This type of cartilage, known as articular cartilage, is bathed in a lubricating fluid known as synovial fluid, which introduces its own problems when your cartilage becomes damaged.

This damage can occur in several ways: as the result of a fall, for example, or direct contact with the joint when playing a physical contact sport such as football or soccer. It can also become damaged through wear and tear over a period of time, such with long distance runners or soccer players (again), and is also associated with age. Many years of continual use, especially amongst those with active rather than sedentary occupations, eventually lead to wear and damage.

Problems with the joint structure itself, known as osteoarthritis, can also damage the cartilage, as can being overweight for a lengthy period. You can also experience cartilage damage if you are bedridden or other wise immobile for long periods, because the cartilage needs regular movement to function correctly. This is connected with the blood supply, which will be discussed shortly.

Cartilage is constructed of cells known as chondrocytes that generate a fibrous matrix known as collagen, a mixture of amino acids known as elastin that allows the cartilage to return to its original shape after deformation, and non-collagenous matrix tissue containing proteins, water and proteoglycans that contain sulfated glycosaminoglycan chains. That last mixture is often referred to as ‘ground substance.’

One of the problems with cartilage is its lack of a direct blood supply, and it relies on the compression and decompression of the articular cartilage, or on the flexing of elastic cartilage, to create a pumping action that drives blood to the chondrocytes. This is why inactivity can cause cartilage damage, due to a lack of blood supply, and why it is repaired more slowly than other body components.

Once an injury or wear and tear damages a joint, the body’s natural defense, the immune system, is activated, and the major part of that involved in cartilage damage is the inflammatory response. The joint becomes inflamed, the quantity of synovial fluid is increased to provide more protection and swells the joint, and enzymes (hyaluronidase) are produced which, although part of the natural defense system, actually degrade the synovial fluid and the cartilage.

This increases the amount of inflammation and the process becomes self-perpetuating, leading to the condition known as degenerative joint disease (DJD) because the body is unable to produce enough glucosamine to generate the proteoglycan needed for repair.

This is where glucosamine sulfate enters the scene. Glucosamine is a precursor for glycosaminoglycans (GAG), which as mentioned as above are components of proteoglycans in the cartilage matrix ground tissue. It has been shown to stimulate the biosynthesis of proteoglycan, and analysis has shown its presence within articular cartilage after administering it orally to patients with cartilage disease. It therefore makes its way to the right place.

Glucosamine is administered in the form of glucosamine sulfate, the highly electrically charged sulfate groups believed to aid in the compression properties of cartilage. It is rapidly absorbed into the bloodstream, although only about a quarter of the oral dose is eventually available to the body, and high concentrations accumulate in the liver, kidneys and in articular cartilage where it is used in the biosynthesis of GAG.

When in solution, glucosamine sulfate separates into ions: sulfate and glucosamine. Glucosamine ions are involved in the synthesis of GAG, that then combine with proteins to form proteoglycans, a component of the non-collagenous matrix of the cartilage. Although glucosamine is the major active component, there is evidence that the sulfate group contributes the stability of the matrix of the connective tissue since the uptake of sulfate ions increases with the amount of glucosamine sulfate used.

Another consideration here is that sulfate is an important part of proteoglycans, and glucosamine sulfate promotes not only the synthesis of glycosaminoglycans, but also of proteoglycans in general. Glucosamine is also active in regenerating the lubricating properties of the synovial fluid, and in hindering the activity of hyaluronidase, the enzyme that breaks down the hyaluronic acid in the synovial fluid.

Some people find that glucosamine, taken either alone or in conjunction with chondroitin sulfate and/or methyl sulfonyl methane (MSM), is more effective than the non-steroidal anti-inflammatory drugs (NSAIDs) used to reduce inflammation (e.g. Aspirin and Ibuprofen) and without the side effects of these substances. MSM contains dietary sulfur, which is necessary for cell structure and healthy cell repair. Methyl sulfone methane is know to be beneficial for painful conditions such as arthritis, and also improves the blood circulation. Gamma Oryzanol is extracted from rice bran oil, and is a mixture of substances that includes ferulic acid and sterols. It is not restricted to rice barn oil, and is also found in the bran of other grains, and some fruits and vegetables. It is commonly used as a sports supplement, although possesses other uses including treatment of menopausal symptoms and high cholesterol levels.

