Recent research has shed new light on dietary factors that can help to promote acne symptoms, if not being the sole cause of them. Before discussing that, it is important to understand why acne gives rise to the symptoms that it does: lesions in the form of whiteheads and blackheads, pustules and cysts. While not necessary to discuss the biochemical details, the part that your diet has to play will not be understood without considering the effect of hormones on acne. The reason that teenagers in particular appear to be more prone to acne is that an increased production of hormones has an effect on the condition that causes the symptoms of acne. Fundamentally, acne is characterized by the infection and inflammation of a mass of oil and dead skin cells within the pores of the skin, particularly on the face, neck, chest buttocks and back. In order to determine how diet and acne are connected, it would be necessary to determine what components of our diet can either stimulate sebum production, or stimulate androgen secretion. If no such link could be found, then it would be fair to descry any connection between acne and the food you eat. However, there is a connection, and it is a positive one. In addition to their main function, insulin and a hormone known as IGF-1 (Insulin-like Growth Factor 1 that helps promote growth in children) promote the secretion of testosterone, a male hormone or androgen. Knowing, as we now do, that androgens promote the secretion of sebum, then anything that increases the levels of insulin or IGF-1 within the body will also lead to sebum production and hence to acne. What that infers is that any foodstuffs that increase the insulin levels in the blood can also lead to acne. This inference is supported in many ways. For example, it has been found that while drinking milk promotes a greater risk of acne, eating yoghurt does not. Why is this? It is known that milk can increase insulin levels because of its high sugar content. The effect of bacterial activity to produce yoghurt reduces the amount of sugars in the milk it is made from because the bacteria live on the lactose. The same argument applies to cheese, which promotes lower insulin levels than milk, if not as low as yoghurt. This being the case, then a diet low in sugars and carbohydrates should reduce the incidence of acne generally. Recent research has indicated that a diet high in refined carbohydrates and sugar increases both IGF-1 and insulin levels in the blood. This then creates a surge in male hormones which in turn leads to excessive sebum secretion and intermittent shedding of skin cells and so on to the growth of bacteria and acne. It is a logical progression, supported both by theory and by observation. So how should a person with a propensity for acne change their diet? Switch to fruits, vegetables and grains. Non-fatty meats are also acceptable, and .lots of fish and other seafood. Studies have concluded that diets rich in seafood lead to very low acne rates. The Japanese and coastal Chinese suffer very little acne in comparison with those taking a Western diet, particularly an American diet. One of the reasons for this is that omega-3 fatty acids help to reduce both inflammation and sebum production. The same is true of green tea that contains antioxidants that reduce the blood levels of dihydotetosterone and hence of sebum secretion by the sebaceous glands. We could go on, and list supplement after supplement that contain antioxidants and other substances that can reduce the production of sebum and hence of the symptoms of acne. Vegetable oils, on the other hand, with their high omega-6 fatty acid content, can drive up sebum production and the activity of the immune system and the inflammatory response. Among these are a number of flavonoids in addition to icariin, sterols and the isoquinoline alkaloid magnaflorine, that possesses antioxidant properties and reduces LDL cholesterol. Diindolylmethane is an indole contained in brassicas such as broccoli, cabbage, kale, brussel sprouts and cauliflower. It is a dimer of indole-3-carbinol, which is itself formed by the autolytic degradation of glucosinolate glucobrassicin which is found in brassicas. Both this and the dimeric diindolylmethane are currently being tested for their antioxidant, anticancer and antiatherogenic properties, although this type of research has been mainly made on animals. When the cell walls of brassicas are ruptured, the enzyme myrosinase is released, that breaks down the glucobrassicin. Because diindolylmethane is oil soluble it is more bioavailable to the body in the presence of vitamin E and other lipophilic substances, and these are normally included in oral supplements. However, back to estrogen and its effects upon human estrogen balance. Estrogen is the name for a group of steroids that function substantially as the female sex hormone. They have many effects on the body, including regulation of the menstrual cycle, breast development and promote the formation of secondary female characteristics such as reduced height and muscle mass. Estrogen is also present in the male, where it regulates several factors pertaining to the reproductive system. It does not, however, take part in the process that determines Sexual desire. The substance has been found to have a positive effect on the balance between estrogen and progesterone, although eating broccoli will not be sufficient to do the job properly. Insufficient DIM is released