L-Arginine is an amino acid that is one of 20 needed by the body for its existence. To some, it is not what is known as an essential amino acid, since it can be biosynthesized by the body, but arginine is termed a conditionally essential amino acid in that we must include some in our diet because our biochemistry does not produce all that our body needs, particularly during the growing years. Amino acids are the building blocks of life, and are the units from which proteins and ultimately our DNA are built. When a supply of a particular protein is needed, the DNA template provides the sequence of amino acids needed to produce it. Without an adequate supply of essential components, we cannot survive, and if the essential amino acids are depleted in our diet then the body will break down muscle tissue to release them. Although L-arginine is termed a 'conditionally' essential amino acid, it is included by many among the 10 regarded as being essential. Hence, depending upon who you read, it can be either essential or non-essential. It also appears to promote the absorption of many antibiotics, which again helps in recovery. Bromelain is relatively safe to use with few side effects, although, curiously, among the side effects are some conditions it is also used to treat. Bromelain has been proposed for cancer therapy, its potential use being recommended due to its effect of the adhesion of cells, its regulation of the immune function and its effect on the immunosuppressive cytokine TGF-beta that is involved in several types of cancer and their metastasis (spread to other parts of the body). However, a lot more work is needed on this for definite conclusions to be drawn. On a more practical note, the effect of bromelain on proteins is put to use as a steak tenderizer. If the product is sprinkled in powdered form onto meat, and then forked into the tissue, the enzymes will break down the protein of the meat and make it tender when cooked. All in all, bromelain is a useful supplement for many medical conditions, and does to food in your stomach what it does to steak on the plate. You are probably best not to take if you are pregnant since it could cause you to abort. Otherwise, however, it has been shown to be effective in many severe cases. Feverfew is bitter, and can be used as a tea with other herbs, such as rosemary, lavender and sage. If you are taking warfarin, aspirin or other blood-thinning drugs, see your doctor first, and the same is true of NSAIDs (non-steroidal anti-inflammatory drugs). If you are taking these for arthritis, the feverfew itself might be more effective! If you are taking feverfew as a supplement using commercially available dried leaf then the recommended dosage is around 125mg of the dried leaf. It should contain at least 0.2% of parthenolide (0.25mg/daily), although other studies have indicated that a total of 0.7 mg parthenolide a day should be taken (100mg at 0.7%). Parthenolide is unstable so don’t take it after its sell by or best by date. BUTTERBUR Butterbur is also useful in the treatment of migraines. The active ingredient here is petasin, a sesquiterpene ester that also has anti-inflammatory properties, all though it works in a different manner to feverfew. Here, it inhibits the activities of leukotriene generation in neutrophils and eosinophils, which indicates that it also possesses anti-allergenic properties that could help asthmatics that also suffer from migraines. It does not prevent the release of histamine, but does appear to reduce smooth muscle spasm. You should never take the raw herb, only the extract, because it contains alkaloids that can harm you. The butterbur extract, however, is a perfectly safe supplement and found effective by many people in the treatment of migraine. GINGER If your migraine is at the front of the head, then you should try ginger. Unlike feverfew, ginger works on a migraine as it is occurring, and improvement can be achieved for a bad migraine within half an hour. It works simply by rubbing the leaves on the area when you have a fall or a hard knock. The active ingredients here are again flavonoids, and sesquiterpene lactones along with tannins, carotenoids and thymol. These, along with the flavonoids, stimulate the circulation and carry away any fluids trapped in bruises and swellings. The sesquiterpene lactones act as anti-inflammatories and boost the immune system, helping to reduce swelling and pain. In fact terpenoid chemicals are common to many of the herbs and flowers that have found a use in the relief of pain in swelling and bruises. The same is true of Ledum, better known as Rosemary, traditionally used for the treatment of burns, ulcers dandruff, and dry skin and to get rid of lice among many other internal and topical applications. Considering that they are among the most common antioxidants and anti-inflammatory agents in the plant world; it is no coincidence that flavonoids just happen to be contained in the vast majority of natural treatments for scratches, grazes and bruises. They reduce swelling, pain and inflammation, and also act as