Athletes use gamma oryzanol to increase their muscle bulk through it increasing the levels of testosterone and other anabolic hormones. The effect of gamma oryzanol on cholesterol levels has been demonstrated, and complement the same effect offered by the fatty acid component of the bran oil. It appears to prevent the absorption of cholesterol by the digestive tract, and so allow it to be excreted naturally before doing any harm. It is believed that the phytosterols present in rice bran oil block the cholesterol absorption sites in the intestine, so is must continue down the intestinal canal until it is evacuated.

Cholesterol itself is essential to human metabolism and biochemistry, and without it we could not survive. Cholesterol is not soluble in water, and it has to be bound to low density lipids (LDL) to enable it to be transported round the blood to where it is needed: usually in the arteries to heal up arterial damage, a bit like a sticking plaster.

It also acts as an anti-inflammatory, specifically in the stomach and can be used to treat gastritis, in that it reduces the inflammation of the stomach lining. There is some evidence from studies on animals that the substance could be effective in treating gastric ulcers, although the results with animals have not yet been tried on human subjects. Another mechanism, other than the anti-inflammatory route, is through the normalization of the secretion of the gastric juices.

Another use to which gamma oryzanol has been successful put is in the treatment of menopause symptoms. This is another of those situations where some trials have proved unsuccessful, but those that use it has found it be effective. Discovered by Phoebus Levene in 1905, D-ribose is a monosaccharide, and is a component of RNA that is needed for genetic transcription. It is a stereoisomer of ribose, and although not regarded as an essential nutrient since it is synthesized in the body, it is essential for life. It also takes part in human metabolism, helping to generate energy from food, and is a component of all living cells – animal or vegetable. It is contained in ATP and NADH and we could not live without it.

Because it is contained in all living organisms, D-ribose is a common component of the human diet, and required as a supplement only for specific needs. It for this reason that the human species developed and survived: the chemicals needed for life are either commonly found in nature or are manufactured by the body itself from other raw materials which themselves are common in our diet. The other stereoisomer is L-ribose, and this plays no part in human metabolism.

Energy is needed not only to enable us to walk and to run, but also to drive every part of human physiological activity, and all of the physical and chemical processes of life. Because of its nature energy cannot travel round the body in our bloodstream as if were a chemical entity, but must be released or generated where and when it is needed. For that reason every cell in our body contains an area called the mitochondria in which energy is generated. The molecule of energy, if there is such a thing, is called ATP, or adenosine triphosphate.

ATP controls the production of energy everywhere in the body, and allows muscles to contract and relax. This muscular activity is responsible for all movement, including the circulation of our blood through heart contractions, the diaphragm movement that creates the vacuum that allows us to breathe, and the operation of the digestive system by means of the peristaltic motion of the esophagus and the intestines. Each of the two ways in which ATP is generated involves D-ribose.

One is by means of D-ribose being used to produce new ATP, and the other is the recycling of used ATP. In the latter, D-ribose and ATP react to form ADP (adenosine diphosphate and D-ribose-5-phosphate with the release of energy. This diphosphate then reacts again with the ADP to produce new ATP and D-ribose. The reaction is catalyzed by an enzyme known as a ribokinase, a phosphotransferase that specifically catalyzes reactions involving phosphorus groups with an alcohol receptor group.

The heart muscle is operated by means of the calcium pump that depends upon both calcium and ATP for its operation. When ADP and phosphate are created by the effect of calcium and ATP coming together, then energy is provided to the muscle fibers of the heart. ADP then reacts with the phosphate and D-ribose in the presence of magnesium to reform the ATP. ATP is therefore essential for the continuation of the pumping action of the heart that maintains life in all mammals.