by breaking the cell walls of broccoli through chewing to have much effect on the balance, about a kilo of broccoli a day being needed to have any noticeable effect. It is the diindolylmethane supplements that are able to have the desired effect in restoring the hormone balance and eliminating the undesirable effects of estrogen imbalance. But why should estrogen imbalance occur? What are the factors that cause this dangerous condition, and if it is so common how did our ancestors manage without today's science? The answer to the second question lies in that to the first, but before we discuss that don't go thinking that your ancestors managed through thick and thin. Perhaps yours did, but your life expectancy is significantly greater than that of your parents when they were born. In fact, you don't have to go too far up your family tree to when you would be doing well if you hit forty! So the argument about your ancestors doesn't wash. In fact, an estrogen imbalance can be caused by several factors: a slow estrogen metabolism that leads to an excess of estrogen because it is not broken down quickly enough is common, but far more common is the effect of pollutants such as pesticides, car exhaust fumes, emulsifiers, household cleaners and even some soaps. They contain chemicals known as xenoestrogens that our body believes to be real estrogens. They can stick to estrogen receptors on healthy cells, and can also prevent true estrogen from binding to the cell it should bind to. This results in an increase in free estrogen that cannot bind to the estrogen receptors that it should. Your ancestors did not have these problems with such 'estrogen mimics' and so managed just fine. There is also the fact that estrogen can be broken down in the liver by means of two different routes: one leads to a form of estrogen that metabolizes to provide many benefits, such as strong bones and a reduction in heart disease, while the other has the opposite effect of causing cancers, depression, osteoporosis and problems with your immune system. That is by no means the full list: you don't want to see that, believe me! You might think that if estrogen is dominant over progesterone, then taking progesterone would resolve the problem. It would seem to be a good cure. In fact, it is not a good idea since progesterone can be metabolized into yet more estrogen, making the problem even worse. The real answer is to take a supplement containing the substance that solves the problem by changing the estrogen metabolism pathways in the liver: diindolylmethane. More of the so-called "good" estrogen metabolites are formed, and less of the bad. The actual estrogen levels are not changed, but the way in which the excess estrogen causes harm is changed. That way, it doesn't matter what the imbalance is, the estrogen cannot do ham to your body. DIM possesses many potential positive benefits when used as a supplement. It can reduce your chances of getting breast, cervical and ovarian cancers, and also acts as a strong antioxidant, and helps to strengthen your immune system. One group of compounds in maca is the isothiocyanates-aromatics constituents that are responsible for the "hotness" of mustards and radishes - fellow members of the brassica family. Isothiocyanates from other members of the brassica family may reduce the risk of breast and stomach cancer. Although the same constituents specifically from maca haven't beentested, it's plausible that they could have the same effects. Q. I've heard a little about guarana extracts - is it just caffeine? A. Guarana is widely loved for its mild stimulating effect, which is due to caffeine. But this is by no means this Amazonian herb's sole beneficial compound. Guarana (Paullinia cupana) is so logically ingrained in the culture of Brazil that it's practically a rival (actually out-sells) Coca-Cola in its soft-drink form. Like many other indigenous herbs, guarana was in use locally well before European settlement. Its Latin name comes fromthe German botanist C.F. Paullini, who first encountered the herb in the 1700s. This evergreen vine typically climbs fairly far up the Brazilian forest trees. The seed is the part that gets used. In one clinical study, guarana boosted the memory alertness of participants, even when the caffeine level per dose was a low 9 mg., as compared with approximately 100 mg for a cup of coffee. This effect suggests that other agents than caffeine contribute to a feeling of well being. Today, a man age 65 is just as likely to be found hiking in the hills, running in a marathon, or even dancing in the streets than rocking in that proverbial front porch rocker. Because it’s becoming more and more evident that the older a man gets, the healthier that man has been. Eating healthy, exercising, and kicking harmful habits (like smoking) can add years to a man’s life. Aging research is proving over and over again, that we can prevent and delay heart disease, cancer, diabetes, and Alzheimer’s disease – the major causes of disability and death in men over 50. Now, it’s very true that good clean living from early on is preferable to sixty five years of bad habits and five years of good. But it’s also true that it’s never too late for men to make changes and vow to take better care of themselves. And one of the easiest and most effective ways men can improve their health is the addition of high quality nutritional supplements. The best foundation supplement is a high quality