Composed of cysteine, glutamic acid, and glycine, it plays a crucial role in the synthesis and repair of DNA and protein, as well as the synthesis of prostaglandins. While the benefits of supplementing with glutathione are numerous, two particularly compelling reasons are its positive impact on liver health and beautiful skin, which are the key focus of this article. However, before diving into the specifics of liver health and skin benefits, it's important to review the data on glutathione depletion and absorption. In February 2006, the findings of an $18 million double-blind placebo-controlled study were published in the New England Journal of Medicine. This study was conducted by the Women’s Health Initiative (WHI) and was on the protective effect of calcium and vitamin D. The New York Times reported on this study, declaring that the study found that there were no clear benefits to calcium pills. In this article, the fact that the women who stuck to their supplementation regime experienced 29% reduction in hip fractures was dismissed. This result is rarely achieved, even with use of the strongest pharmaceuticals. This study was designed to determine whether postmenopausal women who were given calcium and vitamin D would have a lower risk of hip fracture. The intervention group was given 1,000 mg each day of calcium carbonate, along with 400 IU of vitamin D. Although these women portrayed a greater preservation of hipbone density, the decrease in risk of fracture of 12% was not significant as a whole. The fact that many of the women who were included in the study were under sixty, and therefore, not typically at risk for fractures, causes these results to be unsurprising. The results that were found were also skewed as a result of the fact that compliance with the prescribed daily intake was only 59% by the end of the study. 41% of the study participants had fully stopped taking the prescribed daily dosage of calcium and vitamin D, with 24% having discontinued the supplementation altogether. With such an unexpectedly low compliance rate, along with the fact that the projected hip fracture rate was over twice what was actually observed, the power of the study was reduce to only 48%. As a result, the trial had less chance than a simple flip of a coin to find anything but the largest of differences in a risk for fracture. Despite these shortcomings, the researchers looked at the subgroups, and found different pictures emerging. Looking only at the women who mostly stuck to their prescribed regime, researchers found that a reduction in fractures of 29% was experienced. Additionally, those women over sixty experienced a reduction in the risk of fracture of 21%. These results are actually remarkable, especially after considering the many problems which clouded the accuracy of the data. Unfortunately, reporters did not look at these findings, causing a slanted account of the study to be published. Because of this, the media failed to acknowledge what were actually significant findings. Additionally, the design of the WHI study disregarded the fact that a reduction of fracture risk is actually dependent on several factors other than calcium. Studies have actually shown that magnesium is also of equal important in the treatment and prevention of osteoporosis, as a deficiency plays a central role in the development of the disease. Postmenopausal women and those women with osteoporosis usually have low bone-magnesium content, exhibiting other indicators of magnesium deficiency that are not seen in non-osteoporotic women. Also, calcium competes for absorption with magnesium, meaning that postmenopausal women who increase calcium intake without also increasing magnesium intake can impair their absorption of magnesium. With this knowledge, the failure of the researchers to include magnesium supplementation along with calcium and vitamin D caused the potential for a study that could cause harm on the patients. Inositol is a member of the vitamin B complex family, being referred to as vitamin B8, but is not strictly a vitamin because it is biosynthesized in your body. Vitamins are essential substances that are not manufactured by your natural biochemistry, and must be taken in your diet. However, to all intents and purposes it works like a member of the vitamin B family. The main function of myo-inositol (the commonest isomer of inositol) is in the health of cell membranes, particularly those that comprise the marrow, eyes, intestines and the brain. Without proper regulation of the cell membrane, the cell cannot function effectively. Some of its effects include healthy hair and controlling estrogen levels. It is also believed to help to reduce cholesterol levels in the blood. A deficiency will result in hair loss, eczema, increased blood cholesterol levels and eye abnormalities. You might also suffer constipation, although this is not as serious a condition as those preceding. It is present at highest levels in the heart and brain, which indicates where it is mostly used, although it also helps the liver to break down fats and enables the nerves and muscles to operate as they should. Those that are depressed are frequently found to have low inositol levels in their spinal fluid, so it is believed to play a part in that condition. It is known that the substance takes part in the function of the neurotransmitter serotonin, which is known to play a part in depression, and initial signs are that its use in the treatment of depression could be effective. Neurotransmitters are responsible for passing messages across the gap (synapses) between nerve cells, their messages being decoded by the neuroreceptors. A healthy nervous system depends on healthy neurotransmitters. For these reasons, inositol has also been tried on other conditions of the nervous system. These include bipolar disorder, bulimia, panic disorder, obsessive compulsive disorder and attention deficit disorder. So far, results have been inconclusive as to its effectiveness, but it is early days yet and field tests are continuing. A test carried out in Beersheva, Israel, in 1997, found that treatment with inositol produced significant improvement in the depression of 28 patients after four weeks on the Hamilton Depression Scale1, and 21 patients tested with panic disorder (with and without agoraphobia) showed significant improvements in their condition, including agoraphobia. Results on 13 patients with obsessive compulsive disorder also showed significant improvement. These were all double-blind tests. However, not all tests have been so conclusive, and a study on 42 people with sever depression who did not respond to conventional antidepressant, also failed to respond when inositol was added to their medication.2 Results are therefore not conclusive. Four hundred people took part in a double-blind test that indicated a possible improvement in the symptoms of polycystic ovary syndrome when treated with inositol 3,4 and another that inositol treatment on patients taking lithium could help reduce the symptoms of psoriasis, a skin condition believed to be caused by a reaction of the immune system and nerves.5 The supplement has also been found to be just as effective as Luvox (fluvoxamine – similar to Prozac) after four weeks treatment. Although these results are good, they are inconclusive, and more data is need before any indisputable conclusions can be drawn. However, treatment with inositol might be worth considering if conventional treatment for these conditions has been unsuccessful. The most common natural form of inositol is myo-inositol, an isomer of cyclohehexanehexol, a carbocyclic polyol that form the structural basis for secondary messengers in the cells of eukaryotes. A secondary messenger system is one whereby a signaling molecule is released in response to a signal from a primary messenger such as a neuroreceptor, which then activates certain intracellular proteins known as effector proteins that exert a response from the cell. An example is cAMP (cyclic adenosine monophosphate) that is a secondary messenger that activates protein kinases and allows them to phosphorylated proteins. Eukaryotes are organisms that contain cells composed of complex components contained within a cellular membrane, and that also contain a nucleus. Examples are fungi, plants and all animals. Examples of non eukaryotes include the bacteria family. It is frequently recommended that inositol is most effective when taken with an equal amount of choline, although this might be due to the fact that when inositol deficiency is detected, choline is also frequently deficient. Both are vitamin B family like, and both are lipotropic, in that they aid the breakdown of fats in the body. It is not clear whether this is true or not, but taking both would certainly not harm you, and might be of great benefit. Inositol is not essential, because it can be obtained from beans, nuts, seeds, whole grains, cantaloupe, brewer’s yeast, liver and vegetables. Bacteria in the gut also act on the phytic acid (inositol hexaphosphate) contained in citrus fruits to form inositol. However, be careful if you drink a lot of coffee. It destroys inositol, and if you are taking the supplement medicinally, steer clear of coffee during your period of treatment because it will lose its effect. Excessive coffee drinking can also result in a general depletion of inositol from your diet, and hence a deficiency. In such a case you are advised to take a supplement, preferably along with choline that might also be deficient. Although there have been no adverse side effects reported, no specific longer term safety studies have been carried out on inositol. Because of the way it works, inositol should be avoided by people with liver or kidney disease, and also by expectant or nursing women. It should be avoided by young children until safety tests have been carried out, and it is believed that it can cause manic effects in those suffering bipolar disorder. Other than for these specific cases, trials with many times the average daily intake of the substance, it currently appears safe to take inositol as a long-term supplement. Guggul is a natural herb supplement that may help lower cholesterol, yet few in the Western hemisphere know much about it. Guggul is otherwise known as the Mukul myrrh tree, and is a plant of the Burseracae family with small red or pink flowers. It is found across central Asia over to North Africa, although is very common in the northern areas of India where the climate is more semi-arid than equatorial. Guggul does not like a lot of water and can thrive in ground where the soil has few nutrients. Its Latin name is Commiphora wightii, and it grows about 12 feet high. It has been predominantly used in the Ayurvedic medicine of ancient India, and like many such ancient remedies and treatments, is now used in modern medicine to treat specific conditions: conditions such as some forms of heart condition, where it has been found to be able to lower your blood cholesterol levels, weight loss and some forms of arthritis and other inflammatory conditions. However, it has been used so successfully over the years, particularly in India, which it is now in danger of extinction and is contained in the Red Data List of the World Conservation Union that lists endangered species. So what is so special about this plant that makes it so popular? To get the answer to that we have to go back a bit in its history, although not quite as far as the two or three thousand years that it is known to have been used in traditional Hindu medicine. The active ingredient is found in the sap of the tree, and is used to fight against obesity and other diseases that can be caused by excess weight or cholesterol, such as arthritis, obesity and atherosclerosis. Until recently it has had very little support for its claims from conventional medicine. It was in the 1960s that an ancient Sanskrit text was found that recommended guggul as a treatment for high cholesterol levels. Since that discovery, research has focused on the plant's anti-cholesterol properties, and a great deal of evidence has been gathered supporting the claims of that ancient text. So much so that the Indian government has approved the use of guggul for the treatment of high cholesterol levels, largely because it has been found very effective in reducing the levels of 'bad' LDL cholesterol in the blood while increasing the levels of the beneficial HDL cholesterol. Several trials have supported this, including one study involving 228 patients that showed the extract to be equally as effective as the anti-cholesterol drug clofibrate. That is not all, and other studies included one in which a decrease in LDL cholesterol of almost 13% was measured in a double blind study involving 61 subjects, of which around half received a placebo. An average 12.7% reduction in LDL cholesterol, 12% in triglycerides and 11.7% in total cholesterol was experienced by the group given the guggul extract. Every 1% drop in total cholesterol is associated with a 2% decrease in the risk of heart disease. Guggul reduces the levels of harmful cholesterol in your blood by converting it into bile. The plant extract contains substances given the name guggulsterones that block the activity of a protein that regulates the metabolism of cholesterol in your body known as FXR (the Farsenoid X Receptor). This protein can increase the risk of you contracting heart disease by preventing the liver from converting cholesterol into bile acids, so that the concentration of cholesterol in your blood continues to build up. The problem with bile acids is that once they reach a certain concentration in your body, the FXR comes into play and stops more being produced. Guggulsterones prevent the FXR from doing this, and so helps the liver to destroy more cholesterol. There is a reason for the body not allowing too much bile acid to be generated, but for those with excess cholesterol, it is more beneficial for this regulation to be prevented, and more cholesterol to be destroyed by the liver. It is the resin of the plant that is prized, being extracted from the bark in much the same way as rubber is tapped. It is also used in fragrances and perfumes in addition to its medicinal uses, and the dosage generally recommended is 1500 mg (1.5 grams) twice daily. However, it is not recommended for those suffering liver disease, irritable bowel syndrome, Crohn's disease or any form of diarrhea, and should not be taken by those on beta blockers. It is not only for its cholesterol-lowering properties that guggul is prized, however. Another property it possesses is its ability to render blood platelets less sticky, and so reduce the risk of coronary disease, and prevent the formation of blood clots and thrombosis. Another use it has found is in the field of weight loss, where it has been found effective in reducing the weight of obese adults. It does so by the activation of lipolytic enzymes and increased levels of triiodothyronine (T3), believed to be due to the formation of T3 from T4 (thyroxine) in the liver. T3 increases the metabolic rate, and the rate of the breakdown of glycogen and gluconeogenesis: the biosynthesis of glucose. It also causes cholesterol to be broken down and increases the rate of lipolysis - the breakdown of fats stored in fat cells in the body. Studies have shown that those taking guggul lost up to 6 times the weight of a control group within 15 days, and the practice is going along with the theory. Not only that, but when you are on a diet, your body is likely to respond by decreasing levels of triiodothyronine, and so reducing the rate at which fat burns. Hence, your diet does not help you top reduce weight as quickly as it could. Guggul, however, stimulates the production of T3, and so you are not only taking less fat into your body, but are also burning it up at an accelerated rate. Other uses to which the resin has been put are based upon its anti-inflammatory properties. It has been found to be an effective treatment for some forms of arthritis and also in the treatment of acne. The active inflammatory ingredient is believed to be myrrhanol A, a polypodane-type triterpene, which would also explain the antioxidant effect of guggulipid on lipid peroxidation.
