrogen, testosterone, and DHEA. Boron supplementation prevented bone loss in experimental studies. In human volunteers consuming a low-boron diet, boron supplementation decreased urinary calcium excretion by 25-33%, a change that may indicate reduced bone loss. Silicon: Plays a role in the synthesis of the connective-tissue components of bone. Silicon deficiency has been associated with bone abnormalities. In an observational study, higher dietary silicon intake correlated with higher bone mineral density. In a clinical trial, administration of an organic silicon compound increased bone mineral density of the femur (or thigh bone) in postmenopausal women. B vitamins (folic acid, vitamin B6, and vitamin B12): These three B vitamins have been shown to lower blood levels of homocysteine, a breakdown product of the amino acid methionine. An elevated homocysteine concentration is a strong and independent risk factor for fractures in older men and women. Homocysteine levels increase around the time of menopause, which may explain in part why bone loss accelerates at that time. In a 2-year double-blind trial, supplementation of elderly stroke patients with folic acid and vitamin B12 reduced the number of hip fractures by 78%, compared with a placebo. Strontium: This trace mineral is incorporated into bone and appears to increase bone strength. It also stimulates bone formation and inhibits bone breakdown. Controlled trials have demonstrated that strontium supplementation of postmenopausal women increases bone mineral density and decreases fracture risk. Vitamin K: Best known for its effect on blood clotting, vitamin K is also required for the creation of osteocalcin, a unique protein found in bone that participates in the mineralization process. The amount of vitamin K needed for optimal bone health appears to be greater than the amount needed to prevent bleeding. Vitamin K levels tend to be low in people with osteoporosis. In randomized clinical trials, supplementation of postmenopausal women with vitamin K prevented bone loss and reduced the incidence of fractures. Q. Which form of vitamin K is best? A. Two forms of vitamin K compounds are present in food: vitamin K1 and vitamin K2. Vitamin K1 (also called phylloquinone) is present in leafy green vegetables and some vegetable oils, and vitamin K2 is found in much smaller amounts in meat, cheese, eggs, and natto (fermented soybeans). To make things a little more complicated, Vitamin K2 itself can occur in more than one form. The two most important to this discussion are menaquinine-4 (MK-4, also called menatetrenone), which is licensed as a prescription drug in Research suggests that MK-7 from natto may be an ideal form of vitamin K. The biological activity of MK-7 in laboratory studies was 17 times higher than that of vitamin K1 and 130 times higher than that of MK-4. After oral administration, MK-7 was better absorbed and persisted in the body longer, compared with MK-4 and vitamin K1. Although both have shown ability to prevent osteoporosis in laboratory research, a much lower dosage (600 times lower) of MK-7 is required, compared to MK-4, to obtain beneficial effects. Thus, MK-7 has greater biological activity, greater bioavailability, and possibly more potent effects on bone, compared with other forms of vitamin K. The potential value of MK-7 for bone health is supported by an observational study from Q. Why is strontium so important in building strong bones? A. Strontium is of great interest to bone health researchers and has been studied in very high doses. Surprisingly, lower doses are not only safer for long-term supplementation, but may in fact have a greater impact on bone health than very high doses. Too little, and bone density is impaired; too much and health may be impaired. This is a case where dosing needs to be just right for optimal impact. Therefore, until more is known, it is wise to keep supplemental strontium at less than 6 mg per day. Q. Can people taking osteoporosis medications also take bone-building nutrients? A. Because nutrients work by a different mechanism than osteoporosis drugs, nutritional supplements are likely to enhance the beneficial effect of these medications. Calcium or other minerals may interfere with the absorption of biphophonates such as alendronate (Fosamax) or etidronate (Didronel). For that reason, calcium and other minerals should be taken at least two hours before or two hours after these medications. Also, it is always best to discuss the supplements you are using with your healthcare practitioner to create an integrated health plan. Final thoughts… Bone health ramifications extend beyond osteoporosis and fractures. Bone health is essential for freedom of movement, safety, comfort, independence and longevity. Weak bones do not heal well – sometimes they never heal at all. Osteoporosis-related fractures rob us of our mobility and consign thousands of Americans to walkers and wheelchairs every year. In fact, 40% of people are unable to walk independently after a hip fracture, and 60% still require assistance a year later. The most terrible consequence of fractures related to osteoporosis is mortality. The impairment of the ability to move around freely can cause pneumonia and skin damage leading to serious infections. It is estimated that suffering a hip fracture increases the risk of dying almost 25%. Making bone health a priority now will allow you to reap health dividends for many years to come.