antiseptics by disrupting the cell walls of bacteria. Hypericum is well known by its alternative name St. John’s Wort, where it is used in the treatment of depression. However, the active ingredients here are mainly hyperforin and hypericin, which have little to do with the topical benefits of the plant. If you have suffered from insect bites and stings, then you would have been thankful had you brought some Apis Mellifica with you. This is a fast paced world. We are all busy; living our full lives, burning the candle at both ends. We all get tired. We all get sick from time to time and maybe even depressed. But the illness called chronic fatigue syndrome is not like the normal ups and downs that we experience in everyday life. People with chronic fatigue syndrome feel overwhelming fatigue, and often pain as well. This is an illness that does not go away with a few good nights’ sleep. It drags on and on and doesn’t resolve itself. It steals vigor and energy over months, and sometimes even years. In this issue of Ask the Doctor, we will talk about powerful vitamins, minerals, amino acids, and herbs combined in scientifically validated formulas that people with chronic fatigue syndrome or fibromyalgia can use every day. These nutrients help address some root problems of chronic fatigue syndrome and fibromyalgia by restoring energy and health to sufferers. Q. What is chronic fatigue syndrome? A. FMS is basically a sleep disorder characterized by many tender knots in the muscles. These tender knots, called tender or trigger points, are a major cause of the achiness that people with fibromyalgia and CFS feel. Approximately 80 percent of chronic fatigue syndrome patients have received and overlapping diagnosis of fibromyalgia syndrome. For most people, fibromyalgia and chronic fatigue syndrome are the same illness. Q. What causes chronic fatigue syndrome? A. There are many causes that can trigger CFS. Current research is looking at the roles of neuroendocrine dysfunction, viruses, environmental toxins, genetic predisposition, food sensitivities, yeast overgrowth, faulty digestion, or a combination of these factors. For many people, CFS is triggered by a bout with a viral illness (like a cold or the flu), or even a stressful event. CFS is usually a mix of underlying causes. It is like a domino effect in that each problem can trigger another problem, and so on. For example, fatigue and poor sleep can trigger a weakened immune system, which can, in turn, trigger yeast or bacterial infections. Q. Who gets chronic fatigue syndrome? A. CFS is more common than you might expect. It strikes people of all ages, racial, ethnic, and socioeconomic groups. Approximately 800,000 people nationwide have CFS and over six million have fibromyalgia at any given time. It is important to stress that CFS is a real illness; it is not “just in your head.” Unfortunately, sufferers of CFS may find that many healthcare practitioners discount the symptoms of this illness or misdiagnose it as another disease. This can lead to additional emotional suffering. Q. How long does chronic fatigue syndrome last? A. The illness varies greatly in its duration. Some people recover after a year or two. More often, those who recover are more likely to do so three to five years after onset. Yet for some people, the illness seems to simply persist. There are rare cases of spontaneous improvement after five years without undergoing any treatment. However, this is very unusual. Q. What are the complications of chronic fatigue syndrome? A. The patterns of CFS vary from individual to individual. However, many common patterns of symptoms are seen in CFS sufferers. These symptoms and problems interact and create new symptoms and problems. For example, infections and disrupted sleep can lead to digestive, hormone, and immune problems. Infections The most notorious pattern seen in CFS is the one in which a person suddenly comes down with a flu-like illness that doesn’t go away. These viral or bacterial infections can suppress the body’s master gland, the hypothalamus. Since the hypothalamus controls the other glands, including the adrenals, ovaries, testes, and thyroid, suppression of this gland will lead to a subtle but debilitating decrease in the functioning of all glands and their hormones. Suppressed hypothalamic function from chronic infections can then trigger sleep dysfunction. Disrupted Sleep The suppression of the hypothalamus gland can lead to poor sleep because the body confuses its day/night cycles. Because of this, people with CFS have trouble staying in the deep, restorative stages of sleep that “recharge their batteries.” Poor sleep can cause immune suppression, which may lead to secondary bowel infections. The bowel infections seen in people with CFS can cause decreased absorption of nutrients, which can lead to chronic vitamin and mineral deficiencies. Q. Is there a cure for chronic fatigue syndrome? A. Treating chronic fatigue syndrome presents a significant challenge to people with CFS and their healthcare practitioners. Recently, a published placebo-controlled