Guggul is a versatile plant, and a good supplement to take for anybody suffering increased lipid or cholesterol levels, and who wants to increase the weigh-loss effect of their diet. However, make sure that you purchase a supplement standardized on its guggulipid content.

Grapefruit pectin fiber is a source of soluble fiber that can bind to cholesterol and inhibit the formation of atherosclerosis. It is also effective in maintaining good blood sugar levels, and promoting a healthy colon. However, before discussing how grapefruit pectin fiber works in these ways, it is first necessary to explain how they occur in the first place. CHOLESTEROL LEVELS First cholesterol. This is a steroid lipid, most of which is biosynthesized by your body rather than being present in your diet. Although it has a bad name health wise, cholesterol is actually essential to good health because it helps your digestive system break down fast, it strengthens cell walls and is a major ingredient in the manufacture of vitamin D by your liver. It is also essential for the formation of several hormones, including cortisone, cortisol and aldosterone by the adrenal glands, and also of estrogen and testosterone. A bloodstream without cholesterol is therefore not good for you. Given that cholesterol is essential for good health, then why does it have such a bad press? The root cause of that is that cholesterol is not water soluble, and to be carried round the bloodstream to where it is needed it needs some transport. For that, it uses low density lipoproteins (LDLs). However, it’s not a return journey, and to get back they must use another form of lipid known as a high density lipoprotein (HDL). The protein part is water soluble, wile the lipid part can attach to the cholesterol and carry it around the bloodstream. The problem arises with free radicals, which are oxygenated molecules that are generated by the body’s metabolism, and also created from pollutants, such as pesticides and cigarette smoke, and the UV portion of daylight. These can oxidize the low density lipoproteins so that they can be attacked by phagocytes (while blood cells) deposited as plaques in the artery walls. This hardens and thickens the arteries, reducing the heart’s blood supply which in turn can cause strokes and heart attacks. This can be prevented in a number ways, the major of which are by consuming plenty antioxidants in your food, mostly contained in phytochemicals (plant chemicals), reducing your LDL cholesterol levels to the minimum needed by your body, and also by increasing the HDL levels in your bloodstream by as much as possible. Grapefruit pectin fiber can bind to cholesterol, and prevent it from being deposited in the artery walls by the oxidized LDL. A sixteen week double blind study (neither the subjects nor those carrying out the test and analyzing the results knew who were given the placebo) found that grapefruit pectin successful reduced the low density cholesterol level of 27 subjects by 10.8% and overall plasma cholesterol by 7.6%. These volunteers were at a medium to high risk of atherosclerosis due to a high LDL cholesterol concentration in their blood. Animal studies have also been carried out, whereby animals were fed a high cholesterol diet, and those fed a high fat diet were found to have an average of 45% narrowing of their arteries, and those fed the same diet, but with added grapefruit pectin fiber, were found to have only 25% narrowing, thus corresponding with the results of the human volunteers. BLOOD SUGAR It is essential that you maintain a stable level of sugar in your blood since the body reacts to low blood glucose by making you feel hungry, and thereby forcing you eat. If you are trying to lose weight you will find it very difficult if your blood sugar levels fluctuate. Grapefruit pectin can reduce the rate at which carbohydrates enter your bloodstream, and therefore the rate at which sugars are absorbed. Studies have also indicated that pectin can even out your blood sugar concentration, so that you experience fewer fluctuations or sudden spikes that can make you feel either very hungry, and so binge on carbohydrates, or very satisfied so that you eat nothing at all. The maintenance of a stable level of glucose in the blood is necessary for a normal life since it is that that determine your need to eat food. If your blood sugar is low, the desire to eat high calorie carbohydrate food is uncontrollable. This is the theory behind the so-called ‘grapefruit diet’. You eat a grapefruit prior to a meal, and allow it to be digested. The grapefruit pectin will make you feel full and so eat less, so that you can eat normal foods, but still lose weight. Many people take the grapefruit pectin in supplement form for its ease and economy of use. It is also more convenient since you don’t have to carry grapefruit around with you everywhere you go. A Healthy Colon Like any other soluble fiber, grapefruit pectin fiber is a dietary fiber that can help to maintain the health of your colon. In fact it is the only one of the basic four fibers of lignin, cellulose, hemicelluloses and pectin that is soluble. Although referred to as soluble, pectin softens and expands and provides substance and added weight to faeces in order to allow it to pass easier through your colon. Other than ensuring the easy passage of your stool through your colon, and avoiding constipation, grapefruit pectin fiber can help to prevent excessive growth of bad bacteria in the intestine, and because it accelerates the passage of substances through the colon, there is less time for toxic or cancerous substances to be in contact with your bowel lining. The American Academy of Pediatrics (AAP) today announced that it has doubled the amount of vitamin D recommended for infants, children and adolescents. The increase, from 200 international units (IU) to 400 IU per day, starting in the first few days of life, was detailed at the group’s annual meeting in Boston. The new advice replaces an academy recommendation issued in 2003. "We are doubling the recommended amount of vitamin D children need each day because evidence has shown this could have life-long health benefits," said Frank Greer, M.D., FAAP, chair of the AAP Committee on Nutrition and co-author of the report. “Supplementation is important because most children will not get enough vitamin D through diet alone.” "Breastfeeding is the best source of nutrition for infants. However, because of vitamin D deficiencies in the maternal diet, which affect the vitamin D in a mother’s milk, it is important that breastfed infants receive supplements of vitamin D,” said Carol Wagner, M.D., FAAP, member of the AAP Section on Breastfeeding Executive Committee and co-author of the report. The new advice is based on mounting research about potential benefits from vitamin D besides keeping bones strong, including suggestions that it might reduce risks for cancer, diabetes and heart disease. But the evidence isn't conclusive and there is no consensus on how much of the vitamin would be needed for disease prevention. "We know 400 IU a day is safe and prevents rickets," Greer said. "We don't have any idea if that amount of vitamin D is enough for other diseases. We also don't know if anything over 400 is safe." The AAP also made these recommendations: Infants who are breast-fed or partially breast-fed receive 400 IU a day of vitamin D in supplements, beginning in the first few days of life, continuing unless the infant starts taking at least one quart a day of vitamin D-fortified formula or whole milk, although whole milk should not be introduced until the child has turned 1. Non-breast-fed children and older children should also receive a vitamin D supplement of 400 IU per day. Adolescents who do not obtain 400 IU of vitamin D per day through foods should receive a supplement containing that amount. Children at increased risk of vitamin D deficiency (for example, those taking anti-seizure medications) may need higher doses, but this should only be done in consultation with a health-care professional. The new recommendations were expected to be published in the November issue of the journal Pediatrics.