D-ribose is also part of the base of RNA and DNA, without which there could be no life. Because the cardiac muscle is easily depleted of ATP, the presence of D-ribose is of extreme importance and it has been proved that cardiac failure begins with the reduction in energy levels of the heart muscle that can be brought about by a lack of ATP in the muscle cells.

An important property of D-ribose is that it is unchanged by the liver, which is the chemical plant of your body in which a large part of the biochemistry needed for life takes place. This means that D-ribose can be taken in the diet and pass through the liver virtually unchanged, and then be transported in the bloodstream to where it is needed for the maintenance of ATP levels. That is why D-ribose is frequently given to treat heart attacks: because it can rapidly replace lost ATP and enable the calcium pump to get to work and keep the heart beating at its normal strength. There is no warning signal that lets you know if you have a calcium deficiency. Actually, a calcium deficiency is usually undetectable until damage that is irreversible has already occurred. There are many natural sweeteners to choose from if you want to avoid sugar, but don’t want any of the artificial sweeteners over which there are a few questions. You can choose from xylitol, luo han, stevia and others, but before discussing these, let’s have a look at the problems with sugar, artificial sweeteners and the American sugar industry.

Sugar as most people know it originates either from sugar cane or sugar beet, though by far the biggest American industry is in the cane. There are many different types of sugar, though that obtainable from cane sugar is sucrose. Sucrose is a disaccharide and carbohydrate, stored by plants as a reserve energy source to be used when needed. Humans cannot directly use sucrose, and it is metabolized in the body to glucose which needs the hormone insulin to help convert it into energy.

Insulin is produced in the pancreas, and a lack of it, or the body’s failure to use it properly, is referred to as diabetes. There are two types of diabetes:

Type 1: A total lack or deficiency of insulin due to the pancreas producing insufficient quantities of insulin, or even none at all. This is often seen in young people and is generally cause by the immune system attacking the insulin-producing sells in the pancreas. The treatment for type 1 diabetes is to introduce insulin to the blood, normally by means of injections, plenty exercise and the adoption of a high carbohydrate low fat diet.

Type 2: This is strongly associated with obesity and weight, and is due either to insufficient insulin production by the pancreas (but not as deficient as for Type 1 diabetes) or an inability of the cells of the body to properly use insulin. Type 2 diabetes does not always require insulin injections, and can be treated by exercise, diet and weight control. However, there are occasions where insulin injections are also required. It tends to affect people older than those with Type 1 diabetes and 90% of cases are of this type.

Both types, however, are connected with an excess of glucose in the blood, into which most sugars are converted. A diet low in sucrose will go a long way towards helping people that suffer from either type of diabetes, and control of carbohydrate intake should include a reduction in the intake of sucrose in the form or beet or cane sugar. This accepted, then if you need a sweetener, a saccharide free natural sweetener would appear to be the logical choice.

You could opt for a synthetic sweetener, but they also have their problems. There is evidence that saccharin could be a carcinogen, and ‘aspartame’ disease is not a myth. The other artificial sweeteners also have sufficient questions that natural sweeteners would appear to be the obvious choice. But which? That is the question. Let’s have a look at some and check out their pros and cons.

The first is Stevia. This is a South American herb that is 400 times sweeter than sucrose and yet is very low in calories and does not affect diabetics. Used throughout most of the world, it has not been approved by the FDA as a food additive due mainly, it would appear, to lobbying by the American sugar corporations. This is understandable, since mass substitution of sugar by stevia in processed foods is entirely possible, but would cost the sugar corporations billions.

However, it is available to purchase from health food stores, and does not appear to possess the problems that aspartame does. Unlike that sweetener, it can be used in baking and cooking, and is ideal for diabetics and people suffering from yeast infections such as candida. Such infections are aggravated by sugar in the colon, since yeasts love sugar, but they cannot survive on stevia. The safety of stevia has been proven through hundreds of years of popular use without any problems.