study ( of which I was the lead investigator) showed that when using an integrated treatment approach, over 85 percent of CFS and fibromyalgia patients can improve, often dramatically. An editorial in the April 2002 issue of the Journal of the American Academy of Pain Management noted that this treatment, which I developed, is now a highly effective and excellent part of the standard of practice for treatment of fibromyalgia. Since this treatment addresses many different problems associated with CFS/FMS, it needs to be individualized to each patient. Medical Treatments Medications that provide symptom relief are frequently the first line of treatment chosen by healthcare practitioners for the person with CFS. These include medications for pain, sleep disturbances; digestive problems such as nausea, depression and anxiety, and flu-like symptoms. However, medications have not been universally successful because they tent to put a bandage on symptoms instead of addressing the root problems. Because of this, medications may need to be supplemented by the other supportive therapies that can address the root problems. Supportive Treatments People with CFS/FMS may be depressed, given the catastrophic lifestyle disruption these diseases may cause. They may also feel guilt and frustration because their symptoms were not taken seriously for such a long time. Fear can be a factor as employment and family relationships may be jeopardized by this illness. Decrease Occasional Constipation Constipation is defined as infrequent or difficult defection that can result from decreased motility of the intestines. It is a common problem, particularly in older adults. When the feces remain in the large intestine for prolonged periods, there is excessive water absorption, making the feces dry and hard.7 Insufficient amounts of lactobacillus acidophilus and Bifidobacterium longum may play a role in delayed bowel movement, which can favor the activity of undesireable putrefactive bacteria (bacteria that break down organic matter into potentially harmful compounds) in the intestines. Lactic acid bacteria contribute to the re-establishemtn of healthy intestinal flora (at the expense of pathogen growth) and stimulation of intestinal peristalsis via lactic acid. 10 Support of putrefactive Processes When unbalanced conditions are present in the intestines (ie, unbalanced diet, high acidity, and/or low levels of lactic acid bacteria), organic matter may be putrified (decomposed or rotting) by certain bacteria and produce harmful substances such as ammonia.1,11 Micro flora or the large intestines promote homeostasis (balance) in both the intestine and the vagina.1,12,13 these activities are carried out via support of direct production of antibodies, competition with adhesion to intestinal cells, or indirect modulation of the immune system.2 Support Digestion Normal micro flora of the large intestine help support and complete digestion via fermentation.1,11 The risk of diarrhea, for example, increases when the capacity of colonic micro flora to ferment carbohydrates decreases.8 Oral ingestion of probiotics produces a stabilizing effect on the guy flora.2 Additional Benefits The benefits of probiotics extend beyond digestion support and immune support. Borage oil, extracted from the seed of the blue, star shaped borage flower, is gaining much attention by alternative health practitioners and main stream medicine alike for its profound medicinal properties. Whereas the oil is getting all the redit, it is actually the oil’s active component, gamma linoleic acid (GLA), which has drawn the interest of researchers. The majority of the early studies done on GLA, dating back to the late 1940’s, were conducted with the oil of evening primrose. For rezones cited in the title, more bank for the buck, borage seed oil is now thought to be s superior source of GLA compared to evening primrose oil. Other plants forms and food concentrates that contain appreciable amounts of GLA include black currant seed oil and spirulina. Why GLA Supplementation? A body with healthy biochemistry has the ability to produce GLA from the most essential fat linoleic acid. Linoleic acid is found most abundantly in the omega 6 family of oils, including safflower, sunflower, and corn oil. While it has been estimated that a majority of Americans and Europeans consume far to many omega 6 fatty acids in proportion to the beneficial omega 3 fatty acids (by approximately 10:1). Most biochemists agree optimal ratio of omega 6 to omega 3 fatty acids should be approximately 1:1. A prime example is the hydrogenation process utilized to convert liquid polyunsaturated oils into semi-solid, altered saturated fats for use as margarine and commercially processed foods. Hydrogenation is accomplished by subjecting the oils to temperatures in excess of 250 C, and bombarding them with hydrogen ions in the presence of the heavy metal nickel, which irreversibly changes the chemical structure of the fatty acid molecule from a healthful “cis” configuration to a dangerous “trans” configuration. When these