Maintains Healthy Visual Function* It has been well established that lutein is present in high concentrations in the retinal tissue of the human eye. However, a study was conducted in human volunteers to determine whether taking lutein in supplement form actually increased the density of the carotenoid pigments present in the macula. In this study of eight individuals, researchers estimated the density of the macular pigments prior to having each individual take 10 mg of lutein daily in supplement form for 12 weeks. Plasma lutein concentrations were measured at 4-week intervals. During the first four weeks of the study, plasma levels increased five-fold from pre-supplement measures, and then remained at this level for the duration of the study. It was also shown that, due to increased deposition of lutein in optical tissues, macular pigment density increased by an average of 5.3% at the 4-week mark, and continued to increase until the duration of the study.1 A study was also conducted to investigate the possible role of specific nutrients in protecting the lens of the eye against aging, a risk factor for compromised visual function. The study was comprised of 376 individuals aged from 18 to 75. Of the nutrients measured, it was found that the lenses of individuals with higher concentrations of lutein and zeaxanthin showed less of an effect from the aging process. The investigators concluded that these carotenoids might play a protective role in supporting the maintenance of healthy vision.2 The Age-Related Eye Disease Study (AREDS) was a landmark study of the effects of diet and antioxidant supplementation on eye health. The study enrolled over 3500 subjects aged 55 to 80 years who were followed for approximately 6 years. Among the data collected in this multi-faceted study was a self-administered Food Frequency Questionnaire (FFQ). The AREDS Report No. 22 examined the data from the FFQs and determined that, of the nutrients evaluated, only lutein and zeaxanthin were directly related to maintaining eye health with statistical significance3. These findings corroborated similar results of an earlier multi-center study published in the Journal of the American Medical Association that also found that those with a higher intake of lutein and zeaxanthin maintained healthier eye function.4 These promising results have spurred the design of a second major clinical trial (AREDS2), which is currently enrolling participants to study the impact of supplemental xanthophylls (FloraGLO® Lutein and zeaxanthin) and other nutrients on age-related eye health.5 In addition, a double-blind placebo controlled trial was performed in ninety individuals who had signs of compromised visual function. Individuals were divided into three groups and received either 10 mg FloraGLO® lutein, 10 mg FloraGLO® lutein plus a multivitamin/multimineral formulation, or placebo for 12 months. In both the FloraGLO® lutein and FloraGLO® lutein plus other nutrients groups, improvements were seen in mean eye macular pigment optical density, visual acuity and contrast sensitivity. No improvements were noted in the placebo group.6 These results demonstrate FloraGLO® lutein’s beneficial effect on maintaining healthy visual function. Newly published research has demonstrated that lutein and zeaxanthin supplementation may enhance visual performance under glare conditions. Forty healthy subjects took daily doses of 10 mg FloraGLO® Lutein plus 2 mg zeaxanthin for six months. They were evaluated for changes in macular pigment, glare disability and photostress recovery at the onset of the study, and at 1, 2, 4 and six months. After six months, subjects experienced an average increase in macular pigment optical density (MPOD) of 39% compared to baseline, and all but two participants experienced some increase in MPOD. This increase in MPOD was also directly related to measured improvements in visual performance after exposure to bright light, as well as photostress recovery.7 This study suggests another way in which lutein and zeaxanthin can help support optimal visual function in healthy individuals. Potent Antioxidant Protection* Most of the beneficial effects of lutein are ascribed to its potent free radical scavenging abilities. It is well-known that lutein is a carotenoid related to beta-carotene and possesses antioxidant activity against a number of reactive oxygen species.8 More direct evidence for the free radical scavenging activity of lutein is found in studies of its effects on human lens epithelial cells. Cell cultures were exposed to ultraviolet light after pretreatment with lutein or alpha-tocopherol. Both nutrients were found to reduce ultraviolet-induced damage to lens epithelial cells. However, lutein was shown to have significantly higher photoprotective activity than alpha-tocopherol9 demonstrating its potential as a high-powered antioxidant. A further review of the mechanisms of lutein in conferring a protective role reveals evidence for its antioxidant activity in various body tissues. Lutein has been shown to be an effective antioxidant in vitro as well as in experimental models of a number of body systems.10 Supports Healthy Skin* A recent randomized, double blind, placebo-controlled study has demonstrated the positive effects of oral and topical administration of lutein on skin health parameters (surface lipids, hydration, photoprotective activity, skin elasticity and skin lipid peroxidation). Forty female subjects were divided into four treatment groups. Treatment options included oral administration of 5 mg of FloraGLO® Lutein twice daily or placebo and topical administration of 50 ppm FloraGLO® Lutein twice daily or placebo. Each treatment group received either an active oral treatment with a placebo topical treatment, a placebo oral treatment with an active topical treatment, both active treatments, or both placebo treatments. Statistically significant improvements were seen in all five parameters tested in all treatment groups compared to the group receiving only placebos. The greatest overall improvements were seen in the group receiving both active oral and topical treatments, while lesser but still significant improvement was seen in both the active oral only and the active topical only groups. Additionally, oral administration of lutein conferred superior photoprotective activity (as measured by skin surface redness after exposure to ultraviolet light) and prevention of lipid peroxidation (as indicated by levels of malondialdehyde in skin lipids after exposure to ultraviolet light) than either topical lutein or placebo.11 Diverse Cinical Benefits* Evidence from various experimental trials suggests that lutein may play a protective role on the circulatory and cardiovascular systems. Its antioxidant activity may also extend to the heart, skin, lungs and blood vessels, making it a nutrient with diverse clinical benefits. Garcinia Cambogia is found naturally in India and parts of Asia, and also on the Pacific coast of South and Central America all the way from Peru up to Mexico, and likes a humid forested environment. Also known as Brindall berries, garcinia is believed to act as an appetite suppressant and allows you to lose weight by diminishing your desire for food. The Malabar tamarind, as it is also known, is about the size of an orange resembling a small pumpkin, and an extract from the fruit and rind is used in several weight loss products. The health risks presented by synthetic diet pills render a natural product extremely attractive were it to be effective. So can Garcia curb your appetite? What is the scientific evidence for it, and what biochemical route would it take? Although tests on animals have been very positive, human results have been inconsistent. In some double blind tests using a placebo, weight loss was up to three times that of the control, but in others there was no apparent difference between those taking garcinia, and those given a placebo. However, doubts have been raised of the validity of some of the negative tests, so what does science tell us? The active ingredient in the cambogia extract is hydroxycitric acid (HCA), which is a powerful inhibitor of ATP citrate lyase, an enzyme that catalyzes the reaction between citrate and Coenzyme A to Acetyl CoA and oxaloacetate. Since the acetyl CoA is necessary in the synthesis of fatty acids and lipogenesis (the conversion of glucose to fatty acids), then anything that inhibits the biosynthesis of acetyl CoA must help to reduce the amount of fat stored in your body. By inhibiting this reaction, that occurs outside the mitochondria so is not a direct part of the Citric Acid Cycle, HCA should theoretically suppress the formation of fats from carbohydrates, reduce food intake and thereby induce weight loss. But that is not the only mechanism. A study at Georgetown University in Washington found that after 8 weeks of taking the garcinia extract, there was a 5.4% reduction in body weight and body mass index, and a significant reduction in low density lipoproteins (LDL) and triglycerides with an associated rise in high density lipoproteins (HDL). This is good news for those suffering from high cholesterol levels, since the LDL lipoproteins are those that carry cholesterol to the major blood vessels, and which when oxidized by free radicals deposit fatty plaques on the artery walls. These plaques constrict the arteries and the resultant atherosclerosis can lead to cardiac problems and strokes. HDL lipoproteins carry cholesterol back to the liver for destruction, and is known popularly as ‘good cholesterol’. The studies also indicated modifications to certain indicators of the status of fat deposits in the body and of appetite modifiers in the brain. In this respect they found 38% decreases in serum leptin and increases in serotonin levels of 44%, and the excretion of fat metabolites in the urine increased from between 32% and 104%. These are significant findings, and further research has indicated that HCA helps to suppress appetite. Serum leptin is an indicator of the level of fat stores in the body, and as the leptin levels in the blood reduces, the hypothalamus is given an instruction to increase the appetite so as to increase the fat levels again. However, it is believed that HCA possesses leptin-like properties, and this signal is either not generated or is modulated. The increase in serotonin has the same effect. It is known that serotonin controls the appetite, although the exact mechanism has not yet been established. What is known is that serotonin activates certain neurons and melanocortin-4 receptors (MC4R) in the brain, that not only curb appetite but also block the effect of other neurons that would normally increase appetite by blocking the effect of MC4Rs. This is how the banned anti-obesity and serotonin inciting drug Fen-Phen operated, and it appears that the hydroxycitric acid in garcinia cambogia extract acts in a similar, but safer, way. The problem with drugs such as Feb-Phen was that they created cardiac problems which could be dangerous to obese people whose hearts might have been weakened. However, now that the biological pathway by which serotonin controls weight is believed to be known, if not fully understood, the way is becoming clearer as to how safer weight loss pills, acting through appetite suppression, might be developed. It also provides a valid scientific explanation for the effect of garcinia cambogia extract which might in itself prove to be that safer way. The biochemistry supports the evidence of its effect on those wanting to lose weight, and also bolsters the claims that those tests and trials found to be negative were in some way flawed. Until the full chemical pathway is understood, the factors that can lead to flawed tests are unknown, although one could be the use of excessive fiber in some trials that could reduce the effect of the extract. One of the effects of HCA is to limit the ability of your body to convert carbohydrates into fat (the Acetyl CoA inhibition mentioned above). That, combined with suppression of your appetite and a higher rate of thermogenesis, prevents the body from storing excess carbohydrates as fat. Instead you will have increased energy levels, so you should exercise to use this up while taking garcinia extract. For this reason it is popular with athletes and bodybuilders seeking an energy source that has not yet been banned from sport. Although diet pills based on the same principle had side-effects and could make the user feel on edge, there are none known with garcinia. However, it is possible to reduce the absorption of some essential nutrients due to appetite suppression, so do not exceed the recommended dose. An excessive amount can also led to gastric discomfort, but none of these effects have been noted when the recommended doses have been adhered to.
However, if you are diabetic, pregnant or a nursing mother, you should consult with your physician or health professional before taking the extract. Garcinia Cambogia presents no specific risk to such people, and this warning should be given for all non-prescriptive treatments that your physician might be unaware of you taking.

DHA is a fatty acid that is essential for many body processes and structures. Fatty acids are the building blocks that create lipids, which are important nutrients that make up 25 to 45 percent of total body energy for most of those who are living in industrialized countries. The majority of dietary fatty acids are made from dairy products, meat, margarine, and vegetable oils. Lipids are important because they store and transport energy from one part of the body to another and they also insulate and mechanically protect certain parts of the body. Any fat that is eaten is converted by the body into fatty acids, where it is then burned for energy, stored, or synthesized into other acids which are called prostaglandins. Some fatty acids are also vital structural components of cell membranes. DHA is part of a special class of highly concentrated, long-chained, polyunsaturated fatty acids called omega-3 fatty acids. Omega-3 fatty acids are looked at as one of the most fundamental molecules in the structure and activity of the membranes of all cells. They have a huge influence on the production of cellular regulatory compounds which are known as eicosanoids. They also are highly specialized in their function in neurological tissues, especially the retina of the eye. It is also very likely that omega-3s have specific and beneficial effects on the heart muscle, while they also influence the production of substances that control immune responses. Fatty acids are short-chained, medium-chained, or long chained. Short-chain fatty acids are water-soluble and absorbed directly from the intestine into the bloodstream. They are generally used for immediate energy and can be found most abundantly in dairy products. Long-chained fatty acids are more emphasized when it comes to health concerns as they are stored in cell membranes and have a huge affect on membrane protein function. Their behavior in tissue membrane is what makes them greatly influential in health. Besides the length of the chain, fatty acids are also described by their saturation. Saturation means how many hydrogen atoms the fatty acid carries. Fatty acids that carry as many hydrogen atoms as they can are called saturated fatty acids, while those that lose a pair of hydrogen atoms become unsaturated. This loss of hydrogen atoms creates a double bond between the two carbon atoms on either side of the missing hydrogen atoms. Those fatty acids that have one double bond are called monounsaturated fatty acids, while those with two or more double bonds are called polyunsaturated fatty acids. Most omega-3 fatty acids are highly unsaturated and have five or more double bonds.