However, there are others. Luo Han Guo is a sweet Chinese fruit of which extracts are marketed as a natural sweetener. The plant it comes from is the Momordica grosvenori, a member of the cucumber squash family that grows in the mountainous areas of southern China. The sweeteners it contains are called mogrosides that are terpene glycosides, of which there are five different forms, the main one being termed mogroside-5.

The extract is available in the form of a powder consisting of around 80% mogrosides, and possesses around 250% of the sweetness of sucrose. This, too, can be used in cooking; because it is stable to heat and contains about 2% of the calories of ordinary sugar (one half teaspoon is equivalent to 25 teaspoons sugar). Not only that but, like stevia, there are no known side effects. It has been used for many centuries in Chinese medicine in the treatment of gastrointestinal conditions and conditions of the respiratory tract. Unlike natural sweeteners, luo han has been found to be useful in helping to manage diabetes since it does not cause insulin levels to rise and are not involved in energy production, so have no effect on your weight. The mogrosides from luo han are also under study as inhibitors of certain tumors, and might be able to inhibit skin tumor growth. Other possible medical advantages include helping to reduce atherosclerosis and heart disease, so would appear to be a useful sweetener to use in your coffee!

Finally, xylitol. It was during World War II that Finnish scientists rediscovered xylitol that had been previously used in Germany as a sweetener in the late nineteenth century. The sugar shortage resurrected this substance that can metabolize without the need for insulin.

Xylitol is a substance that is found in some fruits and vegetables and also in corn cobs. In fact, it is a product that appears in animal metabolism, and so is perfectly safe. It is known to help support the immune system, and to help reduce the effects of aging. It possesses antibacterial properties due to its 5-carbon ring and has been approved by the FDA. Xylitol can replace sugar in most of its domestic uses, including in baking and as a natural sweetener. It is also used extensively in chewing gum as a sweetener that does not cause dental cavities due the acid caused by bacterial attack on the sugar.

However, one use to which xylitol cannot be put, nor any of the other natural sweeteners mentioned here, is in fermentation. Try these for your wine or beer and you will be very disappointed at the low alcohol level of your brew! This is also, however, one of the benefits of xylitol: it cannot feed the yeasts that cause candida or any other yeast infection. Although it is a saccharide, it is the same as the others in this respect.

So, which of these natural sweeteners should you take? The choice is yours since each has its own benefits with very few disadvantages and certainly no recorded side effects that we know of. Use stevia for superior sweetening effects, and make up a concentrated solution of it in water for your cooking. Use luo han if you have gastrointestinal problems, and use xylitol if you want fresher breath and to protect your teeth.

Use none for brewing or winemaking, and use any of them if you are diabetic. The choice is yours. These sweeteners are available at your local or internet health food store.

Artemisinin, also called Ching-hao-su is a herbal medicine extracted from the sweet wormwood herb, also known as the Artemisia annua. Asians have been using the leaf for centuries to cure simple ailments such as colds and parasitic infections, but it has recently become big business for its effect on malaria.

Sweet wormwood is predominantly a Chinese herb, used for over a thousand years for treatment of a large variety of conditions including malaria. It has also been used to treat wound and skin diseases, and has been recorded as far back as 200 BC as a component of Chinese prescriptions for specific illnesses.

Although the origins of sweet wormwood are in Asia however, it is now grown throughout the temperate regions of the world, and reaches its best in midsummer. Although best known today for its use in treating malaria, its medical uses include treating bronchitis, fevers and general feelings of malaise. It is primarily grown now for the supplement industry and as a non-prescriptive natural cure for malaria. It is a common and favorite herb for Chinese herbalists and sold both in the herbal form and as the extracted artemisinin.

Although there is a wide variation in the artemisinin content of sweet wormwood according to where it is grown, and under different agricultural conditions, it is the main active ingredient. Chemically, it is a sesquiterpene lactone containing an endoperoxide bridge that will be discussed later.