Glucosamine Sulfate and Chondroitin Sulfate Osteoarthritis is the most prevalent form of arthritis in the U.S., according to the Arthritis Foundation. One-third of all American adults have X-ray evidence of osteoarthritis of the hand, foot, knee, or hip. Osteoarthritis is responsible for more than 7 million physician visits per year and is second only to cardiovascular disease as the cause of chronic disability in adults. As Baby Boomers age, the number of people suffering from osteoarthritis is expected to rapidly increase in the next 10 years. While osteoarthritis research ahs led to the development of promising new prescription and over-the-counter medications aimed at reducing pain, none has created the excitement of glucosamine sulfate (GS), which actually addresses the underlying joint destruction. Q. What is osteoarthritis? A. Osteoarthritis is a complex, metabolic disorder of the cartilage and bones of certain joints. However, to fully understand how osteoarthritis develops, we need to understand how joints work. A joint is formed when two or more bones are brought together and held in place by muscles and tendons. Some joints have very little range of movement, such as the joints of the ribs, while others have much more range of movement. Hips, knees, elbows, writs, and thumbs are termed synovial joints, and have the greatest range of movement and mobility of human joints. To allow such mobility, synovial joints have a unique structure. The bones that form synovial joints are covered with cartilage. Tough fibrous tissue encloses the area between the bone ends and is called the joint capsule. The joint cavity within the capsule is lined with an inner membrane, called synovial membrane. The membrane secretes synovial fluid, a thick, slippery fluid that fills the small space around and between the two bones. This fluid contains many substances that lubricate the joint and ease movement. The cartilage of synovial joints serves two very important functions. First, it provides a remarkably smooth weight-bearing surface; synovial joints move easily. Secondly, synovial cartilage serves as a shock absorber, providing a soft, flexible foundation. Healthy cartilage absorbs the force of the energy, transmits the load to the bone, and distributes the mechanical stress created by joint movement. Synovial joints function under almost continual mechanical stress. A joint’s ability to withstand or resist this stress is a reflection of its health. When the mechanical stress is too great or the joint’s ability to resist this stress is compromised, physical changes occur in the cartilage covering the bones. Cartilage is a tough, elastic tissue, comprised mostly of water, collagen, and complex proteins called proteoglycans. In osteoarthritis, the cartilage starts to weaken, becomes frayed, and eventually breaks down. This exposes the bones of the joint, which then rub together. A gritty feeling and grinding sound may occur when an osteoarthritic joint is bent and flexed. As osteoarthritis progresses, bits of bone and cartilage often break off and float inside the joint space. The bones may enlarge, causing the joint to lose its normal shape. Tiny bone spurs may grow on the joints’ sides and edges. These physical changes in the diseased joint are responsible for progressive damage and continual pain. People with osteoarthritis most frequently describe their pain as deep and aching. The pain not only is felt in the affected joint but may also be present in the surrounding and supporting muscles. Joint inflammation also may occur, increasing the already considerable discomfort. Joint stiffness is another unfortunate component of osteoarthritis. Exercising the joint most often results in increased pain; however, stiffness tends to follow periods of inactivity. Humid weather often makes all osteoarthritis symptoms worse. As the disease progresses, the pain may occur even when the joint is at rest, creating sleepless nights and miserable days. Q. What causes osteoarthritis? A. Osteoarthritis’ exact cause remains unknown. Researchers know aging doesn’t appear to cause osteoarthritis. Cartilage in people with the disease show many destructive changes not seen in older persons without the disease. However, certain conditions do seem to trigger osteoarthritis or make it worse. Some families seem to have a lot of osteoarthritis, pointing to a genetic factor. This is most commonly seen in people who have osteoarthritis of the hands. Repeated trauma can contribute to osteoarthritis, too. Athletes, extremely active people, and individuals who have physically demanding jobs often develop the disease. Persons who have certain bone disorders are more prone to osteoarthritis due to the continuous, uneven stress in their hips and knees. Obesity also is a risk factor for the disease. In overweight women, osteoarthritis of the knee is fairly common. Excess pounds also may have a direct metabolic effect on cartilage beyond the effects of increased joint stress. Obese people also often have m ore dense bones. Research has shown dense bones may provide less shock-absorbing function than thinner bones, allowing more direct trauma to the cartilage. Q. Can osteoarthritis be prevented? A. While there is currently no sure way to prevent osteoarthritis or slow its progression, some lifestyle changes may reduce or delay symptoms. The Arthritis Foundation states that maintaining a healthy weight, losing weight if needed, and regular exercise are effective osteoarthritis prevention measures. Optimal calcium intake in younger years is vital to ensure a healthy aging skeletal system. Vitamins A, C, D, and E have been studied for their role in osteoarthritis prevention. These vitamins also have shown benefit in individuals who have osteoarthritis. Q. What treatments are available for osteoarthritis? A. The goal of treatment is to reduce or relieve pain, maintain or improve movement, and minimize any potential permanent disability. Typically, non-steroidal anti-inflammatory drugs or NSAIDs (pronounced “n-sayds”) such as aspirin and ibuprofen are used for pain and inflammation relief. These medications are effective in treating only the pain of osteoarthritis. These medications have many side effects, some of which are serious. NSAID-induced gastrointestinal complications cause more than 100,000 hospitalizations and nearly 16,500 deaths annually in the U.S. Aspirin can cause an extremely annoying and continual ringing in the ears. NSAIDs frequently cause damage to the stomach lining, which can produce uncomfortable heartburn and abdominal pain. Continued NSAID use may lead to the development of stomach ulcers. NSAID-related ulcers can perforate the stomach lining and cause life-threatening bleeding. Most NSAIDs also interfere with blood clotting and may cause kidney damage. When older persons take NSAIDs, dizziness, drowsiness, memory loss, and decreased attention span may occur. Acetaminophen (Tylenol and similar medications) is similar to aspirin and other NSAIDs in its pain-relief abilities. However, acetaminophen doesn’t reduce inflammation. And while acetaminophen doesn’t have the same side effects of aspirin and other NSAIDs, if large doses are taken, liver damage can occur. Newer medications called COX-2 inhibitors provide both pain relief and reduce inflammation without the many side effects of acetaminophen, aspirin, and other NSAIDs. More recent research has indicated that, in certain situations. COX02 inhibitors also can cause stomach lining damage and bleeding. While aspirin, NSAIDs, and COX-2 inhibitors may reduce osteoarthritis pain, they do nothing to stop or slow down cartilage deterioration. In other words, these