EPA and alpha-linolenic acid are most commonly found in plant sources. DHA and EPA are made in phytoplankton, which is consumed by fish, mollusks, and crustaceans. The above reason is why these two essential fatty acids are found mainly in the aquatic food chain. LNA and EPA cannot substitute for DHA in all metabolic functions, even though DHA and EPA are more able to cover for each other in certain metabolic functions. It has been found that LNA is less effective than EPA which is less effective than DHA in enriching tissues, meaning that food sources such as vegetables and seed oils are not as good of sources than sea foods or other foods that are enriched with DHA and EPA. DHA in itself provides most of the health benefits displayed in research, to take essential oils that contain no DHA would limit the health benefits discussed above and from other sources. CLA is conjugated linoleic acid, a compound known as a trans fatty acid due to its stereochemistry, and while trans fatty acids are generally regarded as harmful, CLA is not because it is conjugated. This means that it has alternate single and double bonds in the backbone carbon chain, and the overall energy of the molecule is therefore reduced. Linoleic acid itself is one of the omega-6 fatty acids, the 6 referring to the double bond at the sixth carbon from the omega and of the carbon backbone chain. It is believed to be the cause of heart disease and obesity due to its increasing use in the diet at the expense of omega-3 fatty acids. When the molecule is conjugated, however, the fatty acid has different chemical properties to the standard isomer, and natural CLA is mainly found in cattle products, such as beef and dairy products. Conjugated linoleic acid is present in cattle because it is formed when linoleic acid is converted to oleic acid by rumen bacteria, that are responsible for the microbial fermentation of the feed of ruminant animals such as sheep and cattle. When oleic acid is formed, so too is CLA. However, the form used in supplements is manufactured from vegetable oils, and therefore suitable for use by vegetarians. The usual vegetable oils used are safflower oil and sunflower oil. It is believed to possess several beneficial properties, including antioxidant and anti-cancer properties, but it is for its ability to reduce body fat that it is best known to most people. A growing amount of information is being collected on the use of CLA as a supplement in the weight loss industry, although there are as yet no definitive mechanisms that explain its action. However, recent studies have indicated it possess properties that can help to reduce the levels of low density lipoproteins in the blood, and reduce the possibility of atherosclerosis due to LDL oxidation by free radicals. It is also theorized that CLA in some way regulates the prostaglandin biosynthesis that controls the level of hormones in the body that can regulate growth. An increase in growth hormones is one way in which athletes promote an increase in muscle bulk, while reducing their fatty tissue mass. CLA is also purported to increase thermogenesis, and so promote the loss of body fat and overall weight. Although most studies on the effect of conjugated linoleic acid in reducing body weight have been carried out on animals, recent animal studies have indicated that might not so much reduce weight, as to increase muscle bulk while reducing that of body fat. The end result, therefore, is not a loss of weight, but a leaner body that has more muscle and less fat. All it needs is the results on animals to be transferred to humans, and this, of course, is frequently the case. However, initial studies on the use of CLA in the human diet have been very positive, so the signs are good. Most scientific progress in human biochemistry has been obtained by virtue of prior studies on animals. These studies, of course, have been beneficial to the animals, making them leaner and much fitter than they otherwise would have been. It is believed that the same will be true of humans taking CLA as a supplement. In fact, recent studies are split about 50/50 with regard to the effects on humans. While some studies have shown no benefit, about an equal number have shown a positive benefit in the reduction in the mass of fat in the body. Some of the negative studies may have been flawed in measuring total body weight, and not the relative amounts of muscle and fat, and also basing their results on people already with a low level of fat in their body. In that respect, then, the results look very favorable, and taking CLA as a supplement is likely to help you to reduce fat and increase muscle, if not altogether lose total body weight. However, is that not the end result that most people want? They might not want to be lighter in weight, just to have more muscle mass and less fat tissue. In a study shown at a 2002 Experimental Biology meeting, it was shown that is was possible to substantially reduce body fat mass by taking CLA alone, and when it was taken in association with guarana, both the size and the number of fats cells in the body were reduced by 50%. However it has also been shown that CLA can be oxidized by free radicals shortly after ingestion, and that sesame lignans help to prevent this. Since sesame lignans can also be used in conjunction with CLA to reduce fat by increasing the level of fatty acid oxidation in the liver, than the benefit of CLA seems obvious. The antioxidant effect of CLA is one possible explanation for its anti-cancer properties, though there are others. Its antioxidant properties also have an anti-catabolic effect, in that it can help to prevent the wastage of muscle tissue. The FDA has published studies that attest to these anti-cancer properties. Diabetics, however, should consult with their physician before taking CLA as a supplement, since there is a body of thought that it reduces sensitivity to insulin. Butterbur extract is taken mainly from the rhizome, root and leaves of the butterbur, a member of the daisy family. They are very hardy and have creeping underground rhizomes and large leaves like those of rhubarb. Another name given to it is the sweet Coltsfoot, and they generally grow in the temperate climates of Europe, North Africa and South west Asia. They like damp conditions, specifically marshes and ditches, and also riverbanks where there are always plentiful supplies of moisture. It has been used by Native Americans for headaches and inflammation, and has been shown to be an effective remedy for hay fever and to provide relief from painful menstrual cramps. Butterbur has also been used throughout the middle Ages to treat fever and the plague, and has been recorded in the seventeenth century as being used for asthma, wounds and coughs. However, one of its most important applications is in restore bladder function in the incontinent and semi-incontinent. Urinary incontinence is typified by an unusually high frequency of urination – more than 8 times a day, an immediate strong urge to pass water or leaking and involuntary urination. Any two of these three indicates urinary incontinence. As people age their bladders become smaller, and by definition the periods between urination will reduce. This does not, however, suggest that bladder size is the cause of urinary incontinence. Urination is caused by the contraction of the smooth layered muscle that surrounds the bladder, called the detrusor, a contraction in turn caused by neurons both in the brain and in the detrusor itself. This naturally contracts and expands according to the volume of urine in the bladder, and once the bladder is about half full the brain will tell you that the detrusor is ready to contract to expel the urine. However, if the time is not convenient, the cortex will suppress this desire until a more convenient time. In incontinence, the desire is suppressed but the neurons still fire to contract the detrusor, expelling urine at inconvenient moments. Butterbur contains the sesquiterpenes petasin and isopetasin, which are known to reduce spasms in smooth muscle tissue and in vascular walls. It can therefore be used to control the involuntary spasms that cause urine leakage or expulsion against the patient’s wishes. These sesquiterpenes are at highest concentration in the roots of the plant. The effect that the sesquiterpenes have in inhibiting the synthesis of leukotriene in leukocytes tends to support this effect, since leukotrienes can cause contraction of vascular and smooth muscle tissue. Not only this, but the spasmolytic effect could also be explained by the inhibition of cellular calcium caused by the petasin isomers. Many studies have indicated that the effectiveness of butterbur extract is also useful in the prevention of migraines. There has been a lot of research carried out on the use of butterbur extract on migraine sufferers, and the effective dose appears to about 75 mg twice daily. There is little evidence of it being a cure but as a prophylactic there appears no doubt of its efficacy: there have been too many positive results against placebos for its effect to be deniable. It is significant that leukotriene can cause constriction of the small blood vessels in the veins, and so affect the flow of blood. Butterbur, in inhibiting its biochemical production, helps to keep these blood vessels open. Lekotrienes are also important components of inflammation, and altogether it appears that whatever the real cause of migraine, the petasin isomers in butterbur have an effect in inhibiting its initiation. Add to that the potential reduction in calcium content that can cause blood vessels to become less flexible, and the argument for its effectiveness is both irrefutable and well explained. In one example of such a double blind study that is representative of many, a group of patients given 50 mg butterbur extract twice a day for twelve weeks experienced a 60% reduction in the frequency of attacks, a reduction in the severity of the attacks they did have, and a reduction in the length of the attacks. Although the vascular theory of the cause of migraine is no longer supported, maintenance of the vascular system appears to at least reduce the likelihood of attacks. The effect of butterbur on asthma and other allergic reactions is also well documented. This again is due to its anti-spasmodic properties and inhibitory effect on the inflammatory immune response through the inhibition of leukotriene synthesis and the consequent positive effect on the metabolism of prostaglandin. Prostaglandins also constrict vascular smooth muscle cells, regulate the mediation of the inflammatory response and constrict general smooth muscle cells. All of these can lead a to a variety of disorders cause by smooth muscle spasms in additional to urinary incontinence, such as menstrual cramps, liver and gastrointestinal disorders and asthmatic conditions. In one study of allergic rhinitis, administration of butterbur extract appeared to result in a reduction in the histamine and leukotriene content of nasal fluids and no difference was noticed between this treatment and histamine treatment. This was a useful study because histamines causes drowsiness and butterbur can be used as a substitute for histamine without the sedative effect. A study in Germany in 1993 has shown that the stomach ulceration caused by the anti-inflammatory medications for arthritis was reduced by the administration of butterbur extract Cetirizine is a commonly prescribed prescription treatment for allergic conditions, and studies comparing that with butterbur demonstrated them to be equally effecting in reducing the symptoms typical of allergic reactions such as sneezing, runny nose and nasal congestion. 