multivitamin. Research is repeatedly finding that even very healthy men who take daily multivitamins can significantly improve their health. In fact, an article published in the Journal of the American Medical Association (JAMA) recommends that all adult Americans take a vitamin supplement. Look for solid doses of vitamin supplement. Look for sol doses of vitamins and especially minerals. Multivitamins designed to be taken once a day are often woefully deficient in calcium, magnesium, and potassium. The only mineral a man should avoid is supplemental iron. Iron should only be in formulas for women prior to menopause. The prostate gland is about the size of a walnut and is located below the bladder and in front of the rectum. It wraps around the upper part of the urethra and its primary job is the production and storage of semen, the milky fluid that nourishes sperm. BPH is one of the most common health conditions in older men. Half of all men aged 40-60 and more than 90 percent in men over 80 have BPH. BPH causes the prostate gland to enlarge, putting pressure on the urethra. Men have trouble starting or maintaining a stream of urine, find they can’t completely empty their bladders, and have to urinate frequently, even during the night. They may also have episodes of uncontrollable dribbling or complete loss of urine. BPH is caused by the conversion of estrogen to a very potent form of testosterone called, dihydrotestosterone (or DHT). When prostate cells are exposed to DHT, they multiply in number and get much larger. BPH rarely improves. It most often remains the same for years or gets gradually worse. The need to continually urinate, interrupted sleep, dribbling, and loss of urine can significantly interfere with a man’s quality of life. Prescription medications that have been developed to treat BPH are only partially effective. Conclusion No man has the power to stop the passage of time. But every man has the power to make aging more healthy and less harmful. Vitality 101
More and more Americans are feeling overworked, overtired, and overcome by life’s demands. We just do not have the energy we need to meet our responsibilities to the people we care about. More importantly, we don’t even have the energy to have fun! It seems that a constant feeling of fatigue has become part of the American way of life.
Research has shown that the same processes that cause lack of energy can rob us of sleep, saddle us with excess weight, disrupt our hormonal balance, and create significant amounts of stress in our daily lives. Chronic stress can dramatically contribute to fatigue, sleep disorders, irritability, and anxiety. The research simply confirms what most of us already know – uncomfortable stress can really wear us out mentally and physically! It can take away the satisfaction of a job well done. It can take away our ability to believe in ourselves. And, sadly and maybe most importantly, continual stress can take the fun and joy out of life.
In a separate issue of Ask the Doctor, we discuss the energy and sleeping needs of people suffering from chronic fatigue syndrome and fibromyalgia. In this issue, I discuss the 3-step process I call “Vitality 101.” People do not have to accept pain, insomnia, or fatigue. It’s time for everyone to feel great and have a life they love!
Step 1 – Nutrition
Good overall nutrition is important for everyone! As a foundation product to support energy levels, a powdered drink mix is a pleasant, easy way to ensure that you are taking all of the necessary vitamins, minerals, and amino acids that you need to have great energy all through your day.

The same can be said of a chronic stress response. The difference between the two is that the body’s response in an emergency starts and resolves itself quickly. The response to being stuck in traffic may not.
The body makes the “stress hormone”, cortisol, to handle the normal stresses of day-to-day living. But in an emergency situation, the adrenal glands, located above the kidneys, secrete increased amounts of this hormone until the emergency passes. Then the body returns to its normal function. Unfortunately, however, chronic stress is more complex. When our body is subjected to increased amounts of the hormone, cortisol, for an extended time, it can lead to a condition known as “adrenal burnout” or “adrenal fatigue.” While it’s true that very large amounts of cortisol can have damaging effects on our hormones, too little cortisol doesn’t allow us to respond to stress properly. It’s really a matter of balance.
Q. How can I control the stress in my life and re-energize?
A. Many people who are under constant stress may have adrenal burnout. Adrenal burnout occurs when the adrenal glands are constantly producing cortisol in response to chronic stress. Over time, this exhausts the adrenal reserve, meaning the adrenal gland can no longer increase cortisol production in response to stress.
The good news is that changes in our hormone levels can return to normal when stress is decreased. The key in learning how to deal with daily stress is to allow the body to return to its normal state. I discuss additional techniques for coping with stress in my recent book Three Steps to Happiness! Healing Through Joy (see my website, www. jacobeitelbaum .com, for more information). In addition to stress control, it is important to supplement your adrenals with a glandular therapy regimen to ensure healthy cortisol levels and adrenal function.