medications have no effect on the disease itself. That is why many believe glucosamine sulfate (GS) and chondroitin sulfate (CS) are preferable to pain relievers and anti-inflammatory medications in osteoarthritis treatment: they actually improve synovial joint health. And they do this without potentially life-threatening side effects. Q. How do GS and CS work? A. GS improves the health of joints affected by osteoarthritis. This supplement is so effective that even physicians who mostly rely on conventional medications routinely recommend it to their patients with osteoarthritis. In fact, GS is so good at treating osteoarthritis, many physicians use it for their own osteoarthritis joints. There is even more good news. When glucosamine sulfate is combined with low-molecular weight CS, even greater benefits can be achieved. GS and CS are naturally occurring compounds found in human joints. The right GS/CS combination actually reverses damage in joints affected by osteoarthritis, in turn significantly reducing pain and stiffness. Glucosamine occurs naturally in the body and is found in synovial fluid. Glucosamine is a basic building block for proteoglycans, is a basic building block for proteoglycans, one of the important compounds of synovial cartilage. It also is required for the formation of lubricants and protective agents for the joints. In Europe, GS and CS have been used to treat osteoarthritis for more than 10 years. While persons with arthritis felt much better when they took GS and CS, no one really knew how these compounds worked. When European and American researchers first started to study glucosamine, they discovered GS can reduce synovial joint inflammation. This explains why people felt better after taking it. Q. What has additional study of GS and CS revealed? A. As the scientific study of GS progressed, researchers determined it can stimulate the growth of cartilage cells, inhibit proteoglycans breakdown, and rebuild cartilage damaged from osteoarthritis. In other words, GS does not simply make persons with osteoarthritis feel better; GS actually makes persons with osteoarthritis get better. GS is the form of glucosamine used in research. It’s the sulfate salt of glucosamine and breaks down into glucosamine and sulfate ions in the body. The sulfate part of GS plays an important role in proteoglycans synthesis. CS also provides cartilage strength and resilience. CS is an important component of the cartilage proteoglycans of synovial joints. Because CS helps the production of proteoglycans, researchers believe CS works in a similar nature to GS. Q. Couldn’t GS and CS be taken on their own? Is there any benefit in taking them together? A. Research has discovered GS and CS act synergistically (work well together) in improving joint health. Several studies have investigated this action and it’s recommended that GS and CD be taken together. However, there may be times when your healthcare practitioner may recommend using one or the other, but not both GS and CS together. Please follow their recommendations to obtain the best results for your own unique health concerns. Low-molecular weight chondroitin sulfate (CS) is the preferred CS form, and the form that has shown the most promise in studies. Q. Why is it important to take low-molecular weight CS? A. When CS was first studied, it was given to six healthy volunteers, six patients with rheumatoid arthritis, and six patients with osteoarthritis. Researchers then measured the levels of CS in all study subjects. They found no evidence of CS in any of the subjects. This single study led many physicians and scientists to believe CS can’t be absorbed, and was not an effective natural treatment. However, several other studies in healthy volunteers have reported CS can be absorbed. The distinct difference for these findings is thought to be associated with the types of CS used in the studies. Some forms are much more absorbable that others. This was demonstrated in a recent study using CS with lower molecular weight. A higher absorption is observed for low-molecular weight CS. This means CS products with a low molecular weight may be better absorbed, allowing the CS to get into the bloodstream and the synovial fluid of joints where it’s needed. Q. Are there other supplements that can help osteoarthritis? A. Several vitamins, minerals, enzymes, and natural supplements have benefits for individuals with osteoarthritis. To understand glucosamine's role, it is important to understand joint structure and function. Cartilage in the joints acts as a shock absorber to cushion the blows of daily wear and tear. Joint cartilage is made of a unique connective tissue that consists of collagen and proteoglycans. Collagen is a strong, fibrous, insoluble protein. Proteoglycans are large, carbohydrate-rich protein chains made up of 95 percent polysaccharides and 5 percent protein called glycosaminoglycans (GAGs). GAGs are composed of repeating two-sugar units (disaccharides) that contain glucosamine sulfate and other amino sugars. Surrounding the joint cartilage is synovial fluid, which contains many substances including its chief component, hyaluronic acid. Hyaluronic acid forms the backbone of other proteoglycans and is responsible for the thickness of synovial fluid as well as its lubricating and shock-absorbing properties. Synovial fluid also provides nutrients for the joint cartilage. Glucosamine sulfate is a normal constituent of glycosaminoglycans in cartilage and synovial fluid. In essence, glucosamine sulfate provides important building blocks for cartilage production. Laboratory studies suggest that glucosamine may also function to stimulate production of cartilage-building proteins. It is also thought that the sulfate portion of the molecule contributes to the efficacy of glucosamine sulfate in the synovial fluid by providing the elemental sulfur needed for strengthening cartilage and aiding glycosaminoglycan synthesis. 1,2,3 Glucosamine sulfate has been the subject of research for over twenty years. GLUCOSAMINEGlucosamine is composed of glucose (a sugar) and glutamine (an amino acid). It is utilized by chondrocytes to form glycosaminoglycans (GSG) and proteoglycans (PG). Both of these constituents attract and bind water into cartilage, increasing resiliency. Research indicates that glucosamine may actually help your body repair damaged or eroded cartilage. A number of studies have been conducted on glucosamine sulfate and glucosamine hydrochloride, with a preponderance of positive results. Glucosamine sulfate is considered the more effective of the two. One study from the University of Liege in Liege, Belgium studied the effects of glucosamine sulfate on 212 patients with knee osteoarthritis. Participants took either 1,500 mg glucosamine or a placebo once daily for three years. The study compared joint-space width at enrollment, one year, and at the study’s conclusion. The 106 patients on placebo had a progressive jointspace narrowing, while participants taking glucosamine experienced no significant joint-space loss, indicating glucosamine may benefi cially modify cartilage structure.3 A study published in the journal Osteoarthritis and Cartilage in 1998 investigated the in vitro effects of glucosamine sulfate on proteoglycan and collagen production by chondrocytes taken from osteoarthritic articular cartilage. The results showed “a statistically signifi cant stimulation of PG production by chondrocytes from human osteoarthritic cartilage cultured for up to 12 days in 3-dimensional cultures.” 4 Another study from Italy enrolled eighty inpatients with established OA. They received either 1,500 mg of glucosamine sulfate or placebo daily for 30 days. The patients treated with glucosamine sulfate experienced a reduction in symptoms almost twice as large and twice as fast as those receiving placebo. Researchers also used electron microscopy of patient’s articular cartilage to support this hypothesis. Patients who received glucosamine sulfate showed a picture more similar to healthy cartilage. The researchers concluded that glucosamine sulfate tends to rebuild damaged articular cartilage and restore articular function.5