50% of the patients in the group took each and there was no difference in results. Again it was explained by the petasin limiting the production of leukotriene and histamine, both of which are produced by the immune response and promote mucous secretions and inflammation. Ubiquinol, which is the reduced from of Coenzyme Q10 (CoQ10), has been recently added to the supplement offerings of many companies and has generated a lot of confusion along with its excitement. As a supplement, ubiquinol is somewhat new, but as a critical part of human metabolism, our knowledge of ubiquinol goes back to the discovery of CoQ10. Although CoQ10 is often thought of as a “static” nutrient in the context of nutrition, it actually interchanges between two useful states: the oxidized ubiquinone, and the reduced ubiquinol. Coenzyme Q10 is a member of a family of important biological compounds which are referred to as ubiquinones. It is a lipophilic, water-insoluble substance, which takes part in a large array of biochemical oxidation and reduction reactions. It was first identified in 1957 as an essential component of the energy production system in cells. CoQ10 and other members of the ubiquinone family have, since then, been identified as critical metabolic compounds in a range of aerobic organisms. Because of its crucial role in metabolism, humans have the ability to make their own CoQ10, although small amounts can be obtained through diet and as supplements. In humans, CoQ10 is found in each cell in the body, but is particularly abundant in tissues which have large energy requirements such as the heart, liver, kidneys, and skeletal muscles. Smaller amounts can be found in the brain, lungs, and intestines. There are also substantial amounts that can be found in circulation, which are most often associated with lipoprotein particles. In total, CoQ10 in a normal adult has been estimated to be between 0.5 and 1.5 grams. Inside cells, about half of the CoQ10 is found within the mitochondria, where the final steps of CoQ10 production occur. CoQ10 which is not located in areas of the cell and are not charge with producing cellular energy can amount to about 50-60 percent of the total CoQ10 pool. CoQ10 can be found throughout cell membranes and in other cellular structures such as the nucleus, cytoplasm, and endoplasmic reticulum. Some experimentation has also concluded that, while the final steps of CoQ10 production occur in the mitochondria, it can be exported to other sub-cellular locations. While participating in various oxidation and reduction reactions, CoQ10 is cycled between two stable states: a fully oxidized form referred to as ubiquinone, and a fully reduced form called ubiquinol. CoQ10 cycles through these oxidated/reduced forms in order to achieve its metabolic goals. The cycle of CoQ10 is simple. Ubiquinone picks up electrons and then becomes ubiquinol. Ubiquinol then release its electrons and becomes ubiquinone again. Therefore, it would seem that CoQ10 has a very simple function of moving electrons, as the transfer of electrons is a fundamental step in the production of energy, the regeneration of antioxidants in cell membranes, and the construction of other important biological molecules. Each cell that is in the body needs a source of energy in order to survive. Therefore, sugars, fats, and amino acids are broken down in order to make energy. In the mitochondria, CoQ10 is abundant, as it carries electrons to aid in the chemical reactions that burn cellular fuel and produce chemical energy to form ATP. Since substantial amounts of ATP are needed to power our cells, the importance of CoQ10 in human metabolism is easily understood. Both forms of CoQ10 are needed to transfer electrons between energy-producing reactions. Outside of the mitochondria, CoQ10 performs a slightly different role as a membrane and antioxidant. About half of the human body’s total CoQ10 pool may be functioning in this capacity. CoQ10 is one of the major antioxidant elements of the LDL particles and is also one of the first to be depleted when LDL is subjected to oxidation. A discussion of CoQ10 would not be complete without mentioning its documented health benefits. Supplemental CoQ10 has been the subject of a lot of studies over the last half century, especially in applications for cardiovascular health. Many studies have shown benefits of CoQ10 in patients who are diagnosed with chronic heart failure, exercise-induced angina, hypertension, or those who have recently experienced infarction. There is also early evidence showing that CoQ10 may protect the heart from damage during chemotherapy, bypass surgery, or in diabetes. Aside from its cardiovascular uses, CoQ10 has been studied for its benefits in other conditions involving dysfunctions in cellular energetics, neurological degeneration, or oxidative stress damage. Although the clinical evidence for the potential benefit of CoQ10 in many of these applications shows promise, the variability in study outcomes proves it necessary to further research these areas for a more definite answer. As we have previously seen, CoQ10 functions by cycling between two stable forms, ubiquinol and ubiquinone. This cycle results in the generation of cellular energy and the protection of membranes and lipids from oxidation. Dietary or supplemental CoQ10 also takes part in this cycle. Supplemental ubiquinol may have a distinct advantage over ubiquinone in its facility of absorption. Like many fats and lipophilic nutrients, CoQ10 is usually taken up by the intestinal electrolytes, packaged into lipid particles, and then released into the lymphatic system. From there, these particles are transferred into circulation where they are free to be transported throughout the body as needed. The absorption of dietary CoQ10 is actually quite poor since it has limited solubility in lipids and depends on other contents of the gut. Some studies have measured that absorption is as low as 2-3 percent of the total dosage. One of the most thrilling consequences of the development of a stabilized dosage form of ubiquinol is its ability to be absorbed more efficiently than ubiquinone. There is evidence that CoQ10 must be reduced in intestinal enterocytes before the release into the lymphatic system. This, paired with absorption/reduction, may be a rate-limiting step of CoQ10 assimilation. Dietary ubiquinol avoids this reduction reaction, and is directly available for absorption, which explains why ubiquinol-based CoQ10 supplements exhibit enhanced bioavailability over ubiquinone supplements. Preliminary studies in humans have shown that absorption of ubiquinol is at least double the absorption of ubiquinone. Comparisons of blood levels between trials also estimate the improvement in absorption to be significantly higher. Future studies are necessary to more accurately determine ubiquinol’s enhanced absorption, and what effect the patient age or medical condition may have on these results.

Astaxanthin is a member of the carotenoid family responsible for the red color of many types of algae. Being a carotenoid, it is a powerful antioxidant and free radical scavenger as well as support proper eye function. However, such a bland statement belies the true worth of astaxanthin in its fight against free radicals. It is of particular benefit in its ability to absorb the high energy of singlet oxygen, releasing it as heat, and returning the singlet oxygen to its ground neutralized state. Singlet oxygen is a particularly harmful and reactive oxygen species of free radical that is formed in the body as part of our normal metabolism, and that contains a high level of free energy that can be used to oxidize and destroy the cells of your body. In achieving this, astaxanthin is regarded as one of the most potent plant derived antioxidants known, being up to ten times more active than beta carotene, Lutein or its cousin canthaxanthin. So why are antioxidants so valuable to our biochemistry and what would happen if they did not exist? Free radicals are thieves that use your body as their operating ground. They operate by stealing an electron from a molecule that comprises part of you, and in some cases when this occurs the cell from which the electron is removed is destroyed. Generally electrons go around in pairs, but occasionally an electron pair can lose one of the electrons during a chemical reaction. Many such reactions occur naturally inside the body, especially during the production of energy from blood sugars in the mitochondria, and such a molecule containing a single unpaired electron is called a free radical. Free radicals are also generated by the reactive components of many pollutants such as traffic fumes and cigarette smoke, tars and pesticides, and also by the effect of UV radiation in sunlight. The only purpose of a free radical is to steal an electron from the first source it can find. Such reactions occur very rapidly after the free radical has been generated, and if this electron belongs to another body cell, then the cell is destroyed leading to effects such as premature aging or even cancers. Free radical oxidation of the LDL lipids that carry cholesterol around the blood causes the deposition of fatty plaques inside arterial walls that eventually become constricted or even blocked, leading to heart disease or strokes. That is why antioxidants that destroy these free radicals are so prized, and the more of them that there are in your body then the less affect the free radicals will have on you. You will retain your youthful looks longer, and will be less liable to suffer from heart disease, cancers, circulation problems and conditions such as diabetes. Many abnormal conditions can be laid at the door of free radicals. The stronger the free radical, the more harm it can do to you and a strong antioxidant such as astaxanthin is a very powerful weapon in your armory against them. Astaxanthin is a member of the oxygenated xanthophylls, and its high level of antioxidant power likely comes from the ketonic and hydroxyl functional groups of the ionone ring structure. It is more polar than most carotenoids, and this is a likely reason for its ability to span the cell membrane layers, with the active groups close to the hydrophilic-hydrophobic interface. They are thus more readily available at the sites where most free radicals tend to be found and provide immediate protection to the cell membrane and also to the intracellular mitochondrial membrane. Many antioxidants destroy free radicals by donating an electron, and become oxidized themselves. Astaxanthin, however, does not do this, but instead adds the free radicals to its long double bonded chain hence avoiding oxidation and rendering it much more powerful than normal antioxidants. It is unusual among antioxidants in that it is also able to cross the blood-brain barrier, and so reduces oxidative stress that can cause neurological disorders in general, and also problems with eyesight. It can also attach itself to lipoproteins to enable it to be carried throughout the bloodstream, being available anywhere that free radicals are generated. The carotenoid is also active against active oxygen species that are responsible for inflammation. Another property is its ability to neutralize the oxygenated free radicals formed by the photo-oxidation properties of UVA and UVB radiation. Included in these are the previously mentioned highly reactive singlet oxygen and also triplet oxygen that astaxanthin is able to neutralize without becoming oxidized. In fact reactive oxygen species in general can cause oxidative stress, and they have been thought responsible for many forms of disease and health conditions, and the powerful effect of astaxanthin in targeting many of these has led it to be regarded as highly beneficial to the immune system and to health in general. Another benefit is the ability of the substance to help prevent the oxidation of high density lipoproteins (HDL) that are responsible for carrying cholesterol in the blood back to the liver for destruction. Free radical oxidation of HDL impairs its ability to transport cholesterol, and so decreasing the level of such free radical oxidation will by definition increase the quantity of good HDL available, and hence reduce the concentration of cholesterol in the blood. Studies have proved this to be the case, and astaxanthin supplements are very beneficial to those suffering from high blood cholesterol levels, and helps protect them from heart disease and strokes. Parthenolide is unstable so don’t take it after its sell by or best by date. BUTTERBUR Butterbur is also useful in the treatment of migraines. The active ingredient here is petasin, a sesquiterpene ester that also has anti-inflammatory properties, all though it works in a different manner to feverfew. Here, it inhibits the activities of leukotriene generation in neutrophils and eosinophils, which indicates that it also possesses anti-allergenic properties that could help asthmatics that also suffer from migraines. It does not prevent the release of histamine, but does appear to reduce smooth muscle spasm. You should never take the raw herb, only the extract, because it contains alkaloids that can harm you. The butterbur extract, however, is a perfectly safe supplement and found effective by many people in the treatment of migraine. GINGER If your migraine is at the front of the head, then you should try ginger. Unlike feverfew, ginger works on a migraine as it is occurring, and improvement can be achieved for a bad migraine within half an hour. In fact if, like many people, you can feel a migraine coming on, half a teaspoon of freshly ground ginger can stop the pain before it starts. Periwinkles form the plant genus Vinca, of which the lesser periwinkle and the greater periwinkle are the two members. They are also a form of dogbane, known for its alkaloid properties. Periwinkles are of interest to the medical profession due to their biosynthesis of a number of alkaloids that are used to protect themselves from bacteria and are also toxic to the herbivores that would otherwise eat them. Many other plants produce alkaloids, perhaps the best known being poppy, that produces the alkaloid opium that is the precursor to heroin. The alkaloids of the periwinkle, however, have a much more beneficial use for mankind in their action on certain types of cancer such as Hodgkin’s disease, and also on the flow of blood to the brain.