In order to attain the optimum yield of active artemisinin, the plant has to be grown in the right site for cultivation and fed with the correct fertilizers, the proper strain of the herb must be selected and the method of extracting the active chemicals is also critical. These are secrets that have been used for centuries by the Chinese herbalists who are masters of their craft and are now known to the west and being applied to scientific production of the extract.

There are benefits of artemisinin other than its effect on malaria, but that is the best known of its uses, so let’s have a look at that first. The herb has been used for a long time to treat parasitic infections, mainly flukes and worms that place great strains on the body having not only to feed these unwanted creatures, but also to excrete their waste and by-products. Malaria is caused by such a parasite, any one of four types in fact, and artemisinin appears to be just as effective on them.

The chemical contains what is known as an endoperoxide bridge that reacts with iron. The product of this reaction is a free radical, normally unwelcome in the body. However, malaria parasites contain high levels of iron, and the artemisinin reacts with that iron, forming free radicals that then go on to kill off the parasite. The free radicals that we take supplements to destroy, actually work for us in the destruction of the parasite that cause malaria. There is a silver lining in every cloud!

The reason that the medical world is so excited with this material is that malaria is second only to tuberculosis in its impact on world health. The problem is that the parasites that cause it have become largely resistant to the normal cures. They are not, however, resistant to the endoperoxide chemistry contained by artemisinin. It is now the major hope of world leaders in the fight to stop the inexorable increase in deaths due to malaria.

Now for the other applications. Another major use of the chemical is in the fight against cancer. Those cancers that involve iron rich cells can be disrupted by artemisinin by the same mechanism that kills off malaria bacteria. It is not only parasites that are susceptible to free radical action, but also human cells, and if these can be selectively destroyed, then it is a step forward towards a cure for cancer. Such a cure does not yet exist, but mechanisms such as the endoperoxide bridge provide a means of controlling at least some cancers, the best results being obtained with leukemia and colon cancer. That is not to say, however, that cures for these cancers are currently available since tests are still under way. However, it would do no harm to use sweet wormwood or the artemisinin extract as a supplement.

Cancer cells tend to accumulate iron because it is needed in the cell division process that cancer takes advantage of. However, if cancer cells could be persuaded to accumulate more surface iron, or were artificially exposed to it, then perhaps the endoperoxide mechanism would be more effective. Studies on this mechanism of controlling cancer are currently under way.

At a more mundane level, if you suffer from intestinal parasites, then one or two milligrams each day should see them off. It has also been effectively used to deal with colds, coughs and other general conditions, but most effectively against those that cause fever of one kind or another. It has been used in Chinese medicine as a form of ‘cure-all’ to be used when specific remedies failed. Its greatest current uses are, however, in cancer and especially malarial treatments.

You have to be careful with artemisinin since it can be toxic if taken in the wrong form. The least toxic, and most active, is the water soluble form known as artesunate. However, it lasts the least time in the body and has to get to work quickly. The oil soluble form, artemether, is the most toxic, but can cross from the blood to the brain. However, the parent form, artemisinin is the very safe itself, and can also cross the blood-brain barrier and while some tend to use a combination of the three, others prefer just the parent form.

If you take too high a dose you can suffer neurotoxicity which makes you wobble when you walk, and makes you impervious to certain levels of pain. However, such doses are next to impossible accidentally, and there have been very few reports of adverse effects in humans. It should not be used for at least a month after you have had radiation therapy because that tends to release iron that can be attacked to form free radicals. The recommended dose is 200 – 100 mg daily though the doses should be spread throughout the day. It should also never be taken within 2 hours of any other antioxidant such as vitamins A, C and E. Otherwise artemisinin can be safely used for better health and wellness in the way that the Chinese have been using it for centuries. As the public becomes increasingly aware of the health ramifications of sleep disturbances, more and more people are turning to sleeping pills. In fact, according to the IMS Health research, about 42 million prescriptions for sleeping pills were filled last year in the U.S. This represents a nearly 60 percent increase over the past five years alone. Some projections anticipate that the current $2.7 billion in annual sleeping pill sales will more than triple by 2010.