CHONDROITINChondroitin is classifi ed as a glycosaminoglycan. It bonds with collagen to form the basis of connective tissue. Chondroitin helps attract fl uid into proteoglycans, thereby bringing nutrients into cartilage and providing shock absorption. While glucosamine helps manufacture and maintain cartilage, chondroitin keeps cartilage from becoming malnourished. Chondroitin works synergistically with glucosamine, and these two nutrients form the basis of most joint health supplements on the market today. A 6-month randomized, multi-center, double-blind, doubledummy study published in 1996 compared the effectiveness of chondroitin versus a popular non-steroidal anti-infl ammatory drug (NSAID) in patients with knee osteoarthritis (OA). One hundred and forty-six patients with knee OA were recruited and separated into two groups; an NSAID group and a chondroitin sulfate (CS) group. The NSAID group was given the NSAID and a placebo for the fi rst month, then placebo alone for months 2-3. The CS group was given the NSAID and CS for the fi rst month, and then CS alone for months 2-3. Both groups were then given 1200mg of CS for months 4-6. “Patients treated with the NSAID showed prompt and plain reduction of clinical symptoms, which, however, reappeared after the end of treatment; in the CS group, the therapeutic response appeared later in time but lasted for up to 3 months after the end of treatment. Two groups of researchers have conducted analyses of trials that utilized SAM-e for mood enhancement. One meta-analysis was published in 1994. The researchers analyzed the efficacy of SAM-e in oral or injection forms based on published trials dated between 1973 and 1992. The authors concluded that there was a significant improvement of 17 to 38% seen in trials of SAM-e compared to placebo response. They state that the efficacy of SAM-e was superior to placebo and its administration caused few side effects.5 A second review was published in 2002. The authors analyzed studies in which SAM-e doses ranged from 200 to 1600 mg daily. They also found a significant effect of SAM-e in comparison to placebo, with an evident rapid onset of effect at enhancing mood.6 Promotes Joint Comfort and Mobility* As a sulfur donor to connective tissue, SAM-e plays a major role in protecting the integrity of cartilage tissue. An in vitro trial assessed the actions of SAM-e in cultured human articular chondrocytes. Based on these results, it was shown that SAM-e has a positive influence on the growth and health of cartilaginous connective tissue.7 In a double-blind trial with 734 individuals with compromised joint health. SAM-e given orally at a dose of 1200 mg daily for 30 days was shown to significantly promote joint comfort compared to placebo, with a high level of tolerability and low incidence of side effects. The researchers concluded that SAM-e is a highly effective supplement for enhancing joint comfort.8 Another trial evaluated the response of individuals experiencing discomfort in the joints to a regimen of 1200 mg SAM-e for 1 week followed by 800 mg for the second week, and then 400 mg for weeks 3 through 8. This open trial of 20, 641 people showed a strong ability of SAM-e to enhance feelings of comfort within the joints. The treatment was rated as “very good” or “good” in 71% of the participants, with an additional 21% rating the treatment effect as “moderate”.9 In a long-term trial lasting 24 months, SAM-e was given to 108 participants with compromised joint function. Individuals were given 600 mg orally per day for the first two weeks followed by 400 mg daily for the remainder of the trial. Individuals experienced significant enhancements in joint comfort, with dramatic improvements noted after 2-4 weeks of treatment. Improvements continued to 6 months and beyond.10 In addition to the above studies, a review was conducted in 1987 to assess the results of SAM-e supplementation in clinical trials for enhancing joint mobility and function. Over 22,000 individuals had participated in the clinical trials that were the subject of this review. The author concluded from his analysis that SAM-e was shown to be highly efficacious, rivaling or surpassing the effectiveness of other treatments, and also possessing a high level of safety.11 Because of this, SAM-e may be the treatment of choice for enhancing joint function. In another controlled study, 192 subjects took chondroitin sulfate or a placebo daily for one year. At the end of the trial, chondroitin sulfate maintained healthy joint cartilage thickness, while those on placebo had decreased cartilage. Improvements in joint function also occurred. The researchers reported that chondroitin exerted a clear chondroprotective effect.3 COLLAGEN TYPE II A number of studies have also been conducted on the administration of collagen type II to individuals that have various joint issues. Much of this research has been conducted on animal models of joint conditions while there are also studies showing the effectiveness of oral collagen type II preparations in humans for maintenance of healthy joints. A randomized controlled trial conducted on 60 patients with joint health issues in 1993 found that oral administration of chicken collagen type II for 3 months led to a significant decrease in swollen and tender joints in this group, as compared to no measurable improvement in the placebo group. There were also no side effects seen with the treatment.4 A second multicenter, double-blind, placebo-controlled trial in 274 individuals with joint issues was published in 1998. The participants were given collagen type II orally for 24 weeks. Positive effects of the treatment were noted while no adverse effects were seen.5 A paper published in 2000 reviewed the literature to assess the role of hydrolyzed collagen in joint and bone health. It was found that hydrolyzed collagen when administered orally was able to support joint health in most of the trials reviewed while the author concluded that, “Its high level of safety makes it attractive as an agent for long-term use.”6 HYALURONIC ACID (HA) Most of the literature on hyaluronic acid and joint health deals with its intra-articular use, or injections of HA directly into the joints. In this realm, there is good evidence for the effects of HA on joint function. A study was conducted with injectable HA in individuals with TMJ (temporomandibular joint) conditions. Participants received two injections, each one week apart, or placebo injections with saline. In the HA group, the researchers found decreased clicking sounds and increased function of the joint at 1 month (90% of patients showed improvement) and 6 months (63%) of follow-up, compared to about 26% of the placebo group showing improvement at 6 months.7
A pair of researchers also conducted a literature review of the trials using HA for improving joint health that was published in 2005. Their findings indicate a positive role for HA in modifying the structure of the joint and slowing progressive deterioration of joint function and mobility.8 Hyaluronic acid seems to have a natural affinity for joint tissue, and is therefore able to help support healthy joint structure and function.