The components of the periwinkle include vinblastine, which is used in the treatment of Hodgkin’s lymphoma, and it is the only plant known to produce the alkaloid, although only in very small quantities after synthetic seeding. This involves introducing modified tryptamines to the plant so that they produce the desired alkaloid, although only in quantities of 0.002% of the weight of the plant. Although small, this is still less expensive that synthesizing the alkaloid in a chemical plant rather than a biological one! Chemists are now seeking more readily synthesizable alternative forms of the alkaloid that have the same effect, again using the periwinkle as a botanical chemical factory to provide them with a lead as to possible synthetic routes. However, it is for the application of another alkaloid of this amazing little plant that we are more concerned with here. In addition to vinblastine, the periwinkle produces the alkaloid vincamine, a type of tryptamine that can be extracted from the leaves of the lesser periwinkle, the Vinca minor. This periwinkle has been used through the ages as an invigorating tonic and as an astringent to treat bleeding gums and sores in the mouth. Vincamine promotes the aerobic glycolysis that is essential to cerebral health. Normal glycolysis, or the conversion of glucose to energy in the mitochondria of the cells of the body, occurs best in the absence of oxygen (anaerobic glycolysis). However, the brain produces up to and over 90% more energy through aerobic glycolysis in the presence of oxygen than through anaerobic. Hence one of the needs for such a rich supply of oxygenated blood being needed by the brain. As people grow older, or their brain tissue becomes damaged, then the supply of blood can be reduced and more and more anaerobic glycolysis is switched on. This results in an increasing loss of brain energy and hence brain function. Vincamine can reverse this effect, or at least maintain the status quo, and so enable people to maintain their brain function for longer. It is also what is known as a vasodilator, and dilates the blood vessels in the brain allowing a greater blood flow. It is known to be beneficial in relation to tasks requiring focused concentration such as mathematical problem solving, and has also been found effective in the treatment of people with poor memory. Vinpocetine (ethyl apovincaminate) is a derivative of vincamine, obtained by slightly modifying the molecule to produce a commercial form of the alkaloid. This possesses all of the beneficial effects of its precursor, including its positive effects on memory, believed to be due to the stimulation of serotonin production that improves the rate at which the brain can process information. Tinnitus, or ringing in the ears that has severely reduced the quality of life of so many people, can also be treated with vinpotecine. Its vascodilation effect is thought to be through its action as a phosphodeaterase 1 (PDE1) inhibitor that results in an improvement in the plasticity of neurons. The mechanism is complex, but the end result is an improvement in the cognitive abilities of the subject. This is supplemented by the effect of vinpocetine on the calcium levels in the blood vessels. This renders them more plastic in much the same way that some anti-hypertension treatments work to reduce blood pressure by increasing the plasticity of the blood vessels by modification of their calcium levels. It can also reduce the viscosity of the blood through its action on the “stickiness” of leucocytes and so allow the blood to run more freely through the capillaries of the brain. These effects have been demonstrated through double blind studies, and there is little doubt that vinpocetine helps to maintain a healthy cerebral circulation. So let’s have a recap on its various effects, and then finish with a summary of the conditions that the alkaloid can be used to treat or improve. The theory and biochemistry behind these effects is known, but is complex, but here are the major actions: a) It improves blood flow by modifying the calcium content of the blood vessels and so improves their elasticity, hence allowing a freer movement of blood. In effect it helps the blood vessels to dilate easier. b) It inhibits the enzyme PDE1 and by doing so helps to restore the elasticity of neurons in aging brain cells, allowing them to relax more and operate more effectively. c) It promotes aerobic glycolysis, and so the rate at which the mitochondria of brain cells produce ATP (adenosine triphosphate) that is the chemical manifestation of energy. d) It helps to maintain a good supply of glucose and oxygen in the brain to allow (c) to occur. Not all of these may be separate effects, but the outcome is an improvement in conditions such as short term memory loss, dementia and other conditions associated with a reduction in the blood supply to the brain including Menière’s syndrome and vertigo. There is also evidence that it helps with hearing problems, macular degeneration of the eye, and fatigue. It is a supplement that should be taken by the elderly to help with age-related memory problems, but has also been found to be beneficial in normal healthy people. Coconut is extracted from coconuts, the coconut palm often being called the tree of life and for good reason. However, its benefits were lost to America for many years due to the politics of the vegetable oil industry claiming that all saturated fats are bad. This is not based on scientific or medical fact, and coconut oil was recently rediscovered in the USA, and the health benefits enjoyed by the rest of the world are once again available to Americans. Coconut palms grow in most tropical climates such as Southern California and Florida, the Philippines and the Caribbean. Refined coconuts oils are mass produced and much of the beneficial nutrients are lost by the refining process. Virgin coconut oil is by far the better form, though is a slower process. It is generally obtained by shredding and dry milling the meat, and then cold pressing to produce the milk. The milk is fermented for a day or so and the separated oil collected. Such oils are analyzed to contain over 50% lauric acid and high levels of phenolic antioxidants. Much higher than from refined oils. It is said to be the healthiest oil on earth. So why did the American authorities legislate against its use? It is because of the bad press received by saturated fats and oils. You can have saturated, monounsaturated and polyunsaturated fats. The majority of the fats and oils in our diet are composed of long chain fats and oils. The chain length refers to the length of the hydrocarbon chain, whether saturated or unsaturated. It is long chain saturated acids that are bad for your cardiovascular system, and that should be avoided. However, there are also short chain and medium chain fatty acids that are more easily metabolized by the body. Coconut oil consists predominantly of medium chain fatty acids that help to protect against heart disease and cholesterol rather than promote it. The highest levels of medium chain fatty acids are found in coconut oils and palm kernel oils, and these are far more nutritious than the saturated long chain fatty acids found in animal fats and just about all vegetable fats. The main MCFA in coconut oil is lauric acid. Lauric acid is generally regarded as being responsible for the vast majority of the health benefits of coconut oil. The only other source of this fatty acid other than coconut oil is in human breast milk. After absorption, lauric acid is metabolized into monolaurin that is the monoglyceride used by the human body to destroy viruses and microbes, and attack the fat coated viruses that cause such diseases as HIV. It destroys flu viruses, a number of dangerous bacteria and protozoa such as giardia lamblia. However, let’s have a look at some of the lesser known effects of coconut oil, such as its effects on the thyroid. Anything that blocks the efficient operation of the thyroid and secretion of the thyroid hormones can lead to excess estrogen in the body that can contribute to excessive blood cholesterol. There is a body of evidence that polyunsaturated fats and oils, such as soy, can have this effect is many people. This is not to infer that all hypothyroidism can be cured through the use of coconut oil, only those cases caused by consumption of excessive amounts of polyunsaturated fats. Although this is refuted by some medical practitioners that are against the use of supplements in general, it has been supported by medical evidence. Coconut oil is also good for the digestion, and helps the cure of many digestive problems including Crohn’s Disease. It helps to build lipoproteins, fats and bile in the liver, the latter being essential for digestion. It helps to create a much healthier digestive tract that makes for generally better digestion and more efficient use of the foods you eat. The omnivore vs vegan argument as to who is right and who is wrong can be argued from a number of different platforms. There is the ethical issue of whether we should eat other animal life, and also the argument as which is ‘better for you’, based on arguments such as vitamin B12 is not available from a vegan diet. There is even the ‘lifestyle’ argument: does our lifestyle define our diet? However, strictly, the only argument for or against either diet should only be made upon human biochemistry. Do both meet the needs of our biochemistry, or does one or the other lack something essential in our biochemical pathways? Obviously omnivores will lack nothing except by choice, since all foods are available for their consumption. If vegans do lack a specific chemical need, then is that available as a supplement in a form that can be effectively used in the chemistry of our bodies. The one argument accepted by both sides is that it is essential for all animals to consume living things in order to stay alive themselves. These living thinks need not be alive at the time of consumption, but it is necessary that they eat the flesh of plant or animal life that at one time was alive and contained DNA. What that infers is that it is only vegetables that can survive on non-living tissue and this appears to be borne out in practice. No living animal known can live on inorganic matter only, but most plants can and do. Not all though, the Venus fly trap being an example. It is easy to extend the moral problem of eating living tissue to living vegetable tissue that also contains DNA, and the argument must lie between animal and the derivatives of animals, and non-animal tissue. It has not yet been found that any organism has yet crossed the animal-vegetable divide, so the division is a valid one. That might seem obvious, but it is necessary to establish that for the argument between vegan and omnivore diets to be valid. The consumption of protein derived from meat is not a prerequisite for size and muscle bulk, since the largest dinosaurs in the world were all herbivores, the largest being a member of the sauropod family at more than 175 tons, eclipsing the largest meat eater, the gigantosaurus at 8 tons. Thus, meat does not mean bulk. However, what has been proved is that the fastest creatures are carnivores. Hence if you want to be a top class sprinter, eat meat! Carnivores, with their lean muscle mass and highly efficient quick use of available energy, have very short digestive tracts which are not good for digesting vegetable matter, but make best use of animal proteins and expel unnecessary mass from the body quicker. The argument in favor of the vegans is that the human digestive tract is not that of a carnivore. In herbivores, the food takes longer to digest, and hence it remains in the digestive system longer. This means a longer alimentary canal, longer than humans have. Herbivores also move slowly, and a good example is the comparison of speed between the omnivorous chimpanzees and other small monkeys and the herbivorous gorillas and orangutans. On the one hand you have lean fast moving machines, while on the other you have large bellies and slow moving larger animals. Check out cows and sheep and compare their body fat with ours. Nor are we like herbivores. So what are humans? Omnivores! Our teeth and intestines are those of omnivores, the teeth designed for ripping and tearing meat, and stripping leaves from trees, but also for grinding grains, and our intestines are something between the long and the short. People are able to eat and live on every type of food imaginable from brains to intestines to leaves to roots to ants and grubs. The argument is therefore futile to consider historically. Let’s then study the advantages and disadvantages of each type of diet. Human beings are capable of life through consuming either animals or plants, or both. The argument seems, therefore, to be one of morality rather than biochemistry. However, is that really so? The vegan refusal to eat dairy products should not be taken as extremism, since the human being is the only animal species to drink milk of another species, or to use it to make other products. It is a practice born long after cattle were husbanded by humans for food. The problem with eating animal products lies not in the meat itself, but in the fat. Animal fat is saturated, which means that the fat molecule has no active double bonds in the chemical structure that can be used to break the fat down. Animal fat also contains cholesterol, yet we cannot survive without cholesterol. It is the human band aid, used by the body to patch up damage to the cardiovascular system. Only, sometimes, too much is laid down and the arteries get blocked. However, many vegetable products have more saturated fats and cholesterol than many animal products, so a balance is called for. The unsaturated fats and oils for humans are said to be derived from seeds, such as flax seed and fish, especially oily fish. These are the Omega-3 oils. Although they can be obtained from some seeds and nuts, it has been proved that the best come from oily fish, such as wild salmon, mackerel and sardines. The B vitamins are essential for life. The best sources are animal sources, though you get them from some vegetable sources such as brewers years (who eats lost of that?) and others, but animal sources are the best. Also, there is no evidence to suggest that vegans live longer than omnivores. In fact all of the evidence indicates that a middle road is the best. For human beings the healthiest diet includes both meat and vegetable tissue. The best solution to good health is neither vegan nor carnivore. Nor is it traditional vegetarian, since it is the dairy products that cause many of our dietary products. Studies of the biochemical pathways have demonstrated that all chemicals need to sustain healthy human growth and life are not available from a classic vegan diet. Some animal protein and B vitamins are essential that cannot be obtained form a normal vegan diet. It is possible, however, to maintain life by means of supplements. However, for the healthiest form of human life, our biochemistry, history and physiology indicate that there is a balance somewhere between the extremes of both views that is right for us, and that either diet can be sustained with appropriate supplementation based upon what is missing from one diet or the other. Breast Cancer and Nutritional Supplements
There is probably nothing more frightening for a woman than the discovery of a lump in her breast. Cancer and all its consequences quickly come to mind. This quick association may materialize, in part, because no woman is immune. Most have a friend, a sister, a mother, or a coworker who has been diagnosed with the disease. And they know how difficult dealing with this disease can be. Fortunately, 80% of all breast lumps are not cancerous. Most are cysts or a benign clump of tissue.