products are eaten in excess they are known, in some cases, to block the enzymatic conversion of linoleic acid to GLA. In addition to the damaging effects of hydrogenation, certain essential fatty acid nutrient cofactor deficiencies may exist to further complicate the conversion. Vitamins pro-A, A, C, E, B-2, B-3, B-6, pantothenic acid, B-12 biotin and the minerals calcium, magnesium, potassium, sulfur and zinc are all involved in essential fatty acid (EFA) metabolism. Particular interest should be given to those afflicted with diabetes, cystic fibrosis, asthma, alcoholism, multiple sclerosis. For a myriad of reasons, including cofactor deficiencies, these individuals lack the enzymatic ability to convert linoleic acid to GLA and must obtain a direct source of GLA for proper hormonal regulation. In addition, excessive consumption of animal fats containing acachidonic acid competes for the same metabolic pathways occupied by GLA, thus minimizing its biological action. These potential negating effects can simply be diverted to consuming a food source or supplement containing GLA which bypasses any previously necessary enzymatic conversion and floods the metabolic pathways with beneficial GLA. Nutrient deficiency should also be addressed. Here lies the true power of GLA. Prostaglandins, biochemical regulators Whereas GLA is the power, the prostaglandins deliver the punch in this biological equation. Just as linoleic acid is normally converted to GLA, GLA is further converted into the prostaglandin E1 (PGE1). Many of the benefits derived from GLA supplementation are a result of the hormonal regulatory action of the prostaglandins. Just like the “parent” precursors they are made from, the hormone-like prostaglandins orchestrate a host of important biochemical activities. Washington—Jointly, the Council for Responsible Nutrition (CRN), American Herbal Products Association (AHPA) and National Nutritional foods association (NNFA) issued a statement in November reaffirming and calling for responsible use and marketing of dietary supplements, and encouraged consumers and retailers to avoid supplements purporting to prevent or treat the avian flu. “The dietary supplement industry is keenly aware of public concern regarding avian flu and of the desire of the public to protect itself against this recent health epidemic, which can cause serious illness and, ultimately, death,” the statement read. “We do not believe any dietary supplements have been specifically shown to prevent or treat avian flu.” The trade coalition futher issued the following unified guidance for marketers, retailers and consumers:

In addition to helping decrease neural tube defects,1 folic acid can also help treat inflammatory bowel disease 2 improve memory 3 and help decrease an amino acid in the body, homocysteine,4 that increases heart disease risk.5 Now a new study 6 has found another way that folic acid can help us age more gracefully: by helping strengthen our immune system. Recognizing the importance of nutrition in the overall health of the immune system 7 and knowing that certain types of immune system cells, called “T cells”, decrease with age,(8,9) researchers fed 11-month-old and 23-month-old male rats either a control diet or a diet fortified with 35.7 mg per kg of folic acid for three weeks. Researchers found “a significant” increase in immune system strength in the folic acid group, specifically that of increased T cell levels, other immune system proteins called IL-2, IL-4, and anti-cancer proteins called “tumor necrosis factor”. A. Treating chronic fatigue syndrome presents a significant challenge to people with CFS and their healthcare practitioners. Recently, a published placebo-controlled study (of which I was the lead investigator) showed that when using an integrated treatment approach, over 85 percent of CFS and fibromyalgia patients can improve, often dramatically. An editorial in the April 2002 issue of the Journal of the American Academy of Pain Management noted that this treatment, which I developed, is now a highly effective and excellent part of the standard of practice for treatment of fibromyalgia. Since this treatment addresses many different problems associated with CFS/FMS, it needs to be individualized to each patient. Medical Treatments Medications that provide symptom relief are frequently the first line of treatment chosen by healthcare practitioners for the person with CFS. These include medications for the pain, sleep disturbances’ digestive problems such as nausea, depression and anxiety, and flu-like symptoms. However, medications have not been universally successful because they tend to put a bandage on symptoms instead of addressing the root problems. Because of this, medications may need to be supplemented by the other supportive therapies that can address the root problems. Supportive Treatments People with CFS? FMS may be depressed, given the catastrophic lifestyle disruption these diseases may cause. They may also feel guilt and frustration because their symptoms were not taken seriously for such a long time.