Whereas the cells on their own had a low rate of renewal, hyaluronic acid added to the cells resulted in increased proliferation of skin cells in the collagen matrix. This showed that supplementing skin cells with HA caused a significant increase in the ability of cells to go through the cell cycle.10 One of the major benefits of this may be hyaluronic acid’s ability to continually renew skin tissue to help maintain a youthful appearance of the skin. Under-Reported (and Underappreciated) Solutions for Cholesterol and Triglyceride Controlby Richard Conant, L.Ac., C.N.Fat and human existence are inseparable. Setting aside the fear and loathing over fat in the body that pervades our culture, we understand that fat is our friend. We cannot live without fat. The human body contains many different kinds of fats and fat-like molecules. Collectively known as "lipids" these fatty substances include fatty acids, lipoproteins, phospholipids, glycolipids, triglycerides, steroid hormones and the infamous, dreaded cholesterol. Lipids (fats) are found everywhere in the body, performing a variety of vital functions. The brain is a fat-rich organ. Brain neurons and all other nerve cells are protected by a myelin sheath, made largely out of fatty material. Cell membranes consist almost entirely of phospholipids (lipids that contain phosphorus) arranged in a sandwich-like double layer embedded with proteins. Sex hormones are lipids, belonging to the group of complex lipid molecules known as "steroids." Vitamin D is a lipid. The body stores and transports fatty acids in the form of triglycerides. A triglyceride contains three fatty acid molecules, which have a chain-like structure, linked to glycerol. (There are also mono- and di-glycerides, which have one and two fatty acid chains, respectively, attached to glycerol.) Like many other things necessary to life, fat is a two-edged sword. Fat insulates us from the cold, cushions and protects our vital organs and serves as a storehouse for energy. Yet, when present in excess to the point of obesity, fat threatens health, happiness, self-esteem, social standing and longevity. The same is true of other lipids, most notably triglycerides and cholesterol. Transported throughout the body in the bloodstream, these essential lipids become a health liability when the blood contains too much of them. Keeping fat in it its proper place, not eliminating or drastically reducing it, is the goal we should seek. In the blood, lipids must be maintained at healthy levels and ratios. When they are, an important foundation of good health is established. How do we keep the blood lipids we need——triglycerides and the various forms of cholesterol——balanced at healthy levels? Diet and exercise are indispensable, these basics must come first. Along with the recommended dietary practices, a number of nutritional approaches offer help for maintaining healthy blood lipids. We will now give several of these a closer look. In 1990, an herb used for centuries in the Far East was introduced to U.S. consumers. This herb, called "gum guggul," is proving to be one of the most effective natural cholesterol-lowering agents ever discovered. It also brings triglycerides down and raises HDL, the "good" cholesterol. The changes are substantial; gum guggul single-handedly normalizes the entire blood lipid profile, even in people with high starting levels of cholesterol and triglycerides. Gum guggul, also called simply "guggul," is a gummy resin tapped from the Commiphora tree. A cousin of myrrh gum, guggul has been used by Ayurvedic herbalists of India for at least 3,000 years; texts dating from around 1,000 B.C. mention the herb. Guggul was traditionally given for rheumatism and poor health caused by excess consumption of fatty foods. One ancient Sanskrit text describes in detail what happens in the body when blood fats are out of balance, due to sedentary lifestyle and overeating. The name of this condition has been translated as "coating and obstruction of channels." Intrigued by the obvious similarity between "coating and obstruction of channels" and arteries clogged by fatty plaque, Indian researchers initiated a series of experimental and clinical studies in the 1960's to see if gum guggul would lower excess blood lipids.1 Both human and animal studies consistently showed cholesterol and triglyceride reductions. Detailed pharmacological studies showed that guggul's lipid-lowering effects are produced by compounds in the resin called "guggulsterones."2 An Indian pharmaceutical firm then patented a standardized extract of gum guggul under the trade name "Gugulipid." The product contains a uniform 2.5 percent guggulsterones, which is higher than guggul resin in its natural state. Because Gugulipid guarantees the necessary intake of guggulsterones needed for blood fat reduction, it has become the product used in clinical research. Phase I efficacy safety trials and Phase II efficacy trials have yielded more positive data.3,4,5 Most of the studies on gum guggul have used relatively small numbers of subjects; this tends to make mainstream medical scientists reluctant about natural remedies. A large, well-publicized double-blind Gugulipid trial on 400 to 500 people would go a long way toward giving this herb the credibility it deserves. Another effective natural solution for blood fat control that should be better known is a relative of pantothenic acid (vitamin B5). Pantethine is the active form of pantothenic acid in the body. Pantethine forms CoA, an essential co-enzyme for utilization of fat. CoA transports "active acetate," an important byproduct of fat metabolism that provides fuel for generating cellular energy. By promoting the burning of fats for energy, pantethine helps keep triglyceride levels down.6 Pantethine also helps regulate cholesterol production, by facilitating the conversion of fat into other lipid-based molecules needed in the body.6 Japanese researchers began studying the effect of pantethine on blood fats nearly twenty years ago. They reported their promising results at the Seventh International Symposium on Drugs Affecting Lipid Metabolism, held in Milan, Italy in 1980.7 Few in the medical or scientific communities took notice. Italian researchers followed up with several small clinical trials that confirmed the preliminary reports.6,8,9 An excellent cholesterol and triglyceride lowering agent that is safe and free of side-effects, pantethine remains, for the most part, ignored by mainstream science, although its usage is growing in alternative medicine circles. Pantethine it will no doubt prove to be one of the most important supplements for maintaining healthy blood fat levels. When taken in high enough doses, niacin (vitamin B3) substantially lowers cholesterol. This has been known to medical science for many years.10 studies on niacin as a cholesterol-lowering agent go back to the 1950's. There was a fair amount of initial enthusiasm for niacin because it improves, unlike most lipid-lowering drugs, all parameters of the blood lipid profile. Niacin reduces total cholesterol, LDL cholesterol and triglycerides. It also raises HDL cholesterol quite well. Interest in niacin has faded, in part because the necessary dose, 1200 milligrams a day or more, can cause flushing and gastrointestinal disturbances. Very high doses may be harmful to the liver if taken for too long. There is a solution to the side-effect problem with niacin which, again, has failed to gain widespread attention. Inositol hexanicotinate is a flush-free form of niacin composed of six niacin molecules bonded to one molecule of inositol, another B-complex nutrient. Absorbed