Over her lifetime, a woman’s breasts undergo many, many changes. From before puberty and on, breast tissue is continually evolving. Breasts often feel different before menstrual cycle, returning to normal a few days after. Pregnancy certainly causes changes in a woman’s breasts, as does breastfeeding. And as women age, breast tissue becomes less dense.
Because of these continual changes, breast tissue especially requires adequate nutrition. While everyone benefits from a healthy diet, there are additional nutrients from which women can specifically benefit.
In this issue of Ask the Doctor, we will discuss breast cancer and the vast amount of research that has explored the role nutrition plays in this serious and still deadly disease. Specifically, we will discuss how two B vitamins, calcium D-glucarate, broccoli extract, green tea, maitake mushrooms, and iodine can all help prevent breast cancer.
Q. How can these nutrients prevent breast cancer?
A. Scientists learn a lot about disease from simply observing what is happening around them. One observation that has been recognized for many years is that certain cultures have very low incidence of breast cancer. Women n
Moms (and dads) have also learned a lot about diseases simply by observing what is happening in their families. They have notices that certain vegetables play a large role in the prevention of all types of diseases, including cancer. And, accordingly, they have been urging their offspring to eat their vegetables for several generations.
Building on these observations, scientists have designed and carried out many studies to determine what it is about these nutrients that can prevent breast cancer. What they have discovered, so far, follows. Let’s start with the B vitamins.
Vitamin B12
Deficiencies of this vitamin can result in a serious type of anemia. Nerve damage can also occur if B12 levels are too low. Researchers are now investigating whether breast cancer may, in part, be caused by a B12 deficiency as well.

Another study, this one taking place in a laboratory setting, discovered that vitamin B12, applied directly against experimental breast cancer cells, actually stopped the cancer cells from growing. The researchers conducting the experiment believe that giving vitamin B12 to women with breast cancer as part of a chemotherapy regime, might help keep the cancer in check.
Folic Acid
Low folic acid intake is linked to the development of all cancers. This is because folic acid is crucial to the making and continual repair of DNA, the molecule that carries our genetic code. A recent study discovered that high intakes of folic acid might actually reduce the risk of breast cancer. The researchers looked at the diets of over 2600 women. During interviews with the researchers, the women reported what they usually ate. Once the data was collected, the results showed that women, who ate lots of foods that contained folic acid, had much lower rates of breast cancer.
There is no clear-cut, single cause of breast cancer. Many factors are required for the disease to appear. One such factor is estrogen. A recent study showed that women who developed breast cancer tended to have higher levels of estrogen circulating in their bodies than women without breast cancer. This means that women who got their periods before age eleven or entered menopause after age fifty-five have a higher risk of breast cancer. This also supports the theory that the number of menstrual cycles a woman has affects her risk for breast cancer.
Another factor is drinking alcohol. Because alcohol raises estrogen levels, if a woman consumes even moderate amounts of alcohol her risk of breast cancer also is increased. The link between alcohol and breast cancer may even be stronger than other dietary links. However, an important study has discovered that folic acid may uncouple this link.
A very large study of over 34,000 women recently studied the effect of folic acid on the risk of breast cancer. This project was part of the Nurses’ Health Study, an ongoing, long-term study that looks at nutrition’s role in the development of disease. The women in the folic acid and breast cancer study were followed for 12 years. The participants completed detailed food questionnaires that provided the researchers with important data.
The women were divided into four groups:
1. Women with low folic acid levels and drink alcohol
2. Women with high folic acid levels and rink alcohol
3. Women with low folic acid levels and don’t drink alcohol
4. Women with high folic acid levels and don’t drink alcohol
Within these four groups the women were further divided into subgroups according to the amount of alcohol they consumed each day and their specific folic acid intake.
The researchers found that women who consumed the lowest amounts of folic acid and drank at least one alcoholic beverage a day had the highest rate of breast cancer. In contrast, women who had high intakes of folic acid and also drank at least one alcoholic beverage a day, had the same rate of breast cancer as the women with high folic acid intakes who did not drink. In other words, women who had high levels of folic acid in their diet erased their alcohol-related increase in breast cancer risk.
Calcium D-Glucarate
It seems estrogen can be both friend and foe. While women need the hormone to soften skin, thicken hair, and fill out hips and breasts, estrogen can also nourish breast tumors, helping them grow bigger, stronger, and more deadly. Thanks, in part, to good nutrition, American women get their periods early and go through menopause alter in life. Women today also have fewer pregnancies; families with one or two children are quite common.
All of these factors increase the time women’s bodies are exposed to estrogen. As we discussed before, longer exposure means increased opportunities for estrogen to cause trouble. It is also a troubling fact of modern life that we are continuously exposed to cancer-causing chemicals and toxins. These toxins come in part from contaminants in the food we eat and pollutants in the air we breathe.
The body does have a system that eliminates some of the excess estrogen and toxic chemicals before they can cause harm. In the liver, they are bound or attached to a chemical called glucuronic acid. The bound toxin or estrogen is then excreted in bile and eventually eliminated as a waste product in the stool.
However, an enzyme called beta-glucuronidase can break this bond between estrogen and glucuronic acid. When this happens, the hormone or toxin is released from its bone, capable of causing harm once more. Increased beta-glucuronidase activity is associated with an increased risk for various cancers, particularly hormone-dependant cancers like breast cancer.
Fortunately, scientists have discovered that a natural substance found in foods calcium D-glucarate (CDG) can stop the activity of beta-glucuonidase. CDG keeps the harmful estrogen bound to glucuronidase. While CDG is found in fruits and vegetables, the amounts may not be sufficient to maintain effective levels to stop beta-glucuronidase.
CDG has been shown in experimental studies to significantly stop beast cancer growth. And several human trials are currently underway with CDG to determine its capability to decrease the breast cancer risk in women at high risk for the disease.
Iodine
There are some very interesting connections between breast tissue and thyroid tissue. Iodine is an essential trace element present in a hormone of the thyroid gland and is involved in several metabolic functions. One iodine function is the protection of breast tissue from cancerous cells.
In a laboratory study, researchers exposed breast cancer cells and breast tissue without any cancer to a type of seaweed that contains high levels of iodine. The seaweed killed all of the cancerous cells, yet did not harm the normal breast cells. Japanese women frequently eat this type of seaweed and have very low rates of breast cancer. The study’s researchers believe one reason for this low incidence of breast cancer may be the iodine in the seaweed.
And, for some as yet unknown reasons, women who have thyroid cancer are at higher risk of developing breast cancer. While they are unsure why this happens, researchers are continuing to study this link, and support of healthy thyroid function remains an important consideration.
Broccoli
For quite some time, scientists have observed that cruciferous vegetables, such as broccoli, cabbage, and cauliflower, significantly reduce the risk of disease, including cancer. It seems a phytochemical in broccoli sulforaphane, is one of the chemicals responsible for this beneficial activity. Sulforaphane increases certain enzymes in the body called phase 2 enzymes that deactivate cancer-causing chemicals.
Breast cancer cells exposed to sulforaphane in several lab experiments showed that the compound inhibited the growth of the cancer cells up to 80 percent. Researchers are in the process of setting up clinical trials to study sulforaphane’s effect in women who have breast cancer.
Green Tea
There is a tall amount of research, including finding from the Nurses’ Health Study, that suggest green tea beverage consumption is associated with a lower incidence of breast cancer. In fact, researchers have long noted the low rates of breast cancer in
The active compound in green tea responsible for breast cancer inhibition is epigallocatechin-3 gallate or EGCG. When breast cancer cells are exposed to EGCG in lab experiments, the cells stop growing, lose their ability to replicate, and die.
In a recent study, researchers discovered that drinking green tea prevented the recurrence of breast cancer in women who have previously been diagnosed and treated for the disease. This study involved over 1100 Japanese women. The women who drank green tea every day had very low rates of their breast cancer returning.
Maitake Mushrooms
For thousands of years, maitake mushrooms have been linked to good health in those who eat them. Called “dancing mushrooms” (possibly due to their wavy, rippling appearance or possibly due to the little dance of joy mushroom hunters perform when they find them in the woods), maitakes contain an important compound called D-fraction.
Not only does the D-fraction in maitake mushrooms stop the growth of cancerous tumors, it also alerts and stimulates immune cells (including macrophages and natural killer cells) to fight the disease. Maitake also inhibits some of the mechanism that promotes metastasis, or spread, of cancer cells in the lymph and bloodstream.
Because of this success, maitake is now being used in clinical trials of women with breast cancer. One study reported significant improvement of symptoms, including reduction of the tumor. The maitake was given to breast cancer patients in addition to standard chemotherapy.