But instead of solving the problem, sleeping pills often make sleep problems worse. Sleeping pills commonly result in dependence. They can alter normal sleep architecture, cause amnesia and residual daytime “hangovers,” and they often result in rebound insomnias when discontinued. Some sleep specialists argue that sleeping pill use is further associated with significant increases in mortality. Given the sense of desperation that can accompany insomnia, even such very serious concerns have not prevented sales of sleeping pills from skyrocketing in recent years.

We are currently witnessing an unprecedented advertising campaign on the part of the pharmaceutical industry designed to convince the public that sleep medications are indeed a safe and effective strategy for addressing sleep problems. Despite clever and seductive advertising, however, it remains highly questionable whether sleeping pills can truly offer us sleep. I believe it is more accurate to say that they result in a kind of artificial or simulated sleep. Compared to natural slumber, sleeping pills cause a chemical knockout. Unfortunately, so many people have slept poorly for so long, they have forgotten what it is like to experience truly restorative, deep and refreshing natural slumber. Instead, many people now hold a naïve, limited sense of healthful sleep, confusing it with being knocked out. And sleeping pills satisfy that very naïve notion of sleep.

Supplementing sleep

Rather than artificially simulating sleep with chemical knockouts, sleep-promoting supplements such as melatonin, valerian, and other botanicals support that body’s own sleep-facilitating mechanisms more naturally. Such products work in greater harmony with nature and, unlike conventional drugs, they do not stimulate sleep, they supplement sleep. I think of natural supplements inviting us to sleep. The very potency of many natural products lies in their very gentleness, which works cooperatively with both body and mind to induce healthful sleep.

I believe that the potential benefits and markets for such supplements remain largely untapped. Consumers’ expectations that sleep aids should knock them out rather than gently assist them in letting go into sleep must be addressed through targeted education and information campaigns. Consumers also need to learn how to use alternative sleep supplements in the context of a healthy sleep lifestyle or positive night health.

Supplementing supplements

As helpful as they can be in promoting night health, sleep supplements alone will not do the trick. In fact, I believe many people get discouraged and discount the potentially positive benefits of sleep supplements after using them without proper guidance and understanding. Sleep supplements work best when they are geared to work synergistically as a part of a larger night health promotion program.

The availability of a wide range of over-the-counter health supplements offers an important freedom in healthcare choices. But with increased freedom comes increased responsibility. Consumers need to become significantly more informed. Particularly with regard to night health, such supplements need to be personalized and prescriptive. When it comes to sleep health, one size does not fit all. Whether we choose melatonin or valerian or a specific blend depends upon who we are and exactly what we need. By prescriptive I do not mean ordered by a physician, but specifically tailored to the needs of the individual.

Because of a significant shortage of health care professionals knowledgeable about sleep and the alarming trend towards increased use of sleeping pills, I have assisted in the development of a unique software program that provides sleep solutions that are both personalized and prescriptive. After more than a decade in development, the sleep advisor—an expert software system that thoroughly evaluates and provides personalized comprehensive recommendations for improving sleep—is now available.

AOAC adopts NOW Glucosamine Method

NOW Foods’ glucosamine HPLC method has passed the AOAC’s 2-year validation process to become the fi rst offi cial method for testing the standards of this popular joint health ingredient. It was selected by the expert review panel in April of 2003 as the “best method for further laboratory validation”. It will be published in the Journal of AOAC International and become available to the industry this summer, 2005. “Having NOW’s method for glucosamine become the offi cial AOAC method is a testament to the expertise of our staff, our focus on the science, and our commitment to quality,” said Michael Lelah, technical director of NOW Foods.

DuPage County Forest Preserve District’s Shooting Star Award.

The DuPage County Forest Preserve District of Illinois recently presented NOW Foods their Shooting Star Award at their annual Forest Preserve District volunteer banquet. This award recognizes the long-term efforts that our entire organization has made in order to improve and support the quality of the East Branch Forest Preserve in Glendale Heights, IL.