as an intact structure, inositol hexanicotinate is metabolized slowly, releasing free niacin into the bloodstream over a period of hours following ingestion.11 Inositol hexanicotinate has all the benefits of niacin for controlling blood fats. The flushing effect of ordinary niacin, which metabolizes much more rapidly, does not occur. Taking as much as four grams per day has not been reported to raise liver enzymes or cause other side-effects, but prudence dictates that people with liver problems should avoid very high doses of inositol hexanicotinate, or any form of niacin.12 We often think of vitamin E as synonymous with d-alpha tocopherol. Vitamin E is actually a whole family of compounds that includes various tocopherols and a group of lesser known but highly beneficial substances called "tocotrienols." All have vitamin E activity. Tocotrienols are similar in chemical structure to tocopherols, but they have important differences which give them unique and highly beneficial properties for human health. Vitamin E is one of the most recognized antioxidants, nutrients that deactivate potentially toxic byproducts of oxygen metabolism known as free radicals. Vitamin E neutralizes peroxides, which result from the free radical oxidation of lipids, making it a key antioxidant in cell membranes. While d-alpha tocopherol has generally been regarded as the form of vitamin E with the strongest antioxidant activity, tocotrienols are even stronger. The tocotrienol story is another example of a natural product slow to gain recognition. A Univeristy of California research team discovered that d-alpha tocotrienol is over six times more effective than d-alpha tocopherol at protecting cell membranes against free radical damage.13 In the presence of vitamin C, which recycles vitamin E-like compounds, its antioxidant activity is 40 to 60 times higher than d-alpha tocopherol. This study was published in 1991. Its safe to say few cardiac physicians know about tocotrienols, and we have yet to see 60 Minutes do a piece on "the powerful new form of vitamin E." It would be a tremendous service to public health if they did, because the benefits of tocotrienols go far beyond their stellar antioxidant ability. Tocotrienols also lower total cholesterol and LDL, by impressive percentages. In one double-blind controlled study, tocotrienols reduced total cholesterol by 16 percent and LDL by 21 percent after twelve weeks. Another study recorded drops of 15 to 22 percent in total cholesterol along with 10 to 20 percent decreases in LDL levels.14 Now appearing on health food store shelves, tocotrienols are a health-protecting nutrients whose long overdue time has come. Derived from food oils such as palm oil and rice bran oil, tocotrienols have the same lack of toxicity as ordinary vitamin E. References 1. Satyavati, G. It's in the BloodNatural alternatives abound for managing cholesterol levels, backed by a growing body of research ©VR By Paul BubnyThe National Cholesterol Education Program (NCEP) last July lowered the threshold for considering the use of statin drugs—a move which some say was motivated more by profits than scientific evidence. For example, the Center for Science in the Public Interest pointed out that eight of the nine authors behind the new recommendations had financial ties to statin manufacturers, which stand to reap billions of dollars more from a category that grossed $14 billion in the U.S. last year. And though the Food and Drug Administration (FDA) in January decided against authorizing over-the-counter (OTC) sales of statin drugs, drug companies would still like to see this happen. “The medical establishment’s pushing of these drugs to becoming the number one category of prescribed drugs in the world has led them to keep lowering the total cholesterol number that triggers the drug recommendation,” said Neil E. Levin, C.C.N., D.A.N.L.A., nutrition educator, product formulator, and “Truth Advocate” for NOW Foods (Bloomingdale, IL), which produces a number of supplements for addressing cholesterol. “This is despite the lack of evidence that total cholesterol means much as regards cardiovascular risks. “Other tests are much more important in terms of predicting risks, including CRP (C-reactive protein), the balance of different cholesterol fractions, and homocysteine,” he continued. “Add adult-onset diabetes to the risk factors for cardiovascular disease (CVD).” At the same time, the allegation that enormous sales potential lay behind the lower threshold for prescribing statin drugs illustrates how widespread the problem of hypercholesterolemia (elevated total cholesterol) is. More than 100 million Americans have elevated cholesterol (total cholesterol values of 200 mg/dl and higher), and of these, more than a third have high cholesterol (levels of 240 mg/dl and higher), according to the American Heart Association. Those numbers have unfavorable implications for the incidence of CVD, as high cholesterol is considered a risk factor for coronary heart disease and stroke. While statin drugs haven’t garnered the same degree of negative publicity that COX-2 inhibitors have suffered lately, safety concerns have arisen nonetheless. For one thing, these drugs lower the liver’s production of coenzyme Q10 (coQ10) along with its production of cholesterol. “CoQ10 is related to energy production and immune functions, is an antioxidant, and [is] an important cardiovascular nutrient,” Levin said. “It is not good to lower one’s coQ10 levels by half!” Moreover, said Levin, statins increase the tendency of muscle tissues to break down. “Combined with inactivity or certain drugs, this can stimulate muscle wasting,” he said. “Muscle is where a good deal of calories are burned, so a loss of muscle could affect mobility and energy production, potentially adding to obesity problems. These muscle changes occurred in patients and persisted for years after treatment was discontinued, as shown by muscle biopsies, even if no obvious muscle problems were observed by the patients.” And the last word on the subject may not have been spoken. Predicted Dr. Frank King, Jr. president of King Bio Natural Medicine (Asheville, NC), “Once the appropriate studies are finished, these drugs, along with hypertensives, will hit the fan bigger than the COX-2 inhibitors.” Also looking toward the future, Levin said that of the 20 million Americans who will be “targeted” for statin drug prescriptions under the new NCEP guidelines, “Some of these will want to try natural methods first. Others will rebel at the side effects of the drugs and experiment with alternative products.” King and Levin both saw opportunity for natural products in the fallout from drug safety concerns, with King projecting that sales of his company’s cholesterol-related homeopathic remedies will double in 2005. “The reports of deaths from drugs will always overshadow the trumped-up studies and news reports blasting dietary supplements,” said Levin. “Vioxx knocked vitamin E off the media’s radar screens pretty rapidly, though we still see ignorant reporters citing that [Johns Hopkins] vitamin E analysis as if it were true. But the comparable safety of supplements means that open-minded people will want to at least try natural therapies before signing in to a lifetime of drug therapies. Meanwhile, the studies on natural products will continue to build our credibility.” Those studies keep coming in, with at least four major findings published in the past few months, plus a heart-health claim on walnuts authorized by FDA. They join a raft of earlier findings that link natural products—branded and otherwise—to healthy cholesterol levels.