Q. Should these nutrients be used in place of traditional treatment for breast cancer?
A. Absolutely not. None of these nutrients can cure breast cancer. However, they can be a part of a validated plan of treatment. If you have breast cancer, talk to your health care practitioner about these nutrients. Remember, nutritional supplements are just that: supplements to food, medication, and treatment. They are intended to enhance and prevent, not replace.

Bone Health Approximately 44 million American women and men aged 50 and older have osteoporosis (severe bone loss) or osteopenia (mild bone loss), with women being affected about twice as often as men. At least 1.5 million fractures of the hip, vertebra (back or neck), or wrist occur each year in the United States as a result of osteoporosis, and the annual cost of treating this disorder is nearly $14 billion and rising. Unfortunately, the toll in human suffering and loss of independence is even greater. In this issue of Ask the Doctor, we will discuss the risk factors for osteoporosis and some key nutrients you can add to your diet that can minimize bone loss and reduce your chances of developing this disease. Q. What are the risk factors for osteoporosis? A. Small body frame, underweight, Caucasian or Asian race, a sedentary lifestyle, cigarette smoking, excessive alcohol or caffeine intake, high intake of carbonated beverages (especially colas), and having other family members with osteoporosis all increase personal risk of developing the disease. Certain medical conditions, including diabetes, celiac disease, hyperthyroidism, rheumatoid arthritis, chronic obstructive lung disease, hyperadrenalism, and hyperparathyroidism, are all associated with an increased risk of osteoporosis. Some medications increase the rate at which bone is lost; these include drugs prescribed for the treatment of seizures, drugs used for blood thinning, steroids such as prednisone, aluminum-containing antacids, and loop diuretics (furosemide {Lasix}). Q. Isn’t bone loss just a normal consequence of aging? A. Although bone mass normally declines after the age of 35, bone loss severe enough to cause fractures after just minor trauma (such as bump or fall) seems to be a relatively new phenomenon. Osteoporosis was rare in the late 19th century, and it was not until around 1920 that the condition began to attract attention among doctors. Since that time, the percentage of people who develop osteoporosis has continued to increase. For example, the age-adjusted prevalence of osteoporosis in Q. Can osteoporosis be prevented? A. Engaging in regular weight bearing exercise, avoiding excessive consumption of alcohol and caffeine, and quitting smoking will slow the rate of bone loss. Eating adequate, but not excessive, amounts of protein also enhances bone health. In addition, a growing body of research has shown that supplementing with various vitamins and minerals may not only help prevent, but in some cases actually reverse, bone loss. At least 15 different nutrients have been found to play a role in bone health. Q. What type of calcium is best? A. For most people, calcium salts are absorbed about the same, between 30% and 40% of the administered dose. People who low stomach acid (hypochlorhydria) should not use calcium carbonate, because that form of calcium is absorbed poorly in the absence of stomach acid. Calcium phosphate may be preferable for many older people, because phosphorus is necessary for normal bone formation, the phosphorus intake of older people is often low, and calcium supplements inhibit the absorption of phosphorus. Also, calcium bound to phosphorus is the form in which calcium in the bone is stored, and it has a much greater bone activity than other forms. Q. How much vitamin D is needed to promote strong bones? A. Because vitamin D is produced when the ultraviolet rays from the sun hit skin, people who stay out of the sun, wear sunscreen, or live in a northern latitude (such as Boston or Seattle) where less ultraviolet light reaches the skin, are at increased risk of vitamin D deficiency. In addition, aging decreases a person’s ability to synthesize vitamin D in the skin. Results from five research trials on vitamin D found that supplementation with 700-800 IU of vitamin D per day decreased the number of hip fractures by 26%, but 400 IU per day was ineffective. In addition to enhancing bone health, vitamin D improves nerve and muscle function in older people, thereby reducing their chances of falling down. Supplementation of elderly women with 800 IU of vitamin D per day has been shown to decrease the number of falls by about 50%. Q. Is that much vitamin D safe? A. The Food and Nutrition Board of the Q. Why would nutrients besides calcium and vitamin D is important? A. Bone is living tissue, constantly remodeling itself and engaging in numerous biological functions. Like other tissues in the body, bone has a wide range of nutritional needs. The typical refined and processed American diet has been depleted of many different vitamins and minerals, some of which play a key role in promoting bone health. Not getting enough of one or more of these micronutrients may be and important contributing factor to the modern epidemic of osteoporosis. In addition, supplementing with calcium may cause a loss of magnesium, zinc, silicon, manganese, and phosphorus, unless these nutrients are also provided. Q. What nutrients besides calcium and vitamin D promote healthy bones? A. Magnesium, zinc, copper, manganese, vitamin K, boron, strontium, silicon, folic acid, vitamin B6, vitamin B12, phosphorus, and vitamin C have all been shown to play a role in bone health. Following is a brief description of the role that each of these 15 nutrients play in building healthy bones. Calcium: A component of the mineral crystals that make up bone. Vitamin D: Enhances calcium absorption, prevents falls by improving nerve and muscle function. Magnesium: Important for bone mineralization (accumulation of minerals which form bones). Magnesium deficiency is associated with abnormal bone mineral crystals in humans. In an open clinical trial, magnesium supplementation increased bone mineral density by an average 5% after 1-2 years in postmenopausal women. Copper: Laboratory research has found that copper promotes bone mineralization and decreases bone loss, and that osteoporosis can develop if the diet is deficient in copper. Western diets often contain less copper than the amount recommended by the National Academy of Sciences. In a 2-year double-blind trail, copper supplementation reduced bone loss by 90% in middle-aged women, compared with a placebo. Zinc: Like magnesium, zinc is important for bone mineralization, and also has been shown to decrease bone loss. Low dietary zinc intake was associated with increased fracture risk in a study of middle-aged and elderly men. The zinc content of the diet is frequently low; a study of elderly low-income people found they were consuming only half the Recommended Dietary Allowance for this mineral. Manganese: Plays a role in the creation of the connective-tissue components of bone. Manganese deficiency in laboratory tests resulted in low bone mineral density and weak bones. Manganese deficiency may be associated with the development of osteoporosis. Boron: Supports creation of bone-protecting hormones such as estrogen, testosterone, and DHEA. Boron supplementation prevented bone loss in experimental studies. In human volunteers consuming a low-boron diet, boron supplementation decreased urinary calcium excretion by 25-33%, a change that may indicate reduced bone loss. Silicon: Plays a role in the synthesis of the connective-tissue components of bone. Silicon deficiency has been associated with bone abnormalities. In an observational study, higher dietary silicon intake correlated with higher bone mineral density. In a clinical trial, administration of an organic silicon compound increased bone mineral density of the femur (or thigh bone) in postmenopausal women. B vitamins (folic acid, vitamin B6, and vitamin B12): These three B vitamins have been shown to lower blood levels of homocysteine, a breakdown product of the amino acid methionine. An elevated homocysteine concentration is a strong and independent risk factor for fractures in older men and women. Homocysteine levels increase around the time of menopause, which may explain in part why bone loss accelerates at that time. In a 2-year double-blind trial, supplementation of elderly stroke patients with folic acid and vitamin B12 reduced the number of hip fractures by 78%, compared with a placebo. Strontium: This trace mineral is incorporated into bone and appears to increase bone strength. It also stimulates bone formation and inhibits bone breakdown. Controlled trials have demonstrated that strontium supplementation of postmenopausal women increases bone mineral density and decreases fracture risk. Vitamin K: Best known for its effect on blood clotting, vitamin K is also required for the creation of osteocalcin, a unique protein found in bone that participates in the mineralization process. The amount of vitamin K needed for optimal bone health appears to be greater than the amount needed to prevent bleeding. Vitamin K levels tend to be low in people with osteoporosis. In randomized clinical trials, supplementation of postmenopausal women with vitamin K prevented bone loss and reduced the incidence of fractures. Q. Which form of vitamin K is best? A. Two forms of vitamin K compounds are present in food: vitamin K1 and vitamin K2. Vitamin K1 (also called phylloquinone) is present in leafy green vegetables and some vegetable oils, and vitamin K2 is found in much smaller amounts in meat, cheese, eggs, and natto (fermented soybeans). To make things a little more complicated, Vitamin K2 itself can occur in more than one form. The two most important to this discussion are menaquinine-4 (MK-4, also called menatetrenone), which is licensed as a prescription drug in Research suggests that MK-7 from natto may be an ideal form of vitamin K. The biological activity of MK-7 in laboratory studies was 17 times higher than that of vitamin K1 and 130 times higher than that of MK-4. After oral administration, MK-7 was better absorbed and persisted in the body longer, compared with MK-4 and vitamin K1. Although both have shown ability to prevent osteoporosis in laboratory research, a much lower dosage (600 times lower) of MK-7 is required, compared to MK-4, to obtain beneficial effects. Thus, MK-7 has greater biological activity, greater bioavailability, and possibly more potent effects on bone, compared with other forms of vitamin K. The potential value of MK-7 for bone health is supported by an observational study from Q. Why is strontium so important in building strong bones? A. Strontium is of great interest to bone health researchers and has been studied in very high doses. Surprisingly, lower doses are not only safer for long-term supplementation, but may in fact have a greater impact on bone health than very high doses. Too little, and bone density is impaired; too much and health may be impaired. This is a case where dosing needs to be just right for optimal impact. Therefore, until more is known, it is wise to keep supplemental strontium at less than 6 mg per day. Q. Can people taking osteoporosis medications also take bone-building nutrients? A. Because nutrients work by a different mechanism than osteoporosis drugs, nutritional supplements are likely to enhance the beneficial effect of these medications. Calcium or other minerals may interfere with the absorption of biphophonates such as alendronate (Fosamax) or etidronate (Didronel). For that reason, calcium and other minerals should be taken at least two hours before or two hours after these medications. Also, it is always best to discuss the supplements you are using with your healthcare practitioner to create an integrated health plan. Final thoughts… Bone health ramifications extend beyond osteoporosis and fractures. Bone health is essential for freedom of movement, safety, comfort, independence and longevity. Weak bones do not heal well – sometimes they never heal at all. Osteoporosis-related fractures rob us of our mobility and consign thousands of Americans to walkers and wheelchairs every year. In fact, 40% of people are unable to walk independently after a hip fracture, and 60% still require assistance a year later. The most terrible consequence of fractures related to osteoporosis is mortality. The impairment of the ability to move around freely can cause pneumonia and skin damage leading to serious infections. It is estimated that suffering a hip fracture increases the risk of dying almost 25%. Making bone health a priority now will allow you to reap health dividends for many years to come.