"Blur of Products"With so many natural alternatives to cholesterol drugs available, it can be hard to keep track. “As with any other category, the blur of products as they cascade over several shelves means that the retailer needs to have a good sense of what works and what they want to recommend to their customers,” Levin said. “Really, each person needs a protocol that would include antioxidants, fiber, vitamins, herbs, and oils. The pre-mixed cholesterol support formulas are a good starting place.” To help retailers get a sense of “what works,” here is an alphabetical discussion of several nutrients that have demonstrated benefits in serum cholesterol levels. They include the following: Barley may help lower cholesterol, according to a report in the American Journal of Clinical Nutrition (2004, vol.80, no.5: 1185-1193). Twenty-five adults with mild hypercholesterolemia consumed a controlled diet low in total fat, saturated fat, and cholesterol for 19 weeks. They then added whole-grain products containing barley to their diets that contained low (0 g), medium (3 g), or high (6 g) amount of beta-glucan per day for five weeks. Total cholesterol was reduced by 4 percent 9 percent, and 10 percent, respectively. The diet with the highest amount of beta-glucan led to a decrease in LDL cholesterol of 17 percent. Chromium. There’s evidence, Levin said, that chromium in doses of 500 mg a day may decrease levels of low-density lipoprotein (LDL, the so-called “bad” cholesterol) and total cholesterol while raising levels of high-density lipoprotein (HDL, or “good” cholesterol). At the annual meeting of the American College of Nutrition last October, a poster presentation on the safety of Benicia, CA-based InterHealth Nutraceuticals’ ChromeMate niacin-bound chromium won first prize; among other things, the presentation cited chromium’s role in maintaining healthy blood lipid levels. Fatty Acids. The latest in a long line of studies demonstrating the benefits of fatty acids in heart health is a study published in The International Journal of Clinical Pharmacology and Therapeutics in December 2004. It showed that docosahexaenoic acid (DHA), an omega-3 fatty acid, can restore normal blood vessel function in children with inherited high cholesterol. The study, which used Martek DHA produced from microalgae, concluded that restoration of normal blood vessel function has the “potential for preventing the progression of early coronary heart disease in high-risk children.” “The evidence continues to accumulate on the cardiovascular benefits of DHA for people of all ages,” said Henry “Pete” Linsert, Jr., chairman and CEO of Martek Biosciences, an ingredient supplier based in Columbia, MD. “This study clearly indicates that DHA played an important role in healthy blood vessel function in the children in this study.” On the Omega-Research.com Website maintained by fish oil manufacturer Nordic Naturals (Watsonville, CA) can be found summaries of several earlier studies linking omega-3 fatty acids to maintaining healthy blood lipid levels, as well as related benefits such as elasticity of the arteries. In a 2003 study published in the American Journal of Clinical Nutrition, it was found that women receiving a mixture of 4 g eicosapentaenoic acid (EPA) and DHA along with 2 g of gamma-linolenic acid (GLA) had lower levels of LDL cholesterol after 28 days compared to those who received either the EPA/DHA supplements without DHA, EPA/DHA with a smaller dose of GLA, or GLA alone. Flax is another source of omega-3s, and Arkopharma/Health From The Sun (Bedford, MA) offers FiProFLAX in a variety of forms. Marketing director Hugues P. Mas said the flax is “QAI [Quality Assurance International] certified organic and guaranteed GMO [genetically modified organism]-free.” On its Website, the company offers a cholesterol quiz geared to consumers, discussing the importance of omega-3s as well as other nutrients. Garlic. Adding to an already considerable body of research demonstrating that garlic can lower total cholesterol, LDL cholesterol, and triglycerides while increasing HDL cholesterol, researchers at UCLA in 2003 reported that Kyolic aged garlic extract reduced or inhibited plaque formation in the arteries of 19 cardiac patients taking statin drugs. Lead researcher Matthew Budoff, Ph.D. commented at the time that the study “suggests that aged garlic extract may be a useful and beneficial dietary addition for the people who have high cardiovascular risk or who have undergone heart surgery.” Budoff has since presented several trade show seminars sponsored by Los Angeles-based Wakunaga of America, the makers of Kyolic. Guggul. In use for centuries as a component of Ayurvedic medicine, guggul—a gummy resin tapped from the Commiphora mukul tree, which is native to India—has been studied since the early 1960s for its hypolidemic (blood-lipid lowering) properties. Sabinsa Corp. (Piscataway, NJ), an ingredient supplier which produces a standardized extract under the brand name Gugulipid, says the studies on guggul indicate that its hypolipidemic activity can be attributed to more than one mechanism of action. Among the possible mechanisms are: inhibition of cholesterol biosynthesis, enhancing the rate of excretion of cholesterol, promoting rapid degradation of cholesterol, thyroid stimulation, alteration of biogenic amines, and “high affinity binding and anion exchange.” Homeopathy. “Homeopathy activates the body’s own control system to work properly,” said King. “This is the safest and most curative approach to take. “Forcing the body into biochemical change even naturally doesn’t actually have the curative action of homeopathy,” King continued. “Homeopathy can even correct the genetic predispositions to disease we may have inherited from as deep as a thousand years into our family chain.” King Bio makes Artery/Cholesterol/BP, a homeopathic formula intended to help tone heart muscles and blood vessels. Low glycemic index foods. In a study published in the February issue of the American Journal of Clinical Nutrition, researchers found that high glycemic load is negatively correlated to serum levels of HDL cholesterol. Assessing the relationship between blood levels of lipids and diet in a test population of 32 healthy males and females ages 11 to 25, the researchers found that glycemic load accounted for 21.1 percent of the variation in HDL cholesterol. They concluded that glycemic load appears to be an important independent predictor of HDL cholesterol in youth and noted that dietary restrictions without attention to glycemic load could unfavorably influence blood lipids. Medicinal Mushrooms. Although its product SX-Fraction is intended primarily to address high blood sugar, Maitake Products, Inc. (MPI, Ridgefield Park, NJ) found in a clinical study that LDL cholesterol in diabetic patients declined modestly (from 142 mg/dl to 133 mg/dl) over a two-month period. Those taking SX-Fraction also lost about 7 lbs. in the same time period. “The more impressive lowering of cholesterol, however, comes from the dietary fiber that is found in all medicinal mushrooms,” said Ellen Shnidman, manager of scientific affairs at MPI. She cited animal studies which documented the cholesterol-lowering properties of four different mushrooms: maitake, shiitake, agaricus, and enokitake. For example, a study reported in the September 1996 issue of Alternative Therapies showed “a 44 percent reduction in total cholesterol in rats consuming maitake mushroom in their diet,” said Shnidman. “This cholesterol reduction is accompanied by weight loss, relative to rats eating a similar high-choelsterol diet without mushrooms. Apparently, cholesterol is excreted by the rats in sufficient quantity to aid in weight loss.” Oat bran. A 2004 consumer study conducted by the Natural Marketing Institute (NMI, Harleysville, PA) for Nurture, Inc. (Devon, PA), which produces the ingredient OatVantage, found that 63 percent of consumers managing their cholesterol levels prefer oat-based ingredients. Oat bran is the subject of a health claim authorized by FDA in 1999, and NMI research found that 69 percent of respondents preferred the FDA-permitted health claim, “Helps Lower Cholesterol,” over the model structure-function claim, “Helps Maintain Healthy Cholesterol Levels.” “This is significant for food, beverage, and dietary supplement manufacturers who want to increase sales by using a more consumer-desired claim on the product label,” said Griff Parker, Nurture CEO. Plant sterols. Also the subject of an FDA-approved claim for heart health, plant sterols (structurally similar to cholesterol in humans) can block the absorption of cholesterol, according to a number of studies. In an “Ask the Doctor” publication (available online at www.atdonline.org), Decker Weiss, N.M.D. noted that sterols enter the same receptor sites that cholesterol enters on its way to the bloodstream. “The cholesterol, being blocked from absorption, remains in our intestines where it is eventually excreted,” Weiss wrote. General Mills has just introduced Yoplait Healthy Heart, a yogurt high in plant sterols. Policosanol. A mixture of fatty alcohols derived from sugar cane or beeswax, policosanol has been favorably compared in clinical studies to several types of prescription drugs for managing cholesterol. On its own, policosanol was found in a 1999 study to reduce LDL cholesterol while raising levels of HDL cholesterol. Probiotics. “Several studies have indicated that consumption of certain cultured dairy products resulted in reduction of serum cholesterol, as well as triglycerides,” wrote Dr. S.K. Dash, president of probiotic manufacturer UAS Laboratories (Eden Prairie, MN), in his Consumer Guide to Probiotics. Among other studies, Dash cited two controlled clinical studies from the VA Medical Center at the University of Kentucky. “In the first study, fermented milk containing [Lactobacillus] acidophilus was accompanied by a 2.4 percent reduction of serum cholesterol concentration,” he wrote. “In the second study, a different L. acidophilus strain reduced serum cholesterol concentration by 3.2 percent. Since every 1 percent reduction in serum cholesterol concentration is associated with an estimated 2 to 3 percent reduction in risk for coronary heart disease [CHD], regular intake of fermented milk containing an appropriate strain of L. acidophilus has the potential of reducing risk for [CHD] by 6 to 10 percent.” Dash said his company’s DDS Probiotics contain DDS-1 L. acidophilus, “which has been researched and demonstrated to show cholesterol-lowering effect.” Psyllium. “Internal cleansing is very important” in maintaining healthy cholesterol levels, “especially if you do it with a lot of fiber,” said Sunil Kohli, vice president of Chino, CA-based Health Plus, Inc. The cholesterol-managing ability of fiber in general and psyllium in particular is “very well-established,” he said. However, Kohli said, “It will probably do you no good if it’s random. It should be done on a regular basis, and it should be supervised. Consulting the doctor or pharmacist is important.” Soy. The protein in soy “has evidence of lowering total cholesterol and LDL cholesterol, based on reviews of studies using over 20 g of soy protein per day,” said Levin. “Soy isoflavones are considered only partly responsible for this effect.” Sytrinol. A patented proprietary formula derived from natural citrus and palm fruit extracts and containing citrus polymethoxylated flavones and palm tocotrienols, Sytrinol has been shown in clinical trials to improve total cholesterol, LDL cholesterol, and triglycerides by up to 30 percent, 27 percent, and 33 percent, respectively. Having just wrapped up Phase III of a long-term trial of Sytrinol, Chicago-based SourceOne Global Partners, which owns the exclusive worldwide license for intellectual property associated with the ingredient, is commencing a study that combines Sytrinol with plant sterols. Tocotrienols. On its Website discussing the science and benefits of tocotrienols (www.tocotrienol.org), ingredient supplier Carotech Inc. (Edison, NJ) identifies several benefits for blood lipid levels. Tocotrienols, according to the Website, have been shown to “inhibit cholesterol production in the liver, thereby lowering total blood cholesterol;” “[suppress] hepatic HMG-CoA reductase activity [and result in] the lowering of LDL cholesterol levels;” and “inhibit cholesterogenesis by suppressing HMG-CoA reductase.”
New WeaponsThere are also nutrients that are emerging as potential weapons in the fight against cholesterol. Levin cited rice bran oil, resveratrol, pantethine, l-carnitine, and niacin as showing promise. With all of this, Levin said, it’s important for retailers to remember that “they are not allowed to discuss diseases and remedies unless there is an approved FDA health claim allowed on the label, as with soy protein and plant sterols. What is allowed are structure-function claims such as ‘cholesterol support,’ ‘promoting normal, healthy circulation,’ ‘homocysteine regulators,’ etc.” Supplementation is only one tool for managing cholesterol levels, manufacturers pointed out. “Besides nutrition, lifestyle is a key to controlling cholesterol,” Levin said. “Eating a variety of antioxidant-rich foods will prevent the liver from churning out cholesterol as a ‘cheap’ antioxidant.
